Résumé

Objective To systematically evaluate the safety and effectiveness of interventional uterine artery embolization (UAE) in treating symptomatic uterine fibroids by comparing it with surgical therapy. Methods Computer retrieval of the Cochrane Library, PubMed, EMbase, Web of Science, China National Knowledge Internet (CNKI), Wanfang data, etc. was conducted. The retrieval time was from the inception to July 2014. The academic papers that were related to the randomly controlled test (RCT) of UAE and surgical treatment for symptomatic uterine fibroids were collected, and the reference literatures were also searched for. Two reviewers screened the literatures according to the inclusion and exclusion criteria, the useful data were extracted and the qualities were evaluated. Meta-analysis was performed by using RevMan 5.2 software. Results A total of 6 RCTs were finally obtained, including 732 patients in total. Meta-analysis indicated that both the hospitalization time and recovery time in UAE group were shorter than those in surgery group. No significant differences in health -related quality of life, patient’s satisfaction, intra -operation complications, the recurrence rate and main complications within one year existed between UAE group and surgery group; while the differences in the incidence of minor complications within one year and the need of further intervention rate during 2 -5 years were statistically significant, the occurrence of these minor complications in UAE group being higher than that in the surgery group. The pregnancy rate and live-birth rate in UAE group were significantly lower than those in the surgery group. Conclusion Compared with surgery, UAE has many advantages, such as technically-safe, excellent therapeutic effect, simply-manipulated, easy-nursing, mini-invasive, quick-recovery and easily-accepted by patient, etc. although there is risk of receiving intervention treatment again. The influence of UAE on the fertility, the influence of different embolic agents on the therapeutic effect of UAE, and the effectiveness of UAE for multiple uterine fibroids need to be further studied.

Résumé

OBJECTIVE To discuss the role of head hygiene for nosocomial infection control in operating room.METHODS The head band was weared to improve head hygiene, and enhance the consciousness in control nosocomial infection.RESULTS From Mar 2004 to Mar 2006 there were 2534 cases operated and 23 cases suffered incision infection(0.91%) when surgeon was without use of head band;but from Mar 2004 to Mar 2006 there were 3014 cases operated and the incision infection rate was 0.40%(12 cases) when surgeon was with use of head band.It was an effective method for reducing sweat-inducing contamination by wearing head band to control nosocomial infection in operating room.CONCLUSIONS To improve the head hygiene by wearing head band is arelatively simple practice,it can control nosocomial infection in operating room.

Résumé

@# ObjectiveTo adopt a new suture technique the core long-distance buried for repair of flexor tendon rupture which was designed by author and could lessen adhesion.Methods60 human cadaver flexor digitorum tendons were divided into 3 groups and sewed by core long-distance buried suture, Kessler's technique and Bunnell's technique respectively. Instron-testing machine was used to test gap formation force, 2 mm gap formation force and maximum load. 30 hen second and third toe flexor profundus tendons were divided into 2 groups, each group of tendons were transected and repaired by core long-distance buired suture and Kessler's technique respectively. Information of tendon healing and tendon adhesion was observed in the 3rd, 4th and 6th weeks after operation.ResultsIn the group of using core long-distance buried suture technique, the gap formation, 2 mm gap formation and maximum load in were higher than that using Kessler's technique and Bunnell's technique, the effects of lessening adhesion and promoting tendon healing were better than that in the Kessler's technique group.ConclusionCore long-distance buried suture can offer great tensile strength needed by clinic, and allow early motion and lessen adhesion after operation.

Résumé

Objective To investigate the efficacy of radiofrequency ablation in the treatment of meniscus injury. Methods 42 cases of damaged meniscus of knee joint were treated with radiofrequency ablation under arthroscopy. Results After 15 to 30 months(mean 25.2 months) follow-up, 38 knees ( 90%) were evaluated as excellent, and 4 (10%) as good. Conclusion Radiofrequency ablation was a safe, simple and effective technology for the treatment of meniscus injury of knee joints.

Résumé

Objective To observe the early clinical outcome of the lumbar spondylolisthesis treated with decompressive laminetomy, intertransverse process arthrodesis with bone grafting, interbody fusion cage and solid transpedicle SOCON instrumentation. Methods From December 1997 to December 1999, 38 cases of lumbar spondylolisthesis were treated with decompression for spine canal stenosis. And intertransverse process arthrodesis was done by means of bone chip grafting obtained from the iliac crest, or the interbody fusion cage and the SOCON system. According to Meyerding classfication of lumbar spondylolisthesis, 32 cases were classified as degree 1, 6 cases as degree 2. The levels of lumbar spondylolisthesis was between L 4 and L5 in 29 patients and between L 5 and S 1 in 9 cases. Results The patients were followed up from 14 to 26 months with an average of 18.2 months. The results showed that the pre operative clinical symptoms disappeared completely in 31 of 38 patients. Pain relief was seen in 81.6%. 32 cases with degree 1 and 4 of 6 cases with degree 2 obtained the complete reduction of the slide intervertebral body. Anatomical reduction rate was 94.7%. The other 2 cases of the degree 2 patients obtained 90% of reduction. No infection or neurologic complication occurred in this series. Conclusion Treatment of lumbar spondylolisthesis with SOCON system has satisfactory reduction rate and excellent clinical results.
