Objective To explore the relationship between the establishment of collateral circulation caused by iliac vein compression syndrom(IVCS) and the deep venous thrombosis (DVT). Methods Different types of ideal collateral circulation models and IVCS patient-specific models were numerically simulated using computational fluid dynamics (CFD) in combination with the blood stasis model. The relationship between blood retention and collateral types and cross-sectional area was studied, and the relationship with thrombosis was explored. Results Wall shear stress (WSS) at the distal end part of each ideal model was 0.3 Pa. After four cardiac cycles, the residual blood stayed at the stenosis and the distal end part for the lumbar ascending and pelvic type models, the old blood volume fraction (OBVF) varied with collateral cross-sectional areas, ranging from 5%-90% and 70%-80%, respectively. The OBVF of the coexistence model was above 80%. The WSS at the distal end part of the patient-specific model was 0.9 Pa, and the OBVF at the distal end part was 51.5%. Conclusions The stenosis and the distal end part are most prone to blood stasis, and closely related with DVT. The larger the collateral cross-sectional area, the more serious the blood stagnation. Blood stagnation of the coexistence model is higher compared with the model with lumbar ascending type and pelvic type.

Spinal metastases may cause pain,pathological fracture and/or neurological symptoms.The most common treatment of spinal metastases is conservative therapy.Surgical intervention is necessary when there are pathological and/or neurological deficits.Total en bloc spondylectomy (TES) has lower local recurrence rate and may be applicable to a patient with single metastatic tumor and a long survival expectancy,While debulking surgery is less risky and higher recurrence rate.Conventional radiotherapy is a kind of effective supplement to reduce the local recurrence rate in spinal metastases.However,the local dose is often failed to reach the effective dose because of the close connection of tumor and spinal cord and it is less effective for relatively radioresistant histologies.With the development of radiotherapy technology,spinal radiotherapy has become increasingly accurate to avoid of spinal cord injury.In addition,conventional radioresistant tumors has become treatable with high-dose stereotactic body radiotherapy (SBRT).At the same time,SBRT is also an important basis for the implementation of separation surgery of spinal metastases.Separation surgery is to separate the tumor and dura and achieve circumferential decompression and stabilization through posterior approach.As a result,a high-dose SBRT can be performed because of the small interspace to protect the spinal cord,which greatly reduces the local recurrence rate.Compared with TES,separation surgery combined with high-dose radiotherapy not only reduces the difficulty of surgery,but also has a good local control rate,which is a relatively "minimally invasive" strategy.Separation surgery is particularly suitable for patients with high-grade epidural spinal cord compression and moderately sensitive or insensitive to radiotherapy.Although the concept of separation surgery has been proposed for several years,only a few applications have been reported in foreign literature and fewer in domestic literature.This article aims to summarize the relevant concepts and clinical applications of separation surgery in spine metastases and to provide a reference for the treatment of spinal metastases.

<p><b>OBJECTIVE</b>To investigate the molecular?mechanisms by which triptolide induces apoptosis of human acute T lymphocytic leukemia Jurkat cells.</p><p><b>METHODS</b>MTT assay was employed to detect the proliferation inhibition of Jurkat cells by triptolide, and the IC50 was calculated by OriginPro8. Flow cytometry was used to analyze apoptosis of Jurkat cells. Np9 mRNA levels were detected by RT-PCR and analyzed quantitatively by Kodak 1D 3.6 software. Correlation between the inhibition of Np9 transcription and the cell apoptosis was analyzed by SPSS 19.0.Western blotting was employed to determine Np9 downstream signaling molecules c-myc, β-catenin, ERK, AKT and Notch1 protein level in Jurkat cells after exposure to different concentrations of triptolide for 48 h.</p><p><b>RESULTS</b>Triptolide treatment resulted in dose-dependent inhibition of Jurkat cells proliferation and its IC50 was 12.7 nmol/L. Triptolide induced apoptosis of Jurkat cells in dose- dependent manner. Furthermore, triptolide inhibited Np9 mRNA transcription level in Jurakt cells in a dose-dependent manner. There was a correlation between the triptolide-mediated the apoptosis and the inhibition of Np9 transcription of Jurkat cells (R(2)=0.907). Western blotting results displayed that triptolide inhibited transcription levels of Np9 mRNA with a concomitant decrease of its downstream signaling molecules c-myc, β-catenin, ERK, AKT and Notch1 at protein levels.</p><p><b>CONCLUSION</b>Inhibition of HERV-K Np9 mRNA and its downstream signaling molecules c-myc, β-catenin, ERK, Akt and Notch1 protein might be one of important molecular?mechanisms by which triptolide induces apoptosis of human acute T lymphocytic leukemia Jurkat cells.</p>

Résumé

DHA (22:6n-3) is a Ω-3 polyunsaturated fatty acid with 22 carbon atoms and 6 double bonds, which has important biological functions in human body. Human and other mammals synthesize only limited amounts of DHA, more requirements must be satisfied from food resources. However, the natural resources of DHA (Mainly deep-sea fish and other marine products) are prone to depletion. New resources development is still insufficient to satisfy the growing market demand. Previous studies have revealed that the mammals can increase the synthesis of DHA and other long-chain polyunsaturated fatty acids after transgenic procedures. In this study, mammalian cells were transfected with Δ6, Δ5 desaturase, Δ6, Δ5 elongase, Δ15 desaturase (Isolated from nematode Caenorhabditis elegans) and Δ4 desaturase (Isolated from Euglena gracilis), simultaneously. Results show that the expression or overexpression of these 6 enzymes is capable of conversion of the o-6 linoleic acid (LA, 18:2n-6) in DHA (22:6n-3). DHA content has increased from 16.74% in the control group to 25.3% in the experimental group. The strategy and related technology in our research provided important data for future production the valuable DHA (22:6n-3) by using genetically modified animals.

Objective To evaluate the efficacy of solifenacin with or without tamsulosin in patients with overactive bladder (OAB).Methods Fifty-three patients with OAB were randomly divided into two groups (group A 27 patients, group B 26 patients).The patients received either solifenacin 5 mg/day in group A or combined with Tamsulosin 0.2 mg/day in group B for two weeks.During the treatent period all the patients recorded voiding and adverse events in a diary.The symptoms of urgency, frequency, incontinence were evaluated by OABSS scores at the begining and end of the therapy period.The results of the efficacy and safety were analyzed by using SPSS 13.0.Results The OABSS scores at the begining and end for the two groups were significantly different ( P < 0.01 ).In both groups the efficacy was obvious.The OABSS scores at the end of the therapy between group A and B were significantly different (P <0.05 ).The efficacy of group B was better than that of group A.Conclusion The efficacy of solifenacin combined with tamsulosin was better than solifenacin alone in OAB.

Objective To afford an new effective imaging technique for the morphological observation of the normal male pelvic floor by observing the normal male puborectalis with transrectal ultrasound and magnetic resonance imaging (MRI). Methods Thirty-six normal males were examined by transrectal ultrasound and MRI,data of puborectalis were recorded and compared including starting point, end point,appearance, structure, thickness and the relationship with peripheral organs. Results The results including the appearance,direction and internal echo of puborectalis and the relationship with peripheral organs can be clearly displayed by transrectal ultrasound compared with MRI. The thickness of right and left puborectalis at rest measured in ultrasound and in MRI were (6.22±0.57)mm, (6.19±0.59) mm, (6.20±0.61) mm and (6.17±0.53) mm, respectively. The thickness by two methods and two sides had no significant differences (P>0.05). The thickness of puborectalis measured by two methods did not correlate with age,height,body weight,the volume of prostate and interspinal diameter respectively. Conclusions Transrectal ultrasound is a new imaging method to observe male puborectalis. Identification for nomal puborectalis sonography is the foundation of diagnosis pelvic floor disease.
