ABSTRACT Objective: To assess the attitudes, experiences, training levels, and interest in future education regarding the use of hypnosis by dentists working in Dental Clinic of Monastir, Tunisia. Material and Methods: A cross-sectional study was conducted among 330 dentists working in Dental Clinic of Monastir in November 2019. Students, internship, residents, and professors were randomly selected. This was a survey with a structured questionnaire mailed to all dentists. The questionnaire was pre-fabricated, simple, and close-ended. Data were analyzed using SPSS 24.0 statistical software. Results: A total of 202 dentists respond to this survey. 54% of the participants were interns with a female predominance (66.5%). Findings revealed that beliefs toward hypnosis in the sample were generally positive. Using a visual analogue scale, dentists reported having moderate acknowledge in hypnosis (3.4). 60% of the participants in our study consider that hypnosis is useful in dentistry and, more particularly, in pediatric dentistry. 78.2% of respondents want to follow training courses in hypnosis. Conclusion: Our survey highlights the weak knowledge of participants in hypnosis and reveals certain misconceptions about this procedure. More efforts are required to better educate dentists about hypnosis's benefits in their practice.

Résumé

Forearm fractures are common injuries in childhood. Median nerve entrapment is a rare complication of forearm fractures, but several cases have been reported in the literature. This case report discusses the diagnosis and management of median nerve entrapment in a 13-year-old male who presented acutely with a both-bone forearm fracture and numbness in the median nerve distribution. Following the delayed diagnosis, surgical exploration revealed complete nerve entrapment and a nerve graft was performed.

Background: Cerebral AVMs are very rare lesions, and this rarity contributes to the difficulty of treating them. There is no consensus concerning the method of treatment to be chosen among neurosurgery, radiosurgery or embolization.


Onyx embolization could serve as a curative option with accepted morbidity and mortality. The introduction of Onyx and of catheters with detachable tips has no doubt increased the rate of endovascular occlusion, and decreased the risks associated with treatment in our experience


Objective: The aim of the study was to assess the outcome of the use of Onyx in the treatment of intracranial AVMs as curative embolization or before neuro- or radiosurgery


Patients and Methods: This analytical prospective study was conducted on 25 patients who were diagnosed with cerebral arteriovenous malformations and underwent endovascular embolization with EVOH copolymer with curative intent during the study period. Interventional procedures were done in the neuro - endovascular unit, neurosurgery department Ain Shams University Hospitals and associate neuroendovascular unit in El Matarya Teaching Hospital in the period between September 2014 and April 2017


Results: Actually comparing these results especially concerning the cure rate along with other studies was somewhat confusing and problematic owing to the diversity in results between studies across the last 15 years


Conclusion: For the cases that are not fulfilling these criteria, embolization should be offered as preparing step for other modality of treatment. In our experience, for curative embolization, the AVM should be small sized [< 3 cm], supplied by one vascular territory, with feeders that can tolerate reflux up to 2-3 cm, with clear proximal parts of the draining veins, and not located in deep structures

Objective To evaluate the effects of ethanol–water (80:20) extract of Marrubium vulgare (M. vulgare) on the hematological parameters, macroscopic and histological aspects of the uterus and fetus in non-pregnant and pregnant rats. Methods Female rats were divided into 4 equal groups (n = 9), group N (normal rats) and group G (pregnant rats) considered as control groups, group NE (normal rats treated with the ethanol–water (80:20, v/v) extract of M. vulgare) and group GE (pregnant rats treated with the extract). The ethanol–water (80:20) plant extract was administered in a single daily dose 1 g/kg at the morning, during 19 d. On the 19 day of the experiment, animals were sacrificed, the uterus and fetuses were removed for the morphological and histological studies and the blood was collected in EDTA tubes for the measurement of hematological parameters with the use of an automate ‘HORIBA ABX Micros 60 Hematology Analyzer’. Results Our results showed, in group NE and GE, a significant decrease on hematological parameters: red blood cells (NE: 18.6%; GE: 38.4%), hematocrit (NE: 13.8%; GE: 20.4%), hemoglobin (NE: 12.1%; GE: 8.3%) and mean corpuscular volume (NE: 6.4%; GE: 2%) with P more less a 0.05. Indeed, the extract of M. vulgare caused a significant decrease on the mean implantations of fetuses (82.5%, P < 0.001) and their size (47.2%, P < 0.01). As for the macroscopic and histological appearance of uterus, our data showed no change in normal treated rats. In contrast, the treated pregnant rats showed a severe histological change characterized by the existence of location of stopped gestation. Furthermore, it was also found in the uterus of these rat lyses placental and embryo tissue. Conclusions All these results support the hypothesis of an abortifacient effect of M. vulgare.

Résumé

Background: acne vulgaris is a common chronic skin disease involving blockage and/or inflammation of pilosebaceous units [hair follicles and their accompanying sebaceous gland]. Desnutrin is the major triglyceride lipase in the adipose tissue of mice and excessive secretion from adipocytes results in decreased triacylglycerol storage and increased lipolysis, fatty acid oxidation and thermogenesis


Objective: the aim of this study is to evaluate serum level of Desnutrin in acne vulgaris patients and correlate it with disease severity


Patients and Methods: this study was performed on 40 patients with active acne lesions and 40 healthy subjects with no previous history of acne and no active acne lesions as controls. The control group was composed of age, gender, and Body mass index [BMI] matched individuals. All the patients were recruited from the outpatient clinic of Dermatology and Venereology Department, Ain Shams University hospitals; from March 2016 till August 2016.Serum desnutrin assessment was done by ELISA kit using Sandwich-ELISA as a method. The Micro elisa stripplate has been pre-coated with a Horse Radish Peroxidase antibody specific to desnutrin. The optical density was measured spectrophotometrically


Results: there was a significantly lower level of serum desnutrin among cases compared to that of control group, while the fasting blood glucose level was significantly higher among cases compared to that of control group. The collective data from both study groups showed a significant negative correlation between the mean serum fasting blood glucose level and desnutrin level. There was no significant correlation between the severity of acne and serum desnutrin level


Conclusion: the level of serum desnutrin can affect the occurrence and the progression but not the severity of acne among susceptible individuals. The level of fasting blood glucose is also of value regarding the occurrence of acne and has a negative effect on the level of desnutrin

Objective: To isolate and characterize the bioactive secondary metabolites from aerial parts of widespread Chenopodiaceae taxa growing in Saudi Arabia: Salsola villosa Delile. ex Schul. Methods: Antibacterial activities of chloroformic extract, fractions and isolate compounds was evaluated against five bacterial strains (Escherichia coli, Pseudomonas aeruginosa, Salmonella typhimurium, Staphylococcus aureus and Staphylococcus epidermidis), using a paper disc diffusion method. The purification of compound(s) of chloroform extract was done by chromatographic column of silica gel. The structure elucidation was determined by extensive spectroscopic analysis (1H and 13C nuclear magnetic resonance, correlation spectroscopy, heteronuclear multiple bond correlation, heteronuclear multiple quantum coherence and nuclear overhauser enhancement spectroscopy) and high resolution electrospray ionization mass spectroscopy analysis. Results: Bioactivity guided fractionation of the chloroformic extract led to the isolation of two bioactive compounds: 4-(4'-hydroxy-2'-methylcyclopent-2'-enyloxy)- 4-methylcyclopent-2-enol (1) named salsolanol and 4'-[3-(hydroxymethyl)oxiran-2-yl]-3- [(E)-3-hydroxyprop-1-en-1-yl]-6, 2'-dimethoxy [1, 1'-biphenyl]-2-ol (2) named biphenylsalsinol. The antibacterial effects of the chloroform extracts, fractions and isolated compounds 1 and 2 were also evaluated in this work. Results showed that the compounds 1 and 2 exhibited antibacterial activities against four strains: Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli and Pseudomonas aeruginosa with diameter of zone of inhibition ranging between (9.33 ± 0.94) to (26.33 ± 0.94) mm. Conclusions: Based on data presented here, two new natural compounds secondary cyclic alcohol 1 and biphenylpropanoid 2 isolated from bioactive chloroformic extract from aerial parts of Salsola villosa can be responsible for its antibacterial activities.

The complications of the hydatid cyst of the liver are dominated by infection and rupture. The compression of adjacent organs [mainly the inferior vena cava, the portal vein and the bile ducts] can be seen, when the cyst is located in the dome, in the hilum or within the hepatic parenchyma. Upper digestive stenosis by compression of the duodenum by the hydatid cyst is an exceptional complication. A 63 year-old patient had, for two months, upper digestive stenosis associated with a sensation of weight in the right hypochondrium. Digestive endoscopy showed an extrinsic compression of the second portion of the duodenum. Biopsies were negative. Abdominal CT showed up a hydatid cyst in the segment VI of the liver, adhering to the duodenum, with an exo-vesiculation compressing it. The patient was operated on: There was a hydatid cyst of the right lateral sector compressing the duodenum. A partial intralamellar pericystectomy was performed. Hydatid cyst of the liver, a parasitic disease described as benign, may give mechanical complications related to compression of adjacent organs [especially the bile ducts and veins]. Compression of the digestive tract is exceptional. This is due to the proximity of the cyst to the duodenum and the thickness of the cyst wall

Résumé

Parkinson's disease [PD] is a progressive neurodegenerative disorder. The etiology of this disease is still not fully clear, but free radicals have been proposed to cause neuronal injury. Metals play a key role in the intracellular oxidative balance. However their implication in the degeneration process remains unknown. To assess Cu, Zn and Se concentrations in serum of a group of PD patients in order to determinate, in comparison with age-matched controls, whether alteration in their levels could be involved in PD. A serum level of 3 trace elements [Cu, Zn and Se] was investigated in 48 patients with PD and 36 matched controls using plasma atomic absorption spectrometry. We compared these parameters in PD patients with controls, and we also compared the variations within the PD group according to age, illness duration, stage of the disease and levodopa intake. Patients with PD had significantly lower Cu levels compared to controls. The mean Zn and Se levels in PD patients did not differ significantly from those of controls. Levodopa therapy, age, stage, and illness duration did not significantly influence the measured parameters. These results suggest that a disturbance of the plasmatic rate of Cu could be a marker of PD or at least, a risk factor for the development of this disease. Although zinc participates to the reduction of oxidative stress and the antioxidant role of the selenium, their implication in the onset of PD is not clearly established. Perspectives for the future could include antioxidant therapy. For this reason, other prospective studies should be conducted on this subject to elucidate the implication of trace elements in PD

Résumé

We report a case of a right temporal epidural hermatoma complicating a Sylvian arachnoid cyst in a 25-year-old man after head injury. The diagnosis of this association was .confirmed by MRI. Outcome is favourable with spontaneous resolution of the haematoma. Few cases of association epidural haematoma arachnoid cyst were reported in literature but our case is, to our knowledge, the first one with a conservative treatment and spontaneous favourable outcome

Résumé

Rheumatologic manifestations occurring during inflammatory bowel disease are the most frequent extra intestinal features. They are dominated by spondyloarthropathies, sacro-iliite and peripheral arthritis. To identify in a group of inflammatory bowel disease, the frequency of axial manifestations, to describe the clinical and the radiological features and to identify the risk factors of their occurrence and/or worseness We have established a transversal prospective study including 50 cases of inflammatory bowel disease seen in the department of the internal medicine in the interior forces security hospital during a period of 5 years between January 2001 and December 2006. The prevalence of axial involvement in the case of inflammatory bowel disease was 26%especially spondyloarhropathies which were diagnosed in 11 cases according to the criteria of European Spondyloarthropathy study Group [ESSG], 8 of them fulfilled the modified criteria of new york for the diagnosis of spondyloarhropathy. Isolated sacro-iliitis was found in 4%of cases. By univariate study, only the age under 35 years with a relative risk of 5, 8 and the colic involvement in chron's disease was significativelly associated with the presence of spondyloarthropathy. Systematic checking of spondyloarhtropathies in the course of inflammatory bowel disease through a clinical exam and systematic radiological should be recommended especially if we are in front of a man aged more than 35 years old with a crhon disease involving the colon. This screening offers an early management and avoid ankylosis

Résumé

Since January 2000 till 2004 we performed nephrectomy on 33 patients having large renal tumors [more than 8cm] at Damanhour Oncology Center. We excluded patients who had lung metastasis or adherence to nearby organs. Thirty of the tumors were found to be malignant and only 3 were benign. The tumor was slightly more common in females [17 patients] than in males [16 patients]; also it was affecting the right kidney in 48.5% and the left kidney in 51.5% of the patients. Out of 30 patients having malignant tumors 7 patients died and 23 patients survived till June 2005. The mean follow-up period was 31.95 month +/- 4.97. The microscopic picture of the renal tumors of the surviving patients was clear cell type [23 patients 76.6%]. Meanwhile four of the patients who had died also had clear cell type renal cell carcinoma, and the other three had collecting duct carcinoma. We found that lymph node involvement was significantly positive in the patients who had died. Neither the capsular infiltration nor the tumor size was significantly different between the patients who had survived and the patients who had died. Lymph node involvement was significantly positive in the patients who died [86%]. Neither the capsular infiltration nor the tumor size was significantly different bet%veen the surviving patients and the patients who had died. Among large renal tumors benign tumors could be found [9%]

Résumé

Two cases of a congenital epidermoid cyst located over the anterior fontanel are reported in Tunisian white boys, aged 10 and 12 months. In both cases, cyst had manifested at birth as a small soft mass over the head which gradually enlarged. Radiological examinations revealed no intracranial extension. The cysts were completely excised without complications with good outcome. Review of the literature demonstrates that epidermoid cysts at this location have many common features and would appear to form a district entity

Résumé

Cushing syndrome is a pro-thrombotic state that contributes to an increase in mortality. The antiphospholipid syndrome is characterized by repetitive thrombosis associated with the presence of autoantibodies against phospholipids or membrane proteins. Their association has not yet been described. We report the case of a 47 year old patient in whom the hormonal exploration of an adrenal incidentaloma led to the diagnosis of Cushing syndrome. He returned five years later with cerebral thrombophlebitis, bilateral lower limbs phlebitis and bilateral pulmonary emboli. An antiphospholipid syndrome was confirmed by positive anti-cardiolipin and anti-beta2 glycoproteine-I antibodies. The patient recovered after anti-coagulant treatment and resection of the adrenal adenoma. Thrombo-embolic complications during Cushing syndrome, especially when severe and extensive, must lead to the search for another acquired thrombotic condition, especially the antiphospholipid syndrome
