Résumé

PURPOSE: The purpose of this study was to investigate nurses' awareness of psychological distress and delirium in cancer patients and job stress. METHODS: The participants were 256 nurses in a cancer general hospital. The nurses' awareness of psychological distress and delirium was investigated using a self-report questionnaire (1–4 scale) developed by the researcher. Psychological distress was measured by depression, anxiety, and insomnia. Job stress was measured using the Korean version of the Expanded Nursing Stress Scale (1–4 scale). RESULTS: The analysis of 9 questionnaires about nurses' awareness was divided into two factors: nurses' competence and importance. Nurses' competence results were 2.06 (depression), 2.17 (anxiety), 2.29 (insomnia), and 2.41 (delirium). Importance results were 3.23 (depression), 3.20 (anxiety), 3.15 (insomnia), and 3.37 (delirium). Most nurses have experienced nursing psychological distress and delirium in cancer patients, but only about a quarter have received the relevant education. Job stress was 2.52 and in subcategories, work load received the highest score 2.92. CONCLUSION: Nurses recognize that psychological distress and delirium in cancer patients are very important, but they are less aware of their competence. Therefore, it is suggested to develop an educational program to improve nurses' competence when dealing with psychological distress and delirium.

OBJECTIVE: This study aims to identify self-reported stressors in the Korean community-dwelling elderly and examine their effects on subjective well-being. METHODS: The subjects of this research were 1,010 elderly people who lived in Suwon, Korea. Each subject was administered the questionnaires regarding the socio-demographic characteristics, self-reported stressors, Concise Measure of Subjective Well-Being (COMOSWB) consisting of life satisfaction, positive emotion and negative emotion, Mini-Mental State Examination for dementia screening and Short Form Geriatric Depression Scale-Korean version. Statistical analyses including the Pearson's correlation test and multiple linear regressions were performed in this study. RESULTS: In our adjusted model for confounding variables, stress from physical problems (S-PP) (ss=-0.133, p<0.001), stress from loneliness or solitude (S-LS) (ss=-0.077, p=0.007), stress from family members (S-FM) (ss=-0.083, p=0.001) were negatively associated with the total score of COMOSWB. In subscale analyses of COMOSWB, we also found that S-PP (ss=-0.184, p<0.001), S-LS (ss=-0.077, p=0.014), stress from economic problems (ss=-0.072, p=0.021) were negatively associated with life satisfaction score. While S-PP (ss=-0.140, p<0.001) and S-FM (ss=-0.079, p=0.003) were negatively associated with positive emotion score, S-FM (ss=0.105, p=0.001) and stress from friends and neighbors (ss=0.072, p=0.016) were positively associated with negative emotion score. CONCLUSION: Stresses from physical problems, loneliness, and family members might be associated with subjective well-being in the elderly. While life satisfaction appears to be affected by one's perceived physical health status, loneliness, and economic conditions, positive and negative emotions appear to be affected by relationships with family members, friends, or neighbors.

OBJECTIVES: The aim of this study was to describe subjective physical and mental health assessed by the single item self-rated physical health (SRPH), and self-rated mental health (SRMH) had influence on the cognitive decline in community-dwelling elderly. METHODS: This study was based on the Suwon Project, a cohort comprising of non-random convenience samples of ethnic Koreans aged 60 years and above. 1,356 individuals were assessed at 2 time points 2 years apart for questionnaire which included demographic characteristics, medical history, Korean Mini-Mental State Examination (K-MMSE), Korean version of Geriatric Depression Scale (SGDS-K), SRPH, and SRMH. Multiple logistic regression was used to assess K-MMSE score change from the SRPH, and SRMH. RESULTS: At baseline, both SRPH (beta=0.18, p<0.01) and SRMH (beta=0.19, p<0.01) score positively associated with the K-MMSE score. At follow-up, subjects with baseline lower SRMH score had significantly greater decline in K-MMSE score (beta=0.09, p=0.007). However, baseline SRPH score showed no effect to follow up K-MMSE score before and after adjusting age, sex, education, number of chronic disease, SGDS-K, and baseline K-MMSE score (p=0.89). CONCLUSION: Brief and easily collected subjective mental health may predict the risk of cognitive decline, but subjective physical health may not.

OBJECTIVES: Among non-pharmaceutical measures for preventing cognitive impairment, involving in social activities is known to be very effective. We tried to examine the correlation between social activity and cognitive function. METHODS: This study was based on the Suwon Project, a cohort comprising of non-random convenience samples of ethnic Koreans aged 60 years and above. All the subjects completed the study questionnaire which included demographic characteristics, current and past history of illnesses, drug history, Korean version of Mini Mental State Examination (K-MMSE), Korean version of Short Form Geriatric Depression Scale, and Beck Anxiety Inventory. We checked the time consumed in each 9 social activities through the checklist. RESULTS: On multiple logistic regression analysis, the total time spent in social activities wasinversely associated with the K-MMSE score after adjusting confounding variables like age, sex, education level, depression, anxiety and vascular risk factor (beta=-0.041, p=0.024). On the other hand, correlation was found between K-MMSE score and time spent in participating in religious party (beta=0.053, p=0.003) as well as time spent in elder's university (beta=0.040, p=0.025) after adjustingage, sex, education, depression, anxiety and vascular risk factor. CONCLUSION: Our results suggest that involving in quality social activities might be more effective in improving cognitive function than spending longer total time in any social activity.

Chromatin structure has a crucial role in a diversity of physiological processes, including development, differentiation and stress responses, via regulation of transcription, DNA replication and DNA damage repair. Histone deacetylase (HDAC) inhibitors regulate chromatin structure and activate the DNA damage checkpoint pathway involving Ataxia-telangiectasia mutated (ATM). Herein, we investigated the impact of histone acetylation/deacetylation modification on the ATM-mediated transcriptional modulation to provide a better understanding of the transcriptional function of ATM. The prototype HDAC inhibitor trichostain A (TSA) reprograms expression of the myeloid cell leukemia-1 (MCL1) and Gadd45alpha genes via the ATM-mediated signal pathway. Transcription of MCL1 and Gadd45alpha is enhanced following TSA treatment in ATM+ cells, but not in isogenic ATM- or kinase-dead ATM expressing cells, in the ATM-activated E2F1 or BRCA1-dependent manner, respectively. These findings suggest that ATM and its kinase activity are essential for the TSA-induced regulation of gene expression. In summary, ATM controls the transcriptional upregulation of MCL1 and Gadd45alpha through the activation of the ATM-mediated signal pathway in response to HDAC inhibition. These findings are important in helping to design combinatory treatment schedules for anticancer radio- or chemo-therapy with HDAC inhibitors.

Résumé

The term "entropion" is defined as a turning inward of the eyelid. Depending on the mechanism of its causation, it may be classified into four types; mechanical, spastic, senile, and cicatricial. Cicatricial entropion follows scarring of the palpebral conjunctiva, which may be caused by trauma, chemical injuries, infections such as trachoma, benign ocular pemphigus, Stevens-Johnson syndrome, and eyelid surgery. Distichiasis is a rare congenital anomaly in which aberrant separate row of lashes usually composed of hairs smaller than normal, appears along the posterior lid margin. Numerous surgical procedures have been described for surgical correction of cicatricial entropion and distichiasis in accordance with the severity of the condition. We have treated cicatricial entropion in 6 eyes(8 lids) and distichiasis in 3 eyes (3 lids) with sliding tarsoconjunctival grafting, and the results were satisfactory.

Résumé

In systemic hypertension, capillary resistance is highly elevated and' contractility of vascular smooth muscle is elevated. This is due to increased calcium uptake into the sarcoplasmic reticulum of the vascular smooth muscle. Recently, nifedipine, an antihypertensive medication which inhibits calcium uptake into the vascular smooth muscle is commonly used. It decreases the contractility of the vascular smooth muscle and causes the vessels to relax, which in turn lowers the blood pressure. In 1987, according to Rodriguez-Sargent et al. the increased incidence of cataract in Dahl-salt sensitive rats(DSR) was due to increasd water and sodium contents and decreased potassium content. In this investigative study on hypertension, the water, Sodium and potassium contents in Spontaneous Hypertensive Rats(SHR) were measured. After treatment with nifedipine in SHR, we observed decreased blood pressure along with the changes in water, sodium and potassium contents of the lenses. The following results were obtained from our experiments. 1. In normal SDR, The blood pressure did not decrease with nifedipine injection. 2. After 3-7 days of nifedipine injection bidaily, the blood pressure of SHR did not decrease. 3. After 2 weeks of nifedipine injection bidaily, the blood pressure of the SHR decreased slightly and after 4 weeks of nifedipine injection bidaily, the blood pressure of SHR decreased significantly. 4. Water and sodium contents of SHR were higher than that of SDR and the potassium content was similar to that of SDR. 5. The elevated water and sodium contents of SHR decreased along with decrease of blood pressure, and after 2 weeks of nifedipine injection, showed similar level as the contents of SDR lenses. 6. Potassium contents of SHR lenses showed no change were similar to SDR lines except the 4 weeks nifedipine injection group which showed higher potasslum contents.

She was examined with slit-lamp, direct and indirect ophthalmoscope, ultrasonography, fluor escein angiography, computerized tomography, and then diagnosed as malignant melanoma of choroid. So enucleation was performed and the tumor was confirmed to be a choroidal malignant melanoma composed of spindle cells and epithelioid cells by light microscope.
