Résumé

Abstract Objective: To assess the need for orthodontic treatment among Nepalese high school students. Material and Methods: This is a quantitative, cross-sectional descriptive study. The sample comprises 938 children (537 males and 401 females) with an age group above 14 years. The subjects were selected voluntarily from seven different schools of Kathmandu valley using a multistage sampling technique. The Index of Orthodontic Treatment Need comprises two components: Dental Health Component (DHC) and Aesthetic Component (AC). Two trained and calibrated examiners performed the oral examination. Results: On analysis of the DHC component, it was found that 21% had no need, 18.1% had mild/little need, 24.3% had moderate/borderline need, 35.8% had severe need, and 0.7% had extreme treatment need. Similarly on analysis of AC component, it was found that 33% were AC-1, 30.8% were AC-2, 7.2% were AC-3, 8.2% were AC-4, 2.1% were AC-5, 3.6% were AC-6, 1.8% were AC-7, 7.4% were AC-8, 1.8% were AC-9, and 3.9% were AC-10. Conclusion: The Index of Orthodontic Treatment Need can be used as a tool for planning dental health resources and prioritizing the treatment need of different populations (AU).

Résumé

We report a rare presentation of transitional cell carcinoma of kidney with paraneoplastic fever, leukemoid reaction and thrombocytosis. Description of the case highlights an unusual clinical scenario where fever, leucocytosis, pyuria and raised procalcitonin levels at presentation in a patient with transitional cell carcinoma of kidney may mislead diagnostic work up toward an infective cause (i.e. pyelonephritis). This case will guide clinician to keep a high index of suspicion, in case they encounter such a situation.

Objectives: To describe central venous catheter (CVC) colonizations and catheter-related bloodstream infections (C-RBSIs) among patients admitted in the Intensive Care Unit (ICU) of a tertiary care teaching hospital. Materials and methods: This was a cross-sectional study conducted among critically ill patients admitted in ICU. The semi-quantitative method was used for catheter tip culture. The definitions of catheter infection and colonization were based on the Centre for Disease Control Blood Stream Infection Guidelines. Results: The study population comprised 75 ICU patients whose CVCs had been placed. The incidence of CVC-related colonizations and bloodstream infections was observed to be 42.7% (32/75) and 17.3% (13/75) respectively. Coagulase-negative staph was the most common organism found causing CVC colonization (50%) and C-RBSI (61.5%). Conclusion: Coagulase-negative staphylococci are the most frequent microorganisms which colonize the CVC. The findings of this study may help with implementation of educational and training programs on central line-associated bloodstream infections (CLABSIs) for health care personnel and enable better management of these devices with regard to the prevention, diagnosis, and treatment of CLABSIs.

Résumé

Background & objectives: Serum prostate specific antigen (PSA) levels are known to vary with race and ethnicity, environmental factors, lifestyle, metabolic and physiologic changes and advancing age. This study was designed to determine age specific serum PSA level in healthy Indian men and its comparison with that reported in different populations of the world. Methods: A total of 1300 adult men who attended Executive Health Check-up programme in a tertiary care hospital in Haryana, India, were included in the study. Forty seven men were excluded from the analysis because of urological conditions affecting PSA values. Overall, 1253 men were analyzed for age specific PSA values. Results: The age specific reference range of serum PSA values was 0.71 ng/ml in those younger than 40 yr; 0.85 ng/ml in 40-49 yr; 1.13ng/ml in 50-59 yr group; 1.45 ng/ml in 60-69 yr group; 1.84 ng/ml in 70-79 yr group and 2.35 ng/ml in men older than 80 yr. Interpretation & conclusions: Our study provided the age-specific reference range of serum PSA in healthy men in India. The data suggested that the PSA levels were associated with increasing age.

Résumé

Liposarcoma is the most common pathology seen in the soft tissue sarcoma of retroperitoneum. These tumors have been traditionally treated with radical surgery sacrificing adjacent organs to achieve clear margins. We have reported our experience of renal sparing surgery for perirenal liposarcoma in two patients with more than 24 month disease free survival.

Résumé

Eggshell calcification of kidney in case of ureteropelvic junction obstruction (UPJO) is an uncommon finding with only a few cases reported in literature. We report a thirty-year-old symptomatic man with curvilinear calcification in hydronephrotic right kidney. Thorough investigations to rule out genitourinary tuberculosis and hydatid disease of kidney were performed prior to the definitive management by laparoscopic approach.
