INTRODUÇÃO: Neurocitoma central é um tumor neuroectodérmico raro, geralmente localizado nos ventrículos laterais. RELATO DE CASOS: Uma mulher de 26 anos e um homem de 33 anos apresentaram-se com hipertensão intracraniana. Exames de imagem revelaram tumor intraventricular heterogêneo, que impregnava por contraste, ocupando os ventrículos laterais e causando hidrocefalia. A mulher faleceu no pós-operatório e o homem está livre de recidiva após três anos. HISTOPATOLOGIA: Ambos os tumores eram sólidos, com células arredondadas, lembrando oligodendroglia, positivas para sinaptofisina, cromogranina e NSE e algumas para GFAP, vimentina e proteína S-100. Microscopia eletrônica mostrou neurópilo entre os corpos celulares, mas sinapses eram raras.

A encefalomielite aguda disseminada (ADEM) é doença monofásica inflamatória difusa do sistema nervoso central, que geralmente ocorre após infecção ou vacinação. Neste estudo, apresentamos a análise da ressonância magnética (RM), líquor e aspectos clínicos de quatro pacientes com diagnóstico presuntivo inicial de ADEM. O achado de lesões desmielinizantes na RM foi importante, mas não por si só suficiente para o diagnóstico definitivo. O seguimento clínico e realização de RM de controle, para excluir o aparecimento de novas lesões e reavaliar as anteriores, assim como análises liquóricas, foram impotantes para o diagnóstico diferencial com outras doenças desmielinizantes, particularmente a esclerose múltipla. Além disso, mostramos que a introdução precoce de metilprednisolona foi eficaz tanto para a melhora do quadro clínico quanto para redução ou desaparecimento das lesões na RM.

We studied 11 patients (9 males) with cytogenetic diagnosis of fragile X syndrome (FXS) with the purpose of investigating the neural circuitry involved in this condition. The ages ranged from 8 to 19. All the individuals presented large ears, elongated faces and autistic features. Ten patients had severe mental retardation. Attention disorder was found in 10 individuals. Electroencephalographic recordings were abnormal in 6 of 10 patients examined, showing focal epileptiform discharges predominantly in frontal and parietal areas. All patients underwent magnetic resonance imaging studies which were abnormal in 8 of them. The most important abnormalities were reduction of the cerebellar vermis and enlargement of the IV ventricle. Single photon emission computerized tomography (SPECT) was performed in 7 patients and was abnormal in all of them, the most frequent finding being a hypoperfusion of the inferior portions of the frontal lobes. Based on the clinical picture, neuropsychological findings and functional and structural imaging studies we suggest that FXS presents with a dysfunction involving a large area of the central nervous system: cerebellum - basal frontal regions - parietal lobes. The literature points to a disturbance involving the same neural circuitry in patients with austin.
