Résumé

Background: A prospective study was carried out in our hospital to predict morbidity and mortality in middle and old aged surgical patients by adding echocardiography to standard scoring system with hemodynamic studies. Methods: A total of 50 patients of either sex ranging from 40-70 years of ASA grade 1 & 2 scheduled for various types of noncardiac surgeries were enrolled for the study in our hospital. Patients were divided in two groups according to echocardiographic examinations. The patients with normal echocardiographic values were kept in control group and the patients with abnormal values were kept under study group. The patients in study group were further divided in three groups according to LVEF. Group1-LEVF≥60%, Group2-LVEF≥50-59%, Gr3≥40-49% Tab lorazepam was given to all the patients’ orally prior night of surgery. All the patients were induced with same type inducing agents according to body weight. All the patients were maintained on IPPV by anaesthesia machine with supplemental fentanyl, N2O, O2 and muscle relaxant. SPO2, electrocardiograph (ECG), Non-invasive/invasive blood pressure (BP), Spirometry, Capnography and temperature were monitored. At the end of the research project data’s were compiled systematically and were subjected to statistical analysis using odd’s ratio(OR),95% confidence interval (CI), z value and p value, two statistical software programme were used. Results: Significant difference in the results seen between the three study groups (Gr1, Gr2, Gr3) for perioperative ischemic changes, CHF and arrhythmias. Conclusions: In conclusion preoperative TTE before non-cardiac surgery can predict the risk of perioperative cardiac complications in known or suspected cases of cardiac disease patients.

Résumé

Variations of the muscles pose a challenge to diagnosis and knowledge about them is important during surgeries and other interventions. At times these variations may cause symptoms per se. Variations of the Flexor digitorum superficialis: a muscle of the anterior compartment of forearm, are not very common. We present a rare variation of this muscle in the right upper limb of a male cadaver. The variant was an accessory belly arising from and under the Flexor digitorum superficialis and inserting into the base of proximal phalanx of the index finger. We also review the evolutionary and embryological concepts of such variations.

India, the country which incorporated its efforts towards (RAT) the innovation of light (AABHA) and that’s why it was named BHARAT. It depicts the country proceeding towards light of Truth (SAT), Consciousness (CHIT) and Bliss (ANANDA). Parallel to spiritualism, Indian Intelligentsia tried to protect the body from diseases, which is supposed to be temple of God. Ayurveda is a holistic approach to cure the diseases by natural means. The Objectives of the work is to cover historical aspect of disease treatment in ancient India, to appreciate customs and practices prevalent in ancient Indian societies for malaria control which was primarily based on traditional knowledge system by carrying out multi-sectoral inter disciplinary work. It is tried depict the approach of ancient Indian wisdom in treatment of malaria by different means i.e. Ayurveda, Herbs, Yoga, Mudras etc. This shows the vastness of Indian understanding of health which is worth following by modern science. However, Modern medical science is now embracing ancient Indian Health wisdom and pursuing enormous researches after experiencing inappropriateness of modern medical science. The burning example of it is AYUSH, to led researches in Ayurvedic, Yunani, Sindhi and Homeopathy ways of treatments.

Résumé

The study was undertaken to evaluate the efficacy of multivitamin and micronutrient supplementation in azoospermic patients with maturation arrest. A total of 35 azoospermic patients showing maturation arrest on testicular biopsy were recruited in this study. The patients were divided into two groups. Untreated group (n=11) without any treatment and treated group (n=24) who received multivitamins, micronutrients and co-enzyme Q10. The sperm concentration, motility and morphology were evaluated at monthly interval. The results showed reduction in liquefaction time and relative viscosity of the semen in the treated group. Further, in treated group there was appearance of spermatozoa (4.0 million/ml) exhibiting progressive motility (7%) and normal morphology (6%), even in the first follow up visit. The sperm count, motility and normal morphology increased significantly on subsequent visits. Within 3 months (3 visits) 2 pregnancies were reported. These observations indicate that multivitamin and micronutrient supplementation improve the qualitative and quantitative parameters of seminogram in patients with azoospermia of maturation arrest.
