Résumé

Background: One of the indispensable components of spine surgeries is the relieving of postoperative pain. When used as an adjuvant in regional anaesthesia, α2 adrenergic agonists have both analgesic and sedative properties. With the addition of alpha agonist to epidural local anesthestics, the degree of pain relief increases because of their analgesic properties and augmentation of local anaesthetic effects. Hence, we comparatively evaluated the effect of ropivacaine versus dexmedetomidine and ropivacaine in epidural anaesthesia in lower limb orthopedic surgeries. Materials and methods: The present study included 200 patients in the age group of 25 to 65 years. The patients who were scheduled to undergo lower limb orthopedic surgeries under epidural anaesthesia were included for the present study. All the patients were randomly divided into two study groups; Group I and group II with 100 patients in each group. Pre-operatively pulse rate, non-invasive systolic and diastolic blood pressure (DBP) and respiratory rate was recorded. Assessment of sensory block was done was observing the loss of sensation to pin prick in the midline. Assessment of the motor block was done by analyzing the modified Bromage score. Assessment of analgesia was done by analyzing VAS score. Post-operative values of the parameters were also recorded after surgery. Page 37 vomiting etc were noted in the first 24 hours after surgery. All the results were analyzed by SPSS software and chi square test was used for assessment of level of significance. Results: Mean age of the patients in group I and II were 40.45 and 41.23 years respectively. No significant results were obtained on comparing the mean age and mean weight of the patients. Majority of the patients in both the groups were males while rest were females. 74 % of the patients in group I belonged to ASA grade 1 while in group II, 69 percent of the patients belonged to ASA grade 2. Conclusion: For shorter surgical procedures, dexmedetomidine may be undesirable as it produces prolonged duration of motor block and sedation.

Background: Integrated Child Development Services (ICDS) is one of the world’s largest community based schemes running in India for over three decades. Frequent evaluations of the scheme have been conducted to make it more effective to promote early childhood care. Objectives: Comprehensive assessment of services provided under ICDS in urban slums of Jamnagar city of Gujarat state. Methods: It was decided to study 15% of the total 297 AWCs of the city through Simple Random Sampling technique. The AWCs visited were evaluated with respect to infrastructure facility of the centre, record keeping activity & knowledge of AWWs, availability of essential drugs & logistics. Results: A total of 48 centers were evaluated. 24 centers operated from Kutcha or semi-pucca buildings and toilet facilities were lacking at 20 of the centers. Only about 44% of the enrolled 3-6 years children were present at the AWC on the day of visit. Nearly 40% of the enrolled children had varying grades of malnutrition. Unavailability of medicine kits & other logistics, was observed. Three fourth of the AWW described providing non-formal preschool education & supplementary nutrition as their only responsibilities forgetting other essential components of their service. One fourth of the AWW did not know proper time to initiate Breast Feeding and over one third (37.5%) of them did not know the Universal Immunisation Program schedule fully. Less honorarium & poor quality of supplementary food were their main difficulties. Conclusion: the AWC currently acts merely as a food distribution centre with minimal provision of other services. Regular growth monitoring of the children along with supervision of the services provided would be far more effective in improving the nutritional status of the children than supplementary nutrition alone.

Background & objectives: Developing country like India shows high prevalence of hepatitis C on account of lack of universal precautions while treating patients. This kind of transmission is seen more in cases of Thalesemias on regular blood transfusion, with renal diseases on haemodialysis and co-infection with hepatitis B along with patients admitted in various wards for treatment. Study was conducted with the objective to find out prevalence of HCV infection in patients with hepatitis B, Thalesemias, patients undergoing haemodialysis, patients admitted in hospital for other diseases who are hepatitis B negative, Ante-natal cases and voluntary healthy blood donors. Methods: Blood was collected of 1040 persons from various groups with their consent. All the sera were tested for anti HCV antibody by ELISA and Signal HCV test. Results: Total twelve participants were found positive for HCV infection, majority were belonged to Thalesemic group and rest of the participants belonged to voluntary healthy blood donor group, patients of hepatitis B and patients on haemodialysis. Interpretation & conclusion: those who are at risk of getting Hepatitis C should undergo for HCV testing on regular interval.

Iodophenyl and anthryl retinal analogues have been synthesized. The transisomers have been isolated and purified by high pressure liquid chromatography. The purified isomers have been further characterized by nuclear magnetic resonance and ultraviolet-visible spectroscopy. Incubation of these retinal analogues with apoprotein (bacterioopsin), isolated from the purple membrane of Halobacterium halobium gave new bacteriorhodopsin analogues. These analogues have been investigated for their absorption properties and stability. The iodophenyl analogue has been found to bind to bacterioopsin rapidly. The pigment obtained from this analogue showed a dramatically altered opsin shift of 1343 cm-1. The anthryl analogue based bacteriorhodopsin, however, showed an opsin shift of 3849 cm-1. It has been found that bacteriorhodopsin is quite unrestrictive in the ionone ring site. The apoprotein seems to prefer chromophores that have the ring portion co-planar with the polyene side chain. The purple membrane has also been modified by treatment with fluorescamine, a surface active reagent specific for amino groups. Reaction under controlled stoichiometric conditions resulted in the formation of a modified pigment. The new pigment showed a band at 390 nm–indicative of fluorescamine reaction with amino group (s) of apoprotein –besides retaining its original absorption band at 560 nm. Analysis of the fluorescamine modified bacteriorhodopsin resulted in the identification of lysine 129 as the modified amino acid residue. Fluorescamine-modified-bacteriorhodopsin suspension did not release protons under photolytic conditions. However, proteoliposomes of fluorescamine-modifiedbacteriorhodopsin were found to show proton uptake, though at a reduced rate.
