Résumé

Discovery of Helicobacter (H.) pylori has led to a fundamental change in our understanding of gastric diseases in humans. Previous studies have found various Helicobacter spp. in dogs and cats, and pets have been questioned as a zoonotic carrier. The present study surveyed the Helicobacter infections and investigated the presence of H. felis and H. pylori infections in domestic and feral cats in Korea. Sixty-four domestic cats and 101 feral cats were selected from an animal shelter. Saliva and feces were evaluated by Helicobacter genus-specific polymerase chain reaction (PCR). Genus-specific PCR positive samples were further evaluated for H. felis and H. pylori using specific primer pairs. Thirty-six of 64 (56.3%) samples from domestic cats and 92 of 101 (91.1%) samples from feral cats were PCR positive; the positive rate of feces samples was higher than that of saliva samples in both groups. H. felis and H. pylori species-specific PCR was uniformly negative. The prevalence of Helicobacter spp. in feral cats was approximately two-fold higher than that of domestic cats. The fecal-oral route may be more a common transmission route not only between cats but also in humans.

Résumé

Background: Lumbar epidural anesthesia is widely used regional blockade method. But postepidural back pain is most common cause that experienced patients refuse epidural anesthesia. We planned this study to evaluate the effect of epidural dexamethasone injection for postepidural back pain. Methods: Adult patients free of back pain were randomly allocated into two groups. In both group anesthesia was induced with 2% lidocaine 20~25 ml mixed with 1:200,00 epinephrine and fentanyl 0.1 mg. After surgery, epidural catheter was removed after epidural dexamethasone injection in one group (group 2). In other group (group 1), catheter was removed without treatment. We visited patients and asked existence of back pain, and the intensity of back pain was measured by visual analogue scale at 24, 48 and 72 hours after surgery. Statistical analysis was done by Student's t-test and Mann-Whitney test. Results: In group 2, there was significant decrease in number of patients suffering from back pain and intensity of back pain. Conclusion: Epidural dexamethasone injection results in decreased number of patients suffering from back pain and also less in intensity of back pain.

Résumé

Takayasu's arteritis is one of the most important causes of the renovascular hypertension in orientals. Among the multiple treatment modalities, percutaneous transluminal renal angioplasty(PTRA) has become the treatment of the choice for major renal artery stenosis and is a safe, repeatable, effective procedure for the treatment of renovascular hypertension due to Takayasu's arteritis. We experienced a case of percutaneous transluminal balloon angioplasty of Takayasu's arteritis involving the proximal left renal artery. After PTRA, clinical and angiographical improvements were achieved.

BACKGROUND: One of the main disadvantages of caudal block is the long latent period before a satisfactory blockade is obtained. Many investigators have used various preparations of local anesthetic solutions to improve the speed of onset. This study was performed to evaluate the effectiveness of prewarming of lidocaine HCl for caudal block. METHODS: Fifty healthy young patients (ASA I) were allocated into two groups, A and B. In group A, the local anesthetic solution were injected at room temperature (25 degrees C), while in group B, they were injected at 37oC. All the caudal block were performed using 2% lidocaine HCl 20 ml with fentanyl 100ug and epinephrine 1:200,000. The onset time was defined as the period from completion of injection until the patient first noticed loss of sensation to pin-prick on perianal region. Assessment of sensory loss was made at 15 seconds interval. We have compared the onset of sensory blockade between groups. The duration of analgesia and any significant side effects were also recorded. RESULTS: The onset of sensory blockade was significantly faster in group B (3.5 +/- 0.5 minutes) than group A (6.2 +/- 0.9 minutes). The duration of analgesia were not significantly changed between groups. Side effects of urinary retention, pruritus and nausea were noted between both groups but the difference was not significant. CONCLUSIONS: We have found that the onset time was 44% faster with warm lidocaine-fentanyl mixture (37 degrees C) than with the room temperature (25 degrees C). The improved clinical usefulness was achieved with no increase in side effects. The technique is a safe and effective method to reduce the latency of onset.

Résumé

The fiberscope is no longer an experimental instrument, and it is a state-of-the-art technique for airway management in the operating room, recovery room, and intensive care unit. Changing the oral tube to a nasal one is often necessary if the trachea is to be intubated for a long period. A nasal tube is better tolerated by the patient, is easier to secure when the patient is moved, and is easier to attach to the respiratory care equipment. We describe three cases report changing the oral tube to a nasal one in neurosurgical pa- tient post operatively under flexible fiberoptic laryngoscope, while the interruption of venti- lation is minimized.

Résumé

Blind nasotrscheal intubation with preformed tube has been attempted in 20 conscious young male scheduled for surgical reduction of mandibular fracture. The patient was premedicated in the usual fashion with glycopyrrolate -midszolam- fentanyl. Airway anesthesia was performed with topical lidocaine ; Nasopharyngeal and translaryngeal. Of the 20 patients in whom intubation was performed, 12 (60%) required one to three attempts, 6 (30%) required four to six, and only two failures occurred. Contrary to our expectation, this procedure met with almost positive acceptance by the patients in the series, and the surgical staff had no adverse criticism. In addition to the effectiveness and ease of learning, this technique is simple and free from major morbidity.

Résumé

Hypercarbia can occur in spite of adequate ventilation if removal of carbon dioxide from the inspired gases is inadequate. Neither blood pressure nor heart rate is a reliable indicator of hypercarbia. A 53-year old male, ASA physical status II with mild hypertensive patient underwent primary repair because of rotator cuff tear of the shoulder. Anesthesia was induced with thiopental-succinylcholine and maintained with pancuronium- Halothane-N2O-O2 in a semiclosed circle system. We report occurrence of hypercarbia due to replacement with an incorrect unidirectional valve which is too small to function properly.

Résumé

A 68 year old, ASA physical status 11, hypertensive male patient underwent open reduction, angulated blade plate fixation, and bone graft of femur intertrochanteric fracture due to non-union. Anesthesia was induced with thiopental and succinylcholine and maintained with vecuronium- Halothane-nitrous oxide-oxygen. Three uints whole blood was transfused and any significnat change in vital signs and EKG were not found during 4 hours operation. Soon after the patient was recovered. But he was found to be hemiplegia and sensory disturbance on the next day. Brain CT revealed infarction of middle cerebral artery territory. He expired 7 days following the surgery despite strenuous supportive measure.

Résumé

Pulmonary thromboembolism is more common in medical than in surgical wards. However, it can occur in the postoperative period especially in those undergone orthopedic surgery of the hip or knee. We had an opportunity to give witness in a law court the relation between the pulmonary infarction and general anesthesia, which case had been occurred in a local clinic. A case of pulmonary thromboembolism with infarction m a 28 year-old male is presented which developed at the near of surgery and died 1 hour after surgery of the maxillary open reduction with a plate-wire fixation. Anesthesia was performed under the general nasotracheal with Halo-O2. No case has been yet reported. Diagnosis was made by autopsy.

Résumé

General endotracheal anesthesia with mechanical ventilator is a common anesthetic practice since introduction of various anesthetic ventilator. A mechanical not only deoreases workload but also allows the anesthesiolosist to better nonitor and manage the patient during the operation. However, acoidental complications can be encountered during the wse of ventilator as with all other modern equipments. Several authors have described that increased pulmonary pleasure from poaitive pressure ventilation mar lead to alveolar rupture with subsequent dissection of air along the vascu- lar structure of the lung into the hilum and then into the tissue plane of the mediastinum. We report a case of pneumomediastinum, pneumoperitoneum and subcutaneous emphysema without pneumothorax which occured during discoidectomy under the endotracheal ventilator anesthesia.

The authors presented a family whose 4 siblings had been suffered from oculopharyngeal muscular dystrophy of autoscmal recessive trend, with symptoms of progressive ptosis, external ophthalmoplegia, dysarthria, dysphagia and facial muscle atrophy, and we performed HLA study on these 9 family members which showed no interrelationship between oculopharyngeal muscular dystrophy and HLA Haplotypes.

Résumé

This study was carried out to determine the effect of long term administration of vitamin A on the changes of lipids in liver tissue and serum in rat. The animals were fed with control diet(200 ug/day) and high vitimin A diet(2, 000 ug/day) for 24 weeks. Vitamin A contents of liver in high vitamin A fed rats were increased linearly accordance with duration of the vitamin A administration, but levels of serum vitamin A showed unremarkable changes during the experimental period. Contents of cholesterol and triglycerides of vitarnin A fed rats were significantly higher than those of controls. Levels of HDL-triglycerides in v:itamin A fed rats were significantly higher than those of control rats during experimental period, while levels of HDL-cholesterol and HDL-phospholipids showed unremarkable changes during experimental period. Levels of individual cholesteryl esters showed undremarkable changes during experimental period in both animal groups.

Résumé

The capacity of tobacco infusions of Korean cigarettes to activate the complement pathway was investigated in vitro by hemolytic assay. Complement activity was studied using samples of normal rabbit serum incubated at 37 degrees C with tobacco infusions. In all mixtures containing tobacco infusion A, B, C, D, E, F, G, H and I at a concentration of 0.1 : 1, total hemolytic complement showed a progressive decrease of 55.92%, 48.62%, 42.89%, 49.12%, 42.26%, 52.26%, 71.96%, 60.90%, and 61.14% respectively, after 60min of incubation. This study suggests that all tobacco infusions of Korean cigarettes are activators of complement.

Résumé

This is a report of anesthesia done for 24 cases of renal transplantation performed at St. Mary's Hospital, Catholic Medical college, Seoul, Korea, between 25 March 1969 and 19 August 1976. The recipient of renal transplantation show many critical conditions which require special considerations in performing anesthesia. The patients preoperative condition should be evaluated more carefully than most other kinds of surgical patients and anesthesia technic also must be selected specially. All kidney grafts were obtained from living donors. The family linkage of 24 donors was: 7 cases(29.1%) were from parents, 3 cases(12.5%) from children, 9 cases (37.5%) from siblings, 3 cases(12. 5%) from other relatives and 2 cases (8. 3%) were from nonrelated prsons. In the sex distribution of the donors, each sex was equally distributed.: All donors were anesthetized by means of endotracheal anesthesia using halothane, nitrous oxide and oxygen. As to complications during the nephrectomy of donors, a case of hiccup and two cases of pleural tear were encountered. The removed kidney was perfused with Hartmanns solution before transplantation and the choice of anesthetic agents caused no problems in the function of transplanted kidneys. The recipients had histories of chronic glomerulozephritis or pyelonephritis from 6 months to 32 years and showed uremic conditions. Their mean age was 38 years, ranging from 18 years to 59 years. The most frequent age group of the recipients was the 30 and 40 year group. Most recipients were males; among 24 cases, only 3 were females. Most of the recipients had decreased function of heart and lungs, severe anemia difficult to correct by blood transfusion(Hb, 7. 3 gm%, Hct. 24, 2%), increased BUN(70. 36+/-31. 34 mg/dl), hyperkalemia (4. 67+/-0. 63 mEq/L), poor and fragile veins, bleeding tendency and decreased kidney function(serum creatinine 10. 48 edmea, +/-2. 99 mg/dl). Prolonged medication to the recipient was another hazard to anesthesia. To lessen the risk during the anesthesia maintenance, preoperative preperation of the recipient The Journal of The Korean Society of Anesthesiologists Vol. 10, No. 1, 1977 Premedication of the recipients was perfor med by valium(515 mg) and atropine(0. 4 0. 5 mg) one hour before anestl esia. For tlie anesthetic technic, endotracheal anesthesia was applied to aI1 cases. Induction was done with intravenous pentothal soium, l00~ 200 mg or Epontol 250~500mg. As to the muscle relaxant to facilitate intubation, succinylchline for ll cases of the initial period of the transplantation, later gallamine for one case and pancuronium for the recent 12 cases. All anesthesia was maintained with halothane, nitrous oxide and oxygen. Muscle relaxants were used in al) cases during the surgery: succinylcholine drip for the initial 7 cases, d-Tubocurarine for 2 cases, gallamine for 2 cases and pancuronium for the recent 12 cases. Galllamine seems to be not contraindicated, as far as the active diuresis could be anticipated after the transplantation. Amount of blood transfused during operation was 2, 041 ml in average, ranging from 800 ml to 5000 ml and fluid infused was 1, 242 m] of Hartmann's sol. and/or other solutions. Respiration was controlled manually or mechanically. During the anesthesia, 16 cases of hypertension, 7 cases of arterial hypotension, 3 cases of arrhythmia, one cas of tight chest and one case of tachycardia, were encountered. Emergence of anesthesia was within 15 minutes. After the transplantation, diuresis started in 33 minutes on the average and most patients showed marked diuresis in which urine volume of 24 hours was 10, 164 ml on the average. The total surgical procedure lasted 4 hours and 50 minutes on the average, ranging from 3 hours 50 minutes to 7 hours. The ischemic tine of the removed kidney was 32 minutes in average. Followup of the patients showed that 10 cases died out of 24 cases between 4 months and 4 years after the surgery Patients surviving more than 1 year were 11 cases out of 15 cases(73. 3%), two cases out of 8 cases(25%) survived more than 2 years and two cases out of 8 cases(25%) more than 3 years. One case had the longest survival of 7 1/3 years.
