@#现阶段传统的放化疗给胃癌带来了一定的生存获益,但胃癌总体生存率仍不理想。迫切需要继续挖掘新的治疗靶标 和诊断治疗方案。密封蛋白(claudin)是细胞连接复合体中紧密连接的重要组成成分,其异常表达导致的紧密连接功能失常会促 进肿瘤的增殖和转移。Claudin家族蛋白如claudin1、 4、 7、 18.2等的突变和表达水平异常影响胃癌细胞的增殖、侵袭、转移和凋亡 等,具有作为胃癌诊断、治疗和预后的生物标志物的巨大潜力。以靶向claudin18.2的药物IMAB362为代表,目前已开发出多种 针对claudin蛋白的靶向药物,相关的临床试验已在胃癌、胰腺癌、生殖细胞瘤以及卵巢癌等恶性肿瘤患者中开展,针对claudin蛋 白的研究可能有助于探寻胃癌的早期诊断和精准治疗的新策略。

<p><b>OBJECTIVE</b>To evaluate the feasibility and safety of the modified urethral pull-through procedure for the treatment of posterior urethral stricture or atresia.</p><p><b>METHODS</b>We retrospectively analyzed 212 cases of posterior urethral stricture or atresia treated by the modified urethral pull-through procedure. The length of the stricture or atresia was 1.5 - 12 cm, and 66 cases had experienced 1 - 4 previous unsuccessful urethral repairs. Simple transperineal approach was adopted in 208 cases and transperineal-inferiorpubic approach in the other 4. And 15 of the patients underwent urethral construction with grafts.</p><p><b>RESULTS</b>Satisfactory voiding was achieved in 198 (93.4%) of the patients, of whom 16 received 3 - 15 urethral dilations. Of the 14 cases that failed, 10 succeeded after a second and 2 after a third operation. Of the 15 cases that underwent substitution urethroplasty, 14 achieved satisfactory voiding, and only 1 needed repeat dilation. No serious complications were observed in any of the patients.</p><p><b>CONCLUSION</b>Modified urethral pull-through procedure, with its advantages of safety, mini-invasiveness, simple operation and high success rate, is feasible for the treatment of posterior urethral stricture or atresia, while for that with the length >5 cm, substitution urethroplasty should be considered.</p>

<p><b>OBJECTIVE</b>To study the relationship between the serum levels of adipocyte fatty acid-binding protein (A-FABP) level and type 2 diabetes mellitus in a community population.</p><p><b>METHODS</b>A total of 255 residents (aged 45-85 years) were randomly selected from 4 communities in Guangzhou to examine the fasting plasma glucose (FPG), 2-hour postprandial blood glucose (2hPG), glycosylated hemoglobin (HbA1c), body mass index (BMI), waist circumference (WC), hip circumference (HC), waist-to-hip ratio (WHR), blood pressure (BP), triglyceride (TG), cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C). The insulin resistance index (HOMA-IR) was calculated and enzyme-linked immunosorbent assay (ELISA) was used to determine serum A-FABP and fasting insulin (FINs) levels. The cases were divided into 3 groups according to blood glucose level, namely the normal group (group A, n=90), impaired glucose tolerance group (group B, n=85), and diabetic group (group C, n=80), and the A-FABP levels were compared between them.</p><p><b>RESULTS</b>Compared with group A, the subjects in groups B and C showed significantly increased FPG, 2hPGh, HbA1C, HOMA-IR, systolic blood pressure (SBP), and waist circumference (P=0.000) as well as FINs, WHR, diastolic blood pressure (DBP) , TG, and HDL-C (P=0.038, 0.047, 0.01, and 0.046, respectively). Compared with group B, group C showed significantly higher FPG, 2hPG, HbA1c, TG, and SBP (P=0.00), with also higher levels of FINs, BMI, WC, DBP, and HDL-C (P=0.012, 0.006, 0.03, 0.019, and 0.029, respectively). A-FABP increased significantly in the order of group A, B, and C (P=0.00), and this result was not affected by the differences in age between the 3 groups (P>0.05). A-FABP level was positively correlated to FPB, 2hPG, FINS, WHR, BMI, WC, SBP, and HOMA-IR, but inversely to TG and HDL-C (P=0.00).</p><p><b>CONCLUSION</b>Elevated serum A-FABP is closely related to glucose metabolism disorder, and A-FABP may serve as a useful marker for the occurrence and development of type 2 diabetes in the community population.</p>

<p><b>OBJECTIVE</b>To investigate the protective effects of preconditioning morphine on rabbit myocardium during ischemia-reperfusion.</p><p><b>METHODS</b>Thirty New Zealand male white rabbits were randomly assigned to three groups: control, I/R and morphine groups. In morphine group 1.0 mg/kg morphine was given preoperationaly, in control and I/R groups 1.0 ml/kg NS was given. Twenty-four hours later rabbits in morphine and I/R groups underwent 40 min of coronary occlusion followed by 2 hours of reperfusion; for control group only sham operation was performed. At the end of the reperfusion, infarct size (IS) and area at risk (AAR) were defined by Evans blue and TTC staining. At the end of the reperfusion blood samples were taken for determination of plasma SOD activity and MDA levels. The heart was harvested and levels of the HSP27 were determined by Western blot, and the heart ultrastructures were observed under the electron microscopy.</p><p><b>RESULTS</b>Compared with I/R group,morphine significantly reduced infarct size (21.5%+/-2.4% Compared with 37.8%+/-1.7%, P<0.05). The morphine had a lower level of MDA and higher levels of SOD and HSP27 than those in I/R.</p><p><b>CONCLUSION</b>Preconditioning of morphine demonstrates cardioprotective effect on ischemia/reperfusion injury, which may be associated with increased HSP27 levels in the heart.</p>

<p><b>OBJECTIVE</b>To study the clinicopathologic features and radiologic findings of dysembryoplastic neuroepithelial tumor (DNT).</p><p><b>METHODS</b>The clinical presentations, radiologic findings, histologic features and immunophenotype of 9 cases of DNT were analyzed.</p><p><b>RESULTS</b>The age of patients ranged from 12 to 51 years (mean age = 32 years). Most presented with partial seizures, sometimes accompanied by transient aphasia, agraphia and decreased visual acuity. One case was asymptomatic and discovered incidentally during a routine check-up. All patients had no neurological deficit found on physical examination. All tumors were located in the supratentorial cerebral cortex. There was no peritumoral edema or space-occupying effect on radiologic examination. The tumors involved either frontal lobe (number = 4), temporal lobe (number = 4), frontoparietal lobe (number = 1) . Two cases showed cystic changes. Two histologic variants of DNT were recognized: simple (number = 3) and complex (number = 6). Simple variant was composed mainly of the glioneuronal element, accompanied by surrounding oligodendrocyte-like cells, and the complex variant contained a low-grade glioma component, in addition to the glioneuronal element and sometimes foci of cortical dysplasia.</p><p><b>CONCLUSIONS</b>DNT is a benign tumor with excellent prognosis after surgical excision. Local recurrence is rare. Complex variant of DNT needs to be distinguished from other types of low-grade glioma.</p>

AIM: To study the effects of aspirin-niacinamide-zinc complex (WUY) on platelet aggregation and experimental thrombosis. METHODS: With adenosine diphosphatethe (ADP), arachidonic acid (AA) and collagen, the effects of aspirin-niacinamide-zinc complex (WUY) on platelet aggregation in vitro or in vivo were investigated by Born's method. The mouse mortality caused by intravenous injection of AA and experimental thrombus formation in rats were observed. Radioimmunoassay was used for measuring thromboxane B_2 (TXB_2) and 6-keto-PGF_(1?) in plasma of rabbits. RESULTS: In high, middle and low dose groups, drugs, in vitro, inhibited ADP-, AA-and collagen-induced platelet aggregation and the effect of WUY was stronger than that of ASP in high dose groups. In vivo, WUY showed more potent inhibitory effects on AA-induced aggregation in 1 h and 3 h. WUY had a powerful inhibitory effect on mouse death as a result of pulmonary thrombi induced by AA injection into the tail vein and ED_(50) was lower than that of ASP. In addition, WUY exhibited strong inhibitory effect on thrombus formation in rat arteri-venous shunt and significantly reduced plasma level of TXB_2 while it markedly increasing 6-keto-PGF_(1?) in high dose groups and ASP significantly reduced plasma level of both TXB_2 and 6-keto-PGF_(1?). CONCLUSION: The effect of WUY on platelet aggregation and experimental thrombosis is stronger than that of ASP and can increase plasma level of 6-keto-PGF_(1?).

Objective To evaluate the effect of PTH gene polymorphisms on bone mineral density (BMD) at multiple skeletal sites in normal females.Methods PTH gene phenotype was determined by PCR-RFLP of restriction enzyme Bst BⅠin 596 females aged (46.3?13.7) years (20-80 years),and PCR products with or without enzymolytic site were considered as genotype B or genotype b respectively.BMDs of the anteropesterior spine (AP) and supine lateral spine (Lat) of lumbar vertebrae (L_1-L_4),femoral neck (FN),total hip (T-hip), Ward's triangle (Ward),Trochanter (Troch),forearm [radius+ulna ultradistal (RUUD) and total area of radius + ulna (RUT) ] were measured by DEXA (QDR4500A).Results (1) Hardy-Weinberg equilibrium was evident for PTH polymorphisms.The frequencies of genotype were BB 0.784,Bb 0.208,bb 0.008 and frequencies of alleles B,b were 0.888 and 0.112 respectively in 596 normal females.Frequencies of BB,Bb,bb genotypes were 0.781,0.210,and 0.009 respectively in 347 postmenopausal women and their frequencies of alleles B,b were 0.886,0.114.No significant difference was found between post- and premenopausal women in genotype frequen- cy.(2) Comparing their BMDs of AP,Lat,FN,T-hip,Ward,Troch,RUUD and RUT,there was no significant difference between BB and Bb genotypes of pre- and postmenopansal women groups.(3) Logistic regression analysis failed to show any statistical difference between normal and osteoporosis women with regard to PTH phenotype.Conclusion PTH gene polymorphism has little effect on BMD in normal females.

Objective To observe apelin and its receptor (APJ) expressions in human osteoblasts and evaluate the effect of apelin on osteoblasts.Methods The expressions of apelin and APJ in human osteoblasts were tested by RT-PCR and Western blot.After human osteoblasts were treated with apelin,cell proliferation was measured by [~3H] thymidine incorporation and cell counting.Cell function was measured by alkaline phosphatase (ALP) activity,the secreted osteocalcin level and typeⅠcollagen production .The activation of signaling cascades was tested by Western blot.Small-interfering RNA (siRNA) to blockade APJ was applied to observe effects of apelin on cell proliferation and the activation of signaling cascades.Results Both apelin and APJ were expressed in human osteoblasts.Apelin increased the proliferation and did not show the influences on ALP activity, osteocalcin secretion and type I collagen production in human osteoblasts.Apelin induced activation of phosphatidylinositol-3 kinase (PI3K) downstream effector (Akt),but not mitogen-activated protein kinase (MAPK) such as c-jun N-terminal kinase (JNK),p38 and ERK1/2 in human osteoblasts.Suppression of APJ with siRNA or LY294002 (PI3K inhibitor) abolished the apelin-induced cell proliferation and the activation of Akt.Conclusion Human osteoblasts express apelin and APJ.Apelin stimulates the proliferation of human osteoblast via APJ/PI3K/Akt pathway,but has no effect on osteoblast differentiation.
