Objective:To explore the key points and implementation of establishing a whole-process clinical research management system.Methods:Based on the problems in practice, combined with project management experiences, this article analyzed the construction of the whole-process clinical research management system.Results:The establishment of the management system provides a comprehensive and sustainable safeguard for clinical research, as well as the improvement of efficiency and quality of clinical research.Conclusions:The establishment of an effective whole-process management system for clinical research project is a useful exploration of the research service model in China.

Jeavons syndrome is a rare type of childhood onset photosensitive epilepsy syndrome, which is characterized by eyelid myoclonic and absences seizure, photosensitivity, eye closure-induced seizure or eletroencephalographic (EEG) epileptiform discharges. Jeavons syndrome is often underdiagnosed and misdiagnosed. Jeavons syndrome has its unique epileptogenic mechanisms, and theclinical classification is always closely related to its treatments and prognoses. This article reviews the latest development of Jeavons syndrome in order to enhance the understanding of Jeavons syndrome.

Recent advance in mechanism of epilepsy induced by hypothalamic hamartoma / 中华神经医学杂志

Hypothalamic hamartoma is a rare congenital malformation that can cause precocious puberty, epilepsy, cognitive behavior and mental disorders. Hypothalamic hamartoma is the best in vivo model for studying subcortical epilepsy in human. The eplieptogenesis of hypothalamic hamartoma has already been highlight in epilepotology. With deeply exploring and revealing its intrinsic eplieptogenesis of hypothalamic hamartoma, it can expand our research views and ultimately bring new therapies to patients. The latest research progress on epilepotology induced by hypothalamic hamartoma is reviewed as follows.

Recent advance in anti-epileptic drugs aggravating seizures and its mechanisms / 中华神经医学杂志

Epilepsy is a chronic paroxysmal neurological disorder characterized by irregularity and unpredictability of seizures. Anti-epileptic drugs (AEDs) are currently the first-line method for the treatment of epilepsy, but AEDs are a double-edged sword. Correct use can control seizures and even cure epilepsy; wrong choices can aggravate seizures and even worsen epilepsy. The mechanism of action includes enhancing gamma-aminobutyric acid (aminohexenic acid, tigababine, and gabapentin) or blocking voltage-gated sodium channels (carbamazepine, oxcarbazepine, and phenobarbitone). This article will review the existing clinical evidences of AEDs exacerbations and their mechanisms of aggravation.

Effect of cytochrome P450 2C19 genotypic polymorphism on treatment efficacy for gastroesophageal reflux disease by rabeprazole / 中国基层医药

Objective To investigate whether the effect of rabeprazole in treating gastroesophageal reflux disease is related to CYP2C19 gene polymorphisms.Methods 278 patients with gastroesophageal reflux disease confirmed by endoscopy and proton-pump inhibitor testing were enrolled in this study,including non erosive reflux disease (NERD) in 122 cases,the reflux esophagitis (RE) in 98 cases and Barrett esophagus in 58 cases.They were treated with rabeprazole for 8 weeks.GerdQ scores before and after the treatment were completed,endoscopy was performed again in patients with RE after treatment.The blood CYP2C19 genotyping was detected by matrix assisted laser desorption ionization time of flight mass spectrum(MALDI-TOF-MS).Results According to the genotype of CYP2C19,they were divided into extensive metabolizers,intermediate metabolizers and poor metabolizers,accounted for 39.57%,42.45% and 17.98%,respectively.There was no significant difference in GerdQ scores of three groups before treatment,and also had no significant difference after 8 weeks treatment,but in each subgroup GerdQ scores after treatment was decreased significantly than before treatment.The total effective rate of 98 patients with RE by endoscopy was 86.73%,but there was no significant difference in total effective rate of the three groups after treatment.Conclusion Rabeprazole is effective in the treatment of gastroesophageal reflux disease.Moreover,rabeprazole is less affected by CYP2C19 genotype and therefore its curative effect is more stable.

Chemical constituents of Viola tianshanica / 中国中药杂志

<p><b>OBJECTIVE</b>To study the chemical constituents of Viola tianshanica.</p><p><b>METHOD</b>Compounds were isolated by silica column, pharmadex LH-20 column and polyamide column, and their structures were elucidated by UV, IR, ESI-MS and NMR.</p><p><b>RESULT</b>Six compounds were isolated and identified as daucosterol (1), kaempferol-7-O-beta-D-glucopyranoside (2), kaempferol- 3-O-beta-D-glucopyranoside (3), isorhamnetin-3-O-beta-glucoside (4), kaempferol (5) and quercetin (6).</p><p><b>CONCLUSION</b>Compounds 2-5 were isolated from this plant for the first time.</p>

Studies on the Specificity of Determination Method for Catechin and Epicatechin in Catechu / 中药新药与临床药理

Objective: To study the specificity of the determination method for catechin and epicatechin in Catechu. Method: Contents of catechin and epicatechin in 7 kinds of crude drugs and 3 kinds of preparations of Catechu were determined. Results: As sampling 100 times as much as Catechu, catechin and epicatechin could be both detected in Semen Arecae, and catechin could be detected in Radix et Rhizoma Rhei, Herba Ephedrae and Radix Sanguisorbae. Conclusion:This method can be used for the quality control of Catechu and its preparations and is of specificity.
