Résumé

Vaginal agenesis is rare gynecologic condition, and the most common etiology is Mayer-Rokitansky-Kster-Hauser (MRKH) syndrome, characterized by the absence of uterus and vagina and presence of normal ovaries and tubes. In such patients, the evaluation for associated malformations as well as careful non-surgical and surgical approach are essential. The neovaginoplasty is an important issue for these patients in regard of functional and psychological standpoint. There are many options available for creation of neovagina. We report six cases of laparoscopic assisted neovaginoplasty using pelvic peritoneal flap.

Résumé

Vaginal evisceration is a rare but life threatening complication of gynecologic surgery. Most of them occur after abdominal or vaginal hysterectomy. The incidence after laparoscopic hysterectomy is unknown. The incidence of evisceration in the patients undergoing laparoscopic hysterectomy in our hospital was 0.1 %. The rapid recovery after laparoscopic hysterectomy, compared with abdominal hysterectomy, is widely acknowledged. Swift return to everyday activities and early resumption of intercourse could predispose to rupture of vaginal vault. Upon discharge, patients undergoing laparoscopic hysterectomy should be given careful instructions to avoid coitus until complete healing of vaginal vault. We described three cases of vaginal evisceration after laparoscopic hysterectomy including a review of the literature.

Pneumocystis carinii pneumonia (PCP) is an infectious disease of immune-compromised host. Sometimes it is difficult to differentiate PCP with diffuse pulmonary hemorrhage. Association between PCP and diffuse pulmonary hemorrhage has been reported in 30% of PCP with HIV positive patients. But association between PCP and diffuse pulmonary hemorrhage has not been reported in non-HIV positive patients without any known underlying causes of diffuse pulmonary hemorrhage. We report a case of PCP with diffuse pulmonary hemorrhage in 66 years old male patient. We confirmed PCP and diffuse pulmonary hemorrhage with bronchoalveolar lavage. We can exclude the possible other causes of diffuse pulmonary hemorrhage except PCP. PCP may be one of possible cause of diffuse pulmonary hemorrhage in non-HIV immune compromised patient.

Résumé

Sudden sensorineural hearing loss (SNHL) is a rarely reported clinical manifestation of systemic lupus erythematosus (SLE). Although the exact pathogenesis is still unclear, several reports suggest that circulatory immune complexes or anti-phospholipid antibodies might play a pathological role in hearing impairment in patients with SLE. We describe a 28-year-old female with SLE who presented with sudden SNHL. She was treated with highdose methylprednisolone and plasmapheresis. After 5 times of the plasmapheresis, her hearing ability was improved from 56 dB to 46 dB by measured of pure tone audiometery.
