Résumé

Seaweeds contain significant quantities of lipids, protein, vitamins and minerals. Aquavac Ergosan contains 1% alginic acid extracted from two brown seaweeds, Laminaria digitata and Ascophyllum nodosum. Both in vivo and in-vitro researches have mainly been focused on the effect of Ergosan on fish growth, survival rate, reproductive performance and innate immunity in blood and epidermal mucus. Despite numerous studies on the effects of Ergosan on immune system of aquatic animals and evaluation of seaweeds in proximate analysis, there is no data available on proximate analysis of irradiated Ergosan extract [alginic acid]. Therefore, the aim of this study was to assess the effect of on different groups of irradiated alginic acid [10, 20, 30, 40 and 50 KGy] for analyzing proximate composition. Alginic acid was prepared from the sonicated Ergosan extract. The Alginic acid extracted from Ergosan was irradiated with a gamma cell. The protein and lipid content and Moisture and ash were recorded. Statistical analysis showed no significant differences among all of groups in terms of protein, lipid, ash and moisture. Radiation processing is a very convenient tool for imparting desirable effects in polymeric materials. The polysaccharide degradation by gamma or ultraviolet irradiation is free of initiators. High energy radiation technique can be effectively used to decrease the molecular weight of different polysaccharides such as alginate and Chitosan. Based on the results of this study, gamma irradiation of alginic acid as natural polysaccharide had no effect on crude protein, crude lipid, moisture and ash

Résumé

Background and Aims: Stroke incidence is 15 million people world wide annually and one third of these patients suffer from aphasia. Aphasia is often associated with significant disability in patients. Costs due to cerebrovascular diseases play an important role in national health expenditures. We aimed to assess the effect of piracetam in treatment of aphasia


Methods: Patients with acute ischemic stroke after completing informed consent form were divided randomly into two groups. The intervention group recieved standard treatment and 4800 mg piracetam daily for 12 weeks and the control group received standard treatment plus placebo. All patients on admission, and one, two, and three months post-stroke were evaluated using Nilipour Farsi aphasia test


Results: A total of 40 patients with stroke and mean age of 60.1 +/- 17.2 years were assessed. Among them, the Broca aphasia had the most frequency and global and Wernicke aphasia had the least. The aphasia test showed significant difference between the intervention and control groups at the first and second months of follow-up; but the difference was not significant at the third month


Conclusion: Totally, piracetam does not affect the rate of recovery from aphasia, despite increasing patients' early recovery

Résumé

Rosa damascena has been traditionally used as an herbal medicine for different therapeutic purposes. In order to preliminarily study the probable toxic effects of the plant, its infusion was orally administered to 5 groups of 5 dogs at doses 0.5-8 times that of human use in traditional medicine [90-1440 mg/kg/d] for 10 successive days. The dogs in the control group [n=4] received placebo. Serum levels of urea, creatinine, alkaline phosphatase [ALP], alanine aminotransferase [ALT], bilirubin, albumin and protein were measured in all experimental groups at days 0, 1, 3, 7 and 10. Except for a transient increase in bilirubin levels [day 3] and a rise in serum ALT at day 10, both with the highest dose of the treatment, there were no statistical differences between different experimental groups compared to the control. The results suggest minimal nephrotoxic or hepatotoxic effects for the infusion of R. damascena, however, the medication may be hepatotoxic at extraordinary high doses

More than 50% of road fatalities occur in the 15 to 44-years old which are the young and active group of the society. One of the effective ways to reduce road accidents is using safety belts. In this study the rate of seat belt use and its associated factors have been estimated in the 15 to 44-years old of Kerman. This study was a cross-sectional descriptive and analytical study performed in 2012 and on 506, 15 to 44 years old people visiting the pre-marriage consulting center in Kerman. Sampling was done by convenient sampling. Data was analyzed by SPSS 16 and by using logistic regression. The rate of seat belt use was estimated to be% 54.4. People who owned private vehicles used safety belts more than non-owners [P <0.001]. Participants claimed that their own safety and preventing injury was the most important reason [59.1%] for using safety belts and forgetting due to mind occupation [33%] were the most important reasons for not using safety belts. Despite the high incidence of road accidents in Iran and despite years passed the first compulsory safety belt regulation, the rate of safety belt use among the society andespecially the youth [which are a high percent of the population] is still lower than other countries. These results emphasize the importance of implementing initiatives in this regard.

Résumé

Linguatula serrata, one of the parasitic zoonoses, inhabits the canine respiratory system [final hosts]. The objective of this study was to determine the prevalence rate of L. serrata nymphs in mesenteric lymph nodes [MLNs] of cattle and buffaloes [intermediate hosts] that were processed in the Ahvaz, Iran abattoir. During November 2010 to March 2011, 223 animals [119 cattle and 104 buffaloes], in different sex and three age groups [<2, 2-<3 and 3->3 years old] were sampled randomly at Ahvaz abattoir. Up to 35 grams of their mesenteric lymph nodes were examined separately for nymphal stages of L. serrata by digesting the samples with acid- pepsin method, collected the nymphs and counted under stereomicroscope. Overall 37[16.6%] of 223 animals were infected with L. serrata nymphs in their mesenteric lymph nodes. Prevalence of the infection in cattle and buffaloes were 16.8% and 16.3% respectively. The number of collected nymphs of MLNs was ranged from 1 to 16. No significant differences were seen in the infection rates between males and females [sexes] and age groups in the cattle and buffa-loes [P <0.05]. Linguatula serrata has an active life cycle in the studied area and a zoonotic potential for transmission between animal and human. Avoiding use of raw MLNs to dogs can help reduce the infection.

Résumé

Photodynamic therapy is a treatment that uses photosensitizer and intense visible light. When photosensitizers get exposed to a specific light wavelength [preferentially in the red region], they produce reactive oxygen species that are toxic to cells. Recently, attention has been focused on porphyrins and their analogs as photosensitizers. Zn [II] tetrapyridinoporphyrazin complex is a water-soluble photosensitizer that has a good potential for application in photodynamic therapy. In this study, phototoxic effect of this complex on HeLa cancer cell line has been investigated. HeLa cell cultures were treated with different concentrations of Zn [II] tetrapyridinoporphyrazin. The cytotoxic effects were measured both in the presence and absence of light using the MTT assay. The light source was a 150W tungsten halogen lamp equipped with a red filter. Our data indicate that porphyrazine's photocytotoxicity is remarkably more significant than its cytotoxycity in the dark. Statistical analysis showed the effective dose [ED[50]] values in the dark and light conditions were 8.6 and 4.2 microM, respectively. In addition, the results imply that in the range of 0-12 microM, the increase in the complex concentration correlates with the increase in the cytotoxicity effect. However, the cytotoxicity decreases at the higher concentration [50microM], which is likely due to aggregation of the complex. Our results show that Zn [II] tetrapyridinoporphyrazin complex may be a promising photosensitizer for innovative photodynamic therapy and may have a high potential application in cancer treatment. Furthermore, it seems to have more benefits compared to other known photosensitizers

Noise-induced hearing loss [NIHL] is one of the most common occupational illnesses. Most of the studies on NIHL were conducted at high noise levels that people are rarely exposed to but in industries. The function of the outer hair cells [OHCs] is impaired after exposure to industrial noise. Distortion product otoacoustic emissions [DPOAEs] are useful in examination of noise-induced level shifts. To assess the function of OHCs by DPOAE temporary and permanent level shifts [TLSdp and PLSdp] in rabbits exposed to white noise at realistic levels typically found in industrial settings over a broad range of frequencies. 12 albino rabbits were divided into two groups: the experimental group rabbits which were exposed to 95 dB SPL white noise at 500-8000 Hz for 8 hrs/day for 5 consecutive days, and the control group rabbits with no exposure to noise. The function of OHCs was examined by DPOAE level [Ldp] in different occasions. The study groups were compared for DPOAE temporary and permanent level shifts [TLSdp and PLSdp] to assess the effect of noise on OHCs function. Noise-induced DPOAE levels [Ldp] were decreased up to 20.65 dB [on day 8] and 18.93 dB [on day 11] at 5888.50 Hz [p=0.081]. TLSdp and PLSdp were significantly decreased up to 17.99 dB and 16.27 dB, respectively in the experimental group. The most and least Ldp were significantly different [p<0.05]; they occurred at 5888.50 and 588.00 Hz, respectively. There were significant differences between temporary and permanent threshold shift at various frequencies [p<0.05]. These differences were mainly related to 5888.50 Hz compared to other frequencies in each ear [p<0.05]. DPOAEs are an attractive tool for obtaining information about small temporary or permanent threshold shifts, even when the pure tone audiogram is normal

Résumé

Protecting households against financial risks is one of objectives of any health system. In this regard, Iran's fourth five year developmental plan act in its 90th article, articulated decreasing household's exposure to catastrophic health expenditure to one percent. Hence, this study aimed to measure percentage of Iranian households exposed to catastrophic health expenditures and to explore its determinants. The present descriptive- analytical study was carried out retrospectively. Households whose financial contributions to the health system exceeded 40% of disposable income were considered as exposed to catastrophic healthcare expenditures. Influential factors on catastrophic healthcare expenditures were examined by logistic regression and chi-square test. Of 39,088 households, 80 were excluded due to absence of food expenditures. 2.8% of households were exposed to catastrophic health expenditures. Influential factors on catastrophic healthcare were utilizing ambulatory, hospital, and drug addiction cessation services as well as consuming pharmaceuticals. Socioeconomics characteristics such as health insurance coverage, household size, and economic status were other determinants of exposure to catastrophic healthcare expenditures. Iranian health system has not achieved the objective of reducing catastrophic healthcare expenditure to one percent. Inefficient health insurance coverage, different fee schedules practiced by private and public providers, failure of referral system are considered as probable barriers toward decreasing households' exposure to catastrophic healthcare expenditures

Résumé

In recent years, plants natural products have gained considerable attention as natural preservers for use in different industries. Due to their free radical scavenging activities, phenolic compounds can prevent a wide range of diseases such as cancers and cardiovascular and neurodegenerative diseases. For a long time, different Ferulago species have been in use as medicinal plants worldwide. The aim of this research was to investigate total phenolics and the antioxidant and antibacterial activities of essential oil and different extracts from F. angulata growing wild in Iran. Antioxidant activity of our samples was examined by DPPH assay and their phenolic content was determined using the Folin - Ciocalteau method. Moreover, their inhibitory effects against five gram-negative and gram-positive bacteria including Shigella boidii, Pseudomonas aeruginosa, Escherichia coli, Staphylococcus aureus and Enterococcus faecalis were tested by using the disk diffusion method. Ethyl acetate fraction and methanol extract contained the highest levels of phenolic compounds [229.2 and 202.9 micro g/mg galic acid] in this study. Also, essential oil of this plant exhibited low free radical scavenging activity [IC[50]= 36129.43micro g/ml]. On the other hand, ethyl acetate and aqueous fractions had the highest antioxidant activity [IC[50] values of 20.153 and 28.28 micro g/ml, respectively]. Finally, studied samples had no significant antibacterial activities against tested organisms. The essential oil of this plant can be used as a flavoring agent while extracts prepared from it have the potential to be used as natural antioxidants in relevant industries

Patellofemoral pain syndrome [PFPS] is a common musculoskeletal condition. Women with patellofemoral pain syndrome would present significant weakness in many of hip muscle groups when compared with the healthy group. The objective of the present study was to compare the strength of hip abductor, extensor and external rotator muscle groups in sedentary women with bilateral patellofemoral pain syndrome to a matched control group of sedentary women without patellofemoral pain syndrome. Twelve women with bilateral patellofemoral pain syndrome as experimental, group and other twelve matched women as control group participated in this study. Maximum isometric strength for three hip muscles including abductor, extensor and external rotator muscle groups was measured bilaterally in all subjects using a dynamometer fixed on a platform and finally averaged between two limbs and compared with the control group. The hip abductor, extensor and external rotator muscle groups of experimental group were statistically weaker than that of the control group [p< 0.001]. It seems that lower limb muscles including hip muscles of women with patellofemoral pain syndrome become weaker due to disuse conditions

In this study the level of IL-23 and IL-27 produced by macrophages derived from peripheral blood mononuclear cell culture collected from patients with healing or non-healing form of cutaneous leishmaniasis lesion were compared before and after treatment with live Leishmania to explore whether IL-23 or IL-27 plays any role in healing process of cutaneous lesions induced by L. major. Twenty patients resident in Isfahan Province, with healing or non-healing form of cutaneous leishmaniasis lesion caused by Leishmania major participated in this study. In vitro productions of IL-23 and IL-27 by peripheral blood derived macrophages, before and after stimulation with live L. major [MRHO/IR/75/ER] promastigotes were evaluated using ELISA method. Patient with healing form of lesion received no treatment and patient with non-healing form of lesion received at least 2 courses of glucantime. The mean production of IL-23 and IL-27 from macrophages of patients with healing form of lesion was significantly higher than patients with non-healing form of lesion. The levels of IL-23 and IL-27 in culture supernatants before and after stimulation in healing form of CL was significantly higher than non- healing form of CL [P < 0.001]. IL-23 and IL-27 might play a role in human leishmaniasis and further studies are needed to understand the role of IL-23 and IL-27 in leishmaniasis

Breast conserving surgery [BCS] followed by local radiotherapy is the standard treatment for early stage of breast cancer. Margin status may have an important impact on local recurrence and overall survival of patients treated with BCS, but this has not been defined adequately. In this study, we investigated the relationship between microscopic margins and outcome of BCS. Three hundred eighty four patients treated with BCS between 1999 and 2010 were included in our study. All surgeries were performed by a single surgical team. Margins were defined as close if surgical margins were less than 2 mm, as free if surgical margins were more than 2 mm and as positive if malignant cells were present in one of the tissue edges. Demographic, clinical, and pathological factors as well as biological markers, recurrence, and survival rates were compared between the close and the free margins groups. The mean age of patients at the time of diagnosis was 54.2 years. There were 34 [9%] recurrence cases and overall survival was 128.7 months and ten years survival rate was 81%. There was no significant difference between the close and the free margins groups in terms of demographic parameters, tumor characteristics, biological factors [estrogen and progesterone receptor status] and lymphovascular invasion. Also recurrence rate did not differ significantly between these two groups. Our study indicates that there is no significant difference between the patients with close and the free margins in terms of treatment results. There is no clear consensus in the surgeons' community over the definition of acceptable margin width in BCS, so additional studies are required to find the right answer

Peripheral neuropathy is among the most common complications of diabetes. Balance deficit in diabetic patients with peripheral neuropathy is due to peripheral nerves injury. The purpose of this study was to investigate the effects of balance exercises on stability indices in diabetic patients with neuropathy. This clinical trial study was conducted on 10 diabetic patients with neuropathy as experimental group, and 10 healthy subjects with the same age as the control group. The subjects were recruited through simple nonprobablity sampling. Stability indecis of all subjects in the two groups were measured on bilateral and unilateral standing with open and closed eyes. Experimental groups were trained for ten sessions by progressive balance exercises and were examined again after the ten sessions. Then balance indices of the experimental group before and after training were compared with the healthy subjects. Overall and anterior- posterior stability indecis were significantly different before and after treatment in the experimental group during different conditions. These significant differences were betv/een 0.01 to 0.0001 for overall stability index with open eyes and antero-posterior index with closed eyes respectivly. Balance indices caused significant differences between the experimental and healthy subjects before the treatment [p<0.006]. After ten sessions of balance exercise therapy in the experimental group there was no significant differences in the indices between the two groups. Based on the results of this study, it can be concluded that balance training could improve stability indices in diabetic patients with neuropathy. The improvement is more significant in eyes open condition. Therefore balance exercise therapy is recommended for rehabilitation of diabetic patients with neuropathy

Résumé

Urinary tract infection is one of the most prevalent complains among elderly in primary care centers. The purpose of this study was to assess urinary tract disorders and prevalence of urinary tract infection in residents of Kahrizak elderly House. In this cross-sectional study, 142 men and 114 women [mean age: 76.03 +/- 8.32 years] were randomly selected for the study. Data were gathered using a questionnaire. Weight and height of the participants was measured and urine cultures were ordered. Data were analyzed using ?[2] and Fisher's exact test. The P<0.05 denoted statistical significance. Findings showed that 34% of elderly had urinary tract infection. Of those, 55% [46 cases] were women and 45% [41 cases] were men. Prostate hypertrophy [32.5%] and gyneacologic disorders [16.9%] were the most diagnosed disorders [P<0.05]. The prevalence of urine tract infection in elderly with urinary system disorders is high. Recognizing at risk elderly and referring them to the health care centers can prevent urinary tract infection and its complications

Increased frequency and length of hospital stay in schizophrenic patients in psychiatric hospitals lead to many problems including the increased hospital costs for the health care system and their families. The aim of this study was to evaluate the effect of nursing process education on the nursing care quality of schizophrenic patients. In this semi-experimental study 30 nurses in psychiatric ward of Shahid Lavasani Hospital in Tehran were randomly divided into experimental and control groups. The nursing care quality in both groups was evaluated using the quality patient care scale [Qual PacS]. The experimental group was trained for nursing process of schizophrenic patients. After one month the quality of nursing care was assessed again. The results of this study indicated that the rate of the nursing care quality in experimental group after training improved from poor to good in physical needs [P<0.0001], from poor to moderate in psychological needs [P<0.0001], from poor to moderate in the relationship with the patient [P=0.002] and poor to good in the presented cares [P<0.0001]. The overall nursing care quality improved from poor to good [P<0.0001], but there was no statistically significant difference in the control group before and after training [P<0.80]. Nursing process education can improve the nursing care quality of patients with schizophrenia

Breast conserving surgery [BCS] is a widely accepted form of operation in patients with early breast cancer. Recurrence remains one of the greatest concerns in breast conserving surgery. It can provoke serious anxiety in the patient and, when treated by mastectomy, negate the objective of conservation. In this study we investigated the prognostic value of demographic, clinical and pathological factors and biological markers in breast cancer patients treated with BCS. This study was performed on 258 patients who underwent BCS from 2002 to 2010. All of the surgeries were performed by a single surgical team. Recurrence and its risk factors were evaluated. The mean age of the patients at the time of diagnosis was 50. The overall 10 year survival was 81%, 5 year survival was 88% and recurrence rate after surgery was 9%. Lymphovascular Invasion [LVI] was observed in 41 [16%] patients and in 11 [48%] patients with recurrence. Our study confirmed that tumors with estrogen receptor negative and LVI had more recurrence rate but other demographic, clinical and pathological factors and biological markers [progesterone receptor, P53, HER-2] did not have any significant effect on recurrence. We recommend considering LVI and estrogen receptor assay as a prognostic factor in the patients treated with BCS

Background: Noise induced hearing loss [NIHL] is one of the most important occupational diseases worldwide. NIHL has been found to be potentiated by some of air pollutants. Exposure to noise plus carbon monoxide is common in occupational and environmental settings. This study was performed to evaluate the effect of noise and carbon monoxide exposure simultaneously on rabbits' hearing system by Auditory Brain stem Response [ABR] assessment


Materials and Methods: In this experimental study, 24 male adult white rabbits were divided to four groups: control group, noise exposure group, noise plus carbon monoxide exposure group and carbon monoxide exposure group. Auditory Brain stem Responses of the four groups were determined: before exposure, 1 hour after exposure and 14 days after exposure by click and tone burst stimulus


Results: The latency mean time of V wave in the ABR test for the group exposed to noise plus carbon monoxide, 1 hour after exposure at 1, 2, 4 and 8 KHz frequency were respectively 5.43+/-0.08, 5.50+/-0.07, 5.71+/-0.07 and 5.75+/-0.07 millisecond. These values were significantly more than the same values in the other groups


Conclusion: NIHL may be potentiated by carbon monoxide in the simultaneous exposure. Thus, it seems occupational and environmental exposure to noise plus carbon monoxide may increases hearing loss risk

Materials and Method: Thirty two intact central maxillary incisors were selected. The coronal aspect of each tooth was removed and the remaining root received endodontic therapy. The specimens were then divided into 4 groups [n=8]. Post spaces were prepared to a depth of 10mm.The posts [HtCo, Iran] received one of the surface treatments including cleaning with alcohol [Alc], air borne-particle abrasion and cleaning with alcohol [Air-Alc], cleaning with alcohol and conditioning with silane treatment [Alc-Sil] or airborne- particle abrasion, cleaning with alcohol and conditioning with silane treatment [Air-Sil]. All the posts were luted with Panavia F after conditioning the canal dentin primer [ED-primer] according to the manufacturer's instructions. After cementation, the specimens were stored at 37[degree sign]C for 30 days and subjected to simulated aging conditions, consisting of 7500 thermal cycles [5[degree sign]C/55[degree sign]C]. Retention of the posts was measured with a universal testing machine with a crosshead speed of 2mm/min. The data were analyzed using one-way ANOVA and Duncan tests [alpha=0.05]


Results: The mean retentive values [N] of the test groups were as follows: Alc=157.63 +/- 39.91, Air- Alc=171.53 +/- 47.80, Alc-Sil= 167.87 +/- 42.49, Air-Sil= 166.53 +/- 40.32. There was no significant difference between the mean values of the four groups


Conclusion: Treating the surface of HtCo Iranian post produced no significant improvement in the retention of the posts

Résumé

Background and Objective: sulfur mustard is a potent chemical vesicant warfare agent that remains a significant military and civilian threat. Inhalation of sulfur mustard gas causes inflammation and injury to airways and bronchioles. Mast cells promote allergic reactions when exposed to some chemical compounds such as HD. Hexamethylenetetramine has been shown to protect human lung cells against HD toxicity and has also been shown to be effective against the chemical warfare agent phosgene in vivo. The aim of this study was to evaluate the effects of HMT on the number of mast cells in the lamina propria of visceral layer of pleura in male rats after exposure to sulfur mustard


Materials and Methods: twenty seven Albino Wister male rats weighting 200+/-20 gr were randomly divided into 5 groups [Normal Saline [N.S], HMT, HD, Pre-exposure and Post-exposure]. HD, Pre-exposure and Post-exposure groups received sulfur-mustard and N.S group received Normal Saline as a solvent by intratracheal catheter. HMT, Pre-exp and Post-exp groups received HMT via intra-peritoneal for 14 days. After the day 14, body weight changes, the rate of lung tissue injury and the number of mast cells measured in the pleura's visceral layer of the rats' lungs


Results: histological examination and mast cells count showed no significant difference when compared among NS, HMT, Pre-exposure and Post-exposure groups. However, significant reduction was seen in the number of mast cells in HMT and NS groups in comparison with the HD group [p < 0/001]. The number of mast cells in the Pre-exposure and Post-exposure groups was also significantly lower than that of the HD group [p < 0/001]


Conclusions: from the results of this study it can be concluded that HMT may have a positive protective and therapeutic effect on lung tissue in rats

Résumé

The effect of different methods of exercise therapy in the treatment of patients suffering from lumbar segmental instability can be challenged in different following treatment periods. Compare of two different methods of exercise therapy in treatment of patients with lumbar segmental instability after three month. In this clinical trial study twenty-four patients' ranging 18-45 years old with signs of lumbar segmental instability were randomly divided into two groups. The first group did only the routine exercise and the second group did the routine exercise plus stabilizing exercises for eight weeks and continued it until three months after treatment. Studied variables included flexor and extensor trunk muscle endurance, pain intensity, functional disability, flexion and extension lumbar range of motion, right and left trunk muscle side were evaluated before, eight weeks and three months after treatment. Data was analyzed using k-s and repeated measurement tests. The findings showed that flexion lumbar range of motion, right and left trunk muscle side increased and pain, functional disability decreased in stabilizing exercise group. There is statically significant difference in the mean of this mentioned variables between two groups after 8 weeks [P=0.001]. The process of pain intensity and functional disability reduction and muscle endurance time increase was more in stabilizing exercise than routine exercise group, following three months exercise. Regarding the prolonged positive effects of stabilizing exercise in reduction of pain intensity and increase of functional ability and muscle endurance until three months after treatment, it is recommended to use this method along with routine exercises
