Objective:To observe the clinical effect of omeprazole based triple therapy in the treatment of children with peptic ulcer.Methods:From September 2016 to September 2018, 86 children with peptic ulcer in the People's Hospital of Qingyuan County were selected in this study.By using the random number table method, they were divided into two groups, with 43 cases in each group.The control group received ranitidine based triple therapy treatment, the observation group was given omeprazole based triple therapy treatment.The treatment course of the two groups was 30 d. The eradication rate, clinical effect, gastrointestinal hormone level and adverse reactions of helicobacter pylori were compared between the two groups.Results:The eradication rate and total effective rate of helicobacter pylori in the observation group were 97.67%(42/43) and 95.35%(41/43), respectively, which were higher than those in the control group[81.40%(34/43) and 79.07%(34/43)], the differences were statistically significant (χ 2=6.081, 5.108, all P<0.05). After treatment, the levels of gastrin and motilin in the observation group were (75.29±8.31)μmol/L, (164.88±15.05)ng/L, respectively, which were lower than those in the control group [(81.07±8.96)μmol/L, (204.15±17.67)ng/L], the differences were statistically significant( t=3.102, 11.095, all P<0.05). The somatostatin level of the observation group was (22.67±3.95)ng/L, which was higher than that of the control group [(18.31±3.31)ng/L], and the difference was statistically significant( t=5.548, P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(χ 2=0.938, P>0.05). Conclusion:Omeprazole based triple therapy is safe and effective in the treatment of children with peptic ulcer, which is helpful to eradicate helicobacter pylori, reduce gastrin, motilin levels and improve the level of somatostatin.

Objective To investigate the effect of piracoxib sodium on analgesia in patients with upper abdominal surgery,and to provide a reference for the choice of analgesia in these patients.Methods A total of 120 patients with abdominal surgery (stomach,liver,pancreas,spleen,etc.) were enrolled using computer random software,including 81 males and 39 females,37-62 years old,BMI 17.6-24.2kg/m2,ASA grade Ⅰ-Ⅱ.They were randomly divided into parecoxib sodium group (P group) and control group (C group) using computer random software,60 patients in each group.The two groups were treated with midazolam 0.05mg/kg,propofol 1-1.5mg/kg,rocuronium 0.6mg/kg,sufentanil 0.6pg/kg for anesthesia induction.TCI propofol 2-4mg · kg-1 · h-1 and intermittent intravenous injection of sufentanil 0.1-0.2μg/kg,rocuronium 0.15-0.3mg/kg to maintain intraoperative BIS value between 40-60.The visual analogue scale (VAS) was recorded at 2h,4h,6h,8h and 12h after operation.The postoperative analgesia of sufentanil and PCIA bolus times were recorded at 12 hours postoperatively.The incidence of complications such as postoperative sleepiness,skin pruritus,postoperative nausea and vomiting (PONV) and other anesthesia-related complications were recorded.Results The VAS scores of the patients in the P group [(1.41 ± 0.29) points,(1.55 ±0.30) points] were significantly lower than those in the C group [(1.86 ±0.33) points,(1.95 ±0.41) points] both at static state and active state immediately out of the operation room (t =7.934,6.099,all P < O.05).The dosage of sufentanil and the PCIA bolus times in the P group were significantly lower than those in the C group at 12 hours after operation (t =3.732,4.205,all P < 0.05).The incidence rates of lethargy,PONV and skin pruritus in the P group were significantly lower than those in the C group (x2 =8.107,3.927,4.227,all P < 0.05).Conclusion In the patients of upper abdominal surgery,the analgesic effect of parecoxib sodium can significantly reduce the dosage of sufentanil and reduce the risk of postoperative complications at the same time,it is worthy of promotion in clinical use.

Objective To investigate the application value of CT perfusion imaging (CTPI) combined with CT angiography (CTA) for determination and diagnosis of transient ischemic attack (TIA).Methods Totally 50 TIA patients from April 2014 to April 2016 in the neurology department of some hospital underwent examinations by CT scan,CTPI and CTA in time,and the values of cerebral blood volume (CBV),cerebral blood flow (CBF),mean transit time (MTT) and time to peak (TTP) were calculated at the uninjured and injured sides of the areas of interest.The relationship between cerebral blood perfusion and cerebral angiostenosis of the TIA patient was analyzed.Results The 50 patients had the values of CBF and CBV at the injured sides lower while the ones of MTI and TTP higher than those at the uninjured sides (P<0.05).The numbers of the abnormal cerebral blood perfusion patients found by CT scan,CTPI and CTA were 28 (56.00%),45 (90.00%) and 38 (76.00%) respectively,who were confirmed clinically simultaneously.There were significant differences between the TIA positive result rates by CT scan,CTPI and CTA (P<0.05).Conclusion Carotid artery hypoperfusion is one of the pathogeneses of TIA,and positive rate by CTPI is closely related to the conditions of TIA.CTPI combined with CTA contributes to the diagnosis of TIA,and can be used for its individualized treatment.

Aims:To explore gastric cancer susceptibility gene profiling in an area in Shanghai,and to assess the risk of gastric cancer susceptibility. Methods:A total of 152 patients with primary gastric cancer at Shanghai Pudong Hospital were enrolled,and 152 demographic characteristics matched patients with non-gastrointestinal diseases,non-tumor were served as controls. Gene polymorphism was determined by allele specific polymerase chain reaction. Susceptibility gene of gastric cancer was screened. Multiple genes interactions were analyzed and multiple genes risk was evaluated by DEMCHUK model. Results:Univariate analysis showed that CYP2E1,NAT2M1,NAT2M2,NAT2,XRCC1194,MTHFRA1298C,VDR TaqⅠ were susceptibility genes of gastric cancer. Multivariate analysis showed that CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2(A/ A),XRCC1194( T/ T)and MTHFRA1298C(A/ C)were susceptibility genotypes. Synergistic effect was found between genes except MTHFRA1298C (A/ C)with NAT2M1(T/ T)and NAT2M2(A/ A)(P ﹤0. 05). Multiple genes risk analysis showed that combination OR of multiple genes was highly correlated with gene frequency,and the risk of gastric cancer was increased with the increasing number of susceptibility genes. Conclusions:CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2( A/ A),XRCC1194( T/ T) and MTHFRA1298C( A/ C) are susceptibility genotypes of gastric cancer. Carrying multiple susceptibility genes can significantly increase the risk of gastric cancer.

Objective To discuss the influence of psychological education on the resourcefulness of perioperative breast cancer patients. Methods Two hundred breast cancer patients from a first-class hospital in Fujian province were randomly assigned to the control group, in which routine care was done, and an intervention group, in which psychological education curriculum was employed at three stages on the basis of routine care. The resourcefulness scale was used in both two groups before and after the intervention to evaluate the score of individual and social resourcefulness and the total score of resourcefulness in these two groups. Results As to the control group, the score on individual resourcefulness and the total score on resourcefulness before and after the intervention had no significant differences (P>0.05), but the difference in the social resourcefulness scores before and after the intervention was statistically significant (P<0.05). The differences in the scores on the individual resourcefulness, social resourcefulness and the total score on resourcefulness in the intervention group before and after the intervention were all statistically different (P<0.001). What′s more, the scores of individual resourcefulness, social resourcefulness and the total score of resourcefulness between these two groups after the intervention were statistically different (P<0.001) as well. The scores of the intervention group were significantly higher than those of the control one. Conclusion The psychological education can significantly improve the resourcefulness level of perioperative breast cancer patients, which can help individuals to manage stress effectively.

[ABSTRACT]AIM:Tostudytherelationshipbetweenhuman8-oxoguanineglycosylase1(hOGG1)gene Ser326Cys polymorphism and severity of coronary artery lesions in the patients with diabetes mellitus .METHODS: We retrospectively analyzed 323 patients with diabetic mellitus receiving coronary angiography .These patients underwent the test of polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP), and were divided into Cys/Cys genotype (n=85), Ser/Ser genotype (n=121) and Ser/Cys genotype (n=117) according to the results of PCR-RFLP. All clinical data including history of diseases , complications and biochemical markers , such as blood glucose , blood lipids and so on, were recorded.hOGG1 mRNA and 8-hydroxydeoxyguanosine (8-OHdG) were measured by RT-PCR and ELISA, respectively.The results of coronary angiography such as number and severity of coronary artery with lesions were analyzed by 2 cardiovascular physicians .Gensini score and SYNTAX score were also detected by the unitary criteria .RE-SULTS:(1)8-OHdG in Cys/Cys genotype was higher than that in Ser /Ser genotype and Ser/Cys genotype (P0.05).(2)hOGG1 mRNA ex-pression in Cys/Cys genotype was lower than that in Ser/Ser genotype and Ser/Cys genotype (P0.05) was observed.(3)The probability of triple vessel lesions in Cys/Cys genotype was high and the probability of single vessel lesions in Ser /Cys genotype was low , but the difference among 3 genotypes was not statistically significant (P>0.05).(4)Gensini score and SYNTAX score in Cys/Cys genotype were 48.7 ±15.3 and 39.5 ±17.2, respectively, and the ratio of complex lesions in Cys/Cys genotype was 73.0%.These scores and the ratio of complex lesions were higher than the other 2 genotypes (P0.05).CONCLUSION:hOGG1 gene Ser326Cys polymorphism has relationship with coronary artery lesions in the patients with diabetes mellitus and Cys /Cys genotype might have some impacts on the severity of lesions .The mRNA expression of hOGG1 in the patients with diabetes mellitus carrying Cys/Cys genotype is lower than that in the patients carrying the other 2 genotypes , then decreases the abil-ity of identification and removal of 8-OHdG and the capacity of repairing DNA oxidative damage , and accelerates the devel-opment of atherosclerosis .

Carcinoid tumors are relatively rare neuroendocrine malignancies most commonly originating from enterochromaffin cells in the gastrointestinal tract.Carcinoid syndrome occurs when tumor cells metastasize to the liver,as the vasoactive substances produced are able to reach the systemic circulation via the hepatic vein.The development of carcinoid heart disease (CHD) is thought to be related to the vasoactive substances secreted by the metastatic tumor cells in the liver,reaching the right heart.This is associated with deposition of fibrous tissue on the endocardial surfaces of the heart.Several series have reported CHD in up to 70% of cases of carcinoid syndrome.Somatostatin analogue use is a therapeutic cornerstone,effective in symptomatic control of the endocrine syndrome.In patients with severe cardiac involvement and well-controlled systemic disease,valve replacement surgery is an effective treatment modality that can relieve intractable symptoms and contribute to improved outcome.

<p><b>OBJECTIVE</b>To study the contribution of genetic factors to the variance of serum leptin concentration in healthy, normotensive twins.</p><p><b>METHODS</b>A total of 57 pairs of twins were investigated: 28 female and 19 male pairs of monozygotic(MZ) twins, and 6 female and 4 male pairs of dizygotic(DZ) twins. The zygosity of twins was determined by comparing the concordance of the genotype of nine fluorescence-labeled microsatellite markers. The genetic analysis was performed using the variance-based method. Serum leptin levels were determined in duplicate by a radioimmunoassay Kit (Linco Research, Inc., St. Charles, Missouri) as previously described.</p><p><b>RESULTS</b>The test of genetic variance revealed a significantly larger within-pair variance of serum leptin in the DZ twins, in comparison with the MZ twins. The corresponding heritability for serum leptin was 8%. Adjusted for BMI, gender, and uric acid (UA), the heritability for serum leptin was 0.18%. Log leptin correlated significantly with blood pressure (SBp r=0.355 P<0.001; DBp r=0.339 P<0.001). Stepwise multiple linear regression analysis revealed that only BMI, gender and UA were linked independently to serum leptin levels(R(2)=0.788, P<0.001).</p><p><b>CONCLUSION</b>The above data indicate that environmental factors other than genetic factors are important determinants of leptinemia in normal subjects.</p>
