Résumé

Objective:To investigate the characteristics of cardiopulmonary exercise testing, and to explore the clinical value of midregional pro-atrial natriuretic peptide(MR-proANP)in elderly heart failure(HF)patients with preserved ejection fraction(HFpEF).Methods:A total of 56 elderly HFpEF patients with New York Heart Association(NYHA)cardiac function class Ⅱ-Ⅲ at our hospital from January 2019 to June 2019 were successively recruited into the HF group, and 49 healthy individuals who took health examinations during the same period were enrolled as the control group. All subjects underwent cardiopulmonary exercise testing.Plasma was collected at rest before exercise for the measurement of MR-proANP and N-terminal B-type natriuretic peptide(NT-proBNP).Results:NT-proBNP and MR-proANP levels were higher in the HF group than in the control group[375(236-541)ng/L vs.31(22-130)ng/L, 78(52-215)pmol/L vs.14(10-23)pmol/L, P<0.05]. MR-proANP levels were higher in HF patients with NHYA class Ⅲ(n=23)than in HF patients with NHYA class Ⅱ(n=33)[228(121-436)pmol/L vs.60(50-92)pmol/L, χ2=0.256, P<0.01], while NT-proBNP levels showed no significant difference between the two subgroups[375(245-743)ng/L vs.382(220-540)ng/L, χ2=10.949, P>0.05]. Compared with the control group, the anaerobic threshold, peak exercise oxygen consumption(peak VO 2), peak VO 2 as percentage of predicted value(peak VO2%pred)and peak oxygen pulse were lower in the HF group( P<0.05), and VE/VCO 2 slope was higher in the HF group( P<0.05). Pearson analysis showed that MR-proANP and NT-proBNP were negatively correlated with anaerobic threshold, peak VO 2, peak VO 2%pred, and peak oxygen pulse( P<0.05), and positively correlated with the VE/VCO 2 slope( P<0.05). MR-proANP had the highest correlation with peak VO 2( r=0.791). Conclusions:Compared with healthy subjects, exercise tolerance and oxygen uptake capacity are lower in elderly HFpEF patients, indicating that their cardiopulmonary reserve function is reduced, .Plasma levels of MR-proANP can reflect the state of cardiac function in HFpEF patients with NYHA class Ⅱ~Ⅲ and can better evaluate the condition of patients.

Résumé

Objective To investigate the effect of life events and coping styles on attitude toward seeking professional psychological help in college students. Methods Nine hundred college students were survey by attitude toward seeking professional psychological help questionnaire (ATSPPHQ),adolescent self-rating life events check list and coping style questionnaire. Results The total score, the scores of expecting and acceptance factor of ATSPPHQ of female were significantly higher than those of male (P < 0.05);there was no statistic difference in scores of need and trust factor of ATSPPHQ between male and female(P <0.05).There was statistic difference in total score and score of each factor of ATSPPHQ among difference grade (P < 0.05).The total score and each factor score of ATSPPHQ of science & engineering profession students were significantly higher than those of literature and history profession students(P < 0.05).There was no statistic difference in total score and score of each factor of ATSPPHQ among another variables(P > 0.05).Problem solving and help-seeking of coping styles were positive correlation with attitude toward seeking professional psychological help (P < 0.05);all factors of life events and self-blame,fantasy,avoidance,rationalization factor of coping style were negative correlation with attitude toward seeking professional psychological help(P < 0.05).The punishment, problem solving,self-blame and help-seeking were the influence factors of attitude toward seeking professional psychological help.Poblem solving and help-seeking had positive predictive effect (P < 0.05);punishment,self-blame had negative predictive effect (P < 0.05).The immature coping style and mixed coping style had intermediary effect between life events and the attitude toward seeking professional psychological help. Conclusions The attitude toward seeking professional psychological help of college students is different in gender, grade and profession.Life events and coping style have a certain impact on the attitude toward seeking professional psychological help.Coping style play an intermediary role between the life events and the attitude toward seeking professional psychological help.

Résumé

Objective To observe the clinical efficacy of acupoint sticking plus hot compress by moxa-salt bag in treating constipation after acute cerebral stroke.Method Eighty patients with constipation after cerebral stroke were randomly allocated to a treatment group and a control group by using random number table method according to registration order, 40 cases each. The control group was intervened by acupoint sticking at umbilicus with rawDa Huang (Rheum Officinale), and the treatment group was additionally given hot compress using moxa-salt bag. The treatment was given once a day, for consecutive 3 days in total. The first defecation time, stool shape and defecation sensation were observed in the two groups. The clinical efficacies were compared between the two groups.Result The total effective rate was 90.0% in the treatment group versus 72.5% in the control group, and the difference was statistically significant (P<0.05). After the treatment, there was statistically significant difference in comparing the first defecation time between the two groups (P<0.05). After the treatment, the incidence rate of soft stool was 85.0% and that of hard stool was 15.0% in the treatment group versus 40.0% and 60.0% in the control group, and the differences were statistically significant (P<0.05). The incidence rate of pleasant defecation sensation was 80.0% and that of difficult defecation was 20.0% in the treatment group versus 35.0% and 65.0% in the control group after the treatment, and the differences were statistically significant (P<0.05).Conclusion Acupoint sticking plus hot compress by moxa-salt bag is an effective method in treating constipation after acute cerebral stroke.
