Résumé

OBJECTIVE: To estimate the long-term skeletal responses to hormone replacement therapy(HRT) with or without active vitamin D(VD) by using short-term changes of bone markers in postmenopausal women (PMW). METHODS: Biochemical markers of bone formation(osteocalcin,OC) and (&) resorption(deoxypyridinoline, Dpd ) at 3 months & lumbar bone mineral density(BMD) at 1 year were measured in 64 natural PMW taking HRT(n=41) & HRT with calcitriol 0.25 microgram/day(n=23). The correlation of percent changes of bone markers after 3 months of Tx with those in lumbar BMD after 1 year was evaluated. RESULTS: 1. serum-OC & urine-Dpd showed decrease of 20.9% & 30.1% at 3months respectively & BMD increase of 3.8% after 1 year of Tx. 2. Among 58 PMW with decreased u-Dpd change, 49 (84.5%) revealed increase in BMD, while 40 (81.6%) among 49 PMW with decreased serum-OC change showed increased BMD. 3. Bone gainers showed significant decrease in changes of serum-OC(18.1% vs 9.2% p0.05) & urine-Dpd (r=-0.091 p>0.05) at 3month with BMD changes at 1year were seen in total PMW, but urine-Dpd changes in HRT without active VD group revealed significantly inverse correlation(r=-0.376 p<0.05). CONCLUSION: Short-term changes of bone markers did not precisely predict the long-term changes of BMD in total PMW except urine- Dpd in HRT without active VD.

Résumé

This study was performed to identify the effect of the hydrosalpinx fluid on sperm motility. It has been reported that the patients with hydrosalpinx show the outstandingly lower success rate than other patients having infertility by different factors. It is unclear that the cause of it is influenced by hydrosalpinx fluid directly or by secondary chronic inflammation of endometrium. We wanted to know if the hydrosalpinx fluid influences sperm motility parameters directly such that it is related to the development of infertility. Therefore, using computer assisted semen analyzer (CASA), we observed, from February to July, 1997, how sperm motility, sperm progressive motility, sperm curvilinear velocity, sperm lateral head displacement, sperm straightness and sperm linearity change after treating normal sperm with hydrosalpinx fluid to evaluate sperm function on infertility. The result was that the study group (n=32) has the tendency to differ from the control group (n=32) on sperm motility, progressive motility, curvilinear velocity, lateral head displacement, straightness and linearity. We concluded that the hydrosalpinx fluid, with varying degree, directly has the harmful effects on sperm motility parameters, that is, curvilinear velocity, lateral head displacement and linearity of sperm which are related to the hyperactivation, hence decreased capacitation.

Résumé

Heterotopic brain tissue is a very rare developmental anomaly in which isolated brain tissue is found outside the cranial cavity. In the majority of cases, the nasal cavity is the most common site of involvement, and only 10 cases which were developed in the palate have been reported in the international literature. Recently, we experienced a case in which heterotropic brain tissue, a congenital tumor-like mass, was observed in the soft palate. This was considered initially as hemangioma under MRI study, and under this impression, we observed this lesion to regress spontaneously for two years. Surgical excisoin was performed for diagnostic and therapeutic purpose since the mass have had no interval change. Histopathological diagnosis of positive immunohistochemical reaction was performed for the heterotropic brain tissue using S-100 protein and glial fibrillary acidic protein (GFAP). At the present time, the patient has no evidence of the disease, recurrence nor complication. We present this case with a review of literature.

BACKGROUND AND OBJECTIVES: The majority of laryngeal tumors are known to be squamous cell carcinoma. Non-squamous cell tumors of the larynx represent various spectrum of diseases with different prognosis and rationales of management from squamous cell carcinoma. Therapeutic options to these tumors depend on their histopathological characteristics and clinical behavior. To assess the unique aspects of the non-squamous cell tumors which can lead to their diagnosis and to provide considerations of management options and prognostic variables. MATERIALS AND METHODS: Eight non-squamous cell tumors of the larynx including 6 malignant tumors (two malignant lymphoma, single instances of adenoid cystic carcinoma, malignant schwanoma, osteosacoma, and spindle cell carcinoma) and two benign tumor (hemangioma, spindle cell lipoma) during the 7-year period from 1990 to 1996 were reviewed retrospectively. RESULTS: The frequently presenting symptoms were hoarseness, dyspnea, sore throat, and lump sensation in the throat. The most common origin site was supraglottis (50%), followed by glottis (38%) and subglottis (12%). All non-squamous malignancies beside non-Hodgkin's lymphoma were treated by surgery and postoperative radiotherapy. Two benign tumors were treated with only surgery. The mean follow-up period was 37.4 months and all patients have survivied with no evidence of recurrence. CONCLUSION: Our experience suggests that non-squamous cell tumors are a group of various pathological types and it may be possible to achieve better results with combined treatment including surgery.
