Objective To investigate the influence of intensified acid suppression scheme on operation-related clinical parameters, re-bleeding rate and laboratory indexes of elderly patients with ANVUGIB underwent endoscopic hemostasis. Methods 150 elderly patients with ANVUGIB underwent endoscopic hemostasis were chosen from June 2015 to June 2017 and randomly divided into control group (75 patients) with conventional dose of pantoprazole and observation group (75 patients) with intensified dose of pantoprazole; and the clinical efficacy, the disappeared time of spitting blood and melena, hemostasis time, total blood transfusion volume, hospitalization time, re-bleeding rate, conversion to open surgery rate, the levels of laboratory index in 24 h and 72 h after treatment and adverse reaction incidence of both groups were compared. Results The clinical total effects of observation group were significantly higher than that in control group (P < 0.05). The disappeared time of spitting blood and melena, hemostasis time,total blood transfusion volume, hospitalization time of observation group were significantly better than that in control group (P < 0.05). The re-bleeding rate and conversion to open surgery rate of observation group were significantly lower than that in control group (P < 0.05). The levels of laboratory index in 24 h and 72 h after treatment of observation group were significantly better than that in control group (P < 0.05). There was no significant difference in the adverse reaction incidence between the two groups (P > 0.05). Conclusion Intensified acid suppression scheme in the treatment of elderly patients with ANVUGIB undergoing endoscopic hemostasis can efficiently achieve hemostasis in early stage, relieve symptoms and signs, decrease the blood loss amount in perioperative period, reduce the conversion to open operation risk and be helpful to improve the levels of PCV and BUN.

Purpose To analyze the MMP-9,MMP-13,HIF-1α expression in lung adenocarcinoma tissue and to explore the relationship with clinical pathologic features,EGFR mutation and prognosis of the patients.Methods The expression of MMP-9,MMP-13,HIF-1α in 629 cases of lung adenocarcinoma were detected by using immunohistochemical of SP method.50 cases of normal tissue adjacent to carcinoma and 50 cases of pneumonia pseudotumor hyperplasia tissues were selected as controls.629 patients with lung adenocarcinoma were detected by real-time fluorescence quantitative PCR and all of them were followed-up.Results The MMP-9,MMP-13,HIF-1α expression in lung adenocarcinoma tissues were higher than controls (P < 0.001).The MMP-9 expression was correlated with lymph node metastasis and the size of the tumor (P < 0.05).The MMP-13 expression was correlated with smoking history,TNM stage,lymph node metastasis and the size of the tumor (P <0.05).The HIF-1α expression were correlated with smoking and lymph node metastasis.Statistically significant differences were found in all these above groups (P < 0.05).But the expression of all has nothing to do with the EGFR mutation (P >0.05).The Kaplan-Meier survival analysis results showed that MMP-9,MMP-13,HIF-1α positive expression groups of 12,24 and 36 months cumulative survival were significantly lower than the negative expression groups.COX multiple factor analysis results showed that EGFR mutation,the expression of MMP-9,MMP-13,HIF-1α,tumor size,TNM stage were independent risk factors for the development of lung adenocarcinoma patients living conditions.Statistically significant differences were found in these groups (P <0.05).Conclusion MMP-9,MMP-13,HIF-1α are overexpressed in lung adenocarcinoma tissues,and its expression are associated with lymph node metastasis,but has nothing to do with EGFR mutations.Patients with positive expression of MMP-9,MMP-13,HIF-1α and with no EGFR mutation have lower survival rates,and they are independent risk factors for the development of lung adenocarcinoma patients living conditions.

<p><b>OBJECTIVE</b>To study expression of regucalcin (RGN) and prohibitin (PHB) genes in cirrhotic rat liver and to investigate the related effects of compound glutathione inosine injection (CGII) intervention.</p><p><b>METHODS</b>Forty male Wistar rats were randomly divided into a control group (n=12) and a model group (n=28).The model was established by injecting sterile porcine serum (0.5 mL) into the rat abdominal cavity, twice weekly for 8 consecutive weeks; the control group rats were treated with physiological saline injection (0.5 mL) into the abdominal cavity with the same frequency and time span. During the modeling period, four rats from the model group were randomly selected at different time points to examine changes in liver pathology. Upon pathology confirmation of liver cirrhosis, the porcine serum injection was terminated. The remaining 24 rats in the model group were randomly divided into a fibrosis group and a CGII treatment group.The CGII group received CGII (intramuscular injection of 0.018 mL 100g-1 body weight) once a day for 6 continuous weeks; the fibrosis rats were treated with the same dosage of physiological saline with the same frequency and time span.Liver tissue morphology was examined by both hematoxylin-eosin and Masson's staining. RGN and PHB expression at the mRNA and protein levels in liver tissues were detected by real time RT-PCR and immunohistochemical staining, respectively.</p><p><b>RESULTS</b>Both the mRNA and protein expression levels of RGN and PHB were significantly lower in the liver tissues of the fibrosis group than in the control group.CGII intervention led to significant alleviation of the liver fibrosis severity; moreover, the mRNA and protein expression levels of RGN and PHB were significantly higher than those in the fibrosis group.</p><p><b>CONCLUSION</b>Down-regulation of regucalcin and prohibitin gene expression might contribute to the pathogenesis of liver cirrhosis.</p>

<p><b>OBJECTIVE</b>To investigate the distribution of somato-types in Chinese medicine and its correlations with body mass index (BMI), blood lipids and hepato-enzymes in patients with non-alcoholic fatty liver disease (NAFLD).</p><p><b>METHODS</b>From January 2008 to December 2008, the somato-types of 1 163 NAFLD patients were categorized, and its correlations with BMI, blood lipids and hepato-enzymes were analyzed.</p><p><b>RESULTS</b>Among the 1 163 patients, 401 were categorized as qi-deficiency type and 371 as phlegm-dampness type, accounting for 66.38%. Levels of BMI, blood lipids (TC, TG, LDL) and hepato-enzymes (ALT, AST, GGT) in patients of phlegm-dampness type were higher than those in patients of other types, the differences were statistically significant (P<0.05).</p><p><b>CONCLUSIONS</b>Qi-deficiency type and phlegm-dampness type are the dominant pathogenetic somsto-types in patients with NAFLD; abnormal BMI, blood lipids and hepato-enzymes may present in those of phlegm-dampness type more frequently.</p>

<p><b>OBJECTIVE</b>To find the chemical diversity and characteristics of A. lancea on two levels--individuals and populations, and to discover the chemical essentials for forming geoherbs.</p><p><b>METHOD</b>47 rhizomes of A. lancea were collected in 7 populations, and 6 naphtha components (1. elemol, 2. hinesol, 3. beta-eudesmol, 4. atractylone, 5. atractylodin, 6. atractylenolid I) in the rhizomes were determined by GC-MS combination. Principal Component Analysis and Cluster Analysis were carried out by SPSS.</p><p><b>RESULT</b>Cluster Analysis of the 6 main components indicated that the chemical components of geoherbs were different from those of the non-geonerbs of A. lancea. Other analysis showed as follows: 1. The general oil of geoberbs were lower than that of non-geoherbs(P < 0.01), but components yielding more than 1% (% of the total oil) were more than non-geoherbs(P < 0.01); 2. Hinesol mixing beta-eudesmol was more in non-geoherbs, which atractylodin mixing atractylone was more in geoherbs(P < 0.001); 3. Principal Component Analysis implied that atractylone was the most important component to discriminate geoherbs and non-geoherbs of A. Lancea.</p><p><b>CONCLUSION</b>The naphtha composing characteristics of geoherbs was the special proportionment sale, viz. atractylone: hinesol: beta-eudesmol: atractylodin being(0.70~2.00):(0.04~0.35):(0.09~0.40):1.</p>
