Cerebellar Nocardiosis: Case Report

The authors report a extemely rare case of primary cerebellar nocardial abscess. Cerebral nocardiosis is arare disease and often fatal, and commonly spreaded from nocardiosis of lung or skin. Total excision of abscess and medication with sulfonamide is the treatment of choice. The patient was treated by continuous external dirainage with intermittent irrigation of abscess cavity by sulfonamide and given with sulfonamide for 10 months.

Microsurgical Posterolateral Approach for a Thoracic Disc Herniation

The authors describe the case of a man who presented with T2-T3 disc herniation. The lesion was successfully identified by computed tomography with iopamidol myelogram and other radiological examinations. A posterolateral approach was tried and made successful result by the aids of microsurgical divices.

Low Back Pain Syndrome in Pregnancy

The authors surveyed one-hundred women within 24-36 hours after delivary for the evaluation of low back pain in pregnancy. It showed that 55% of them suffered from low back pain during pregnancy. Low back pains mostly started form 6-9 months of gestational age. Low back pains were tolerable at 84% of them. And low back pains were not aggravated during pregnancy among the 67% of low back pain pregnant women. Leg pains were noted at 34% of low back pain group.

Tuerculous Brain Abscess on Sellar Region: A Case Report

The authors described a case of tuberculous brain abscess, located on the sellar area with as episode of tuberculous meningitis on the past history. Tuberculous brain abscess, encapsulated collection of pus, is quite rare. Especially, occurrence around the sellar area is extremely rare. On brain CT scan, tuberculous brain abscess on sellar region was differented from pituitary adenoma, craniopharyngioma, germinoma, pyogenic brain abscess, etc. The history of previous tuberculous meningitis was meaningful for the diagnosis of our case. So, we have reported this case with review of the articles.

Intracranial Aneurysmal Rupture during Pregnancy: A Case Report

A multiparous 28-year-old woman, suffering ruptured intracranial aneurysm during pregnancy is presented. Clipping of the aneurysm was performed in 32 weeks of gestational age. Three weeks after operation, she delivered vaginally without complication. Delivery may be managed according to obstetrical indication following surgical correction of the aneurysm. The authors reviewed the literature, and discussed the neurosurgical and obstetric management of such patients.

Traumatic Cerebral Aneurysm: A Case of Traumatic False Aneurysm of the Superficial Temporal Artery

A case of iatrogenic false aneurysm of the superficial temporal artery is presented. Direct injection of Urograffin into the pulsatile mass showed aneurysmal dilatation, 3x2cm sized, of the superficial temporal artery. Aneurysm excision is indicated to reduce the risk of hemorrhage from the subsequent head trauma, to relieve headache, and for any cosmetic defect. The authors have reviewed the literature, and discussed the incidence, classification, pathogenesis, clinical and angiographic diagnosis, differential diagnosis, and treatment.

Multiple Intracranial Aneurysms: A Case Report of Patient with Nine Multiple Intracranial Aneurysms

One case of multiple intracranial aneurysms is presented. On carotid angiogram, nine aneurysmal sacs including a giant aneurysm of left middle cerebral artery were detected on the right distal anterior cerebral artery, anterior communicating artery, bifurcation of both internal carotid artery, both middle cerebral artery, and left posterior communicating artery. The authors reviewed the literature, and discussed the incidence, determining the site of rupture, and the operative methods of multiple intracranial aneurysms.

A Surgical Experience of the Distal Posterior Cerebral Artery Aneurysm

Advances in microneurosurgical treatment of arterial aneurysm have led to increased interest being focused on aneurysm situated within the posterior cerebral fossa. Despite this, reports of neurosurgical intervention in such cases are still rare and there is no report about the distal posterior cerebral artery aneurysm in Korea. An aneurysm of the posterior cerebral artery has been referred to as a "distinct rarity", making up about 0.26 to 2.7% of all cerebral aneurysms. Especially, the incidence of the distal posterior cerebral artery aneurysm is quite rare and most of them are saccular in shape and single in number. The authors report a case of P4 segment aneurysm of left posterior cerebral artery, successfully treated by clipping cross neck and reviewed the literature, and discussed their anatomy, clinical feature, location, and variable operative methods.

Application of Fenestrated Clip in the Intracranial Aneurysms: Report of Four Cases

Some aneurysms with peculiar shapes, large sized or in unusual location cannot be obliterated by ordinary methods. We present four cases using two kinds of fenestrated clips. There were 4 aneurysms in the 4 patients:3 saccular and 1 fusiform aneurysm. Two aneurysms were located in anterior communicating artery, one left carotico-opthalmic artery, and one distal middle cerebral artery. All of the aneurysms were successfully obliterated. Our results suggest that the various shape of fenestrated clips may be benefit for clipping difficult aneurysms, which would not be clipped with ordinary one.

Neuropsychological Analysis of Operated Intracranial Aneurysms

The mortality rate of aneurysmal surgery had recently been reduced due to modern microsurgical technique and refinement of the timing of operation. Despite recovery without neurological deficits, many patients remain with emotional and psychological sequelae postoperatively. We evaluated 33 patients, operated on intracranial aneurysm for recent 2 years, about quality of life and degree of cognitive function. For proper evaluation of outcome of aneurysmal surgery, in addition to the neurological outcome, psychological aspects must be considered.

A Case of Carotid Body Tumor: Case Report

Carotid body tumor is uncommon and has been a source of fear and curiosity to the surgeon for many years. This puzzling tumor attaches itself so intimately to the carotid artery system that its routine removal is often associated with serious consequences. The justification for its extirpation has been debated vigorously. Recently, we have experienced a case of carotid body tumor and obtained good result after total removal. We report our case with a brief review of the articles.

A Case of Intradural Vermian Schwannoma without Attachment to a Nerve Root

A rare case of intradural cerebellar vermian schwannoma has been reported. A 43 years old woman admitted to our hospital because of headache and weakness of upper extremities. CT brain scan showed an irregular lower density with intermixed high density at the middle posterior fossa, and large lower density with peripheral irregular thickness of rim enhancement after contrast media. CT cervical spine showed a lower density with displaced cord to the right side on the dorsal surface at C1 portion. Total enucleation of irregular brownish-gray large cystic mass resulted in uneventful recovery. There was no attachment to any nerve root.

Clinical Analysis of Intracranial Tuberculomas

We have experienced 10 cases of intracranial tuberculoma which were confirmed by CT scan in National Medical Center from Jan. '79 to Aug. '82. The analysis is based on 10 cases with intracranial tuberculoma on whom operation was performed in 7 cases and conservative treatment was done in 3 cases. Extracranial disease or a past history of tuberculosis are evident in half of patients. The common presenting features are intracranial hypertension and papilledema. These are usually located in the supratentorial region. The use of CT brain scanning can be of great help in diagnosis and follow up more than others. The current treatment consists of medical therapy such as combined antituberculous agents, but surgical exploration may be reserved for intractable seizure, suspected brain tumor and medical failure or severe intracranial hypertension.

Chondrosarcoma of the Parasella Area: Case Report

A rare case of chondrosarcoma of the skull in a 57 year old female is reported. The tumor was located in the right parasella region, extra-dural type. A plain skull showed amorphous calcific densities in the right parasella area. Right carotid angiogram showed a slightly opening of carotid siphon on lateral view. CT brain scan showed a irregular mottled calcific density in the right parasella area. Osteoplastic craniotomy on right frontotemporal area was performed for removal of tumor. Then, patient was improved markedly.

Disseminated Calcific Cysticerosis of BRain and its Histopathologic Features: A Case Report

We report a case of disseminated calcific cysticercosis of brain, accompanied with marked hydrocephalus. Histopathologically, the calcific spots consisted of mummified scolex and bladder of cysticercus cellulosae with a diffuse calcification, but no area of cystic change were noted. Surrounding brain tissue showed a dense fibrous reaction and gliosis with little foreign body reaction.
