Résumé

The population of intermediate-risk prostate cancer patients is a large heterogeneous group with diverse prognoses and challenges the struggle to develop more meticulous and standardized treatment recommendations. Furthermore, there are no specific treatment guidelines based on Korean patients although the cancer nature of this patient group is known to be somewhat different from those of western patients. This review will examine the treatment options for intermediate-risk prostate cancer patients in specific clinical situations. The literature provides evidence that combining androgen deprivation therapy (ADT) for 6 months with radiation therapy (RT) may have superior survival than RT alone. Also, patients with adverse pathologic features and lymph node metastasis will benefit from adjuvant RT and ADT respectively after radical prostatectomy (RP). In addition, patients with life expectancy ≥10 years will benefit from lymphadenectomy during RP when lymph node metastasis is suspected as well as neurovascular bundle saving during RP is recommended because of its superior functional results.

OBJECTIVES: Adoption of hospital information systems offers distinctive advantages in healthcare delivery. First, implementation of consolidated hospital information system in Seoul National University Hospital led to significant improvements in quality of healthcare and efficiency of hospital management. METHODS: The hospital information system in Seoul National University Hospital consists of component applications: clinical information systems, clinical research support systems, administrative information systems, management information systems, education support systems, and referral systems that operate to generate utmost performance when delivering healthcare services. RESULTS: Clinical information systems, which consist of such applications as electronic medical records, picture archiving and communication systems, primarily support clinical activities. Clinical research support system provides valuable resources supporting various aspects of clinical activities, ranging from management of clinical laboratory tests to establishing care-giving procedures. CONCLUSIONS: Seoul National University Hospital strives to move its hospital information system to a whole new level, which enables customized healthcare service and fulfills individual requirements. The current information strategy is being formulated as an initial step of development, promoting the establishment of next-generation hospital information system.

Hypertension and atherosclerosis are the most important factors contributing to the development of aortic dissection. Primary aldosteronism is a rare cause of hypertension. The concurrence of aortic dissection is very rare in primary aldosteronism. However, when aortic dissection is found as a life-threatening complication of primary aldosteronism, then the diagnosis of primary aldosteronism is important because the therapeutic intervention can be curative. Only 3 cases of primary aldosteronism with aortic dissection have been reported in the literature. We report here on a case of primary aldosteronism with aortic dissection, which was treated by laparoscopic adrenalectomy. We lowered the blood pressure with antihypertensive drugs and potassium replacement was done to treat the aortic dissection. After stabilization of aortic dissection, we removed his left adrenal mass by laparoscopic adrenalectomy. Postoperatively, the patient's blood pressure has been within normal limits and the serum potassium increased to a normal level without supplementation. The aortic dissection has remained in a stable state
