Objective: We aimed to explore the feasibility of a single-port thoracoscopy- assisted five-step laparoscopic procedure via transabdominal diaphragmatic(TD) approach(abbreviated as five-step maneuver) for No.111 lymphadenectomy in patients with Siewert type II esophageal gastric junction adenocarcinoma (AEG). Methods: This was a descriptive case series study. The inclusion criteria were as follows: (1) age 18-80 years; (2) diagnosis of Siewert type II AEG; (3) clinical tumor stage cT2-4aNanyM0; (4) meeting indications of the transthoracic single-port assisted laparoscopic five-step procedure incorporating lower mediastinal lymph node dissection via a TD approach; (5) Eastern Cooperative Oncology Group performance status (ECOG PS) 0-1; and (6) American Society of Anesthesiologists classification I, II, or III. The exclusion criteria included previous esophageal or gastric surgery, other cancers within the previous 5 years, pregnancy or lactation, and serious medical conditions. We retrospectively collected and analyzed the clinical data of 17 patients (age [mean ± SD], [63.6±11.9] years; and 12 men) who met the inclusion criteria in the Guangdong Provincial Hospital of Chinese Medicine from January 2022 to September 2022. No.111 lymphadenectomy was performed using five-step maneuver as follows: superior to the diaphragm, starting caudad to the pericardium, along the direction of the cardio-phrenic angle and ending at the upper part of the cardio-phrenic angle, right to the right pleura and left to the fibrous pericardium , completely exposing the cardio-phrenic angle. The primary outcome includes the numbers of harvested and of positive No.111 lymph nodes. Results: Seventeen patients (3 proximal gastrectomy and 14 total gastrectomy) had undergone the five-step maneuver including lower mediastinal lymphadenectomy without conversion to laparotomy or thoracotomy and all had achieved R0 resection with no perioperative deaths. The total operative time was (268.2±32.9) minutes, and the lower mediastinal lymph node dissection time was (34.0±6.0) minutes. The median estimated blood loss was 50 (20-350) ml. A median of 7 (2-17) mediastinal lymph nodes and 2(0-6) No. 111 lymph nodes were harvested. No. 111 lymph node metastasis was identified in 1 patient. The time to first flatus occurred 3 (2-4) days postoperatively and thoracic drainage was used for 7 (4-15) days. The median postoperative hospital stay was 9 (6-16) days. One patient had a chylous fistula that resolved with conservative treatment. No serious complications occurred in any patient. Conclusion: The single-port thoracoscopy-assisted five-step laparoscopic procedure via a TD approach can facilitate No. 111 lymphadenectomy with few complications.

Objective: To investigate the clinical characteristics of hospitalized children infected with the Omicron variant in Kunming after the withdrawal of non-pharmaceutical interventions (NPI) and analyze the risk factors of severe cases. Methods: Clinical data was retrospectively collected from 1 145 children with SARS-CoV-2 Omicron infection who were hospitalized in six tertiary grade A hospitals in Kunming from December 10th, 2022 to January 9th, 2023. According to clinical severity, these patients were divided into the general and severe SARS-CoV-2 groups, and their clinical and laboratory data were compared. Between-group comparison was performed using t-test, chi-square test and Mann-Whitney U test. Spearman correlation test and multivariate Logistic regression analysis were used to determine the risk factors of severe illness. Results: A total of 1 145 hospitalized patients were included, of whom 677 were male and 468 female. The age of these patients at visit was 1.7 (0.5, 4.1) years. Specifically, there were 758 patients (66.2%) aged ≤3 years at visit and 387 patients (33.8%) aged >3 years. Of these children, 89 cases (7.8%) had underline diseases and the remaining 1 056 cases (92.2%) had no combined diseases. Additionally, of all the patients, 319 cases (27.9%) were vaccinated with one or two doses of SARS-CoV-2 vaccine, 748 cases (65.3%) had acute upper respiratory tract infection (AURTI), and six cases died (0.5%). A total of 1 051 cases (91.8%) were grouped into general SARS-CoV-2 group and 94 cases (8.2%) were grouped into severe SARS-CoV-2 group. Compared with the general cases, the severe cases showed a lower rate of SARS-CoV-2 vaccination and younger median age, lower lymphocyte count, as well as proportions of CD8+T lymphocyte (36 cases (38.3%) vs. 283 cases (26.9%), 0.5 (2.6, 8.0) vs. 1.6 (0.5, 3.9) years, 1.3 (1.0, 2.7) ×109 vs. 2.7 (1.3,4.4)×109/L, 0.17 (0.12, 0.24) vs. 0.21 (0.15, 0.16), respectively, χ2=4.88, Z=-2.21,-5.03,-2.53, all P<0.05). On the other hand, the length of hospital stay, proportion of underline diseases, ALT, AST, creatine kinase isoenzyme, and troponin T were higher in the severe group compared to those in the general group ((11.6±5.9) vs. (5.3±1.8) d, 41 cases (43.6%) vs. 48 cases (4.6%), 67 (26,120) vs. 20 (15, 32) U/L, 51 (33, 123) vs. 44 (34, 58) U/L、56.9 (23.0, 219.3) vs. 3.6 (1.9, 17.9) U/L, 12.0 (4.9, 56.5) vs. 3.0 (3.0, 7.0) ×10-3 pg/L,respectively, t=-20.43, χ2=183.52, Z=-9.14,-3.12,-6.38,-3.81, all P<0.05). Multivariate regression analysis indicated that increased leukocyte count (OR=1.88, 95%CI 1.18-2.97, P<0.01), CRP (OR=1.18, 95%CI 1.06-1.31, P<0.01), ferritin (OR=1.01, 95%CI 1.00-1.00, P<0.01), interleukin (IL)-6 (OR=1.05, 95%CI 1.01-1.08, P=0.012), D-dimer (OR=2.56, 95%CI 1.44-4.56, P<0.01) and decreased CD4+T lymphocyte (OR=0.84, 95%CI 0.73-0.98, P=0.030) were independently associated with the risk of severe SARS-CoV-2 in hospitalized children with Omicron infection. Conclusions: After the withdrawal of NPI, the pediatric inpatients with Omicron infection in Kunming were predominantly children younger than 3 years of age, and mainly manifested as AURTI with relatively low rate of severe SARS-CoV-2 infection and mortality. Elevated leukocyte counts, CRP, ferritin, IL-6, D-dimer, and decreased CD4+T lymphocytes are significant risk factors for developing severe SARS-CoV-2 infection.

Objective@#To observe the migration of 4,4'-diaminostilbene-2,2'-disulfonc acid-based fluorescent whitening agents ( DSD-FWAs ) in food packaging paper, so as to provide evidence for quality and safety supervision for paper packaging materials.@*Methods@#Forty-one paper samples with DSD-FWAs positive were made into 6 cm2 pieces and were soaked in four food simulants ( distilled water, 3% acetic acid, 10% ethanol and 95% ethanol, 10 mL each ). The experiment was carried out at the specified soaking temperature and time. The migration amounts of eleven DSD-FWAs were detected by high performance liquid chromatography-fluorescence detection. @*Results@#C.I.220, C.I.24, C.I.210, C.I.85, C.I.113, C.I.264, C.I.353 and C.I.357 were found in all the four food simulants. At the same time and temperature, the migration amount was highest in 10% ethanol, followed by distilled water, 3% acetic acid and 95% ethanol. C.I.220 was dissolved in all four food simulants, in the range of 20-90 ℃, the migration amount increased with soaking temperature; at 20 ℃, 40 ℃ and 60 ℃, the migration amount increased first and then stabilized over time.@*Conclusion@#The higher the storage temperature and the longer the storage time of paper packaging, the easier the DSD-FWAs in packaging paper migrate to food.

Objective:To study the influence of communication adequacy of critical patients on their willingness to participate in their own medical safety.Methods:From August 2019 to October 2019, a questionnaire survey was conducted on 209 critical patients.Results:The willingness to participate in the medical safety of critical patients was 3.92 ± 0.57, the willingness to participate was higher, the communication between doctors and patients was more sufficient, and the willingness to participate in the medical safety of patients was increased.Conclusions:Understanding the psychology of critical patients and paying attention to the sufficiency of communication between doctors and patients can increase the flexibility of patients' participation in medical safety and improve their willingness to participate in their own medical safety.

Objective::To rapidly identify and analysis the chemical constituents in the methanol extract of heartwood of Dalbergia cochinchinensis by ultra high performance liquid chromatography-quadrupole-time-of-flight high resolution mass spectrometry (UPLC-Q-TOF-MS/MS). Method::UPLC RRHD SB-C18 column (3.0 mm×100 mm, 1.8 μm) was used for chromatographic separation with acetonitrile-0.1% formic acid solution as the mobile phase for gradient elution (0-0.01 min, 5%B; 0.01-2 min, 5%-22%B; 2-28 min, 22%-35%B; 28-45 min, 35%-44%B; 45-55 min, 44%-100%B; 55-57 min, 100%B; 57-57.10 min, 100%~5%B) at a flow rate of 0.3 mL·min-1. The analytes were determined in negative ion mode with electrospray ionization (ESI) and data collection range of m/z 100-1 500. Result::A total of 101 chemical constituents were identified, including 22 flavonoids, 34 isoflavones, 15 neoflavonoids, 18 other flavonoids and 12 other components. Conclusion::UPLC-Q-TOF-MS/MS technique can quickly, accurately and comprehensively identify the chemical constituents in the heartwood of D. cochinchinensis. Isoflavones, flavonoids and neoflavonoids are the main chemical constituents in the heartwood of D. cochinchinensis, which is of great significance to reveal its internal material basis and provides experimental basis for this plant to be developed as a potential new resource of traditional Chinese medicine.

Objective To explore the application effect of triage-registration information systems in emergency patients. Methods A total of 18 964 emergency patients from 1 December 2016 to 31 December 2016 in Zhongshan Hospital of Fudan University Emergency Department were selected as control group and 18798 emergency patients as experimental group from March 1, 2017 to March 31, 2017. In the control group, the patients were used traditional mode of triage-registration and registration office respectively line. In the experimental group, the patients were used triage-registration information systems in line once. The emergency waiting time, patient satisfaction and medical staff satisfaction were compared between the two groups before and after the application of triage-registration information systems. Results The waiting time in the experimental group was significantly shorter than that in the control group, median(P25, P75):11(6-28)min vs. 10(4, 27)min, The difference between both groups was statistically significant(Z=-14.5433, P=0.000). The patient satisfaction in the experimental group were better than those in the control group, (2.89 ± 0.41 vs. 2.98 ± 0.13), and with statistical difference (Z=-10.317, P<0.05). And the acceptance of the medical staff in the experimental group were better than those in the control group, (2.90±0.35 VS 2.97±0.16), and with statistical difference (Z=-5.527, P<0.05). Conclusions The development and application of the triage- registration information system can effectively shorten the waiting time of patients, improve the experience of emergency patients, improve the satisfaction of medical staff, and realize the optimization of the process of seeking medical treatment.

@#AIM:To investigate the clinical effect of phacoemulsification combined with goniosynechialysis for patients with cataract and glaucoma. <p>METHODS: Totally 73 cases(80 eyes)with angle closure glaucoma and cataract were selected from April 2014 to October 2015. All patients were treated with phacoemulsification combined with goniosynechialysis. The visual acuity, intraocular pressure, anterior chamber depth, anterior chamber angle and complications were compared before and after treatment. <p>RESULTS: Vision: compared with preoperative data, the data at 1wk, 1, 2, 6mo, 1a of postoperative were improved significantly(<i>P</i><0.05). Intraocular pressure: compared with preoperative data, the data at 1wk, 1, 2, 6mo, 1a of postoperative decreased significantly(<i>P</i><0.05). After the surgery, the anterior chamber depth were evidently higher than those before the surgery(<i>P</i><0.05). The angle grading after the surgery were improved remarkably compared with the preoperative ones(<i>P</i><0.05). There were no serious complications. <p>CONCLUSION: Phacoemulsification combined with goniosynechialysis is safe and effective for patients with cataract and glaucoma. The surgery increase the depth of the anterior chamber significantly, improve the openness of angle, improve the vision and reduce the intraocular pressure.

The injury of the penile cavernous nerve is a common cause of erectile dysfunction (ED). Reconstruction of the resected cavernous nerve can restore penile erectile function to normal. The methods for cavernous nerve repair include direct anastomosis, autotransplantation of the nerve, and substitution of the biodegradable artificial nerve, among which only autotransplantation of the sural nerve is used clinically at present. Besides, the nerve growth factor plays an important role in nerve reconstruction. This paper summarizes the methods of cavernous nerve reconstruction in the recent years.

Résumé

The effect of L-proline as a promoter on the condensation reaction of salicylaldehyde or its derivatives with ethyl acetoacetate in neutral ionic liquid [emim]BF4 was studied. All reactions were carried out under mild reaction conditions and achieved high yields. Moreover, the ionic liquid containing L-proline could be recycled and reused for several times without noticeably decreasing in productivity. The results show that the L-proline-[emim]BF4 system has a potential in contribution to the development of environmentally friendly and inexpensive processes in organic syntheses.

Résumé

The binding of pefloxacin mesylate (PFLX) to bovine lactoferrin (BLf) and human serum albumin (HSA) in dilute aqueous solution was studied using fluorescence spectra and absorbance spectra. The binding constant K and the binding sites n were obtained by fluorescence quenching method. The binding distance r and energy-transfer efficiency E between pefloxacin mesylate and bovine lactoferrin as well as human serum albumin were also obtained according to the mechanism of Förster-type dipole-dipole nonradiative energy-transfer. The effects of pefloxacin mesylate on the conformations of bovine lactoferrin and human serum albumin were also analyzed using synchronous fluorescence spectroscopy.

Résumé

<p><b>OBJECTIVE</b>To investigates the surgical treatment for congenital anophthalmia.</p><p><b>METHODS</b>The operation was performed in two steps. At first, the orbit was enlarged and the tarsus was reconstructed with cartilage transplantation. At the second step, blepharoptosis was corrected with levator shortening or frontalis muscle suspension.</p><p><b>RESULTS</b>Five cases have been treated successfully with this method and satisfactory results were obtained.</p><p><b>CONCLUSION</b>Orbit amplification and tarsus reconstruction along with ptosis correction is an effective treatment for anophthalmia both aesthetically and functionally.</p>

<p><b>OBJECTIVE</b>To study the changes of the contralateral testicular histology and germ cell apoptosis after unilateral testicular torsion (UTT) and to determine whether the contralateral testis is injured or not.</p><p><b>METHODS</b>Sixty SD male rats were divided into control group (12 rats) and experimental group(48 rats). The former underwent sham operation of the left testis under general anaesthesia. The latter underwent left testis torsion(720 degrees) for 6 h, and then 4 of them were sacrificed and the other 44 were subdivided into the torsed testis untwisted group (22 rats) and the torsed testis removal group (22 rats), 7-8 rats were sacrificed and both testes (twisted and untwisted) were removed 1 day, 1 week and 4 weeks after surgery. All testes underwent histological and germ cell apoptosis examination.</p><p><b>RESULTS</b>There were significant histological changes in the contralateral testis, and the germ cell apoptosis was increased greatly in the contralateral testis.</p><p><b>CONCLUSIONS</b>UTT can cause contralateral testicular injury, whose mechanism may be related to reperfusion, and torsed testis removal can prevent or reduce damage to the contralateral testis.</p>
