Résumé

OBJECTIVES: The objectives of the study were to assess body composition, physical activity level (PAL), basal metabolic rate (BMR), and daily energy expenditure (DEE) and to examine associations between PAL and body composition, BMR, and DEE of elderly in Busan. METHODS: A cross-sectional study was conducted among 226 elderly aged 65-93 years. Body composition was measured by Inbody 720. PAL was calculated by daily activity diary. BMR was calculated by Harris-Benedict (H-B) formula, Dietary Reference Intakes (DRI) formula, and Inbody 720 measurement. DEE was calculated by H-B formula, DRI formula, Inbody 720 measurement, and estimated energy requirements (EER) formula. RESULTS: The mean fat free mass (FFM) in elderly men was significantly higher than that in elderly women (p<0.001). The mean percent body fat and fat mass (FM) in elderly women were significantly greater than those in elderly men (p<0.001, p<0.001). The mean PAL in elderly men (1.59) was significantly higher than that in elderly women (1.53) (p<0.001). The mean DEEs calculated by 3 methods except for H-B formula in elderly men were higher than EER for elderly men (2000kcal). The mean DEEs calculated by 4 different methods in elderly women were higher than EER for elderly women (1600kcal). Age showed significantly negative correlations with height (p<0.001, p<0.001), FFM (p<0.001, p<0.001), BMRs calculated by H-B formula (p<0.001, p<0.001), DRI formula (p<0.001, p<0.001) and Inbody 720 measurement (p<0.05 p<0.01) and DEEs calculated by H-B formula (p<0.001, p<0.001), DRI formula (p<0.001, p<0.001), Inbody 720 measurement (p<0.05, p<0.05), and EER formula (p<0.001, p<0.001) in elderly men and elderly women. PAL showed significantly positive correlations with FFM (p<0.05), BMR by Inbody 720 measurement (p<0.05) in elderly men and negative correlations with FM (p<0.05) in elderly women. CONCLUSIONS: Based on the results, PAL was associated with greater FFM in elderly men and lesser FM in elderly women. Therefore, nutritional education to increase physical activity for health promotion in late life is needed in the elderly.

OBJECTIVES: This study was conducted to examine blood pressure and other characteristics of a high sodium intake group assessed with 24-hr urine analysis and the dietary factors related to the risk of high sodium intake among Korean adults. METHODS: A cross-sectional study was conducted with adults aged 20-59 years. Subjects who completed 24-hr urine collection (N = 205) were divided into 3 groups (tertile) according to the sodium intake estimated with 24-hour urine analysis. We compared the blood pressure, BMI and dietary related factors of the 3 groups (low, medium, high sodium intake group) with General Linear Model (GLM) and Duncan's multiple range test (p or = 25) (OR = 2.619; 95% CI: 1.368-5.015), current alcohol consumption (OR = 1.943; 95%CI: 1.060-3.564), and having salty soybean paste with salt percentage > 14% (OR = 3.99; 95% CI: 1.404-6.841). The dietary attitude related to increased risk of high sodium intake included 'enjoy dried fish and salted mackerel' (p < 0.001) and 'eat all broth of soup, stew or noodle' (p < 0.001). CONCLUSIONS: Because high sodium intake was associated with higher blood pressure, nutrition education should focus on alcohol consumption, emphasis on related dietary factors such as using low salt soybean paste, improvements in the habit of eating dried fish and salted mackerel or eating all broth of soup, stew or noodle.

The purpose of this study was to assess sodium and potassium intakes and urinary excretion of adults in Busan and to evaluate the relationship of urinary sodium/potassium excretion (UNa/UK) to the status of anthropometric, blood pressure, urine analysis, and nutrient intake of subjects. Nutrient intake by 24-h recall, 24-h UNa/UK were measured with 87 adults aged 20-59 yrs (42 men and 45 women). The mean intakes of sodium and potassium were 3915.4 mg and 3093.9 mg, respectively. The mean 24-h UNa/UK was 3457.0/1680.4 mg. UNa showed significant positive correlations with sodium intake (p < 0.001, p < 0.001), sodium/potassium ratio (p < 0.001, p < 0.01), UK (p < 0.001, p < 0.001), and UNa/UK ratio (p < 0.05, p < 0.01) in men and women and with age, BMI, systolic blood pressure (SBP) and diastolic blood pressure in women (p < 0.05, p < 0.05, p < 0.05, p < 0.05). The UK showed significant positive correlations sodium intake (p < 0.001, p < 0.001), UNa (p < 0.001, p < 0.001) in men and women and with sodium density in men (p < 0.001) and with age, intakes of protein and potassium in women (p < 0.01, p < 0.05, p < 0.05). Mean SBP was lowest in the second quartile and highest in the fourth quartile of UNa. Mean UNa in the second, third, and fourth quartiles were 2821.1 mg, 3621.3 mg, and 5456.4 mg, respectively. Mean SBP in the second, third, and fourth quartiles were 115.8 mmHg, 120.7 mmHg, and 125.9 mmHg, respectively. Based on the results, UNa was related to sodium intake, UK, and SBP. We conclude that nutritional education for the reduction of high sodium intake is needed in the general population to prevent and control adverse blood pressure levels.

The purpose of this study was to assess calcium and sodium intakes and urinary excretion of adults in Busan and to evaluate the relationship between urinary calcium excretion (UCa) and the status of anthropometric, blood pressure, urine analysis, and nutrient intake of subjects. Nutrient intake by 24 hr recall, 24 hr urinary calcium and sodium excretion (UNa) were measured with 87 adults aged 20-59 yrs (42 men and 45 women). The mean calcium intake was 88.0% for men and 103.0% for women of Recommended Intake. The mean sodium intake was 283.4% for men and 250.5% for women of Adequate Intake (AI). The mean 24hr UCa was 127.4 mg in men and 107.3 mg in women. The mean 24 hr UNa was 3650.6mg in men and 3276.4mg in women. The intake and urinary excretion of calcium and sodium were not significantly different by gender. UCa showed significantly positive correlations with sodium intake and UNa in men (p < 0.001, p < 0.05) and women (p < 0.001, p < 0.001) and with age, systolic blood pressure (SBP) and sodium density in women (p < 0.05, p < 0.05, p < 0.01). The UCa/creatinine showed significantly positive correlations with age, sodium intake, sodium density, and UNa in women (p < 0.05, p < 0.01, p < 0.01, p < 0.01). When UCa was stratified into quartile (Q1-Q4), age, SBP, UCa, UNa, sodium intake, and AI percentage of sodium (p < 0.01, p < 0.05, p < 0.001, p < 0.001, p < 0.001, p < 0.001) were significantly higher in Q4. The mean intake and AI percentage of sodium in Q4 were 4768.8mg and 329.0. Based on the results, UCa was related to age, SBP, UNa, and sodium intake. Therefore, nutritional education of decreasing sodium intake for decreasing UCa is needed.

This study was performed to assess the physical activity, food habit and nutrient intakes by gender and age groups in 193 adults aged 20-59 years (84 men and 109 women) in Pusan. Data for physical activity and dietary survey was assessed by a questionnaire and 24hr recall method. The mean BMIs of men and women were 24.0 and 22.2 respectively and BMI of women in the 20-29 years group (20.9) was significantly lower than that of women in the 30-49 and 50-59 years group (22.7, 23.2) (p < 0.01). 56.0% for men and 44.0% for women exercised regularly. The mean exercise duration per once of men (69.7 minutes) was significantly higher than that of women (52.4minutes) (p < 0.01). The mean exercise duration per day was 36.0 minutes for men and 29.9 minutes for women. 67.9% for men and 78.0% for women often skipped meals and 68.4% for men and 69.4% for women skipped breakfast in the main. The mean energy intake of men was 2067.2 kcal and that of women was 1783.1 kcal comprised of 87.2% and 92.1% of the Estimated Energy Requirements (EER). The mineral intakes of men and women were over Recommended Intake (RI) and Adequate Intake (AI) except calcium and potassium. The mean calcium intake was 88.3% for men and 84.0% for women of RI. The mean potassium intake was 63.3% for men and 59.2% for women of AI. The mean vitamin intakes of men and women were over RI and AI except vitamin C and folic acid. The mean vitamin C intake was 92.5% for men and 85.6% for women of RI. The mean folic acid intake was 76.6% for men and 70.0% for women of RI. The mean energy, protein, sodium and zinc intakes of men were significantly higher than those of women (p < 0.01, p < 0.05, p < 0.05, p < 0.05). The mean vitamin B6, vitamin C and folic acid intakes of men in the 20-29 years group were significantly lower than those of men in the 30-49 and 50-59 years group (p < 0.01, p < 0.05, p < 0.05). For energy, proportions of subjects with intake levels less than 90% EER were 64.3% for men and 56.0% for women. For calcium, proportions of subjects with intake levels less than Estimated Average Requirement (EAR) were 52.4% for men and 59.6% for women. For folic acid, proportions of subjects with intake levels less than EAR were 78.6% for men and 83.5% for women. For iron and phosphorus, proportions of women (36.7%, 14.7%)with intake levels less than EAR were significantly higher than those of men (6.0%, 1.2%) (p < 0.01, p < 0.001). For men, age was positively correlated with intakes of potassium, vitamin B6, vitamin C and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.01). For men, weight showed significantly negative correlations with intakes of carbohydrate, phosphorus, potassium, zinc, vitamin B6 and folic acid (p < 0.01, p < 0.05, p < 0.05, p < 0.05, p < 0.01) and BMI showed significantly negative correlations with protein, lipid, phosphorus, potassium, zinc, vitamin E and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05) For men, exercise duration per once showed significantly positive correlations with intakes of calcium, phosphorus, potassium, zinc, vitamin B1, vitamin B2, niacin, vitamin C and folic acid (p < 0.01, p < 0.05, p < 0.01, p < 0.05, p < 0.05, p < 0.01, p < 0.01, p < 0.01, p < 0.05). Therefore, nutritional education for adult health management is needed by gender and age groups.

This study was performed to assess the sodium intakes of Korean adults using a 24-hr urine analysis and dish frequency questionnaire (DFQ) according to each dish group and the regional area. The subjects of this study were comprised of 552 adults (male: 267, female: 285), aged 20-59yr residing in the metropolitan area (N = 200), Chungcheng-Do (N = 117), Jeolla-Do (N = 117), and Gueongsang-Do provinces (N = 118). The subjects were recruited from the residents who once participated or are participating in the various health programs offered by the public health center. The number of subjects who completed the 24-hr urine collection was 205 (male : 110, female : 95).The mean age and BMI of the subjects were 39.0+/-11.7 y and 23.1+/-2.9 kg/m2, respectively. The mean systolic and diastolic blood pressure was 119.5+/-15.4 mmHg, and 77.1+/-11.1 mmHg, respectively. Eighteen percent of the subjects responded that they are currently smoking, 36% drinking and 50.4% exercising. Twenty point six percent of the subjects were assessed as having hypertension according to their systolic or diastolic blood pressure(SBP > or = 140 mmHg or DBP > or = 90 mmHg) measurements in the present study. Salt intake of the subject estimated with 24-hr sodium excretion was 12.7 g/d (male : 13.4 g/d, female : 12.1 g/d) based on the sodium excretion rate as 82%. Salt intake estimated with DFQ was 14.7 g/d (male : 16.2 g/d, female : 13.4 g/d), 2 g more than the salt intake estimated with 24-hr urine analysis. The four dish groups that contributed most to the sodium intake in order were kimchi (I1571.4mg), soup and stew (1260.5 mg), fish and shellfish (706.3 mg) and noodle and ramyeon (644.3mg). Salt intake estimated with DFQ was the highest in the subjects of Gueongsang-Do (17.0 g/d), second highest Chungcheong-Do (16.4 g/d) and the lowest in the metropolitan area (13.0 g/d). Subjects of Gueongsang-Do showed the highest sodium intakes in most of the dish group, whereas subjects of the metropolitan area showed the lowest. Residents of Chuncheong-Do revealed the highest sodium intake with kimchi and ofJeolla-Do the higher sodium intake with the main dish (meat, fish and beans). The highest salt percentage of kimchi (3.0+/-0.8%) and soybean paste (14.5 +/-5.1%) were observed in Gueongsang-Do, whereas individuals of the metropolitan area were observed as having kimchi (1.6 +/-0.5%) and soybean paste (7.4 +/-1.6%) with the lowest salt percentage. Men were observed as having more salty kimchi (2.4 +/-0.1%) than women (2.1 +/-0.1%).

This study was performed to estimate serum 25-hydroxyvitamin D (25-OHD) level in wintertime and to evaluate the relationship between serum 25-OHD level and associated factors in 50 premenopausal working women aged 30 . 49 y in Busan. The serum 25-OHD level was measured by radioimmunoassay. Data for physiological characteristics, lifestyle factors, physical activity and nutrient intake was assessed by questionnaire including information about outdoor activity time, daily activity diary and 24hr recall method. The mean vitamin D intake was 4.24 ug, which corresponded to 84.9% of the Korean RDA. The mean level of serum 25-OHD was 25.7 ng/mL. Vitamin D deficiency (25-OHD or = 75 nmol/L) were not found in the subjects. However, the prevalence of vitamin D insufficiency (25-OHD < 15 nmol/L) and hypovitaminosis D (25-OHD < 30 nmol/L) were 12% and 60% respectively. The serum 25-OHD level showed positive significant correlations with the duration of outdoor activity per weekdays (p < 0.05). Our findings suggest that hypovitaminosis D was common in the subjects in wintertime. So nutritional education for increasing outdoor activities is needed for premenopausal working women to increase vitamin D status in wintertime.

The subjects were 61 premenopausal working women aged 30 - 49 y in Busan. The serum 25-hydroxyvitamin D level was measured by radioimmunoassay. Data for physiological characteristics, lifestyle factors, physical activity and nutrient intake were assessed by questionnaire including information about outdoor activity time, daily activity diary and 24 hr recall method. The mean vitamin D intake was 3.12 ug, which corresponded to 62.5% of the Korean RDA. The mean level of serum 25-hydroxyvitamin D was 31.0 ng/mL. Low 25-hydroxyvitamin D (<25 nmol/L) was not found in the subjects. The serum 25-hydroxyvitamin D level showed positive significant correlations with exercise hours, daily energy expenditure, hours of outdoor activity per weekdays (p< 0.001, p< 0.05, p< 0.05). Exercise hours were found to be the most important determinant of serum 25-hydroxyvitamin D level. Therefore nutritional education for increasing hours of physical activity including indoor and outdoor exercise, is needed for premenopausal working women to increase vitamin D status.

To assess the food habits and the seasonal differnces of nutrient intakes and diet qualities of adult working women aged 30 - 49y in Busan, dietary survey was conducted in summer and in winter by a questionnaire and two-day food record. Anthropometric assessment was also investigated in two seasons. 91.8% of those skipped breakfast in the main. 44.3% had irregular meals. The mean daily energy intake was 1725.8 kcal with 63.3% of energy intake being supplied by carbohydrates, 14.7% by protein, 22.3% by fat in summer and 1598.4 kcal with 62.1% of energy intake being supplied by carbohydrates, 15.6% by protein, 22.1% by fat in winter. Over 70% of iron intake came from plant origin in two seasons. The mean intakes of energy, calcium, iron and vitamin A in summer and energy, calcium, iron, vitamin A and vitamin B2 in winter were below Recommended Dietary Allowance (RDA) for Koreans. As well as insufficiency in iron, the bioavailability of iron is considered to have been low because most of iron intake came from plant origin in two seasons. For calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter, proportions of subjects with intake levels less than 75% of RDA were over 40% in summer and over 50% in winter, respectively. The nutrient adequacy ratios (NAR) were below 0.75 for calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter. NARs of iron (p < 0.05), vitamin A (p < 0.01) and vitamin B2 (p < 0.001) in winter were significantly lower than those in summer. The mean adequacy ratios (MAR), an index of overall dietary quality were 0.85 in summer and 0.80 in winter. The MAR in winter was significantly lower than that in summer (p < 0.05). The indexes of nutritional quality (INQ) were below 1 for calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter. The intake (p < 0.05) and NAR (p < 0.05) of vitamin B2 showed positive significant correlations with height in winter. In conclusion, nutrient intake and diet quality of adult working women were different between the summer and the winter. So nutritional education programs for summer and winter are needed for adult working women.

To assess zinc status by dietary intake and urinary excretion of preschool children in Busan and to evaluate the relationship of intakes of food and nutrient with urinary zinc excretion, zinc food frequencies of 40 common foods affecting intakes of zinc by food fequency method, nutrient intake by 24hr recall and 24hr urinary zinc excretion were measured with 97 preschool children. The mean zinc intake was 4.29 mg and 43.0% of RDA. The mean zinc intake per 1,000 kcal was 3.09 mg.97.9% of subjects had zinc intake less than 75% of RDA. Grains food group was the primary source of zinc intake and supplied 38.9% of the total daily zinc intake. Altogether, plant food products supplied 49.7% of zinc intake. The mean urinary zinc excretion and zinc excretion per gram of creatinine were 0.19 mg and 1.00 mg respectively. The urinary zinc excretion showed positive significant correlations with height and weight (p<0.05, p<0.05) , urine volume and urinary creatinine excretion (p<0.05, p<0.001) , urinary zinc excretion per creatinine (p<0.001) , urinary zinc excretion per weight (p<0.001) , intakes of energy and carbohydrate (p<0.05, p<0.01) and usual intake of zinc from eggs food group (p<0.05) . In conclusion, these results show that the zinc intake of preschool children is low and that sources of dietary zinc are mainly plant foods, suggesting low bioavailability. So nutritional education is needed in order to inc-rease usual intake of animal food group. Interpretation of urinary zinc excretion data is complicated by current uncertainty about "normal" zinc level at this age group. Further studies are needed to obtain extensive data on urinary zinc excretion for this age group.age group.

To assess the dietary fiber intake of preschool children in Busan and to evaluate the relationship between of the intake of dietary fiber and nutrient intake. Nutrient intake using 24 hour recall, and total dietary fiber (TDF) intake based on tables of TDF of common Korean floods developed by the modified Prosky Method, were estimated for 176 preschool children. The mean daily intakes of TDF, and TDF after adjusting energy intake, were 10.20 g and 7.69 g/1,000 kcal, respectively. The mean daily intakes of TDF for children aged 1-3 and 4-6 years were 9.20 g and 11.08 g, respectively. The range of TDF intake was 1.86 to 22.16 g. The major sources of TDF were cereals (31.0%), vegetables (18.9%) and fruits (11.9%). The TDF intake showed positive correlations with nutrient adequacy ratios (NAR) of iron and Vitamin B1, (p<0.05, p<0.05). The TDF intake per 1,000 kcal showed negative correlations with the NARs of protein, calcium, phosphate, iron, Vitamin A, vitamin B1, Vitamin B2, and niacin (p<0.001, p<0.001, p<0.001, p<0.05, p<0.001, p<0.001, p<0.001, p<0.001) and with the mean adequacy ratio (MAR, p<0.001). When children were stratified into quartiles (Q1-Q4) on the basis of their fiber intake per 1,000 kcal, their NARs for calcium, phosphate, iron, Vitamin A, Vitamin B1, Vitamin B2 and niacin (p<0.05, p<0.05, p<0.001, p<0.05, p<0.05, p<0.001, p<0.001), and their U? (p<0.001) were significantly lower in the children with higher fiber intake per 1,000 kcal (the upper quartile). The NARs fur calcium (0.63), iron (0.60), Vitamin A (0.66), Vitamin B2 (0.74), niacin (0.64), Vitamin C (0.65) and the MAR (0.74) were lower than 0.75 in the children with fiber intakes of more than 9.25 g per 1,000 local (0,), the highest fiber intake per 1,000 kcal. Based on these results, the mean TDF intake of children was higher than the 'age (yr)+5g', the minimum recommended level for American children. Meals with a fiber intake of more than 9.25 g per 1,000 local (Q4) could cause a decreased nutritional status for minerals and vitamins. The result of this study could contribute to the establishment of Recommended Dietary Allowances (RDA) for dietary fiber for Korean Preschool children.

Résumé

To assess diet quality by food group intake and to investigate the interrelationship of age, dietary diversity score(DDS), dietary variety score(DVS), dietary frequency score(DFS), food group intake and nutrient intake with food group intake, a dietary survey was conducted with 176 preschool children aged 1 to 6 in Busan using a 24-hr recall method. Food group intake was assessed by food number consumed and intake frequency by six food groups(grain, meat, vegetable, fruit, dairy, sweets group). The mean food numbers consumed and intake frequencies by six feed group were 3.1 and 4.0 in the grain group, 3.6 and 4.0 in the meat group, 3.5 and 4.1 in the vegetable group, 1.0 and 1.1 in the fruit group, 1.3 and 1.5 in the dairy group, 1.4 and 1.4 in the sweets group respectively. As age increased, the intake frequency of the grain group(p<0.05) increased but that of the dairy group(p<0.05) decreased significantly. The DVS and DFS didn't show significant correlations with intake frequency of the dairy group. The grain group intake had significant positive correlations with intakes of the meat, vegetable, and fruit groups. The vegetable group intake had signigicant positive correlations with intakes of the grain and meat groups. The dairy group intake had significant positive correlation with sweets group intake but negative correlations with intakes of the grain and vegetable groups. As the intake frequency of the meat group increased, the NAR(nutrient adequacy ratios) of all nutrients and NAR(mean adequacy ratio) increased significantly. NARs of provein iron, vitamin B1, niacin had the highest correlation with the meat group intake and those of protein, calcium, phosphorous, and vitamin B2 had the highest correlation with the dairy group intake. NARs of vitamin A and vitamin C had the highest correlation with intake of the vegetable and fruit groups respectively. this study was undertaken to compare the riboflavin status of rural women with different physical activity intensity and to determine factors influencing biochemical fiboflavin status. The study was carried out over three different farming seasons : planting (June), harvest(October) and interim(February) in two rural regions of Korea. One was a traditional farming region, the other a commercial farming region with heavier work intensity. Twenty women in the traditional region and eighteen women in the commercial region were involved. The intensity of physical activity was determined by a daily activity record. Body composition was assessed by bioelectrical impedence. Dietary riboflavin intake was measured by the food frequency method. Riboflavin biochemical status was assessed by erythrocyte glutathione reductase activity coefficient (EGR AC) and ruinary riboflavin excretion. The results from the EGR AC and urinary riboflavin excretion during the period showed the overall riboflavin status of the commercial farming women was significantly worse than that of the traditional farming women(EGR AC p<0.0001, urinary riboflavin excretion p<0.05). The traditional farming group had about 40% with risk of riboflavin deficiency, whereas the commercial farming group had about 70%. Overall mean nutrient intake was not significantly different between the two groups, however, overall mean percent lean body mass representing long term physical activity was significantly higher in the commercial farming group (<0.005). It appears that the biochemical riboflavin status of traditional farming women was significantly influenced by riboflavin intake and crude nitrogen balance while the biochemical riboflavin status of the commercial farming women was significantly influenced by riboflavin intake and percent of lean body mass over the three seasons.
