Résumé

To study the role of laparoscopic surgery in the treatment of chronic gastro-oesophageal reflux disease [GORD], fourteen cases were admitted to the department of surgery of Zagazig and Minia University Hospitals. All patients were selected after failure of medical treatment, full investigations especially motility studies were done. All cases were examined endoscopicaly and for 24 hours pH-monitoring. All cases were operated by the same team of authors. Nissen fundoplication was done. The mean operative time was 120 +/- 12 minutes. There was no mortality recorded in our study. There was marked reduction in the postoperative complications in comparison to the open surgery, and there was significant improvement in the reflux problem. In conclusion laparoscopic fundoplication is a well promising technique that can effectively relieve symptoms of GORD with minimal morbidity and mortality as well as the mild postoperative pain. Short hospital stay and other benefits of laparoscopic surgery. However long term follow up is needed for proper evaluation

Résumé

An aural polypectomy was performed on 50 patients with active chronic otitis media in association with aural polyp. It is found that in patients with attic and posterior marginal perforation only 13% became dry but the other 87% remained discharged. On the other hand, in patients with large subtotal perforation, only 25% became dry, but the other 75% remined discharged. Cholesteotoma was found in 87% of cases with attic and posterior marginal perforation and in 16.7% of cases with subtotal perforation. According to such findings it can be concluded that aural polypectomy is relatively ineffective in obtaining a dry ear and it is better to remove the aural polyp during a formal operation to explore the state of the ear

Résumé

The article aimed at demonstrating the diagnostic significance of the endoscopy of the maxillary sinus. It correlates the radiographic with the endoscopic findings in case of chronic maxillary sinusitis. For this purpose 30 patients with frank chronic maxillary sinusitis clinically were picked up. Another 5 normal cases were taken as a control group. Both were examined radiologically and endoscopically. False negative and positive results were found

Résumé

Thirty patients were suffering from trauma to a moving head with no audiological changes [60%], six patients were suffering from trauma to a moving head with audiological changes [12%], ten eases were suffering from mild trauma to fixed head with audiological changes [20%], while four cases were suffering from mild trauma to a fixed with less than 5 hours in all cases. In this study the incidence of hearing loss in the immediate post- injury period was 20% and it was of sensorineural type. The change in hearing was found to be contra or homolateral to the injured side but more commonly bilateral high tone deficit, mostly around 4000 Eight cases [80%] out of the ten cases with sensorineural hearing loss were found to be due to cochlear lesion as manifested by audiological tests, while the other two cases [20%] were found to be due to retrocochlear lesion, but central hearing impairment was not observed. Three months later the situation has changed quite dramatically, as all cases [ten cases] recovered completely in this period. I.e. They have suffered a concussion type injury. The incidence of hearing loss was 27.2% for occipital injuries and 26.3%, 12.5% and 0% for temporal, frontal and vertical injuries respectively

Résumé

Studying the variable epithelial and stromal changes in 50 patients with laryngeal nodules and correlating them with some etiologic factors showed that excessive smoking lead to more dysplasia, and excessive vocal abuse lead to more fibrinoid change in the subepithelium of laryngeal nodules, this fibrinoid change with oedema may dilute the irritating toxin and irritant on the surface epithelium, and so plays a factor in the decrease of the incidence of dysplasia in laryngeal nodules

Résumé

Acute mastoiditis and mastoid abscess are, at present, infrequent complications of acute otitis media. The presentation and course of the disease have changed considerably since the introduction of antibiotic therapy. Nevertheless, it is still a serious disease. This bacteriological study of acute mastoiditis and mastoid abscess was performed on 20 cases suffering from mastoid abscess. The results of bacterial cultures from the abscesses revelaed proteus organisms in 12 cases, Streptococcus fecalis in association with proteus in 3 cases, peptococcus and Streptococcus fecalis in 3 other cases, whereas 2 cases gave negative cultures
