Objective To evaluate the efficacy and safety of DA-EPOCH-R protocol for patients with B-cell non-hodgkin lymphoma (NHL). Methods 43 patients with B-cell NHL received DA-EPOCH-R protocol, and their efficacy and adverse reactions were analyzed. Results 43 patients received a total of 203 cycles of chemotherapy and the median chemotherapy cycle was 6 (2ˉ8 cycles). 32 patients (74.4%) achieved complete remission (CR) after 2ˉ4 cycles of chemotherapy. A further analysis found that age ≤60 years and>60 years, stageⅠ/Ⅱand stageⅢ/Ⅳ, germinal center B-cell (GCB), non-GCB, double expression lymphoma (DEL) and non-DEL patients had no significant differences (P> 0.05). With a median follow-up of 40 months (9ˉ62 mouths), the overall survival (OS) rate of 1-year and 3-year was 97.6 % and 92.8 % respectively. The major toxicity of DA-EPOCH-R protocol was hematologic toxicity. Other toxicities were mild, and no treatment-related deaths occurred. At the end of follow-up, no secondary tumors were found. Conclusions DA-EPOCH-R protocol is an effective and safe protocol for patients with NHL. The result shows that the curative effect of patients in stageⅢandⅣis similar to the patients in stageⅠandⅡ.

The monoamine oxidase mutant A-1 (F210V/L213C) from Aspergillus niger showed some catalytic activity on mexiletine. To futher improve its activity, the mutant was subjected to directed evolution with MegaWHOP PCR (Megaprimer PCR of Whole Plasmid) and selection employing a high-throughput agar plate-based colorimetric screen. This approach led to the identification of a mutant ep-1, which specific activity was 189% of that for A-1. The ep-1 also showed significantly improved enantioselectivity, with the E value increased from 101 to 282; its kinetic k(cat)/K(m) value increased from 0.001 51 mmol/(L x s) to 0.002 89 mmol/(L x s), suggesting that catalytic efficiency of ep-1 had been improved. The mutant showed obviously higher specific activities on 7 of all tested 11 amines substrates, and the others were comparable. Sequence analysis revealed that there was a new mutation T162A on ep-1. The molecular dynamics simulation indicated that T162A may affect the secondary structure of the substrate channel and expand the binding pocket.

Objective To explore the expression of pituitary tumor transforming gene (PTTG) and c-myc gene in patients with non-Hodgkin lymphoma (NHL),its relationship with NHL and its clinical significance.Methods PTTG mRNA and c-myc mRNA levels in bone marrow mononuclear cell (BMMNC) isolated from 38 NHL patients and 10 chronic lymphadenitis patients were quantified by reverse transcriptionpolymerase chain reaction (RT-PCR).Results mRNA expression of PTTG and c-myc gene in BMMNC was significantly higher in NHL patients than those in normal controls (PTTG:0.567 7±0.270 7 vs 0.071 2± 0.020 1,t =4.706,P < 0.05; c-myc:0.352 6±0.185 4 vs 0.107 3±0.043 5,t =3.303,P =0.002).The expression of PTTG and c-myc gene in peripheral T cell lymphoma and diffuse large B cell lymphoma showed no significant difference (PTTG:0.556 8±0.211 3 vs 0.602 8±0.244 6,t =0.640,P =0.527; c-myc:0.350 1± 0.177 6 vs 0.361 0±0.190 2,t =0.302,P =0.765).Both expression of PTTG and c-myc were positively related to NHL clinical stage and IPI.Expression of PTTG mRNA was positively related to the expression of c-myc mRNA.Conclusion There was overexpression of PTTG and c-myc in NHL,which indicates that PTTG might be involved in tumorigenesis of NHL through direct or indirect activation of c-myc oncogene.

Objective To compare the operation and clinical effects of total hip arthroplasty and hemiarthroplasty in the treatment of fracture of femoral neck in elderly patients.Methods 58 elderly patients( ≥85 ) with femoral neck fracture were divided into observation group(n=39) and control group(n=19).The observation group was treated by hemiarthroplasty and the control group was treated by total hip arthroplasty.The operation time,blood loss,hospital stay,postoperative hip functional score and complications were observed and compared between the two groups.Results The operation time and blood loss of the observation group were(71.6 ±9.3)min and (266.8±113.5) ml respectively,which were significantly less than those of the control group ( t=7.89,3.43,all P < 0.05 ).Therer was no significant difference of the hospital stay,postoperative hip functional score and incidence rate of complications between the two groups.Conclusion Hemiarthroplasty was an ideal rmethod in treating fracture of femoral neck in elderly patients,which was effective and simple.

Hemodialysis monitoring and information management software was used to establish an intelligent electronic system to manage hemodialysis patients' information,with hardware and software moderations.The electronic information management system for hemodialysis functioned steadily since 2008.This system changed not only the existing medical process of the center,but also contributed to safer treatments,more accurate operations of doctor’ s instructions,and better medical information records as well The results indicate that the electronic information management system in hemodialysis centers not only irnproves work efficiency and management quality significantly,but also effectively ensures the safety and caliber of hemodialysis treatment with less manpower costs.

Objective Congestive heart failure (CHF) is the final common pathway of various heart diseases.Calcineurin,a calcium/calmodulindependent phosphatase consisting of a catalytic subanit A (CnA) and a regulatory calcium-binding subunit B (CnB),is activated in heart failure.This study aimed to investigate the relationship between mRNA level of calcineurin in circulating T-lymphocyte and that in myocardium in patients with CHF. MethodsA total of 38 patients with CHF (aged from 29 to 62 years) were included in this study.The mRNA levels of alpha-and beta-isoform of CnA in left ventricular anterior papillary muscle and peripheral lymphocytes were determined by semi-quantitative reverse transcription polymerase chain reaction.Pearson linear correlation analysis was performed,and difference was considered statistically significant at a P value <0.05. ResultsCalcineurin mRNA levels in lymphocytes were positively correlated with those in myocardium (for CnA-alpha mRNA,r=0.820;for CnA-beta mRNA,r=0.875;both P<0.01).CnA-beta mRNA levels in both circulating lymphocytes and myocardium increased significantly with increasing NYHA class (r=0.877 for peripheral blood and r=0.805 for cardiac muscle;both P<0.01). ConclusionsThe mRNA level of CnA-beta in circulating lymphocytes is positively correlated with that in myocardium and is a promising marker for the severity of cardiac dysfunction in patients with CHF.

Calcineurin dephosphorylates multiple serine residues near the N terminus of NFAT proteins enabling them to translocate from cytoplasm to nucleus, where they activate a subset of hypertrophic response genes. Transgenic mice over-expressing a constitutively active form of calcineurin or NFAT3, developed obviously hypertrophy and heart failure or sudden death proving its pathogenic role. Here we used literatures on MEDLINE (2000-2011), systematically reviewed the new development of calcineurin signaling pathway in myocardial hypertrophy.

<p><b>OBJECTIVE</b>To investigate the chemical constituents from the extract of the fruits of Ailanthus altissimrna.</p><p><b>METHOD</b>Twelve compounds were isolated by chromatography and identified by spectral data.</p><p><b>RESULT</b>The compounds obtained were identified as (+)-isolariciresinol (1), isolariciresinol 9-O-beta-D-glucopyranoside (2), gallic acid (3), D-sorbitol (4), vanillin (5), 6,7-dihydroxy-coumarin (6), stigmast-4-en-3-one (7), 5alpha, 8alpha-epidioxy-ergosta-6,9 (11), 22E-trien-3beta-ol (8), ergosta-4,6,8 (14), 22E-tetraen-3-one (9), cycloeucalenol (10), lupeol (11), betulinic acid (12).</p><p><b>CONCLUSION</b>Compounds 1-12 were isolated from this genus for the first time.</p>

<p><b>OBJECTIVE</b>To investigate the chemical constituents of the extract of Galium verum.</p><p><b>METHOD</b>The compounds were isolated by chromatography and identified by spectral data.</p><p><b>RESULT</b>The eleven compounds obtained were identified as (+)-pinoresinol 4,4'-O-bis-beta-D-glucopyranoside (1), epipinoresinol (2), (+) -medioresinol (3), isorhamnetin (4), isorhamnetin 3-O-alpha-L-rhamnopyranosyl-(1-6)-beta-D-glucopyranoside (5), diosmetin (6), diosmetin 7-O-beta-D-glucopyranoside (7), quercetin-3-O-beta-D-glucopyranoside (8), ursolic acid (9), ursolic aldehyde (10) and rubifolic acid (11).</p><p><b>CONCLUSION</b>Compounds 1-5 and 9-11 were isolated from this genus for the first time.</p>

<p><b>OBJECTIVE</b>To observe the activity of peripheral blood monocytes (PBMs) derived macrophage scavenger receptors (MSR) and changes of serum inflammatory factor in peripheral blood in patients with coronary heart disease (CHD), and to evaluate the effect of Ginkgo biloba extract (GBE) on the MSR activity, to explore the relationship between inflammatory factor and scavenger receptors activity as well as the possible mechanism of GBE in stabilizing the atheromatous plaque.</p><p><b>METHODS</b>Ninety-seven CHD patients with normal blood lipids were classified into the stable angina group, the unstable angina group and the acute myocardial infarction group, and 29 healthy persons were taken as control. Levels of C-reactive protein (CRP), soluble intercellular adhesion molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1) in all subjects were determined. And their PBMs were isolated, cultured in vitro, and transferred into macrophage to observe the effect of GBE on the expression of scavenger receptors.</p><p><b>RESULTS</b>The levels of MSR activity, CRP, sICAM-1 and sVCAM-1 in patients with acute myocardial infarction > unstable angina > stable angina > control.</p><p><b>CONCLUSION</b>GBE could down-regulate the MSR activity in CHD patients, which was positively correlated with levels of CRP, sICAM-1 and sVCAM-1. MSR activity could be taken as a monitoring criteria for active degree of vulnerable atherosclerosis plaque. GBE has the effect of suppressing MSR activity.</p>

Objective To isolate and identify the chemical constituents of 95% alcohol extract in the fruits of Ailanthus altissima. Methods The compounds were prepared by repurification and their structures were identified by physicochemical properties and spectral analyses. Results Nine compounds were isolated and identified as ailanthone A [11?, 20-epoxy-1?, 2?, 12?-pentahydroxypicrasa-3, 13(21)-dien-16-one, Ⅰ], olean-9(11), 12-dien-3?-ol (Ⅱ), D-friedoolean-14-en-3-one (Ⅲ), ?-amyin (Ⅳ), cycloart-25-ene-3?,24?-diol (Ⅴ), hopane-3-one-22-ol (Ⅵ), n-tetratriacontane (Ⅶ), L-evonymitol (Ⅷ), D-mannitol (Ⅸ). Conclusion Among them Ⅱ- Ⅸ are isolated from the plants of Ailanthus Desf. nom. Conserv. for the first time.

AIM: To investigate the influence of adenosine on human umbilical vein endothelial cells (HUVEC) bFGF protein production and bFGF mRNA expression. METHODS: Immunohistochemistry staining was performed to detect bFGF protein. RT-PCR was performed to detect bFGF mRNA expression. RESULTS: Immunohistochemistry study demonstrated that there was only a small amount of bFGF positive cells and the color was weak in control group (without adenosine). In groups treated with 10~-4 mol/L and 10~-6 mol/L adenosine, bFGF protein was significantly higher than that in control group (P0.05). RT-PCR showed that in 10~-4 mol/L and 10~-6 mol/L adenosine groups, bFGF mRNA expression was higher than that in control group (P0.05). CONCLUSION: Adenosine may promote HUVEC proliferation and angiogenesis partly through inducing bFGF expression.
