Résumé

A 13-year-old girl developed rice to nut-sized, bluish to slate-gray colored, depressed patches in a zosteriform distribution along the right side of her upper back and arm without any symptoms. Microscopically, the lesions showed a normal-appearing epidermis, dermis, and subcutaneous fat tissue except for a thinning of collagen fibers and widening of intervals between them in the lower dermis. There have been no changes to the skin lesions during the last 2 years.

Résumé

A 33-year-old woman developed a pea-sized, painless, pedunculated nodule on the right palm. Microscopically, it consists of an intricate mixture of lipocytes and uniform spindle cells that was closely associated with a varying number of short bundles of collagen fibers. The lesion had been excised and showed no evidence of recurrence.

Résumé

Graft-versus-host disease can develop in immunosuppressed individuals who receive blood product transfusions that contain imrnunocompetent lymphocytes. We report a case of transfusion-associated graft-versus-host disease(TA-GVHD) that developed in a patient with acute lymphocytic leukemia who were undergoing therapy. The groups at risk for development of TA-GVHD, the clinical presentation and course, and methods of diagnosis are summarized. Prevention of TA-CVHD is possible by irradiation of blood products given to patients at risk, but problems remain in determining the groups that warrant such measures. We should be aware of the risk of developing TA-GVHD after routine blood transfusion, especially in areas where the population's HLA types are rather homogeneous.

Résumé

A 26-year-old man presented with a 7-month history of hyperkeratotic skin lesions on both elbows and heels. He also had marked subungal hyperkeratosis and splinter hemorrhage on the finger and toe nails. A biopsy specimen from the elbow revealed lichenoid re-actions with areas of epidermal atrophy with alternating acanthosis and foci of parakeratosis. We made a diagnosis of keratosis lichenoides chronica based on the characteristic clinicopathologic findings. We have treated the patient with etretinate, but only the skin lesions on the elbows and heels responded to treatment.

Résumé

Microvascular endothelial cells were purely isolated from human fetal skin using magnetic particles. The principle of this technique is based on the selective binding of the lectin Ulex europaeus I (UEA I) to the endothelial cell surface via fucose residues. Initially UEA I was covalently bound to tosyl-activated magnetic polydisperse polymer particles (Dynabeads) and then the UEA I-coated beads were collected using a magnetic particle concentrator (MPC). Endothelial cells were isolated by extracting microvascular segments from trypsin-treated fetal skin tissue and were purified by sieving with nylon mesh and by 35% Percoll gradient centrifugation. For further purification, the obtained cells were incubated with UEA I-coated Dynabeads. The endothelial cells bound to the Dynabeads were collected using MPC. This is a simple and reproducible technique for isolating a pure population of microvascular endothelium from the fetal skin.

Résumé

The authors made a clinical study on the changes of cardiovascular functions according to duration of inhalation anesthesia as compared with the preanesthetic status in 250 adults aged from 20 to 50 years who were anesthetized for operations by the Department of Anesthesiology, Pusan National University Hospital from September, 1981 to August, 1982. The results obtained were summarized as follows: 1) There were marked increases in pulse rate 5 minutes after the start of anesthesia in all groups and this change persisted until 30 minutes after anesthesia. Of all groups, the ethrane group(group3) showed the most marked increase in pulse rate 5 minutes after the start of anesthesia. 2) Diethyl either group (group5) showed the greatest change of systolic blood pressure and this lasted until 30 minutes after anesthesia. 3) The halothane (group 1,2) showed variable changes in electrocardiographic patterns. Biphasic T waves appeared more in the ethrane group (group 3,4) and the fewest changes in EKG were observed in the diethyl ether group(group5).

Résumé

The author made a clinical study of postoperative respiratory function and analgesic effect of intrathecal morphine HCI which had been administered to 40 of a total 80 adult male and female patients who underwent abdorminal operation with spinal anesthesia from March, 1980 to September, 1981 at Department of Anesthesiology, Pusan National University Hospital. The results obtained were as follows: 1) Vital capacity during 6 to 12 hours after operation was significantly increased in patients given intrathecal morphine as compared with other groups of patients. 2) Patients given intrathecal morphine showed a significant increase in tidal volume during 6 hours after operation. 3) There was no significant change in the level of arterial oxygen tension at pre-and postoperative period in all groups of patients. 4) The average duration of postoperative analgesia was 33.6 and 33.8 hours respectively in patients who were given intrathecal morphine.
