Résumé

Objective:To investigate any anti-aging effect of repeated transcranial magnetic stimulation (rTMS) and explore the relationship between the effect and relief of clinical symptoms in patients with Parkinson′s disease (PD).Methods:A total of 108 PD patients were randomly divided into an rTMS group and a control group, each of 54, while another 54 healthy counterparts were selected to form a normal group. In addition to anti-PD drug therapy, the rTMS group was given daily rTMS treatment, 5 days a week for 4 weeks, while the control group received sham rTMS treatment, with no treatment of the normal group. Before the treatment and after 4 weeks of treatment as well as and 1 month after the ending of the treatment, the subjects′ clinical exercise symptoms were evaluated using the Unified Parkinson′s Disease Rating Scale (UPDRS), a timed exercise test and the 10m re-entry exercise test. Non-exercise symptoms were assessed using the Hamilton Depression Scale (HAMD), the Hamilton Anxiety Scale (HAMA) and the Mini-mental State Examination (MMSE). Fasting venous blood samples were analyzed to quantify the serum levels of tumor necrosis factor (TNF), interleukin-6 (IL-6), interleukin-1β (IL-1β) and matrix metalloproteinase-3 (MMP-3).Results:Four weeks and 1 month after the treatment, the average UPDRS scores, exercise test times and 10m re-entry exercise test results of the rTMS group were significantly better than those before treatment and significantly better than those of the control group at the same time point. The rTMS group′s average HAMA, HAMD and MMSE scores, as well as its average P300 latency and amplitude were also significantly better than those of the control group at the same time point and significantly better than those before treatment. After 4 weeks, the average MMP-3 content in the rTMS group was significantly lower than the control group′s average, and after a month the average levels of TNF, IL-6, IL-1β and MMP-3 of the rTMS group were all significantly different from those before treatment and those of the control group. The TNF, IL-6, IL-1β and MMP-3 levels were all positively correlated with the average UPDRS total score.Conclusion:High-frequency rTMS therapy can change the phenotypes related to cell senescence, and thus has good therapeutic effect on motor and non-motor symptoms of PD.

Objective:To investigate whether TNM staging combined with systemic immune inflammation index (SII) has a high predictive value for the clinical prognosis of elderly patients with esophageal cancer.Methods:Clinical data of 118 elderly patients with esophageal cancer who received radiotherapy and chemotherapy were retrospectively analyzed, and the SII was calculated. SII and clinicopathological features were included in the Cox proportional risk model, and the prognostic index (PI) equation was obtained. Kaplan- Meier survival analysis was adopted. According to PI, the survival of patients was predicted and the predictive values of PI and TNM were statistically compared. Results:Univariate analysis showed that SII, N staging and TNM staging were closely correlated with the overall survival (all P<0.01). Cox multivariate analysis revealed that SII and N staging were the independent risk factors for overall survival. According to the results of Cox analysis, the equation of PI=0.961 × SII grouping+ 0.523 × N staging was obtained. The receiver operating characteristic (ROC) curve was drawn according to PI and overall survival, and the critical value was obtained and divided into different groups. The 1-, 2-and 3-year survival rates in the low-risk group were significantly higher than those in the high-risk group ( HR=0.365, 95% CI: 0.221-0.604, P<0.001). The prediction of overall survival by SII-N[area under curve (AUC)=0.707] was significantly better than that by TNM staging (AUC=0.560, P<0.001). Conclusion:This study preliminarily proves that the SII-N prognosis score model is better than the traditional TNM staging, which may have guiding significance for the selection of therapeutic strategies for elderly patients with esophageal cancer, and is worthy of further study.

Objective:To investigate the expressions of let-7 and microRNA-26b (miR-26b) in serum of patients with type 2 diabetes mellitus (T2DM) and their relationships with insulin resistance (IR).Methods:60 newly diagnosed patients with T2DM from March 2018 to March 2019 were selected as the T2DM group, and 70 healthy people in the same period were selected as the control group. The clinical data of the two groups were collected and the expression levels of let-7 and miR-26b in serum were measured by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR), the expression levels of let-7 and miR-26b in serum of patients with T2DM and their correlations with other clinical indicators were observed, and the receiver operating characteristic (ROC) curve was used to analyze the predictive values of let-7 and miR-26b levels in serum for IR in patients with T2DM.Results:Total cholesterol (TC), triglyceride (TG), glycosylated hemoglobin (HbA1c), fasting blood glucose (FPG), fasting insulin (FINS) and homeostasis model assessment of insulin resistance (HOMA-IR) in the T2DM group were significantly higher than those in the control group ( P<0.05), while insulin sensitivity index (ISI) and homeostasis model assessment of β cell function (HOMA-β) were significantly lower than those in the control group ( P<0.05). The serum levels of let-7 and miR-26b in patients in T2DM group were significantly lower than those in the control group ( P<0.05). The serum levels of let-7 and miR-26b in patients in T2DM group were negatively correlated with TC, TG, HbA1c, FPG, FINS and HOMA-IR ( P<0.05), and positively correlated with ISI and HOMA-β ( P<0.05). ISI, HOMA-IR and the serum levels of let-7 and miR-26b were independent risk factors for IR in patients with T2DM ( P<0.05). The area under curve (AUC) of serum let-7 expression predicting IR in patients with T2DM was 0.858, and the best cut-off value was 0.73, with sensitivity and specificity 77.50% and 80.00% respectively. The AUC of miR-26b in serum predicted the occurrence of IR in patients with T2DM was 0.908, and the best cut-off value was 0.60, with sensitivity and specificity 80.00% and 92.50%. Conclusions:The expression levels of let-7 and miR-26b in serum of patients with T2DM are significantly decreased, both of them may participate in the occurrence and development of IR, and have certain predictive value for the occurrence of IR in patients with T2DM, while miR-26b is more effective.

Objective To investigate the present situation of midwifery and the appropriate technical application of midwifery in Henan Province.Methods Using convenience sampling method,questionnaire survey was conducted among 32 tertiary hospitals,47 secondary hospitals and 11 primary hospitals via the platform of midwifery section of Henan Province nursing association.Results For hospitals of different levels in Henan Province,the training rate of midwives was (19.55±20.28)%,80.2% of the hospitals mentioned there was a need to increase midwives,the rates of natural delivery and perineal side cutting were (50.84±12.16)% and (40.14±21.15)%,and there were significant differences in the rates of natural delivery and perineal side cutting among hospitals of different levels(P<0.05).The proportion of application of appropriate techniques of midwifery was as follows:Doula delivery was 49.4%,labor in liberal position was 35.8%,epidural anesthesia was 56.8%,water birth was 4.9%,Lamaze breathing was 60.5%,and music therapy was 24.7%;the main reason for no Doula delivery was the shortage of midwives (75.6%),and the reasons for no labor in liberal position were the midwife had not been trained(57.7%) and there was safety hazard (57.7%).Conclusion The rate of standardized training for midwives was low,and there was a need of increasing midwives in hospitals of all levels in Henan Province.The appropriate technologies of midwifery need to be popularized,and the hospital should strengthen the training of related knowledge and skills for midwives in order to improve work ability,pronote natural delivery,and enhance application of appropriate techniques of midwifery.

Objective To assess the therapeutic effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) combined with neuromuscular electrical stimulation (NMES) on poststroke dysphagia.Methods A total of 90 patients with poststroke dysphagia were enrolled.They were randomly divided into either a NMES + rTMS group or a NMES + sham rTMS group (n =45 in each group).The Kuhota water drinking test and Standardized Swallowing Assessment (SSA) were used to assess the swallowing function.Results The scores of Kuhota water drinking test (F=82.001,P<0.001) and the SSA (F =33.743,P <0.001) before treatment,treatment of one course,treatment of two courses,and at 3 months after treatment in the NMES + rTMS group had significant differences.Compared with before treatment,they were improved significantly for treatment of one course (P <0.01 and P <0.05,respectively),two courses (all P<0.01),and at 3 months (all P<0.01) after treatment.The scores of Kuhota water drinking test (F =53.647,P<0.001) and the SSA (F=19.178,P<0.001) in the NMES + sham rTMS group also had significant difference.Compared with before treatment,they had significant improvement for treatment of one course (all P <0.05),two courses (P <0.05 and P <0.01,respectively) and at 3 months (all P<0.01)after treatment.The scores of Kuhota water drinking test for treatment of one course,two courses,and at 3 months after treatment (treatment of one course:t=2.217,P=0.02;treatment of two courses:t =2.406,P =0.019;at 3 months after treatment:t =2.128,P =0.037) and the SSA (treatment of one course:t =2.196,P =0.030,treatment of two courses:t =2.425,P =0.016;at 3 months after treatment:t =2.512,P=0.013) in the NMES + rTMS group were significantly better than those in the NMES + sham rTMS group.Conclusions High-frequency rTMS combined with NMES may significantly improve the swallowing function in patients with stroke.Its efficacy is superior to NMES.

Objective To comparative analysze of the applications value of breast-conserving surgery and improved surgical in breast cancer patients.Methods One hundred and sixty-eight female breast cancer patients from February 2005 to October 2010 in the People's Hospital of Rizhaoour hospital were selected.The patients were divided into breast-conserving surgery group (n =80) and improved surgical group (n =88).The patients in two groups were followed-up and investigated for three years.Operative time,blood loss,postoperative drainage,postoperative drainage time and hospital stay,incidence of complications,local recurrence,distant metastasis and survival after 3 year,satisfaction ratio of breast cosmetology,psychological anxiety scores and quality of life scores after 3 years of the patients in two groups were compared and analyzed.Results The complication rate of breast-conserving surgery group was 12.50% (10/80),lower than the improved surgical group,and the difference was significant statistically (29.55% (26/88),x2 =7.23,P<0.05).The operative time ((72.68±5.28) min),blood loss ((39.21 ± 8.59) ml),postoperative drainage ((162.25 ± 28.56) ml),postoperative drainage time((8.16±2.58) d) and the length of hospital stay((11.43±1.98) d) of the patients in breast-conserving surgery group were lower than those of the improved surgical group,and the differences of two groups were significant statistically ((92.48 ± 6.01) min,(68.26 ± 10.23) ml,(343.29 ± 62.18) ml,(16.59±4.96) d,(18.47±2.94) d;t =22.72,19.99,24.61,13.99,18.35;P<0.05).The satisfaction rate after 3 years of breast-conserving surgery group was 92.50%(74/80),higher than that of the improved surgical group,and the difference was significant statistically (81.82% (72/88),x2 =4.20,P< 0.05).The psychological anxiety scores after 2 and 3 years of breast-conserving surgery group were (40.02 ± 2.63) points and (34.26 ± 3.59) points,lower than those of improved surgical group((42.58±2.37) points and (39.85±2.10) points),the quality of life scores after 3 years of breast-conserving surgery group was (67.58± 11.26)points,higher than that of improved surgical group ((50.85 ± 10.42) points),and the differences were significant statistically (t =6.60,12.16,9.96;P < 0.05).Conclusion Breast-conserving surgery could reduce the incidence of complications and psychological anxiety situations,improve clinically relevant indicators and satisfaction ratio of breast cosmetology and quality of life for the breast cancer patients.

The research aimed to investigate the therapeutic effects and mechanisms of Opuntia dillenii Haw polysaccharide (OPS) on atherosclerosis of rats. First atherosclerotic rat models were established by high-fat and high-calcium diet. Thirty days later, the rats were treated with low dosage of OPS (0.2 g x kg(-1) x d(-1)) or high dosage of OPS (0.4 g x kg(-1) x d(-1)) by intraperitoneal injection for 60 days continuously. At the end of treatment, thoracic aorta rings were prepared and vasorelaxation of rat thoracic aorta in different experiment groups were determined by using 620M multi wire myograph system in vitro. Blood and livers of rats were collected. Then plasma levels of total cholesterol (TC), triglyceride (TG) and low density lipoprotein (LDL) of rats were separately determined using whole automatic biochemical analyzer; protein level of hepatic apolipoprotein B (ApoB) and that of hepatic diglyceride acyltransferase (Dgat1) were measured by Western Blot technique. Results showed that the ability of rat thoracic aorta to relax decreased markedly in the model group compared with that in the normal group, and significant differences existed in vasorelaxation ratios induced by different concentrations of carbamylcholine chloride (Carb) between these two groups (P < 0.01). After OPS treatment, the ability of rat thoracic aorta to relax improved markedly, the vasorelaxation ratios induced by Carb at 5 and 10 μmol x L(-1) were respectively 0.34 ± 0.08 and 0.62 ± 0.15 in the group treated with low dosage of OPS, while the ratios induced by Carb at 1 and 5 μmol x L(-1) were respectively 0.54 ± 0.08 and 0.98 ± 0.02 in the group treated with high dosage of OPS, which were all significantly different with those in the model group (P < 0.01). Plasma contents of TC, TG and LDL reduced significantly by the treatments both with low and high dosages of OPS compared with those in the model group (P < 0.01). Protein level of hepatic ApoB and that of hepatic Dgat1 decreased significantly after the treatment with high dosage of OPS compared with those in the model group (P < 0.01). These results indicate that OPS can markedly improve the vasorelaxation of thoracic aorta of atherosclerotic rats and has significant anti-atherosclerotic effect; inhibiting the expression of ApoB and Dgat1 and thus decreasing the amounts of TC, LDL and TG serving as one of the molecular mechanisms of its antiatherosclerosis effect.

Purpose To explore the expression of monocarboxylate transporters 4 (MCT4) and its clinicopathologic significance in cer-vical squamous cell carcinoma. Methods The expression level of MCT4 in 72 cervical squamous cell carcinoma tissues and 48 cervi-cal normal tissues were detected by immunohistochemistry assay, and its relationship with clinical pathological features was analyzed. Results The positive rate of MCT4 was 81. 9% (59/72) in cervical squamous cell carcinoma tissues and 35. 4% (17/48) in normal cervical tissues. The positive rate of MCT4 in cervical squamous cell carcinoma was significantly higher than that in cervical normal tis-sues (χ2 =26. 848, P<0. 001). In addition, the expression of MCT4 protein was correlated with clinical stage (χ2 =5. 389, P=0. 020) and lymph node metastasis (χ2 =4. 706, P=0. 030). However, it did not correlated with age (χ2 =1. 238, P=0. 266), tumor differentiation (χ2 =0. 530, P=0. 467) or infiltration degree (χ2 =1. 300, P=0. 254) in patients with cervical squamous cell carcinoma. Conclusion The expression of MCT4 is up-regulated in cervical squamous cell carcinoma and correlate with clinical stage and lymph node metastasis of cervical squamous cell carcinoma. MCT4 may be a biomarker for evaluating the biological behavior of cer-vical squamous cell carcinoma.

This study is to investigate the effect of ethyl gallate on invasion capabilities and its mechanism of breast cancer MDA-MB-231 cells. Using cell adhesion and transwell assay, separately, the effects of ethyl gallate on the invasion of MDA-MB-231 cells were measured. The Akt-NF-κB signal pathway protein expressions were analyzed with Western blot. Also, the mRNA levels of MMP-9 and MMP-2 were analyzed by RT-PCR. Ethyl gallate inhibited the abilities of motility, adhesion and invasion of breast cancer MDA-MB-231 cells in vitro (P<0.05), inhibited the mRNA levels of MMP-9, MMP-2, phosphorylation of AKt and protein expression of NF-κB. It is concluded that ethyl gallate can inhibit the abilities of invasion of breast cancer in vitro by inhibiting the mRNA levels of MMP-9/MMP-2, phosphorylation of Akt and protein expression of NF-κB.

Objective To investigate the clinical efficacy of postoperative three-dimensional conformal radiotherapy concurrent with paclitaxel +TS-1 chemotherapy in the treatment of advanced gastric carcinoma.Methods A total of 60 patients with advanced gastric carcinoma were randomly divided into treatment group and control group.In treatment group,30 patients received simultaneous three-dimensional conformal radiations therapy combined with paclitaxel +TS-1 chemotherapy.In control group,30 patients received paclitaxel +TS-1 chemotherapy.The long term survival rate,local recurrence rate and side reactions were observed in two groups.Results The 1,2-year survival rate in treatment group [93.1% (27/29),75.9 % (22/29)] and control group [72.4 % (21/29),48.3 % (14/29)] had significant difference (P < 0.05).The 1,2-year local recurrence rate in treatment group [6.9% (2/29),13.8% (4/29)] and control group [31.0% (9/29),44.8 % (13/29)] had significant difference (P < 0.05 or < 0.01).The side reactions in two groups were mainly manifested as leukopenia,thrombocytopenia,anemia,nausea,vomiting,liver function damage and oral mucositis.The incidence rate of nausea,vomiting in treatment group was significantly higher than that in control group [83.3% (25/30) vs.53.3% (16/30)] (P =0.012).Conclusions Postoperative threedimensional conformal radiotherapy concurrent with paclitaxel +TS-1 chemotherapy in the treatment of advanced gastric carcinoma may improve the long-term survival rate,lower local recurrence rate.But the adverse reactions are increased.

Objective To explore the mechanism of immunomodulatory activity of triptolide on healthy volunteers peripheral blood mononuclear cells (PBMC)-derived plasmacytoid dendritic cells (pDCs). Methods Healthy volunteers-derived pDCs were sorted by flow cytometry, then incubated with triptolide (0, 5, 10, 30 μg/L). After 24 hours, we detected the concentration of IFN-α, IL-6, TNF-α using ELISA. After 5 days, the cultrural cells were collected and analyzed by flow cytometry, light microscope and electron microscope scanning. Results Triptolide-treated pDCs secreted lower level of IFN-α,IL-6 ,TNF-α, triptolide could inhibit pDCs differentiation to DCs which displayed more immature morphology and immunophenotypes than untreated-pDCs. Conclusion Triptolide could decrease the immune function of pDCs, inhibit differentiation and maturation of pDCs.

Idiopathic pulmonary fibrosis(IPF), with unknown pathogeny, is an interstitial lung disease.The pathological features are diffuse epithelial-cell lesion, fibroblast proliferation, myofibroblast differentiation and excessive extracellular matrix deposition.CXCR4 is the predominant chemokine receptor on fibrocytes;CXCL12 is the only ligand of CXCR4.A large number of studies have shown that CXCL12/CXCR4 biological axis plays an important role in the pathogenesis of idiopathic pulmonary fibrosis.Under the regulation of hypoxia, HIF-1α and PI3K-Akt-mTOR path, CXCL12/CXCR4 biological axis promotes lung fibroblast proliferation, myofibroblast differentiation and extracellular matrix deposition, resulting in development and progression of IPF.

OBJECTIVE:To study the effect of Erdong granules on glucose metabolism and lipid metabolism in type 2 diabetes mellitus rats.METHODS:Type 2 diabetes mellitus model was induced by giving high-fat and high-calorie diet with intraperitoneal administration of streptozotcin for eight weeks.Model rats were divided into normal group,model group,streptozotcin group and erdong granules high-dose,middle-dose and low-dose groups.The levels of FBG,LDL-C,FFA,SOD and MDA were detected and immunohistochemistry method was used to determine the morphology change of islet cell.RESULTS:The serum levels of FBS,MDA,FFA and LDL-C in Erdong granules high-dose and low-dose group were significantly decreased while the activity of SOD was increased.Erdong granules could protect islet cell.CONCLUSION:Erdong granules can notably improve glucose metabolism and lipid metabolism,antioxidant enzyme activity and inhibit oxidative stress so as to protect islet cells of type 2 diabetic mellitus rats.

Objective To study the component and dose of polysaccharides compound and its inhibitive effect on the growth of S180 tumor.Methods Proliferative activity of splenolymphocytes was observed to study the component and dose of polysaccharides compound with MTT method. Mice bearing in-vivo transplanted tumor were used to evaluate the antitumor effect of polysaccharides compound.Results Polysaccharides compound, composed of lentinan100 mg/kg, agruecus blazei murill 200mg/kg and pachman 50 mg/kg,improved the activity of splenolymphocytes. Polysaccharides compound had an inhibitory effect on the growth of S180 tumor.Conclusion Polysaccharides compound with appropriate component in proper dose has synergistic immune actions and can inhibit the growth of S180 tumor.

Objective To investigate the antibacterial and antifungal metabolites from the fermentation broth of Streptomyces sp.M095 and classify the strain M095.Methods The structure was evaluated by spectral(ESI-MS,EI-MS,~(1)H-NMR,~(13)C-NMR and APTNMR) analysis.And the strain M095 was analysed with the characteristic of the culture and modality and the sequence of 16S rDNA.Results An antibiotic was found to have the remarkable activity to Staphylococcus aureus and filaceous fungi Mucor miehei(T 284).And its structure was identified as holomycin.The strain M095 was classified as maybe a variety belongs to Streptomyces Globisporus group.Conclusion The activity of holomycin to fungi was reported for the first time in this paper.The antimicrobial spectrum of holomycin was broadened.
