Background: Type 2 diabetes mellitus (T2DM) development involves various interrelated risk factors, including age, gender, obesity, and inactivity. However, research lacks insights into the risk factors within the minor ethnic Bishnupriya Manipuri community in Bangladesh. This study aimed to identify T2DM risk factors within this community. Methods: This survey-based prospective observational study was conducted in different villages of Kamalgonj Upazila under the Moulvibazar district in Bangladesh from March 2023 to June 2023 enrolled 280 individuals using purposive sampling. Data collection involved a questionnaire, verbal agreement, and diagnostic documentation. Analysis utilized Microsoft Office and the student抯 t-test. Results: Participants exhibited a 1:1.2 male-female ratio, with 64% aged 36-65. 43% were housewives, 55% engaged in moderate work, 15% smoked, and 41% had a family history of diabetes. Diabetic cases accounted for 39%, with an average age of 57.39.99 years, significantly higher than non-diabetic cases (46.79.93 years). Diabetic cases also showed significantly higher waist circumference (94.1163 cm versus 84.71.76 cm) and body mass index (BMI) (26.1261 kg/m2 versus 21.4497 kg/m2). Conclusions: T2DM predominantly affects older individuals in the Bishnupriya Manipuri community. Increased waist circumference, overweight, and obesity emerged as significant risk factors for T2DM within this population.

Background: In India, families are the primary source of support in caring for the elderly, whereas the potential group of family caregivers is shrinking. Caregivers need greater recognition and support to help them care for the elderly and to maintain their own health and well-being. Assessment of the perceived burden of caregivers helps identify those in need of support. Methods: A cross-sectional questionnaire-based study was conducted to estimate the perceived level of caregiver burden in families of elderly and to identify the association between selected socio-demographic characteristics of the caregivers and the level of caregiver burden. The caregivers of 50 elderly people categorized as mild and moderate dependents as per the Katz index of Independence were included in the study. The Zarith caregiver burden scale was used to evaluate their perceived level of caregiver burden. Results: Thirty-two (64%) caregivers were found to experience a mild to moderate perceived level of caregiver burden. An ordinal regression analysis between other burdens/responsibilities (predictor) of the caregiver and the perceived level of caregiver burden showed a significant association (?2; p=0.009) between the two. An odds ratio of 6.7 (95% CI, 2.22 to 22.7; p=0.00115) showed that as the predictor increased, the event (caregiver burden) increased as well. Conclusions: The majority of the caregivers suffer from a mild to moderate perceived level of caregiver burden. They experience more burden when they hold additional responsibility of caring for the elderly.

@#In tropical regions, numerous tick-borne pathogens (TBPs) play a crucial role as causative agents of infectious diseases in humans and animals. Recently, the population of companion and pet dogs has significantly increased in Vietnam; however, information on the occurrence of TBPs is still limited. The objectives of this investigation were to determine the occurrence rate, risk factors, and phylogenetic characteristics of TBPs in dogs from northern Vietnam. Of 341 blood samples tested by PCR, the total infection of TBPs was 73.9% (252/341). Babesia vogeli (18SrRNA gene – 30.5%) was detected most frequently in studied dogs followed by Rickettsia spp. (OmpA gene – 27%), Anaplasma platys (groEL gene – 22%), Bartonella spp. (16SrRNA – 18.8%), Mycoplasma haemocanis (16SrRNA – 9.4%) and Hepatozoon canis (18SrRNA gene – 1.2%), respectively. All samples were negative for Ehrlichia canis and Anaplasma phagocytophylum. Co-infection was detected in 31.4% of the samples (107/341) of which, A. platys/Bartonella spp. (34/94,10%), Rickettsia spp./B. vogeli (19/94, 5.6%), and M. haemocanis/B. vogeli (19/94, 5.6%) were recorded as the three most frequent two species of co-infection types. Statistical analysis revealed a significant correlation between TBP infection and several host variables regarding age, breed, and living area in the current study. The recent findings reported herein, for the first time in Vietnam, are essential for local veterinarians when considering the appropriate approaches for diagnosing these diseases. Furthermore, this data can be used to establish control measures for future surveillance and prevention strategies against canine TBPs in Vietnam.

Abstract This study was carried out to evaluate the effect of Glutamine, as a dipeptide or a free amino acid form, on the progression of burn injuries in rats. Thirty male Wistar rats were burned with a comb metal plate heated in boiling water (98 °C) for three minutes, creating four rectangular full-thickness burn areas separated by three unburned interspaces (zone of stasis) in both dorsum sides. The animals were randomized into three groups (n=10): saline solution (G1-Control) and treated groups that orally received Glutamine as dipeptide (G2-Dip) or free amino acid (G3-FreeAA). Two and seven days after burn injury, lesions were photographed for unburned interspaces necrosis evolution assessment. Seven days after injury, glutathione seric was measured and histopathological analysis was performed. By photographs, there was a significant reduction in necrosis progression in G3-Free-AA between days two and seven. Histopathological analysis at day 7 showed a significantly higher stasis zone without necrosis and a higher number of fibroblasts in G2-Dip and G3-FreeAA compared with G1-Control. Also, glutathione serum dosage was higher in G2-Dip. The plasmatic glutathione levels were higher in the G2-Dip than the G1-Control, and there was a trend to higher levels in G3-FreeAA. The reduction in histological lesions, greater production of fibroblasts, and greater amounts of glutathione may have benefited the evolution of burn necrosis, which showed greater preservation of interspaces.

PURPOSE@#The study aimed to identify the risk factors of recurrence in chronic osteomyelitis (COM) and to document the microbiological patterns pre- and intra-operatively and at recurrence, if any.@*METHODS@#We performed retrospective review on COM patients treated with surgical debridement and a 6-week course of antibiotics. The patients with symptoms of osteomyelitis for at least 6 weeks, present or past episodes of discharging sinus, documentation of bone sequestration in operative notes or preoperative images were included in the study. Patients with symptoms of osteomyelitis < 6 weeks, lack of history of discharging sinus or lack of evidence of sequestration in preoperative images or intraoperative notes were excluded. Logistic regression models were used to assess the impact of risk factors of recurrence. Cohen-Kappa scores were derived to see the concordance between pre-operative and intra-operative isolates and at recurrence.@*RESULTS@#Totally, 147 COM patients (115 males and 32 females, mean age (33 ± 19) years) were included in this study. Recurrence was noted in 28 patients (19.0%). Polymicrobial growth and extended spectrum beta-lactamase producing Enterobacteriaceae increased the chance of recurrence. Cierny-Mader stage-1, hematogenous aetiology and negative intraoperative culture reduced the chance of recurrence. Concordance between pre-operative and intra-operative cultures was 59.85% (Kappa score 0.526, p < 0.001) and between index surgery and at recurrence was 23.81% (Kappa score 0.155, p < 0.001). Lack of knowledge of causative organism preoperatively did not affect outcome. At mean follow-up (42 ± 15) months, all patients were apparently infection free for at least 1 year.@*CONCLUSION@#Polymicrobial growth and multi-drug resistant organisms increase the risk of recurrence in COM. Patients' age, gender, diabetes mellitus, previous failed treatment, duration of symptoms, haemoglobin, white cell count, C-reactive protein and erythrocyte sedimentation rate at presentation did not have any impact on the recurrence of infection. Pre-operative isolation of organism is of questionable value. Recurrences of infections do occur and are more of re-infections than relapses. Diligent isolation of organism must be attempted even in re-debridements. Even patients with recurrences do well with appropriate debridement and antibiotic therapy.

Resumo Fundamento A circulação colateral coronária (CCC) proporciona um fluxo sanguíneo alternativo a tecido miocárdico exposto a isquemia e ajuda a preservar as funções miocárdicas.
Abstract Background Coronary collateral circulation (CCC) provides an alternative blood flow to myocardial tissue exposed to ischemia and helps to preserve myocardial functions. Endothelial-derived nitric-oxide (NO) production and vascular endothelial growth factor (VEGF) have been suggested as the most important factors in the development of CCC. Adropin is a peptide hormone responsible for energy hemostasis, and is known for its positive effects on the endothelium through NO and VEGF. Objective The aim of this study is to investigate the association between adropin and the presence of CCC in patients with chronic coronary syndrome (CCS). Methods A total of 102 patients with CCS, who had complete occlusion of at least one major epicardial coronary artery, were included in the study and were divided into two groups: the group of patients (n:50) with poor CCC (Rentrop 0-1) and the group of patients (n:52) with good CCC (Rentrop 2-3). The level of significance adopted in the statistical analysis was 5%. Results Mean adropine levels were found as 210.83±17.76 pg/mL and 268.25±28.94 pg/mL in the poor and good CCC groups, respectively (p<0.001). Adropin levels proved to be positively correlated with neutrophil-to-lymphocyte ratios (r:0.17, p:0.04) and the rentrop scores (r:0.76, p<0.001), and negatively correlated with age (r:-0.23, p:0.01) and Gensini scores (r:-0.19, p:0.02). Adropin level is a strong independent predictor of good CCC development (OR:1.12, 95% CI:(1.06-1.18), p<0.001). Conclusion This study suggests that adropin levels may be a possible factor associated with the presence of CCC in CCS patients.

Aims: The objectives of the research work were to evaluate the effects of mulching on soil water content, maize performance and weed growth in dry land area of Bangladesh Study Design: The experiment was laid out in a randomized complete block design (RCBD) with three replications Place and Duration of Study: Central research field of Sher-e-Bangla Agricultural University, Dhaka-1207 during November 2019 to April 2020. Methodology: The experimental field is divided into 3 blocks to represent 3 replications. There are 6 unit plots (each unit plot consists of 3m 2m area) in the experimental farm. Distances of 1 m and 0.5 m are maintained between replication to replication and plots to plots, respectively. Plant to plant and row to row distances are considered to be 0.25 m and 0.75 m, respectively. The treatments are assigned in random plots. The land is ploughed four times followed by laddering to have fine tilth of the soil. The maize cultivars Shuvra and KS-510 are used as plant materials. Fig. 1 represents the six mulch treatments are imposed on the plants: Control (without mulch, CK), water hyacinth dry (10 t/ha, WH), rice straw dry (10 t/ha, RS), black polythene (4m 3m, BP), white polythene (4m 3m, WP), and transparent polythene (4m 3m, TP) (S2). The mulches are applied as per treatment in each plot just after sowing to maintaining an equal thickness throughout the plot. Dried and cleaned mulches are used before applying to the experiment. The significance of the difference among the treatment means were estimated by the MSTAT-C package programme at 5% level of probability. Results: It is found in the experimental results that the soil water content of the no-mulching treatment is lower as compared to that of the other treatments from 0-10 cm, 10-20 cm and 20-30 cm soil depths. It is also observed that the RS mulching provides the highest soil water content, leaf area index (LAI), chlorophyll contents, and total biomass. Significant enhancement of maize yield (20.55 ton/ha) is recorded for the rice straw mulching. In contrast, the weed dry weight of 7.45g/m2 is observed in the RS mulching, which is lower than the other treatments. Conclusions: Therefore, it can be concluded that the RS mulching, compared to other mulching, would be more efficient for maximum utilization of limited water resources, weed suppression as well as to increase the maize yield. The present research approach would be applicable to manage the soil water for enhancing the maize production in dry land area.

ABSTRACT Objective: To investigate the protective effect of wolfberry (WB) against acetic acid-induced colitis in rats. Methods: The rats were divided into four groups with eight rats in each group: the control group, WB group, colitis group and WB + colitis group. Distal colitis was induced in rats by intracolonic instillation of 4% acetic acid. Wolfberry + colitis group received 100 mg/kg of WB extract dissolved in saline through the intraperitoneal route for 7 days. Acute colitis was created on the 8th day, and the rats were sacrificed 48 hours later. Colonic damage was assessed by macroscopic and histological criteria as well as biochemical markers. Results: Mean total antioxidant capacity (TAC), total oxidant status (TOS), tumour necrosis factor (TNF)-α, interleukin (IL)-1β and IL-6 levels were significantly higher in the colitis group compared with the control and WB groups (p < 0.05). The WB + colitis group had significantly lower TAC, TOS, TNF-α, IL-1β and IL-6 levels compared with the colitis group (p < 0.05). The analyses of the histopathological findings indicated that the colitis group had a significantly higher histopathological damage score than the control group (3.12 ± 0.45, 0 ± 0.00, respectively; p < 0.05). Histopathological damage score was significantly higher in the WB + colitis group than in the control group and statistically significantly lower than the colitis group (1.62 ± 0.44, 0 ± 0.00, respectively; 3.12 ± 0.45, respectively; p < 0.05 for both comparisons). Conclusion: Wolfberry extract is an agent that is effective for preventing acetic acid-induced colitis in rats.

Background: Tuberculosis (TB) is the commonest opportunistic infection among Human Immunodeficiency Virus (HIV) positive patients in India and HIV/TB co-infection poses a major public health challenge in developing countries. It is estimated that 60-70% of HIV positive patients will develop tuberculosis in their lifetime. The aim of the present study is to record the clinical, radiological profile of pulmonary and Extrapulmonary Tuberculosis (EPTB) in HIV positive patients. Methods: This was a prospective study conducted in the department of Pulmonary medicine, Kempegowda institute of medical sciences. All newly diagnosed HIV patients during the study period were included and screened for tuberculosis irrespective of whether they had signs and symptoms. Results: Among 44(15.94%) patients among 276 HIV positive patients were diagnosed to have tuberculosis. Males (72.72%) were affected more than females (27.27%). Most common affected age group was 31-40 years with a mean age of 38.08 years. Unprotected heterosexual contact was the most common mode of HIV transmission. Fever, weight loss and cough were the commonest symptoms at presentation. Pulmonary TB was diagnosed in 10(22.7%) patients, EPTB in 30(68.3%) and disseminated TB in 4(9%) patients. All the pulmonary TB patients had CD4 count below 250, EPTB below 150 and disseminated TB patients below 50.6(13.63%) patients had pleural effusion, 5(11.36%) had abdominal TB, 5(11.36%) had tubercular meningitis, 4(9%) had intra thoracic lymphadenopathy and one (2.27%) patient had pericardial effusion. Low CD4 count (<150) had statically significant association with HIV/TB co-infection.Conclusions: The prevalence of HIV-TB co-infection was high. Moreover, HIV positive patients need early diagnosis and treatment of active TB. The study has shown clear correlation between clinical data and the laboratory parameter of immunodeficiency (CD4 count) and the temporal development of TB.

The liver and kidney are very important organs of the body that play vital roles of detoxification of compounds and removal of waste products respectively. In this research, some liver and kidney function tests were carried out on albino mice infected with malaria parasite to know the effect of treatment with extract on them. Pulverised dried seeds of Picralima nitida were extracted using ethanol. Serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities, total bilirubin, total protein, urea and creatinine levels and histopathological studies on liver and kidney tissues were performed. From the results of the experiment, significant (p < 0.05) reductions were observed in serum levels of urea, creatinine of the treated groups compared to the positive control (group infected with malaria parasite and treated with 3% tween 80) on days 3 and 5 post treatment. There was non-significant (p > 0.05) reduction in serum AST and ALT activities of the groups treated with 20, 40 and 80 mg/kg b.w. of the extract on days 3 and 5 post treatment compared to the positive control. Groups treated with 20 and 40 mg/kg b.w. of the extract showed significant (p < 0.05) reduction in bilirubin level on days 3 and 5 post treatment compared to the positive control. All the treated groups showed significant (p < 0.05) elevation in serum total protein on day 3 post treatment while the group treated with 80 mg/kg b.w. of the extract showed significant (p < 0.05) elevation in total protein on day 5 treatment compared to the positive control. Histopathological studies on the hepatic tissues of the treated mice showed lesser inflammatory cells around the portal areas than the untreated. No lesions were noticed in the renal tubules of both the treated and untreated mice. From the results of this experiment, it is now obvious that Picralima nitida seeds possess good hepato-protective properties.

Background: Side branch occlusion is a well known complication of percutaneous coronary intervention. Although occlusion of small side branches is well tolerated, occlusion of larger side branches may cause more serious complications. After PCI the incidence of complications in patients with compromised side branches smaller than 2 mm is small. Compromising side branches larger than 2 mm can be accompanied by clinical outcomes as non Q-wave MI. This study was undertaken to assess the in-hospital outcomes of compromised small (<2mm) side branch after percutaneous coronary intervention. Methods: This cross sectional analytical study was carried out in the department of cardiology, National Institute of Cardiovascular Diseases, Dhaka during the period of September 2011 to June 2012. A total of 100 consecutive patients with coronary artery disease who underwent elective PCI were included in the study. Study patients were divided into two groups on the basis of presence of compromised small (< 2 mm) side branch. In Group- I, small (< 2 mm) side branch were compromised after PCI and in Group- II, side branches were patent after PCI, with 50 patients in each group. In-hospital outcome were evaluated in both groups. Result: There were no significant differences of the baseline clinical demographics between two groups. Post PCI angina was higher in group I than group II (10.0% vs. 9.0%). Non ST elevation myocardial infarction and significant arrhythmia was identical in both groups (2.0% vs. 2.0%) but hypotension was more in group II than group I (4.0% vs. 2.0%). The findings were statistically insignificant between the study groups. There was no mortality, emergency CABG within 24 hours, ST elevation myocardial infarction, cardiogenic shock or acute left ventricular failure during their hospital course in either group. Conclusion: The present study concluded that compromised small (<2mm) side branch after percutaneous coronary intervention was not associated with adverse in-hospital outcome.

The aim of this study was to investigate the effects of the mushroom extract of Coriolus versicolor on drug resistant strain of S. typhimurium experimental results revealed that hot water ethanol extract and Tris- HCl ethanol extract of C. versicolor were not only effective against drug resistant strain of S.typhimurium (MTCC 3214) but also against other pathogenic microorganisms such as K. pneumoniae, S. typhi, E. coli, S. pyogenes and Aspergillus niger. The extracts exerted their effects on the microorganisms by inhibiting their growth. The optimum temperature and pH conditions for mycelial growth of C. versicolor were found to be 28±1ºC and 5.98 pH respectively. Maximum mycelial growth was observed by employing starch and ammonium sulphate as carbon and nitrogen source and on supplementing the cultivation medium with 20% (v/v) culture filtrate of C. versicolor. The present investigation highlights optimization of culture conditions and determination of antimicrobial spectrum C. versicolor .This indicate that the extracts of C. versicolor is a novel inhibitory source against drug resistant bacteria salmonella typhimurium.

Background: Tuberculosis holds one of the top places on the list of the main cause of death in India. At times the patients fail to respond to treatment with anti tubercular drugs, drug resistance being one of the reasons. The increasing incidence of MDR- and XDR-TB worldwide highlight the urgent need to search for newer anti-tubercular drugs. Objectives: The present study was carried out to assess the antitubercular activity of Lawsonia inermis, and if any, to compare it with ethambutol by "in vitro" method. Materials and Methods: From sputum samples of patients of pulmonary tuberculosis, who had not received any antitubercular drug earlier, the acid-fast bacilli were cultured and identified. Resistance ratio method was followed. For each isolate, (originally from the sputum samples) two sets of L-J slants containing ethambutol, in serially doubling concentration ranging from 1mcg/ml to 16 mcg/ml and L-J slants containing L.inermis leaves aqueous extract (2%) in serial doubling concentration ranging from 3 mcg/ml to 48 mcg/ml were prepared. One set for inoculating with test strain and the other with standard H37Rv strain. Each set had a drug/extract free L-J slant (control). All the L-J slants were labeled with appropriate drug concentrations and incubated at 37° C for 8 weeks for growth. Result & Conclusion: The MIC for ethambutol for both test as well as standard H37Rv strain was 4 mcg/ml by Resistance ratio method. The number of colonies (test and standard H37Rv) on all the L-J slants containing L.inermis extract were more than 100. Thus the aqueous extract of L.inermis leaves used, did not show antitubercular activity in the concentrations of 3 to 48 mcg/ml in the Lowenstein – Jenson media in the present laboratory set up. It is suggested that further studies may be undertaken to confirm the results of the present study.
