Résumé

Type II reaction in leprosy, or erythema nodosum leprosum (ENL), is often characterized by severe clinical symptoms together with nerve function impairment leading to permanent disabilities. Thalidomide has been shown to be a highly effective drug for the treatment of ENL. It is, however, contraindicated for women of childbearing age due to its teratogenicity. On the other hand, pentoxifylline, used to treat hypercoagulable states, is not teratogenic and, like thalidomide, can inhibit the synthesis of tumor necrosis factor-a and other cytokines. In the present randomized double-blind clinical study we compared the effectiveness of orally administered pentoxifylline vs thalidomide in treating type II reaction in 44 patients. Daily doses of 300 mg thalidomide or 1.2 g pentoxifylline were administered for 30 days to multibacillary leprosy patients undergoing type II reaction. Randomly chosen patients were included in the study before, during, and after specific multidrug therapy. Clinical evaluations were performed on the 1st, 7th, 14th, 21st, and 30th days of treatment and laboratory tests were carried out on the 1st and 30th days. As expected, overall, thalidomide proved to be more effective in the treatment of type II leprosy reaction. Nevertheless, continuous treatment with pentoxifylline was effective in relieving the clinical signs of ENL, especially limb edema and systemic symptoms, in 62.5 percent of the patients.

It has been suggested that iron overload may be carcinogenic. In the present study, we evaluated the effect of plasma and prostate carotenoid concentration on oxidative DNA damage in 12-week-old Wistar rats treated with intraperitoneal (ip) ferric nitrilotriacetate (Fe-NTA) (10 mg Fe/kg). Plasma ß-carotene and lycopene concentrations were measured as a function of time after ip injection of carotenoids (10 mg kg-1 day-1 ß-carotene or lycopene) in rats. The highest total plasma concentration was reached 3 and 6 h after ip injection of lycopene or ß-carotene, respectively. After 5 days of carotenoid treatment, lycopene and ß-carotene were present in the 0.10-0.51 nmol/g wet tissue range in the prostate. Using a sensitive method to detected 8-oxo-7,8-dihydro-2'-deoxyguanosine (8-oxodGuo) by HPLC/EC, the level of 8-oxodGuo in rat prostate DNA was significantly higher (6.3 ± 0.6 residues/10(6) dGuo) 3 h after Fe-NTA injection compared with control rats (1.7 ± 0.3 residues/10(6) dGuo). Rats supplemented with lycopene or ß-carotene for 5 days prior to Fe-NTA treatment showed a reduction of about 70 percent in 8-oxodGuo levels to almost control levels. Compared with control rats, the prostate of Fe-NTA-treated animals showed a 78 percent increase in malondialdehyde accumulation. Lycopene or ß-carotene pre-treatment almost completely prevented lipid damage. Epidemiological studies have suggested a lower risk of prostate cancer in men reporting a higher consumption of tomato products. However, before associating this effect with tomato sauce constituents, more information is required. The results described here may contribute to the understanding of the protective effects of carotenoids against iron-induced oxidative stress.

The aim of the present study was to evaluate the effect of first morning urinary volume (collected on three different non-consecutive days), fasting blood glucose (determined on the first and third days of urine collection), and glycosylated hemoglobin (determined on the first and third days of urine collection) on the albumin concentration in first morning urine samples collected on three different days. We found 3.6 per cent asymptomatic bacteriuria in the urine samples; therefore, every urine sample must be tested to exclude infection. One hundred and fifty urine samples were provided by 50 IDDM patients aged 21.9 ñ 7 (l2-38) years with a disease duration of 6.8 + 5.8 (0.4-31) years attending the Diabetes Clinic at the State University Hospital of Rio de Janeiro. There were no differences in albumin concentration (6.1 vs 5.8 vs 6.2 mug/ml; P = NS) or urinary volume (222.5 vs 210 vs 200 ml) between the three samples. In addition, there were no differences in fasting blood glucose (181.9 + 93.6 vs 194.6 + 104.7 mg per cent; P = NS) or glycosylated hemoglobin (HbA 1) (8.4 ñ 1.3 vs 8.8 ñ 1.5 per cent; P = NS) between the first and third blood samples. Six patients (group 1) had a mean urinary albumin concentration of more than 20 mug/ml for the three urine samples. This group was compared with the 44 patients (group 2) with a mean urinary albumin concentration for the three urine samples of less than 20 mug/ml. No difference was found between groups 1 and 2 in relation to fasting blood glucose (207.1 ñ 71.7 vs 187.6 ñ 84.6 mg/dl), HbA 1 (8.1 ñ 0.9 vs 8.6 ñ 1.1 per cent) or urinary volume [202 (48.3-435) vs 246 (77.3-683.3) ml]. Stepwise multiple regression analysis with albumin concentration of first morning urine samples as the dependent variable, and urinary volume, fasting blood glucose and glycosylated hemoglobin as independent variables, showed that only 12 per cent (P = 0.01) of the albumin concentration could be accounted for by the independent effect of morning urine volume on the first day of urine collection. No urine samples showed a change in the cutoff level of 20 mug/ml of albumin concentration as the result of volume. Fasting blood glucose and glycosylated hemoglobin did not influence the urinary albumin concentration. Considerable variability in urinary albumin concentration was found in the three morning urine samples with a mean intraindividual coefficient variation of 56 per cent. Résumé

O presente trabalho traduz estudo prospectivo sobre indicadores de risco gravidico no Piaui. Para tanto, foi elaborada ficha contendo indicadores socio-economicos e medico-cirurgicos, aos quais foram dados valores arbitrarios de 0 a 8. Considerou-se alto risco quando a soma de pontos foi maior ou igual 8, medio risco de 4 a 7 e baixo risco menor 4. Os dados foram obtidos na primeira consulta de gestantes assistidas nos servicos pre-natal da Maternidade Dona Evangelina Rosa (capital do Estado), do bairro Dirceu Arcoverde (periferico da capital) e zona rural da cidade de Barras, que dista 120Km da capital. Esses grupos populacionais apresentaram em comum o fato de pertencerem a classes sociais com limitados recursos financeiros e, por isso, dependentes, quase exclusivamente, da assistencia medica gratuita oferecida pelo Estado. Os resultados demonstraram que os problemas socio-economicos repercutem na aceitacao da gravidez e interferem negativamente na assistencia prenatal. Dentre os problemas medicos destacam-se a sifilis, os estados de hipertensao arterial, as lesoes pre-malignas e malignas do colo uterino e as infeccoes urinarias. Dos tres grupos em estudo, o que apresentou indice mais elevado de gravidez de alto risco foi o da zona rural de Barras, seguido daquele assistido no pre-natal da Maternidade Dona Evangelina Rosa
