Aim To investigate whether alisol A (AA) could improve the blood brain barrier (BBB) mediated cortex cerebral ischemia-repeifusion injury (CIRI) by inhibiting matrix metalloproteinase 9 (MMP-9). Methods The global cerebral ischemia- reperfusion (GCI/R) model in mice was established, and the AA was intragastric injected subsequently for seven days. The modified neurological severity scores (mNSS), open field test and Y-maze test were applied to detect neurological function. Magnetic resonance spectroscopy (MRS) was used to detect relevant neu- rosubstance metabolism in cortex of mice. Transmission electron microscope (TEM) was employed to observe the ultrastructure of BBB in cortex. Western blot and immunohistochemistry were used to detect the MMP-9 level in cortex. The binding possibility of A A and MMP-9 was determined by molecular docking. Results Compared with Sham group, mice in GCI/R group have an increased mNSS score but decreased at total distance and center distance to total distance ratio in open field test as well as alternation rate in Y-maze test (P<0.01). While mice in GCI/R + AA group have a decreased mNSS score but increased at total distance and center distance to total distance ratio in open field test as well as alternation rate in Y-maze test (P<0.01) compared with GCI/R group. MRS results found that in cortex of GCI/R group mice, the level of GABA and NAA significantly decreased while the Cho, mI and Tau level increased (P<0.01). Whereas in GCI/R + AA group mice, the GABA and NAA level increased and the Cho, ml and Tau decreased significantly (P<0.01). By TEM we observed that the basilemma of cerebral microvessels collapsed, the lumen narrowed, the endothelial cells were active and plasma membranes ruffled, gaps between cells were enlarged and tight junctions were damaged and the end feet of astrocytes were swollen in GCI/R group mice. While in GCI/R + AA group mice, the lumen was filled, plasma membranes of endothelial cells were smooth, tight junctions were complete and end feet of astrocytes were in normal condition. Western blot and immunohistochemistry both found that the MMP-9 level increased in GCI/R group mice (P < 0.01) and decreased in GCI/R + AA group mice (P < 0.05). Molecular docking proved the binding between aliso A and MMP9 through TYR-50 and ARG-106, and the binding energy was calculated as -6.24 kcal · mol

Objective@#To explore the pre-exposure prophylaxis (PrEP) awareness, willingness, uptake patterns, barriers and needs among Chinese student men who have sex with men (MSM), so as to provide relevant evidence for targeted interventions with PrEP.@*Methods@#A proportional sampling method was used to conduct a cross sectional survey of MSM aged 16 years and older residing in 21 provinces, municipalities, and autonomous regions in mainland China between October 20 and December 20, 2021, to collect information on demographic and sexual behavioral characteristics, and 923 students of them were selected as the subjects of this study. The chi-square test and Fisher s test were used for univariate analysis, followed by multivariate Logistic regression to analyze the influencing factors of PrEP awareness and uptake.@*Results@#According to the cascade analysis method, the awareness, willingness, uptake and adherence rates of PrEP among the student MSM were obtained as 88.95%, 67.36%, 13.20% and 45.21 %, respectively. HIV testing more than once in the last 3 months, and all of them were aware of the HIV test results of their sexual partners, and those with high frequency of condom use had a higher rate of awareness ( OR =2.32, 1.79, 1.69, P <0.05). Willingness rates were higher for those from the pilot city, using substances, and HIV testing more than once in the last 3 months ( OR =2.13, 1.65, 1.69, P <0.05). Higher rates of uptake were found among those from high risk and pilot areas, presence of commercial sex, substance use, and high literacy ( OR =5.60, 3.54,2.81, 1.92, 4.54, P <0.05). Adherence rates were higher among those who used one pill per day or both ( OR =12.77, 13.26, P <0.05). The barriers and needs faced by student MSM were primarily personal concerns about medication side effects, preventative effects, and confidence in sexual behavioral styles, and the high cost of medication and related service costs.@*Conclusions@#The student MSM population in China is characterized by high awareness, low willingness, low uptake, and low adherence to PrEP. Targeted interventions should be considered and tailored by the departments to facilitate PrEP promotion among student MSM.

Objective@#To explore the performance of machine learning prediction models in forecasting student absenteeism due to respiratory symptoms caused by air pollution in short term, aiming to provide a methodological reference for early warning systems of school diseases.@*Methods@#Utilizing data from shortterm sequences of student absenteeism due to respiratory symptoms in Jiangsu Province from September 2019 to October 2022, the study integrated average concentrations of atmospheric pollutants. A univariate distributed lag nonlinear model was employed to select optimal lag variables for the pollutants. An extreme gradient boosting(XGBoost) algorithm model was developed to predict the frequency of absenteeism due to respiratory symptoms and compared with the seasonal autoregressive integrated moving average with exogenous factors(SARIMAX) model.@*Results@#Between 2019 and 2022, an average of 9 709 students per day in Jiangsu Province were absent due to respiratory symptoms. The daily average air quality index (AQI) was 76.96,with mass concentrations of PM2.5, PM10, NO2, and O3 averaging at 35.75, 61.13, 28.89, 104.81 μg/m3, respectively. Granger causality tests indicated that AQI, PM2.5, PM10, NO2, and O3 were significant predictors of absenteeism frequency due to respirutory symptoms(F=1.46,1.79,1.67,3.41,2.18,P<0.01). The singleday lag effects of PM2.5, PM10, NO2, and O3 reached their peak relative risk (RR) values at lag4, lag0, lag0, lag4 respectively. When integrating these optimal lag variables for the pollutants, the XGBoost model demonstrated superior predictive performance to the SARIMAX model, reducing the mean absolute error (MAE) from 2.251 to 0.475, mean absolute percentage error (MAPE) from 0.429 to 0.080, and root mean square error (RMSE) from 2.582 to 0.713; at the P75 percentile alert threshold, the sensitivity improved from 0.086 to 0.694 and specificity from 0.979 to 0.988, with the Youden index increasing from 0.065 to 0.682.@*Conclusions@#The XGBoost model exhibits robust predictive performance and effective early warning capabilities for shortterm sequences of student absenteeism due to respiratory symptoms caused by air pollution. Schools could timely adopt this model to preemptively detect and control disease outbreaks, thereby enhancing school health management.

Objective@#To assess the effectiveness of online and offline myopia prevention and control health education interventions using wearable behavior monitoring tools for non myopic elementary school students,so as to provide evidence based medical support for public health practices.@*Methods@#From May to June in 2021, two schools were selected within the same county in Jiangsu Province. School 1 conducted online and offline parental health education ( n =111), while school 2 exclusively conducted offline health education activities, representing the traditional intervention group ( n =122). Students from both schools underwent monitoring through wearable behavior tracking tools, with feedback reports provided (eye distance, eye duration, ambient light, and outdoor exposure time). Both schools relied on activities to carry out health education interventions, and organized the distribution of promotional materials and display boards. The intervention group also established WeChat groups to conduct online "Healthy Parents Action" (answering and providing feedback on health knowledge related to myopia prevention and control, myopia prevention and control, science popularization, etc. raised by parents). Evaluation criteria included myopia rates, post dilation refractive error, and axial length, with a tracking period of two years (from 2021 to 2023). Additionally, the study collected refractive parameters from non myopic students who did not participate in wearable tool monitoring in the 12 classes across the two schools.@*Results@#The baseline results indicated that there were no significant differences between the two groups in terms of refractive parameters and wearable tool monitoring results (including screen time, viewing distance, outdoor exposure time, and homework light exposure)( t/Z/χ 2=1.94,1.17,0.58,0.40,0.80,0.69,0.32, P >0.05). After a two-year follow up, in the first and second year, the myopia rate of the online Healthy Parents Action group (11.4%, 29.7%) were lower than that of the traditional group (26.2%, 50.9%), and the degree of refractive change in the intervention group [0.63(0.38,1.19)D] was lower than that of the traditional group [0.91(0.40,1.50)D], and all the differences were statistically significant( χ 2/ Z =4.93,10.37,2.29, P <0.05). However, there were no significant differences ( P >0.05) in axial length changes between the two groups over the twoyear intervention period. Nevertheless, in the second year, the axial length change in the traditional group [0.35(0.20,0.65)mm] was lower than that in the natural observation group [0.55(0.30,0.75)mm], and this difference was statistically significant ( Z =1.92, P <0.05).@*Conclusions@#Online and offline myopia prevention and control health education can effectively reduce myopia rates. The intervention mode combining wearable behavior monitoring tools with online health education may have better effects, but further large sample and multi center studies are needed to provide additional evidence and confirmation.

To discover new structural hits, based on the important role of pyrazole ring and fragment of pyridinone carboxylic acid in drug design, novel title pyrazolo[3,4-b]pyridine-4-one-5-carboxylic acid derivatives (10a-10p) were designed and synthesized, the structures were confirmed by spectral data and elemental analyses. The antibacterial and antitumor activities were evaluated by the measured minimum inhibitory concentration (MIC) values against the tested four strains and half inhibitory concentration (IC50) values against the tested four cancer cells, respectively. The results displayed markedly poor antibacterial activity and observably potent antitumor activity. In particularly, the title difluorophenyl (10d, 10e, 10f), pyridyl (10j), ethyl (10k) and cycloproyl (10l) compounds exhibited comparable activity against Capan-1 and A549 cells to that of the comparison doxorubicin. Thus, pyrazolo[3,4-b]pyridine-4-one-5-carboxylic acid derivatives as promising antitumor hits need to be developed.

Objective:To explore the distribution characteristics of traditional Chinese medicine (TCM) syndromes of abnormal uterine bleeding associated with ovulatory dysfunction (AUB-O). Method:A clinical and epidemiological investigation was conducted in 6 688 patients with AUB-O from the provincial, municipal, and county/district hospitals in 29 provinces, autonomous regions, and municipalities across China to identify the distribution characteristics of their TCM syndromes. Result:The AUB-O patients nationwide were mainly differentiated into the kidney Qi deficiency syndrome (17.34%), the spleen Qi deficiency syndrome (13.25%), the Qi and blood deficiency syndrome (12.62%), the Qi deficiency and blood stasis syndrome (8.45%), and the kidney Yin deficiency syndrome (6.88%). AUB-O resulted from Qi deficiency, Yin deficiency, and blood deficiency, often involving the kidney, spleen and liver. The analysis of the regional distribution of TCM syndromes in AUB-O patients revealed that kidney Qi deficiency, spleen Qi deficiency, and Qi and blood deficiency were the shared syndromes. However, due to regional discrepancy, the TCM syndrome varied widely from one geographic region to another. The kidney Qi deficiency syndrome was more frequently seen in North China, Northwest China, Southwest China, and East China, but less frequently in central China, Northeast China, and South China. The spleen Qi deficiency syndrome occurred most frequently in central China, while the East China had the highest frequency of Qi and blood deficiency syndrome. The spleen and kidney deficiency syndrome was mostly present in central China, North China, and Southwest China, the Qi deficiency (kidney deficiency) and blood stasis syndrome in Northwest China, South China, and North China, the kidney Yin deficiency syndrome in East China, Northwest China, and Northeast China, the deficiency-heat syndrome in Southwest China and East China, the kidney Yang deficiency syndrome in South China, the liver depression and blood heat syndrome in Northeast China, and the liver depression and spleen deficiency syndrome in central China. The diagnostic criteria for the kidney Qi deficiency, spleen Qi deficiency, Qi and blood deficiency, Qi deficiency and blood stasis, and kidney Yin deficiency syndromes were not significantly different from the previous ones. The distinctive symptoms for the kidney Qi deficiency syndrome were irregular vaginal bleeding, heavy menstrual flow, or shortened menstrual cycle, back soreness and pain, and forgetfulness, while those for the spleen Qi deficiency syndrome mainly included the shortened menstrual cycle, mental fatigue, lack of strength, poor appetite, loose stool, and white tongue coating. The Qi and blood deficiency syndrome were mainly manifested as the shortness of breath, laziness to speak, pale complexion, dizziness, and palpitation. The Qi deficiency and blood stasis syndrome were mainly judged by the scanty menstrual flow frequently or occasionally accompanied by blood clots, mental fatigue, lack of strength, and dark purple tongue. The ovulatory bleeding, dizziness, tinnitus, vexing heat in chest, palms and soles, and night sweat were the characteristic signs for the kidney Yin deficiency syndrome. Conclusion:There exist certain rules in the geographical distribution of TCM syndromes of AUB-O patients, which has provided a reference for the clinical treatment of AUB-O in accordance with the local conditions.

BACKGROUND: Osteoarthritis (OA) is one of the most common rheumatic diseases of which clinical symptoms includes swelling, synovitis and inflammatory pain, affect patients' daily life. It was reported that non-coding RNAs play vital roles in OA. However, the regulation mechanism of ncRNA in OA pathogenesis has not been fully elucidated. METHODS: The expression of SNHG7, miR-34a-5p and SYVN1 was detected using qRT-PCR in tissues, serum and cells. The protein expression of SYVN1, PCNA, cleavage-caspase 3, beclinl and LC3 were measured using western blot. The RNA immunoprecipitation (RIP), RNA pulldown, and luciferase reporter assays were used to verify the relationship between SNHG7, miR-34a-5p and SYVN1. The MTT and flow cytometry assay was performed to detected cell proliferation and cell apoptosis respectively. RESULTS: In this study, SNHG7 and SYVN1 expression were down-regulated, but miR-34a-5p was up-regulated in OA tissues and IL-1P treated cells compared with normal tissues and chondrocyte. Functional investigation revealed that up-regulated SNHG7 or down-regulated miR-34a-5p could promote cell proliferation and inhibit cell apoptosis and autophagy in OA cells. More than that, RIP, pulldown and luciferase reporter assay was applied to determine that miR-34a-5p was a target miRNA of SNHG7 and SYVN1 was a target mRNA of miR-34-5p. Rescue experiments showed that overexpression of miR-34a reversed high expression of SNHG7-mediated suppression of apoptosis and autophagy as well as promotion of proliferation, while its knockdown inhibited cell apoptosis and autophagy and promoted cell proliferation which could be impaired by silencing SYVN1. In addition, SNHG7 regulated SYVN1 through sponging miR-34a-5p. CONCLUSION: SNHG7 sponged miR-34a-5p to affect cell proliferation, apoptosis and autophagy through targeting SYVN1 which provides a novel sight into the pathogenesis of OA.

Hepatocellular carcinoma is one of the most prevalent malignancies and a leading cause of cancer-related mortality worldwide. However, the therapies to prevent hepatocellular carcinoma are still limited and the emergence of drug resistance leads to the development of new anti-cancer drugs and combinational chemotherapy regimens. Our study was aimed to explore the anticancer effects of the essential oil extract (EEEO) from Euphorbia esula which has been widely used in traditional Chinese folk medicine and possessed potential cytotoxic effects in several human tumor cells. However, the mechanisms of EEEO-induced anti-proliferation and apoptosis have not been completely elucidated. In this study, EEEO was prepared by hydro-distillation and the main chemical component of EEEO was identified by GC-MS. HepG2 cells were treated with EEEO in vitro and then evaluated with respect to proliferation, apoptosis, and levels of reactive oxygen species (ROS) and apoptotic proteins. Our studies showed that EEEO decreased cell viability, elevated ROS levels, and induced apoptosis of HepG2 cells in a concentration- and time-dependent manner. Furthermore, Bcl-2 was down-regulated, while Bax was up-regulated in HepG2 after EEEO treatment. These results suggest that EEEO induced apoptosis of HepG2 cells and indicate that this apoptosis might be mediated by the mitochondrial pathway.

Objective@#This study aimed to evaluate the validity and predictive value of different methods for myopia screening, and to provide the most effective method of myopia screening.@*Methods@#A total of 7 441 students of grade 1-12 selected from 36 primary and middle schools in 12 counties in Jiangsu Province were enrolled in this study for ophthalmological examination, and uncorrected visual acuity (UCVA, Plan A), noncycloplegic autorefraction (NCAR, Plan B) and combination of UCVA and NCAR (Plan C) were included in the examination. Receiver operating characteristics (ROC) curve was performed to determine the cut-off values (The gold standard is cycloplegic refraction: SE≤0.5 D), and positive predictive value, sensitivities, specificities and other indicators of three screening tests were compared and calculated.@*Results@#The AUC of the combination test for uncorrected visual acuity(UCVA) and noncycloplegic autorefraction (NCAR) was nearly 1(0.91, 95%CI=0.90-0.92), and the sensitivity, specificity, Yoden index, diagnostic coincidence rate, positive predictive value and negative predictive value were 92.4%,89.5%,0.82,91.1%,91.3%,90.8%, respectively, but the index of the younger pupils in primary schools was relatively low. The result of screening plan A for students of middle and high school students was the closest to that of myopia diagnosis.@*Conclusion@#Combination test, which was recommended by China National Health Commission, can be a practical solution for school students for myopic screening. Under the condition of no mydriatic refraction, Plan A could be considered for myopia screening especially for middle and high school students. For the younger pupils, it is suggested to use the internationally recommended myopia screening strategy, that is, SE ≤ 0.50D after mydriasis.

Objective@#To explore the association of social bonds, deviant behaviors and bullying victimization among children, and to provide practical reference for deviant behavior and bullying victimization prevention among children.@*Methods@#A questionnaire survey was conducted among 1 044 students from 8 schools in Hubei Province through empirical stratification and non-strict random sampling. Among them, 612 were left-behind children and 427 were non-left-behind children.@*Results@#There was a positive correlation between deviant behavior and bullying victimization (r=0.17, P<0.01); The average level of positive cognition of left-behind children(2.95) was significantly lower than that of non-left-behind children(3.07) (t=-2.31, P<0.05); Regression analysis showed that participation in daily housework and deviant behavior had a significant positive impact on bullying victimization of left-behind children (β for deviant behavior was 0.55, P<0.01; β for participation in daily housework was 0.27, P<0.05). Relation with peers could significantly negatively predict bullying victimization among left-behind children (β for peer relationship was -0.25, P<0.05). In the deviant behavior model, bullying victimization had a significant positive impact on deviant behavior among left-behind children (β for bullying was 0.58, P<0.01); Parent-child relationship and teacher-student attachment could significantly negatively predict deviant behavior of left-behind children (β for parent-child relationship was -0.34, P<0.01; β for teacher-student relationship was -0.49, P<0.01).@*Conclusion@#The vulnerability of social bonds, deviant behavior and bullying victimization shows intercorrelation among children. Attachment is the common factor that affects deviant behavior and bullying victimization among left-behind children. Participation in daily housework has a unique impact on the bullying experience of left-behind children.
