Abstract This split-mouth randomized controlled trial aimed to evaluate the primary and secondary stability of hybrid implants with different thread configurations and hydrophilic surfaces. Twenty patients with a partially edentulous maxilla were selected. These patients received two types of implants with the same hydrophilic surface: CTP group: Cylindrical-Tapered implant with perforating threads; CTH: Cylindrical-Tapered implant with hybrid threads configuration (perforating and condensing threads). The primary and secondary stability parameters were measured by insertion torque and resonance frequency analysis at the time of implant placement and 7, 28, 56, and 90 days after the surgical procedure. The paired t-test was used to compare the data on the implant's stability between the groups. The statistical analysis was performed with a confidence level set at 95%. It was found that the implants in the CTH group presented higher primary stability values ​​at the time of implant placement, due to the higher ISQ (63.61 ± 9.44 vs. 40.59 ±7.46) and insertion torque (36.92 ± 16.50 Ncm vs. 28.00 ± 14.40 Ncm), than the implants in the CTP group. The CTH group presented higher ISQ values ​​in all follow-up periods: 7 days (68.67 ± 7.60 vs. 41.55 ± 9.07), 28 days (68.61 ± 5.98 vs. 47.90 ±13.10), 56 days (74.09 ± 3.96 vs. 55.85 ± 13.18), and 90 days (75.45 ± 4.02 vs. 63.47 ± 6.92) after implant placement. Hybrid implants with perforating and condensing threads demonstrated greater stability than hybrid implants with only perforating threads.


Resumo O objetivo deste estudo controlado randomizado de boca dividida foi avaliar a estabilidade primária e secundária de implantes híbridos com diferentes configurações de rosca e superfícies hidrofílicas. Vinte pacientes com maxila parcialmente edêntula foram selecionados. Esses pacientes receberam dois tipos de implantes com a mesma superfície hidrofílica: Grupo CTP: Implante cilíndrico-cônico com roscas perfurantes; Grupo CTH: Implante cilíndrico-cônico com roscas perfurantes e condensadoras. Os parâmetros de estabilidade primária e secundária foram medidos por análise de torque de inserção e frequência de ressonância no momento da colocação do implante e 7, 28, 56 e 90 dias após o procedimento cirúrgico. O teste t-pareado foi utilizado para comparação dos dados de estabilidade entre os grupos. A análise estatística foi executada levando-se em consideração um nível de confiança de 95%. Constatou-se que os implantes do grupo CTH apresentaram maiores valores de estabilidade primária no momento da colocação do implante, devido ao maior ISQ (63.61 ± 9.44 vs. 40.59 ±7.46) e torque de inserção (36.92 ± 16.50 Ncm vs. 28.00 ± 14.40 Ncm), do que os implantes do grupo CTP. Implantes do grupo CTH apresentaram valores de ISQ superiores em todos os períodos de acompanhamento: 7 dias (68.67 ± 7.60 vs. 41.55 ± 9.07), 28 dias (68.61 ± 5.98 vs. 47.90 ±13.10), 56 dias (74.09 ± 3.96 vs. 55.85 ± 13.18) e 90 dias (75.45 ± 4.02 vs. 63.47 ± 6.92). Os implantes híbridos com roscas perfurantes e condensantes demonstraram maior estabilidade do que os implantes híbridos com apenas roscas perfurantes.

Aim: This study was performed to compare two different rat defect models (critical calvaria defects versus guided bone regeneration in the mandibular ramus) used to evaluate bone repair in grafted areas. Methods: A total of 12 rats were allocated in two groups according the experimental model used to evaluate the bone repair in grafted areas: a critical sized-calvaria defect of 5 mm filled with bone graft (n=6) and a mandibular ramus filled with the bone graft associated with a Teflon dome-shaped membrane (n=6). Both groups were grafted with deproteinized bovine bone graft. After 60 days, the animals were euthanized and the samples obtained were submitted to histomorphometry analysis to evaluate the relative amount of bone, remaining bone substitute, and soft tissue within the grafted areas. Results: No differences were observed between the preclinical models evaluated in relation to the amount of bone tissue formation (19.93 ± 4.55% in calvaria vs. 21.00 ± 8.20% in mandible). However, there was a smaller amount of soft tissue (43.20 ± 10.97% vs. 57.79 ± 7.61 %; p<0.01) and a greater amount of bone substitute remaining (35.80 ± 5.52% vs. 22.28 ± 4.36 %; p<0.05) in the grafted areas in the mandible compared to calvaria defect. Conclusion: Preclinical models for the analysis of bone repair in grafted areas in the mandible and critical sized-calvaria defects showed different responses in relation to the amount of soft tissue and bone substitute remnants

Abstract Introduction The maintenance of oral health in inpatients is essential, particularly to avoid complications arising from periodontal disease, which not only affect oral health but can also exacerbate systemic issues during hospitalization. Objective: This study aimed to assess nursing staff's knowledge regarding the management of periodontal disease and its association with systemic diseases. Material and method This cross-sectional observational study was conducted in a general referral hospital of the Brazilian National Health System (SUS). A total of 504 nursing professionals completed a self-administered 24-item questionnaire on relating to oral care procedures and the importance of oral health in hospitalized patients. Data were analyzed using descriptive analysis and the chi-squared test (α = 0.05). Result Most respondents (97.8%) believed that oral infections can affect a patient's general health and that oral hygiene is important during hospitalization. However, knowledge deficits were identified. The oral effect of periodontal disease was considered moderate by 49.3% of the participants, and 74.6% believed that sugar was the most common cause of periodontal disease. Conclusion Nursing professionals are aware of the importance of maintaining oral hygiene and the relationship between periodontal and systemic diseases. However, their dental hygiene practice knowledge is limited. A training program on oral healthcare for inpatients and the inclusion of oral hygiene topics in the curricula of nursing professionals are recommended.


Resumo Introdução A manutenção da saúde bucal em pacientes internados é essencial, especialmente para evitar complicações decorrentes da doença periodontal, que não apenas afetam a saúde bucal, mas também podem agravar problemas sistêmicos durante a hospitalização. Objetivo: Este estudo teve como objetivo avaliar o conhecimento da equipe de enfermagem em relação ao manejo da doença periodontal e sua associação com doenças sistêmicas. Material e método Este estudo observacional transversal foi conduzido em um hospital geral de referência do Sistema Único de Saúde (SUS) do Brasil. Um total de 504 profissionais de enfermagem completaram um questionário autoaplicável com 24 questões, abordando procedimentos de cuidados bucais e a importância da saúde bucal em pacientes hospitalizados. Os dados foram analisados por meio de análise descritiva e teste qui-quadrado (α = 0,05). Resultado A maioria dos entrevistados (97,8%) acreditava que infecções bucais podem afetar a saúde geral do paciente e que a higiene bucal é importante durante a hospitalização. No entanto, foram identificados déficits de conhecimento. O efeito oral da doença periodontal foi considerado moderado por 49,3% dos participantes, e 74,6% acreditavam que o açúcar era a causa mais comum da doença periodontal. Conclusão Profissionais de enfermagem reconhecem a importância da manutenção da higiene bucal e a relação entre doença periodontal e doenças sistêmicas. No entanto, seu conhecimento prático em higiene bucal é limitado. Recomenda-se um programa de treinamento em cuidados bucais para pacientes internados e a inclusão de tópicos de higiene bucal nos currículos dos profissionais de enfermagem.

Abstract This study evaluated the influence of a fluoride-modified titanium surface on osseointegration in rats with induced diabetes. One hundred and eighty rats were randomly allocated into 3 groups with 60 animals each: Control group (C): Animals without diabetes; Diabetes Group (D): Animals with uncontrolled induced diabetes; Controlled Diabetes Group (CD): Animals with diabetes induced controlled by the insulin administration. Diabetes was induced by streptozotocin injection. Each animal received 2 implants in the proximal tibial metaphysis, one with the machined surface (M) and the other one with a fluoride-modified titanium surface (F), after 4 weeks of induction of diabetes. The animals were submitted to euthanasia 2, 4, and 6 weeks after the implant placement (n = 20 animals/group). The osseointegration was evaluated by the implant removal torque test and the histometric analysis of the non-decalcified histological sections: 1) Contact bone/implant (%BIC); 2) Bone tissue area between implant threads (%BBT). Implants with F surface showed a higher removal torque than implants with surface M in all groups. There was no difference in %BIC between the groups regardless of the surface used. The F surface showed a tendency to present higher %BBT values for the 3 evaluation periods in the D group. The fluoride-modified implant surface has no impact on the %BIC and %BBT. However, the fluoride-modified implant surface increases the locking of the implants with the bone. The hyperglycemia was associated with lower removal torque values despite the surfaces of the implant used.


Resumo Este estudo avaliou a influência de uma superfície de titânio modificada com flúor na osseointegração em ratos com diabetes induzida. Cento e oitenta ratos foram distribuídos aleatoriamente em 3 grupos com 60 animais cada: Grupo controle (C): Animais sem diabetes; Grupo Diabetes (D): Animais com diabetes induzida descompensada; Grupo Diabetes Controlado (CD): Animais com diabetes induzido controlado pela administração de insulina. O diabetes foi induzido por injeção de estreptozotocina. Cada animal recebeu 2 implantes na metáfise proximal da tíbia, um com superfície usinada (M) e outro com superfície de titânio modificado com flúor (F), após 4 semanas de indução do diabetes. Os animais foram submetidos à eutanásia 2, 4 e 6 semanas após a colocação do implante (n = 20 animais/grupo). A osseointegração foi avaliada pelo teste de torque de remoção do implante e pela análise histométrica dos cortes histológicos não descalcificados: 1) Contato osso-implante (%BIC); 2) Área de tecido ósseo entre as roscas do implante (%BBT). Os implantes com superfície F apresentaram maior torque de remoção do que os implantes com superfície M em todos os grupos. Não houve diferença no %BIC entre os grupos independente da superfície utilizada. A superfície F mostrou tendência a apresentar maiores valores de %BBT para os 3 períodos de avaliação no grupo D. As superfícies de implantes modificadas com flúor não influenciaram nos dados de %BIC e %BBT. Entretanto, essas superfícies aumentaram o travamento dos implantes no tecido ósseo. A hiperglicemia foi associada a menores torques de remoção dos implantes independentemente do tipo de superfície de implante utilizada.

Abstract The aim of this study was to evaluate the osseointegration of a hydrophilic surface (blasting + acid etching + immersion in isotonic solution) in comparison with that of a control surface (blasting + acid etching) using an experimental model of low-density bone. To perform the study, 24 rabbits were submitted to the installation of 4 implants in the iliac bone bilaterally: 2 implants with a control surface and 2 implants with a hydrophilic surface. The rabbits were euthanized at 2, 4, and 8 weeks after implant installation. After euthanasia, one implant from each surface was used to perform the removal torque analysis, and the other implant was used for the execution of non-decalcified histological sections and evaluation of the bone implant contact (% BIC) as well as the fraction of bone tissue area between the implant threads (% BBT). The implants with a hydrophilic surface presented higher %BIC (42.92 ± 2.85% vs. 29.49 ± 10.27%) and % BBT (34.32 ± 8.52% vs. 23.20 ± 6.75%) (p < 0.05) in the 2-week period. Furthermore, the hydrophilic surface presented higher removal torque in the 8-week period (76.13 ± 16.00 Ncm2 vs. 52.77 ± 13.49 Ncm2) (p<0.05). Implants with a hydrophilic surface exhibited acceleration in the process of osseointegration, culminating in greater secondary stability in low-density bone than in implants with a control surface.


Resumo O objetivo deste estudo foi avaliar a osseointegração de uma superfície hidrofílica (jateamento + ataque ácido + imersão em solução isotônica) em comparação com uma superfície controle (jateamento + ataque ácido) usando um modelo experimental de osso de baixa densidade. Para realizar o estudo, 24 coelhos foram submetidos a instalação de 4 implantes bilateralmente no osso ilíaco: 2 implantes com superfície controle e 2 implantes com superfície hidrofílica. Os coelhos foram eutanasiados com 2, 4 e 8 semanas após a instalação dos implantes. Após a eutanásia, um implante de cada superfície foi usado para avaliar o torque de remoção, e o outro implante foi utilizado para execução de cortes histológicos não descalcificados e avaliação de contato osso implante (% BIC) bem como a fração da área tecido ósseo entre as roscas do implante (% BBT). Os implantes com superfície hidrofílica apresentaram maior %BIC (42.92 ± 2.85% vs. 29.49 ± 10.27%) e % BBT (34.32 ± 8.52% vs. 23.20 ± 6.75%) (p < 0.05) no período de 2 semanas. Além disso, a superfície hidrofílica apresentou maior torque de remoção no período de 8 semana (76.13 ± 16.00 Ncm2 vs. 52.77 ± 13.49 Ncm2) (p<0.05). Implantes com a superfície hidrofílica apresentaram aceleração no processo de osseointregração, culminando em melhor estabilidade secundária no osso de baixa densidade em relação a implantes com superfície controle.

Abstract This study aimed to evaluate the effect of nicotine administration on the osseointegration of a superhydrophilic implants surface on rat tibiae. Thirty-two rats were used and divided into 2 groups according to the administration or not of nicotine: HH - Installation of implants with superhydrophilic surfaces in healthy animals; and HN - Installation of implants with superhydrophilic surfaces in animals subjected to nicotine administration. The animals were euthanized 15 and 45 days after implant placement (n = 8). Osseointegration was assessed by means of biomechanical analyses (removal torque), microcomputed tomography (volume of bone around the implants- %BV/TV), and histomorphometry (bone-implant contact -%BIC and the bone area between implant threads -%BBT). The animals subject to the nicotine administration presented lower removal torque than the control animals at the 45-day period (21.88 ± 2.80 Ncm vs. 17.88 ± 2.10 Ncm). The implants placed in the control rats presented higher %BIC (54.26 ± 6.59% vs. 39.25 ± 4.46%) and %BBT (50.57 ± 5.28% vs. 32.25 ± 5.24%) than the implants placed in nicotine animals at 15-day period. The nicotine administration reduces the osseointegration at 15 days, however, the superhydrophilic surface equalized the osseointegration in nicotine-exposed animals compared with healthy animals after 45 days of implant placement.


Resumo O objetivo deste estudo foi avaliar o efeito da administração de nicotina na osseointegração de implantes com superfície modificada por jateamento de oxidos e ataque ácido instalados em tíbia de ratos. Foram utilizados 32 ratos que foram divididos em 2 grupos de acordo com a administração ou não de nicotina: HH - Instalação de implantes com superfícies superhidrofílicas em animais sadios; e HN - Instalação de implantes com superfícies superhidrofílicas em animais submetidos à administração de nicotina. Os animais foram eutanasiados 15 e 45 dias após a colocação do implante (n = 8). A osseointegração foi avaliada por meio de análises biomecânicas (torque de remoção), microtomografia (volume de osso ao redor dos implantes - %BV/TV) e histomorfometrica (contato osso-implante -%BIC e área óssea entre roscas do implante -%BBT). Os animais submetidos à administração de nicotina apresentaram menor torque de remoção do que os animais controle no período de 45 dias (21,88 ± 2,80 Ncm vs. 17,88 ± 2,10 Ncm). Os implantes colocados nos ratos controle apresentaram maiores %BIC (54,26 ± 6,59% vs. 39,25 ± 4,46%) e %BBT (50,57 ± 5,28% vs. 32,25 ± 5,24%) do que os implantes colocados nos animais submetidos a administração de nicotina no período de 15 dias. A administração de nicotina prejudicou a osseointegração no período de 15 dias. Entretanto, a superfície superhidrofílica equalizou a osseointegração em animais expostos à nicotina em comparação com animais saudáveis após 45 dias da colocação do implante.

Abstract This study evaluated the effect of photobiomodulation therapy (PBMT) with a red or infrared laser on the repair of post extraction sockets in rats administered alendronate (ALN). Forty male rats were randomly allocated into four groups: Control Group (CTR): subcutaneous administration of saline solution throughout the experimental period; Alendronate Group (ALN): subcutaneous administration of alendronate during the entire experimental period; Alendronate/Red Laser Group (ALN/RL): administration of ALN and irradiation with a GaAlAs laser (λ 660 nm); and Alendronate/Infrared Laser Group (ALN/IRL): administration of ALN and irradiation with a GaAlAs laser (λ 830 nm). The first lower molars were extracted 60 days after the beginning of the administration of the drugs. The PBMT was applied after tooth extraction (7 sessions with intervals of 48 hours between sessions). Thirty days after tooth extraction, the animals were euthanized. Micro-CT and histometric analysis were performed to assess the bone healing and soft tissue repair of the tooth socket. The ALN group presented with more bone than the CTR; however, most of this bone was necrotic. ALN does not affect the bone microarchitecture. On the other hand, PBMT with IRL enhances the bone density due to the increase in the number and reduction in the spacing of the trabeculae. The amount of vital bone and connective tissue matrix was higher in the ALN/RL and ALN/IRL groups than in the ALN and CTR groups. PBMT enhanced the healing of the post extraction sockets in rats subjected to ALN administration. Furthermore, IRL improved the new bone microarchitecture.

Abstract The purpose of this study was to evaluate the effect of a surface modified by blasting and acid attack and maintained in an isotonic solution compared to a machined surface on osseointegration in normo- and hyperglycaemic animals. Sixty-four animals were allocated into 4 groups with 16 animals each, and they were subdivided into two experimental periods (15 and 45 days), with 8 animals in each group. The groups were divided according to the type of implant that was installed in the animals' tibia and the animals' systemic condition: CM - Machined implants placed in Healthy animals; CH - Hydrophilic implants placed in Healthy animals, HM - Machined implants placed in animals with hyperglycaemia; HH- Hydrophilic implants installed in animals with hyperglycaemia. The following analyses were performed: biomechanical (removal torque), microtomographic (evaluation of the bone volume around the implants- BV/TV), and histomorphometric (evaluation of bone-implant contact BIC% and of the bone formation area between the threads BBT%). It was found that the implants with hydrophilic surfaces presented higher removal torques and quantities of BV/TV% and higher BIC% and BBT% values in normo- and hyperglycaemic animals. The results of this study indicated that the hydrophilic surface accelerates the osseointegration process (~ 15% BIC/BBT at 15-day period), especially in animals with hyperglycaemia. The hydrophilic surface equaled the osseointegration between normo- and hyperglycaemic animals, reversing the negative potential of hyperglycaemia on the osseointegration process.


Resumo O objetivo deste foi o de avaliar o efeito de uma superfície modificada por jateamento e ataque ácido e mantida em solução isotônica em comparação a uma superfície lisa sobre a osseointegração de implantes em animais hiperglicêmicos. No estudo foram utilizados 64 ratos, que foram divididos em 4 grupos com 16 animais em cada, e que foram avaliados em dois períodos experimentais (15 e 45 dias), com 8 animais em cada grupo. Os grupos foram divididos de acordo com o tipo de implante que foi instalado na tíbia dos animais e a condição sistêmica dos mesmos: Grupo CM - Implantes maquinados instalados em animais saudáveis; Grupo CH - Implantes maquinados instalados em animais com hiperglicemia; Grupo HM - Implantes hidrofílicos instalados em animais saudáveis, Grupo HH- Implantes hidrofílicos instalados em animais com hiperglicemia. Foram realizadas análises: biomecânicas (contra torque de remoção dos implantes), histométricas (avaliação do contato do osso com os implantes e da área de formação óssea entre as roscas) e microtomográfica (avaliação da área óssea ao redor dos implantes). Foi verificado que os implantes com superfície hidrofílica apresentaram maiores torques de remoção, quantidade de osso ao redor dos implantes e maiores valores de %BIC e %BBT em animais normo e hiperglicêmicos. Conclui-se que a superfície hidrofílica acelera o processo de osseointegração (~15% de %BIC/%BBT aos 15 dias), especialmente em animais com hiperglicêmicos. A superfície hidrofílica igualou a osseointegração entre animais normo- e hiperglicêmicos, revertendo o potencial negativo da hiperglicemia no processo de osseointegração.

Résumé

The use of selective barriers as resorbable membranes has become a routine clinical procedure for guided bone regeneration. Therefore, the production of membranes with a low inflammatory potential during their resorption process has become the goal of a considerable number of researches. Aim: The purpose of the present study was to evaluate the biocompatibility of poly (L- lactic acid) (PLLA) and biocelulose membranes (BC) inserted in the subcutaneous tissue on the dorsum of rats. Methods: Fifteen animals underwent surgical procedures for the insertion of 4 types of membranes: COL (Collagen membrane) ­ Control Group; BC (Biocellulose membrane); BCAg (Biocellulose membrane impregnated with Silver); PLLA (Poly (L-lactic acid) membrane). All membrane types were inserted into each animal. Animals were euthanized after 3, 7, and 15 days of the surgical procedure. Descriptive histological analyses were carried out to investigate host tissue reaction to membrane presence by assessing the anti-inflammatory process composition associated with the membrane resorption and the presence of foreign-body reaction or encapsulation. Results: The BC membranes showed a higher degree of inflammation and poor pattern of integration with the surrounding tissues than the PLLA and COL membranes. Conclusion: The PLLA and COL membranes present better biocompatibility than the BC membranes

Abstract The study aimed to evaluate the accuracy of Micro-CT in linear and volumetric measurements in native (NB) and grafted bone (GB) areas. A total of 111 biopsies of maxillary sinuses grafted with deproteinized bovine bone (DBB) in humans were evaluated. The linear measurements were performed to measure the length of the NB and GB. Furthermore, the amount of mineralized tissues at the NB and GB was performed. In the histomorphometry analysis the percentage of mineralized tissues at the NB and GB was obtained in two histological sections while the mineralized tissues were measure in the micro-CT varying the thresholds of the grayscale varying from 90-250 to 90-150 with 10 levels of variation between each one was applied. Then these data were correlated in order to check the higher r level between the histomorphometry and micro-CT thresholds intervals. The linear length of the NB was 2.44±0.91mm and 2.48±1.50mm, respectively, for micro-CT and histomorphometry (r =0.57), while the linear length of the GB was 3.63±1.66mm and 3.13±1.45mm, respectively, for micro-CT and histomorphometry (r =0.74) Histomorphometry showed 45.91±11.69% of bone in NB, and 49.57±5.59% of bone and biomaterial in the GB. The total volume of mineralized tissues that were closest to the histometric analysis were 43.75±15.39% in the NB (Threshold:90-240; r = 0.50) and 51.68±8.42% in the GB (Threshold:90-180; r =-0.028). The micro-CT analysis showed good accuracy in the linear analysis in both portions of the biopsies but for volumetric analysis just in NB.


Resumo Esse estudo tem como objetivo avaliar a acurácia da análise microtomográfica em mensurações lineares e volumétricas em osso nativo (ON) e enxertado (OE). Para isso, 111 biópsias removidas de seios maxilares de pacientes enxertados com osso bovino desproteinizado foram coletadas e avaliadas. As medidas lineares foram realizadas para medir o comprimento do ON e do OE. Além disso, foi realizada a mensuração da quantidade de tecidos mineralizados em ON e OE. Na análise histomorfométrica a porcentagem de tecidos mineralizados do ON e OE foi obtida em dois cortes histológicos enquanto os tecidos mineralizados foram medidos em microtomografias variando os thresholds da escala de cinza variando de 90-250 a 90-150 com 10 níveis de variação entre cada. Em seguida, esses dados foram correlacionados para verificar o maior nível de R entre os intervalos dos thresholds testados na análise microtomográfica em relação aos dados obtidos na histomorfometria. O comprimento linear do ON foi de 2,44±0,91mm e 2,48±1,50mm, respectivamente, para análises microtomográfica e histomorfométrica (r=0,57), enquanto o comprimento linear do OE foi de 3,63±1,66mm e 3,13±1,45mm, respectivamente, para para análises microtomográfica e histomorfométrica (r =0,74) A histomorfometria detectou 45,91±11,69% de osso na porção de ON e 49,57±5,59% de osso e biomaterial na porção de OE. O volume total de tecidos mineralizados detectados pela análise microtomográfica que apresentou valores mais próximos da análise histomorfométrica foi de 43,75±15,39% no ON (Thresholds:90-240; r = 0,50) e 51,68±8,42% no OE (Thresholds:90-180; r =- 0,028). A análise microtomográfica apresentou boa acurácia na análise linear em ambas as porções das biópsias, porém a mesma apresentou boa acurácia para análise volumétrica apenas em áreas de ON.

Abstract This study evaluated osseointegration in areas grafted with deproteinized bovine bone (DBB) and biphasic ceramic based on hydroxyapatite and beta-tricalcium phosphate (HA/TCP) in rat tibias. Noncritical bone defects were made in the tibias of 28 rats that were randomly assigned to 2 groups: DBB: DBB-filled defects and HA/TCP: HA/TCP-filled defects. Bone defects were made in the tibias bilaterally and filled with biomaterials. After 60 days, the implants were inserted, and the animals were euthanized 15 and 45 days after the implants were installed. Osseointegration was evaluated by biomechanical, microtomographic and histometric analysis. Implants installed in the defects filled with DBB presented higher removal torque forces (2.33 ± 0.51 Ncm vs. 1.50 ± 0.54 Ncm) and mineralized tissue volume around implants at 15 days (34.96 ± 3.68 % vs. 25.61 ± 2.95 %) and greater bone-implant contact (20.87 ± 8.28 % vs. 11.52 ± 7.42 %) and bone area within implant threads (26.83 ± 12.35 % vs. 11.98 ± 7.56 %) at 45 days compared to the measurements of implants in areas grafted with HA/TCP. Implants installed in defects in areas grafted with DBB had a better osseointegration pattern than implants placed in defects in areas grafted with HA/TCP.


Resumo Este estudo avaliou a osseointegração em áreas enxertadas com osso bovino desproteinizado (DBB) e cerâmica bifásica à base de hidroxiapatita e beta-fosfato tricálcico (HA / TCP) em tíbias de ratos. Defeitos ósseos não críticos foram feitos nas tíbias de 28 ratos que foram divididos aleatoriamente em 2 grupos: DBB: defeitos preenchidos com DBB e HA / TCP: defeitos preenchidos com HA / TCP. Defeitos ósseos foram confeccionados nas tíbias bilateralmente e preenchidos com biomateriais. Após 60 dias, os implantes foram instalados e os animais sacrificados 15 e 45 dias após a instalação dos implantes. A osseointegração foi avaliada por análises biomecânica, microtomográfica e histométrica. Os implantes instalados nos defeitos preenchidos com DBB apresentaram maiores forças de torque de remoção (2,33 ± 0,51 Ncm vs. 1,50 ± 0,54 Ncm) e volume de tecido mineralizado ao redor dos implantes aos 15 dias (34,96 ± 3,68% vs. 25,61 ± 2,95%) e maior contato osso-implante (20,87 ± 8,28% vs. 11,52 ± 7,42%) e área de osso dentro das roscas do implante (26,83 ± 12,35% vs. 11,98 ± 7,56%) no período de 45 dias em comparação com os implantes em áreas enxertadas com HA / TCP. Implantes instalados em áreas enxertadas com DBB apresentaram melhor padrão de osseointegração do que implantes colocados em áreas enxertadas com HA / TCP.

Introdução: o desafio no uso do Micro-Ct tem sido estabelecer e padronizar padrões adequados para escaneamento e tratamento das imagens, para que se obtenha o máximo desempenho do equipamento, e permitir a comparação dos achados entre diferentes estudos. Objetivo: o presente estudo tem como objetivo comparar a porcentagem de volume ósseo em regiões com perda óssea periodontal utilizando diferentes metodologias para definição da área a ser analisada (ROI). Material e método: dez ratos foram submetidos à indução de doença periodontal, e, após a eutanásia, as mandíbulas foram escaneadas com cortes de 9 µ e 18 µm de espessura, com passo de rotação de 0.3mm. As imagens foram reconstituídas utilizando o software NRecon, e em seguida, utilizando o software CTAnalyser - CTAn, foram definidas as áreas de interesse (ROI) ao redor dos segundos molares. A primeira área de interesse (ROI1) foi definida em um padrão retangular que se restringiu às regiões interproximais e furca, onde a área total de tecido ósseo foi obtida somando os resultados dos 3 ROIs. A segunda área (ROI2) foi definida no sentido corono/apical ao redor do segundo molar até suas limitações proximais com os dentes vizinhos, onde a porcentagem de tecido ósseo pertencente às raízes foram excluídas das avaliações. As análises foram realizadas com cinco diferentes valores de thresholds (130-50, 130-60, 130-70, 130-80, 130-90 e 130-100). Resultado: a análise entre os diferentes ROIs demonstrou que em ambas as análises foi observada a tendência a menor porcentagem de tecido ósseo quanto maior o contraste de tons de cinza utilizado, no entanto, nos resultados obtidos no ROI2 essa diferença não foi estatisticamente significante. Conclusão: com os resultados obtidos pode se concluir que a utilização de diferentes thresholds para quantificação óssea, em áreas onde houve doença periodontal, pode trazer resultados divergentes; a definição da área de interesse interfere com os resultados obtidos e que a obtenção de uma área de interesse com a remoção das raízes mostrou-se menos susceptível à variação dos parâmetros de escaneamento.


Introduction: the challenge in the use of Micro-Ct has been to establish and standardize adequate standards for scanning and image processing to obtain the maximum performance of the equipment and to allow the comparison of findings between different studies. Objective: this study aims to compare the percentage of bone volume in regions with periodontal bone loss using different methodologies to define the area to be analysed (ROI). Material and method: ten rats were submitted to periodontal disease induction, and, after euthanasia, the mandibles were scanned with slices of 9µ and 18 µm thickness, with a rotation step of 0.3mm. The images were reconstructed using the NRecon software, and then, using the CTAnalyser - CTAn software, the areas of interest (ROI) around the second molars were defined. The first area of interest (ROI1) was defined in a rectangular pattern that was restricted to the interproximal and furcation regions, where the total area of bone tissue was obtained by adding the results of the 3 ROIs. The second area (ROI2) was defined in the coronal/apical direction around the second molar to its proximal limitations with the neighboring teeth, in which the percentage of bone tissue belonging to the roots was excluded from the evaluations. Analyses were performed with five different threshold values (130-50, 130-60, 130-70, 130-80, 130-90, and 130-100). Result: the analysis between the different ROIs showed that, in both analyses, there was a tendency towards a lower percentage of bone tissue the greater the grayscale contrast used. However, in the results obtained in ROI2, this difference was not statistically significant. Conclusion: with the results obtained, it can be concluded that the use of different thresholds for bone quantification, in areas where there was a periodontal disease, can bring divergent results; the definition of the area of ​​interest interferes with the results obtained; and that obtaining an area of interest with the removal of the roots, proved to be less susceptible to the variation of the scanning parameters.

Introdução: A estabilidade primária é um importante indicador de sucesso da osseointegração. Porém, fatores locais com osso de baixa intensidade podem interferir negativamente na obtenção da estabilidade primária. Objetivo: O presente estudo avaliou o efeito de diferentes direções, velocidades de rotação e sistemas de fresagem na expansão de perfurações e estabilidade de implantes instalados em blocos mimetizando osso do tipo IV. Material e método: Foram instalados 50 implantes em blocos de poliuretano sólido rígido. Esses implantes foram igualmente divididos em cinco grupos (n = 10): 1) Fresa Maximus (utilizadas no sentido horário a 1200rpm); 2) Fresa Maximus (utilizadas no sentido horário a 600rpm); 3) Fresa Neodent (utilizadas no sentido horário a 800rpm); 4) Fresa Neodent (utilizadas no sentido anti-horário a 800rpm); 5) Fresa Neodent (utilizadas no sentido anti-horário a 600rpm). Foram executadas análises de estabilidade dos implantes através de testes de torque de inserção e remoção, além das análises de frequência de ressonância. Adicionalmente, a expansão associada às perfurações promovida pelas brocas foi avaliada por meio de análises tomográficas. Resultado: Verificou-se que os implantes instalados após o preparo da perfuração com as brocas Maximus a 600rpm apresentaram valores de torque de inserção maiores, quando comparados ao grupo de implantes instalados em perfurações confeccionadas com brocas Neodent. Ademais, as brocas Maximus apresentaram valores de expansão maiores que as brocas Neodent. Conclusão: As brocas Maximus são mais eficientes em promover a osseodensificação, e sua utilização está associada ao aumento da estabilidade dos implantes instalados em blocos mimetizando osso do tipo IV.


Introduction: Primary stability is an important indicator to obtain a successful osseointegration. However, local factors like bone with low density can negatively interfere in obtaining primary stability. Objective: This study assessed the effect of different drilling systems, speeds, and movement directions on the expansion of perforations and the stability of implants placed in blocks that mimicked type IV bone. Material and method: Fifty implants were installed in rigid solid polyurethane blocks and equally divided into the following five groups (n = 10): 1) Maximus Driller (on a clockwise direction at 1200rpm); 2) Maximus Driller (on a clockwise direction at 600rpm); 3) Neodent Driller (on a clockwise direction at 800rpm); 4) Neodent Driller (on a counter clockwise direction at 800rpm); 5) Neodent Driller (on a counter clockwise direction at 600rpm). The stability analyses of the implants were performed through insertion and removal torque testing, in addition to resonance frequency analysis. Additionally, the expansion promoted by the drills associated with the perforations was assessed through tomographic analysis. Result: We found that implants placed after drilling preparation with Maximus drills at 600 rpm had higher values of insertion torque than the group of implants installed in drillings made with Neodent drills. In addition, the maximus drills showed higher expansion values than the Neodent drills. Conclusion Maximus drills are more efficient in promoting osseodensification and their use was associated with increased stability of implants installed in blocks that mimicked type IV bone.

Aim: To evaluate the effect of manual (M), electric (E) and ultrasonic (US) toothbrushes on the removal of oral biofilm and control of gingivitis. Also, the roughness and tooth wear production were evaluated in vitro. Methods: For the in vitro analyses, thirty bovine dentin specimens were submitted to a 3-month brushing simulation (9 minutes) with the three types of toothbrushes (n = 10). Subsequently, a randomized controlled clinical trial was performed with 36 patients divided into 3 groups according to the toothbrushes used (n = 12). Gingival index, visible plaque index and the volume of crevicular fluid were evaluated at baseline and 3 months after the beginning of the toothbrush use. Furthermore, the performance of the biofilm removal per brushing cycle of 1 and 3 minutes with each toothbrush was made monthly until the end of the experiment. Results: The US group had the highest dentin wear. Clinically, the US group had a lower plaque index at 3 months than the M group. The M group also showed less biofilm removal efficiency from the second month of follow-up and more worn bristles at the end of the 3 month period than the E and US groups. Conclusion: The ultrasonic, electric and manual toothbrushes showed no differences in gingivitis control in the present study. The ultrasonic and electric toothbrushes had a more significant effect on biofilm removal than a manual toothbrush, but the ultrasonic toothbrush promoted greater dentin tissue wear

Abstract Objective To evaluate the effect of different protocols of low-level intensity laser therapy (LLLT) irradiation on the osseointegration of implants placed in grafted areas. Methodology 84 rats were randomly allocated into six groups: DBB: defect filled with deproteinized bovine bone; HA/TCP: defect filled with biphasic ceramic of hydroxyapatite/β-tricalcium phosphate ; DBB-LI: defect filled with DBB and treated with LLLT after implant placement; HA/TCP-LI: defect filled with HA/TCP and treated with LLLT after implant placement; DBB-LIB: defect filled with DBB and treated with LLLT after graft procedure and implant placement; and HA/TCP-LIB: defect filled HA/TCP and treated with LLLT after graft procedure and implant placement. The bone defects were made in the tibia and they were grafted. After 60 days, the implants were placed. The rats were subsequently subjected to euthanasia 15 and 45 days after implant placement. The pattern of osseointegration and bone repair in the grafted area was evaluated by biomechanical, microtomographic, and histometric analyses. Furthermore, the expression of bone biomarker proteins was assessed. Results The LLLT groups presented higher removal torque, mineralized tissue volume, and a greater degree of osseointegration, especially when LLLT was performed only after implant placement, and these findings were associated with higher expression of BMP2 and alkaline phosphatase. Conclusion LLLT performed on implants placed in grafted areas enhances the osseointegration process.

Abstract The aim of this study was to assess the influence of cyclosporine administration on the repair of critical-sized calvaria defects (CSDs) in rat calvaria filled with diverse biomaterials. Sixty animals were divided into two groups: the control (CTR) group (saline solution) and the cyclosporine (CCP) group (cyclosporine, 10 mg/kg/day). These medications were administered daily by gavage, beginning 15 days before the surgical procedure and lasting until the day the animals were euthanized. A CSD (5 mm Ø) was made in the calvaria of each animal, which was allocated to one of 3 subgroups, according to the biomaterial used to fill the defect: coagulum (COA), deproteinized bovine bone (DBB), or biphasic calcium phosphate ceramics of hydroxyapatite and β-phosphate tricalcium (HA/TCP). Euthanasia of the animals was performed 15 and 60 days after the surgical procedure (n = 5 animals/period/subgroup). Bone repair (formation) assessment was performed through microtomography and histometry, while the analyses of the expression of the BMP2, Osteocalcin, and TGFβ1 proteins were performed using immunohistochemistry. The CSDs not filled with biomaterials demonstrated lower bone formation in the CCP group. At 15 days, less bone formation was observed in the CSDs filled with DBB, a smaller volume of mineralized tissue was observed in the CSDs filled with HA/TCP, and the expression levels of BMP2 and osteocalcin were lower in the CCP group compared to the CTR group. The use of cyclosporine impaired bone repair in CSD, and this effect can be partially explained by the suppression of BMP2 and osteocalcin expression.

Abstract Lipoproteins are important bacterial immunostimulating molecules capable of inducing receptor activator of nuclear factor-κB (RANKL) and osteoclast formation in vitro and in vivo . Although these molecules are present in periodontopathogenic bacteria, their role in periodontitis is not known. In this study, we used Pam2CSK4 (PAM2), a synthetic molecule that mimics bacterial lipoprotein, to investigate the effects of lipoproteins on periodontitis in mice. C57BL/6 male mice were randomly divided into three experimental groups: 1) Negative control group: animals received vehicle injection; 2) Positive control group: animals received injection of Escherichia coli lipopolysaccharide (LPS); 3) PAM2 group: animals received PAM2 injection. All the injections were performed bilaterally every other day into the palatal mucosa between first and second molars. After twenty-four days, the animals were euthanized to assess alveolar bone volume (micro-CT), cellular and extracellular composition in the gingiva (stereometric analysis), and osteoclast numbers (TRAP staining). Treatment with either PAM2 or LPS induced gingival inflammation, as demonstrated by increased infiltration of inflammatory cells and enhanced angiogenesis, associated with a smaller number of fibroblasts and decreased extracellular matrix. Importantly, treatment not only with LPS but also with PAM2 resulted in a larger number of TRAP+ multinucleated osteoclasts and significant loss of alveolar bone. Collectively, our data demonstrate that PAM2 can induce gingival inflammation and bone loss in mice, broadening the avenues of investigation into the role of lipoproteins in the pathogenesis of periodontal disease.

Abstract Purpose To evaluate the local effect of simvastatin (SVT) combined with deproteinized bovine bone (DBB) with hydroxyapatite/β-tricalcium phosphate biphasic ceramics (HA/TCP) and with collagen sponge (CS) on bone repair in critical size defects (CSDs) in rat calvaria. Methods Forty-two 5-mm diameter CSDs were made bilaterally in the calvaria of 18 rats. The animals were allocated according to the type of biomaterial and associations used to fill the CSD. After 8 weeks, the animals were euthanized, and their calvaria were evaluated for repaired tissue composition using histologic and histometric analyses. Results In the histometric analysis, the use of SVT showed to increase bone formation in the CSDs when combined with all the bone substitutes tested in this study (p<0.05). Greater bone formation was observed in the groups with SVT compared to the groups without SVT. Conclusions The use of SVT without the need for a vehicle and combined with a commercially available biomaterial may be a cheaper way to potentiate the formation of bone tissue without the need to produce new biomaterials. Therefore, SVT combined with DBB induced significantly greater new bone formation than did the other treatments.

Abstract Introduction The use of bone substitutes in grafting procedures as an alternative of the use of autogenous bone graft has been indicated, however, the direct comparison between these biomaterials has been little explored. Objective To evaluate the effect of different osteoconductive bone substitutes on the bone repair in critical-sized defects (CSDs) in rat calvaria. Material and method One CSD with an 8 mm diameter was made in each of the 40 rats used in this study. The animals were randomly allocated into 5 groups (n=8), according to the type of bone substitute used to fill the CSD: COA (Coagulum); AUT (autogenous bone); DBB (deproteinized bovine bone graft); HA/TCP (biphasic ceramic composed of hydroxyapatite and β-phosphate tricalcium); and TCP (β-phosphate tricalcium). A microtomographic analysis was performed to evaluate the remaining defect linear length (DLL) of the CSD and the volume of the mineralized tissues (MT) within the CSD at 3, 7, 15 and 30 days after the surgical procedure. In addition, a histometric analysis was performed to evaluate the composition of the repaired bone tissue (% Bone and % Biomaterial) at the 30-day period. Result It was shown that the COA had the lowest DLL and MT within the CSD. In addition, the COA presented the highest % of bone in CSD. The DBB had a higher MT and a higher % of bone substitute particles in the CSD than the AUT and TCP groups. The DBB and AUT groups presented higher % of bone in the CSD than the TCP group. Conclusion The use of the DBB promoted a better pattern of bone volume gain and formation compared to TCP and HA / TCP but was biologically inferior to the AUT.


Resumo Introdução A utilização de substitutos ósseos em procedimentos de enxertia de forma alternativa ao uso do osso autógeno tem sido indicada, entretanto a comparação direta entre esses biomateriais tem sido pouco explorada. Objetivo Avaliar o efeito de diferentes biomateriais osteocondutores sobre o reparo de defeitos críticos em calvárias (DCC) de ratos. Material e método Foram utilizados 40 ratos que foram submetidos a confecção de um DCC com 8 mm de diâmetro. Os animais foram aleatoriamente divididos em 5 grupos com 8 animais, de acordo com o tipo de biomaterial utilizado para preencher os DCC: Grupo COA (coágulo); Grupo AUT (osso autógeno); Grupo OBD (osso bovino desproteinizado); Grupo HA/ TCP (cerâmica bifásica composta de hidroxiapatita e β fosfato tricálcio); Grupo TCP (β fosfato tricálcio). Foram executadas análise microtomográfica para avaliação do comprimento linear remanescente (DLL) do DCC e o volume dos tecidos mineralizados (MT) dentro do DCC nos períodos de 3, 7, 15 e 30 dias após cirurgia. Adicionalmente, foi executado análise histométrica para avaliar a composição do tecido ósseo reparado (% Osso e % Biomaterial) no período de 30 dias. Resultado O grupo COA apresentou o menor DLL e MT dentro do DCC e maior % osso do que os outros grupos. O grupo OBD apresentou maior volume de tecidos mineralizados e maior % biomaterial do que o grupo os grupos AUT e TCP. Os grupos OBD e AUT apresentaram maior % osso que o grupo TCP. Conclusão O OBD promoveu melhor padrão de aumento de disponibilidade óssea e qualidade do osso reparado em comparação ao TCP e HA/TCP, porém biologicamente inferior ao grupo AUT.
