RÉSUMÉ

<p><b>OBJECTIVE</b>To investigate the result of hip arthroplasty for failed internal fixation of femoral neck fractures.</p><p><b>METHODS</b>From June 2007 to January 2014, 29 cases who underwent hip arthroplasty for failed of internal fixation of femoral neck fractures were reviewed. There were 12 males and 17 females. The mean age was 60.3 years (ranged 43 to 83 years) at the time of the fracture. Left hip was in 16 cases, right hip was in 13 cases. The average interval from fracture to arthroplasty was 23.3 months (ranged, 3 to 48 months).</p><p><b>RESULTS</b>All of 29 cases were performed total hip arthroplasty. There were 20 cases of cementless cup,7 cases of cementless cup with bone graft, 2 cases of cemented cup with bone graft; 13 cases of cementless stem, 16 cases of cemented stem. There were no complications occurred such as intraoperative fracture of the greater trochanter. The average operative time was (115 ± 38) minutes,the mean intraoperative blood loss was (420 ± 175) ml, the average postoperative drainage volume (240 ± 119) ml, intraoperative blood transfusion was (200 ± 220) ml, intraoperative fluid volume was (2,200 ± 400) ml, the average postoperative blood transfusion was (300 ± 200) ml. There was 1 case get postoperative dislocation. All patients were followed up for 14.7 months in average (ranged, 5 to 24 months). There was no revision for mechanical failure. Harris Hip Score significantly was improved from 51.1 ± 7.5 before the conversion to 88.5 ± 6.4 points at the final follow-up.</p><p><b>CONCLUSION</b>The effect of the hip replacement for patients with failed internal fixation of femoral neck fractures was confirmed. This method can shorten the time on the bed and reduce the complications. It benefits the patients earlier functional recovery, but it must control operation indication. The long term efficacy is necessary to further observation.</p>

Several previous studies have reported the role variant of ERCC1 rs3212986 and ERCC2 rs13181 polymorphisms in the risk of glioma, but the results of these studies are inconsistent. Therefore, we aimed to conduct a meta-analysis to investigate the role of ERCC1 rs3212986 and ERCC2 rs13181 on the risk of glioma. A comprehensive research was conducted through the databases of Pubmed, EMBASE and the China National Knowledge Infrastructure [CNKI] platforms until June 1, 2014, including 14 eligible case-control studies. Our meta-analysis found that ERCC1 rs3212986 AA genotype was significantly associated with increased risk of glioma compared with CC genotype, and the pooled OR [95%CI] was 1.29[1.07-1.55]. By subgroup analysis, ERCC1 rs3212986 AA genotype was found to be significantly correlated with increased glioma risk in Chinese population [OR=1.37, 95%CI=1.07, 1.55], Similarly, we found that ERCC2 rs13181 GT and TT genotypes were significantly associated with increased risk of glioma in Chinese population, with ORs [95%CI] of 1.47[1.17-1.85] and 1.50[1.02-2.22]. But ERCC1 rs3212986 and ERCC2 rs13181 polymorphisms had no significant association with glioma risk in Caucasian populations. By begg's funnel plot, we found that no publication bias was existed in this meta-analysis. Our meta-analysis suggested that ERCC1 rs3212986 and ERCC2 rs13181 polymorphism play an important risk factor for brain tumor development in Chinese population, but no association in Caucasian populations

<p><b>OBJECTIVE</b>To investigate the results of hip arthroplasty for the treatment of severe comminuted proximal femoral fracture with ipsilateral acetabulum fracture.</p><p><b>METHODS</b>From June 2007 to September 2013, 8 patients (8 hips) with severe comminuted proximal femoral fracture combined with ipsilateral acetabulum fracture were treated with hip arthroplasty. All patients were male and using biological prosthesis. Aged from 33 to 64 years old with an average of 41.9 years. According to Harris score in aspect of pain, function, range of motion to evaluate clinical effects.</p><p><b>RESULTS</b>There was no untoward reaction in 8 patients. And bed rest at 3 months after operation, waiting for acetabulum fracture healed to out-of-bed activity. There was no complications such as pneumonia, bedsore and so on in the patients. Follow-up time was from 9 to 72 months with an average of 35.8 months, the wound healed, there was no the subsidence and loosening of prosthesis, no dislocation and infection. The mean of Harris score was 87.5 points after operation.</p><p><b>CONCLUSION</b>The effect of the hip replacement in treating severe comminuted proximal femoral fracture with ipsilateral acetabulum fracture is confirmed. It can restore motor function and reduce traumatic complication, may serve as a substitute for internal fixation of difficult operation. The long-term efficacy is necessary to further observe.</p>

OBJECTIVES@#To investigate the effect of microRNA (miR-184) on regulating the genesis, development and proliferation of glioma cells.@*METHODS@#Lipidosome was used to transfect miR-184 mimic and inhibitor to glioma cell line, and the cell proliferation ability changes were determined by MTT and plate cloning experiment after the transfection. WB test was used to measure the levels of cyclinD1, p27 and FOXO3. Meanwhile, QPCR was used to detect miR-184 expression in glioma cell line, glioma tissues and adjacent tissues. Luciferase experiment was used to test 3'UTR gene targeting regulation of miR-184 and FOXO3.@*RESULTS@#QPCR results showed a significant lower miR-184 expression level in glioma cell line and glioma tissues than that in juxtacancerous tissue. MTT and plate cloning experiments have shown that after over-expressing of miR-184, the cell proliferation capacity of glioma U87 and T98G was significantly increased, which was significantly inhibited after the inhibition of miR-184. WB results showed a lower expression level of p27 in U87 and T98G cells, and a higher expression level of cyclinD1 after over-expressing of miR-184 was observed. However, a lower expression level of cyclinD1 and a higher expression level of p27 after the inhibition of miR-184. The luciferase activity was inhibited after the over-expressing of miR-184.@*CONCLUSIONS@#MiR-184 can affect the proliferation abilities of glioma cells and regulate the cell cycle related protein. It plays an important role in the occurrence and development of gliomas.

Objective To study the expressions of costimulatory molecule B7-H3 and Ki-67 in human glioma and explore their correlation. Methods Forty-six human brain gliomas, collected in our hospital from March 2008 to January 2010, including 21 with low grade and 25 with high grade, were chosen;and 10 normal tissues, collected from patients with cerebral trauma or cerebral hemorrhage at the same period, were chosen. Immunohistochemistry with SABC and SP methods were employed to detect the expressions of B7-H3 and Ki-67. Results B7-H3 and Ki-67 did not express in the normal brain tissues, but their expressions were detected in all glioma tissues of different grades (positive rate of B7-H3: 65.22%, that of Ki-67: 89.13%), which had significant differences (P<0.05). High-grade gliomas showed no significant differences on B7-H3 and Ki-67 expressions as compared with low-grade gliomas (P>0.05);and a positive correlation was noted between B7-H3 and Ki-67 expressions (r=0.417, P=0.014).Conclusion Being the important molecular biological factors, B7-H3 and Ki-67 participate in the development of human glioma and exhibit a synergic effect in this process.

This study was designed to investigate the relationship between plasma cortisol levels and stress ulcer following acute severe head injury. The plasma cortisol levels were prospectively measured by radioimmunoassay in 68 patients following acute head injury. The diagnosis of stress ulcer was based on clinical evidence and was confirmed by endoscopic examination. Patients with stress ulcer and gastrointestinal bleeding [n = 30, 44.1%] were older than those without stress ulcer [38.2 +/- 7.9 vs. 28.3 +/- 9.7 years, P< 0.01]. The combined rate of poor recovery and death in the stress ulcer patients [70.0%] was significantly higher than in the nonulcer patients [42.1%, p = 0.02]. On each of the first 3 days following the head injury, the average plasma cortisol levels in the stress ulcer patients were higher than in the nonulcer patients [p< 0.01]. Univariate analysis showed a positive relationship between plasma cortisol on admission and stress ulcer [r= 0.329, p = 0.01]. Logistic regression analysis revealed that plasma cortisol levels on admission [OR = 2.326, 95% Cl = 1.982-2.466] and age [OR = 1.064, 95% Cl = 0.861-1.219] were independent predictors of stress ulcer. The data showed that acute severe head injury was associated with a significant increase in plasma cortisol. Plasma levels of cortisol and age were independent predictors of stress ulcer following acute head injury

<p><b>OBJECTIVE</b>To evaluate the efficacy and toxicity of combination therapy with surgery and recombinant adenovirus-p53 injection of recurrent malignant gliomas.</p><p><b>METHODS</b>38 patients with recurrent malignant gliomas were included in this study. Among them, 18 patients of combined treatment group had Ommaya reservoirs placed into the tumor cavities after the resection of the tumors and received regular recombinant adenovirus-p53 injections after the operation. The other 20 patients received surgery alone.</p><p><b>RESULTS</b>The 6-month and 1-year survival rates after the combination therapy were 66.7% (14/18) and 44.4% (8/18), respectively. The median survival time was 9.7 months. Compared with the surgery-alone group, the combined treatment group achieved significant improvement (P < 0.05). The Karnofsky score was significantly improved at 6 months after the combination therapy compared with that before the treatment (P < 0.05).</p><p><b>CONCLUSION</b>The recombinant adenovirus-p53 injection is safe and effective in treatment of recurrent malignant gliomas. The combination therapy of surgery and recombinant adenovirus-p53 injection may improve the life quality and the prognosis in patients with recurrent malignant gliomas.</p>

<p><b>OBJECTIVE</b>To evaluate the treatment results of stereotactic (186)Re intracavitary irradiation in the patients with craniopharyngioma.</p><p><b>METHODS</b>Nineteen patients were treated with stereotactic (186)Re intracavitary irradiation, including 12 males and 7 females (average age, 37.2 years). Among them 12 patients had a solitary cyst, whereas 7 patients with mixed structure (e.g., a large cyst with a small solid portion). The mean volume of the cystic portion of the tumor before irradiation was 8390 mm(3).</p><p><b>RESULTS</b>The patients were followed up for 6 months to 3 years. The retraction of the cyst was complete in 7 patients, cyst volume decreased more than 50% in 5 patients and less than 50% in 7 cases. Among the 8 patients with visual acuity deficit before irradiation, 5 were improved. No hypopituitarism occurred in patients with normal pituitary function before treatment. One of the 4 patients with hypopituitarism was improved, 3 of the 5 patients with diabetes insipidus was improved.</p><p><b>CONCLUSION</b>Stereotactic (186)Re endocavitary irradiation for the treatment of cystic craniopharyngioma is a safe and effective procedure.</p>

<p><b>OBJECTIVE</b>To evaluate the clinical value of serum hypersensitive C-reactive protein (hsCRP) test in early diagnosis of early-onset infection in premature infants.</p><p><b>METHODS</b>Thirty-five premature infants with early-onset infection, 50 premature infants with non-infectious diseases and 40 healthy premature infants 3 hrs after birth were enrolled. Serum hsCRP levels were measured by particle enhanced immunoturbidimetric assay. Diagnostic test of hsCRP was performed.</p><p><b>RESULTS</b>Serum hsCRP levels shown as medium and inter-quartile ranges (M, Q75-25) were 2.19 and 4.96 mg/L respectively in the infection group, 0.41 and 0.61 mg/L respectively in the non-infectious disease group, and 0.24 and 0.28 mg/L respectively in the healthy group. Serum hsCRP levels in the infection group were significantly higher than those in the non-infectious disease and the healthy groups. When serum hsCRP test was used for the diagnosis of early-onset infection, the area under the ROC curve was 0.84, 95% confidence interval was 0.76-0.93, the diagnostic sensitivity and specificity were 74% and 86% respectively, and the positive and negative predictive values were 37% and 97% respectively. ConclusionsSerum hsCRP test is valuable in early diagnosis of early-onset infection in premature infants, but it can not serve as an independent diagnostic marker.</p>

<p><b>OBJECTIVE</b>To study the chemical constituents and the antibacterial activity from n-butanol extract of Sarcandra glabra.</p><p><b>METHOD</b>The compounds were isolated by Diaion HP-20, Sephadex LH-20, MCI CHP-20 and silica gel column chromatographic methods. Their structures were identified on the basis of physicochemical and spectroscopic analysis. The antibacterial effect of the compounds were measured against Staphylococcus aureus by filterpaper slice method, finally the antibacterial ring in each group was recorded after 24 hours.</p><p><b>RESULT</b>Seven constituents were isolated and elucidated as 5, 7, 3', 4'-tetrahydroxy-6-C-beta-D-glucopyranosylflavanone (1), kaempferol-3-O-beta-D-glucuronide (2), fraxidin (3), isofraxidin (4), isofraxidin-7-O-beta-D-glucopyranoside (5), kaempferol (6), pinostrobin (7). Diameters (in mm) of antibacterial ring in the compounds 2, 5, 6 were orderly recorded as follows: 14.67 +/- 0.08, 11.14 +/- 1.06, 8.26 +/- 1.26 and the compound 4 is not effective.</p><p><b>CONCLUSION</b>Compounds 1-3 and 5 were isolated from S. glabra for the first time.</p>

<p><b>OBJECTIVE</b>To investigate the silencing effect of gene encoding peroxisome proliferator-activated receptor gamma (PPARgamma) on the expression of tumor necrosis factor alpha (TNFalpha) by constructing vectors for RNA interference in RAW264.7 cells.</p><p><b>METHODS</b>The pSUPER-EGFP vectors were used to transcribe functional small interfering RNA (siRNA). Four pairs of oligonucleotides (64 nt) targeting PPARgamma gene were inserted into the downstream of the H1 promotor, with their veracity confirmed by double digestion and sequencing. Western blotting and immunofluorescence assay were used to examine the silencing effect of PPARgamma gene in RAW264.7 cells. Meanwhile, the TNFalphalevel was determined by Sandwich ELISA.</p><p><b>RESULTS</b>Compared with other recombinant pSUPER-EGFP vectors (R-pSUPER.EGFP), R-pSUPER.EGFP2 induced the best silencing effect on the expression of PPARgamma in RAW264.7 cells, which played an obvious inhibitory role in down-regulating the TNFalphaexpression after the curcumin and lipopolysaccharide (LPS) stimulation.</p><p><b>CONCLUSIONS</b>PPARgamma-pSUPER-EGFP inducing a silencing effect on the expression of PPARgamma can efficiently play a negative role in controlling the inflammatory responses of RAW264.7 cells.</p>
