Abstract Background: Currently, there is no uniform and official terminology in Portuguese for reflectance confocal microscopy analysis, despite the increasing number of Brazilian dermatologists using this new tool. Objective: To present the terminology in Brazilian Portuguese for the description of reflectance confocal microscopy and establish the first Brazilian consensus on terms related to normal skin and cutaneous tumors. Methods: 10 Brazilian specialists from different institutions and states of Brazil were selected to evaluate the best corresponding terms in Portuguese for normal skin, melanocytic and non-melanocytic tumors. The terms used were translated from international consensuses in the English language. The modified Delphi method was used to create the consensus in 3 steps. Results: The terms considered the most appropriate in the Portuguese language to describe the findings of normal skin, melanocytic and non-melanocytic lesions in the reflectance confocal microscopy analysis were presented. Study limitations: The limitations of the present study include the number of participants and limited regional representation (only two of the five Brazilian regions were represented). Conclusion: This Brazilian consensus represents an opportunity for dermatologists and physicians specializing in cutaneous oncology to become familiar with reflectance confocal microscopy, propagating the technique in clinical and research environments to stimulate national and international publications on this subject.

Abstract Total body mapping comprises photographic documentation of the entire body surface followed by digital dermatoscopy of selected melanocytic lesions, aiming to compare their evolution over time and identify new lesions. As this is an exam based on comparative analysis of serial dermoscopic body images, standardization of the technique for performing total body mapping is essential. Prepared by specialists from the Brazilian Society of Dermatology, using the modified Delphi method, this article provides recommendations for carrying out total body mapping in Brazil, regarding its indications, technical aspects, and the issuing of the report.

Abstract: Background: Dermoscopy is a noninvasive complementary diagnostic method largely used in dermatology. Feasibility, accuracy, and reproducibility are key elements for a diagnostic method to be useful, hence the importance of the terminology used to describe dermoscopic criteria. Objective: To evaluate the reproducibility of the English descriptive terminology proposed for dermoscopic criteria at the 3rd Consensus Meeting of the International Dermoscopy Society in Brazilian Portuguese. Methods: Nine Brazilian dermatologists independently analyzed the translation of sixty dermoscopic descriptive terms proposed at the 3rd Consensus Conference of the International Society of Dermoscopy. Interobserver agreement index was analyzed using the Fleiss' kappa test. Results: The interobserver agreement of the descriptive terminology in Brazilian Portuguese was considered weak (κ = 0.373; p < 0.05). The interobserver agreement of the descriptive terminology used to describe morphology and arrangement of vascular structures was considered moderate (κ = 0.43; p < 0.05). Study limitations: Our study limitations include the small number of participants and limited regional representation (only 2 out of 5 Brazilian regions were represented). Conclusions: The descriptive English terminology proposed at the 3rd Consensus Conference of the International Dermoscopy Society revealed weak reproducibility and the morphology and arrangement of vascular structures presented moderate reproducibility in Brazilian Portuguese. Despite small regional differences, metaphoric terminology in dermoscopy seems to be the most useful and reproducible system to be adopted in Brazilian Portuguese.

Abstract: BACKGROUND: Approximately five to 10% of all melanomas occur in families with hereditary predisposition and the main high-risk melanoma susceptibility gene is the CDKN2A. OBJECTIVES: To describe, after a five-years study, the clinical data of patients (probands) from familial melanoma kindreds, and the pathological characteristics of their melanoma. METHODS: The inclusion criteria were melanoma patients with a family history of melanoma or pancreatic cancer (first- or second-degree relatives) or patients with multiple primary melanomas (MPM). RESULTS: A total of 124 probands were studied, where 64 were considered familial cases and 60 MPM. Mean age at diagnosis was 50 years. Our results show that the following characteristics were prevalent: skin phototype I/II (89.5%), sunburn during childhood (85.5%), total number of nevi ≥50 (56.5%), Breslow thickness ≤1.0mm (70.2%), tumors located on the trunk (53.2%) and superficial spreading melanomas (70.2%). STUDY LIMITATIONS: Analyses of probands' relatives will be demonstrated in future publication. CONCLUSIONS: Our findings are in agreement with previous familial melanomas reports. Fifteen new melanomas in 11 patients were diagnosed during follow up, all of which were ≤1.0 mm. This is the largest dataset of Brazilian melanoma prone kindreds to date, thus providing a complete database for future genetic studies.

Résumé

A detecção precoce do melanoma é crucial para sobrevida maior. A dermatoscopia aumenta a acurácia do exame clínico, possibilitando o diagnóstico em fases iniciais, mas apresenta limitações no diagnóstico dos melanomas incipientes. O mapeamento corporal total e dermatoscopia digital auxiliam o diagnóstico de lesões incaracterísticas e permitem a detecção de lesões novas suspeitas. Os pacientes de alto risco para o desenvolvimento de melanoma são aqueles que mais se beneficiam dessa forma de seguimento. Relatamos caso de paciente de alto risco em que o diagnóstico de melanoma só foi possível através do auxílio da dermatoscopia na análise direcionada da histopatologia.


The early detection of melanoma is crucial for improving survival rates. Dermoscopy improves the accuracy of clinical examination, enabling a diagnosis in the early stages. However it has limitations in the diagnosis of incipient melanomas. Full body mapping and digital dermoscopy help diagnose nonspecific lesions and allow the detection of new suspicious lesions. Patients at high risk of developing melanomas benefit the most from this approach. The authors report the case of a high-risk patient whose melanoma diagnosis was only possible with the help of dermoscopy-guided histopathological analysis.

A incidência do melanoma cutâneo tem aumentado mundialmente e, por tratar-se de neoplasia bastante agressiva e de difícil tratamento em estádios mais avançados, o diagnóstico precoce é fundamental para a cura do paciente. A dermatoscopia surgiu como exame auxiliar in vivo, que tem papel fundamental na realização do diagnóstico precoce e amplifica a acurácia diagnóstica do melanoma. Para a realização do método, é necessário utilizar o dermatoscópio, aparato que permite aumentar a lesão, no mínimo, 10 vezes. A imagem obtida é interpretada utilizando-se o método diagnóstico da preferência do examinador. O método de Análise de Padrões é atualmente o mais utilizado e o que possui maior acurácia para o diagnóstico do melanoma cutâneo, tendo-se demonstrado confiável para o ensino de residentes em dermatologia. Baseia-se em padrões globais e específicos que permitem diferenciar as lesões melanocíticas das não melanocíticas (também importantes no diagnóstico diferencial com o melanoma cutâneo), assim como identificar lesões melanocíticas consideradas benignas, suspeitas ou malignas.

FUNDAMENTOS - A corticoterapia tópica está indicada no tratamento de lesöes eczematosas, como dermatite atópica e dermatite de contato, e de lesöes eritêmato-escamosas, como a psoríase e a dermatite seborréica. A busca de alternativas mais eficazes e mais bem toleradas é plenamente justificável. OBJETIVOS - Comparar a eficácia e a tolerabilidade de um corticosteróide tópico de média potência - o creme de fuorato de mometasona a 0,1 porcento, aplicado uma vez ao dia - em relaçäo a um de baixa potência, o creme de hidrocortisona a 1 porcento, aplicado duas vezes ao dia. MÉTODOS - O estudo clínico incluiu 48 pacientes, de ambos os sexos, com idade entre dois e 14 anos, portadores de dermatoses corticossensíveis, em bom estado geral. O estudo foi aberto, randomizado e prospectivo, com duraçäo de 21 dias. RESULTADOS - O creme de furoato de mometasona foi mais eficaz do que o creme de hidrocortisona na indicaçäo proposta (x=5,48;p<0,01). Näo houve diferença estatisticamente significativa entre os dois fármacos quanto à tolerabilidade. CONCLUSÄO - A mometasona tópica, sob a forma de creme a 0,1 porcento, em aplicaçäo única diária, mostrou-se estatisticamente superior à hidrocortisona, sob a forma de creme a 1 porcento, aplicada duas vezes ao dia, em termos de eficácia, sendo estatisticamente equivalente à droga de comparaçäo, no que se refere à tolerabilidade
