Objective:To explore and construct the nursing quality standards of spine surgery, provide a scientific basis for improving the nursing quality of spine surgery.Methods:Taking the "structure-process-outcome" three-dimensional quality model as the theoretical framework, through evidenced-based literature search and semi-structured interview, the items pool was formed, and finally the nursing quality standards were determined by two rounds of Delphi method of expert inquiry from May to July 2023.Results:The positive coefficients of the two rounds of inquiries to experts were 0.90 and 1.00, respectively. The authority coefficients were 0.946 and 0.951, respectively. The Kendall coefficients of the first, second and third level standards in the first round were 0.161, 0.095 and 0.108, respectively, and the Kendall coefficients of the first, second and third level standards in the second round were 0.162, 0.088 and 0.100, respectively, the differences were statistically significant (all P<0.05). The final nursing quality standards were finally constructed, including 3 first-level standards (structural quality, process quality and outcome quality), 20 second-level standards and 60 third-level standards.Conclusions:The nursing quality standards of spine surgery based on the three-dimensional quality model of "structure-process-outcome" have high reliability and science, which can provide target incentives for clinical nurses, provide a basis for the quality of specialized spine nursing, and promote the development of nursing specialization and the construction of nursing discipline.

Objective:To investigate the effect of rehabilitation exercise combined with resistance training on the postoperative recovery and quality of life among patients with pulmonary nodules.Methods:A randomized controlled study was conducted on 90 patients with pulmonary nodules who underwent thoracoscopic resection of pulmonary nodules at Zhejiang Veteran Hospital between January 2022 and April 2023. Patients were randomly allocated into an observation group and a control group, with 45 patients in each group using the random number table method. The control group underwent routine rehabilitation exercise, whereas the observation group received resistance training combined with routine rehabilitation exercise. All patients were treated for 1 week. The incidence of postoperative complications and the changes in lung function, exercise endurance, and quality of life from baseline levels were compared between the two groups.Results:The incidence of postoperative complications in the observation group was significantly lower than that in the control group [6.7% (3/45) vs. 24.4% (11/45), χ2 = 5.41, P = 0.020). After intervention, the forced expiratory volume in 1 second, maximal voluntary ventilation per minute, and forced vital capacity in the observation group were (83.84 ± 4.35)%, (96.53 ± 3.45) L/min, and (2.87 ± 0.16) L, respectively, which were higher than those in the control group [(78.98 ± 4.01)%, (92.13 ± 3.08) L/min, (2.62 ± 0.19) L, t = -5.51, -6.38, -6.75, all P < 0.001]. Additionally, the modified Medical Research Council dyspnea score in the observation group was (0.42 ± 0.13) points, which was significantly lower than that in the control group [(0.87 ± 0.19) points, t = 13.11, P < 0.001). The modified Barthel index score in the observation group was significantly higher than that in the control group [(89.53 ± 3.67) points vs. (82.94 ± 4.23) points, t = -7.89, P < 0.001). Conclusion:The combination of rehabilitation exercise and resistance training can effectively enhance lung function, exercise endurance, and overall quality of life in patients with pulmonary nodules. Furthermore, this combined therapy markedly reduces postoperative complications.

ObjectiveTo investigate the effects of whole body vibration training on postural stability in patients with chronic ankle instability (CAI) using meta-analysis. MethodsRandomized controlled trials about whole body vibration training on patients with CAI in relation to postural stability were retrieved from PubMed, Cochrane Library, Web of Science, Physiotherapy Evidence Database (PEDro) scale, CNKI and WanFang data from inception to November, 2022. Reference lists of included studies were also traced to supplement the relevant literature. Two researchers independently searched the literature, and extracted relevant data. The PEDro scale was used to assess the quality, and the Cochrane Handbook was used to assess the risk of bias. The meta-analysis was conducted using Review Manager 5.3. And Stata 15.0 was used to conduct the Egger's test to assess publication bias. ResultsTwelve randomized controlled trials involving 440 individuals were included. Compared with the other exercises or no exercise, whole body vibration training could improve the center of pressure-area (MD = -0.70, 95% CI -0.97 to -0.43, P < 0.001) and the Star Excursion Balance Test in the direction of anterior (MD = 6.16, 95% CI 3.88 to 8.44, P < 0.001), posteromedial (MD = 6.22, 95% CI 3.68 to 8.76, P < 0.001), posterolateral (MD = 6.68, 95% CI 4.28 to 9.08, P < 0.001), anteromedial (MD = 8.78, 95% CI 6.71 to 10.85, P < 0.001), medial (MD = 4.16, 95% CI 1.46 to 6.86, P = 0.002), posterior (MD = 6.69, 95% CI 3.81 to 9.58, P < 0.001), lateral (MD =12.37, 95% CI 8.09 to 16.65, P < 0.001) and anterolateral (MD = 5.29, 95% CI 1.44 to 9.13, P = 0.007). However, there was no significant difference in the overall stability index (MD = 0.02, 95% CI -0.05 to 0.09, P = 0.643) and hop-test (MD = 6.24, 95% CI -8.80 to 21.28, P = 0.416) between whole body vibration training and other exercises or no exercise. ConclusionWhole body vibration training can improve the range of all directions of the Star Excursion Balance Test in individuals with CAI. However, more studies are needed to determine the improvement of center of pressure-area.

Early-onset scoliosis(EOS) refers to scoliosis diagnosed under 10 years old, and early intervention should be carried out for progressive EOS. Surgery is one of the main treatments. Due to the physiological and psychological characteristics of children such as organ structure and function are still in the stage of growth and development, young age and poor compliance, perioperative nursing strategies are different from adults, this article focuses on 13 aspects, including admission and preoperative assessment, safety management, vital signs observation, neurological function monitoring, fluid and electrolyte balance, general anesthesia awakening delirium, pain management, nutrition management, tubing management, position and early mobilization, surgical site infection prevention, identification and treatment of hemopneumothorax, and discharge-related nursing.

  Objective  To evaluate the clinical outcomes and complications of posterior spinal fusion surgery in the treatment of neurofibromatosis type 1(NF1)thoracolumbar kyphoscoliosis, and to explore the mode of perioperative care for nurses provided to the patients.  Methods  We used the retrospective analysis on the 134 patients with NF1 thoracolumbar kyphoscoliosis admitted to our department from March 2012 to April 2022 and analyzed the clinical outcomes and perioperative complications by using the related statistics. We evaluated the Perioperative care by the nurses in the treatment of NF1 to explore the mode of nursing to the patients with the NF1, by using specific observation points and evaluation indicators.  Results  NF1 kyphoscoliosis patients had poor preoperative nutritional status and lung function. NF1 kyphoscoliosis underwent longer operation time, lost more blood in operation, had higher osteotomy grade and more postoperative complications. All the patients successfully completed the operation in our group. The correction rate of scoliosis was (52.8±22.7)%, and the correction rate of kyphosis was (57.3±34.6)%. 25 patients had complications but no such serious complications as nerve damage.  Conclusions  The practice of the perioperative nursing to NF1 type scoliosis patients facilitates the shortening of the recovery period, the prevention or timely detection of complications, and improvement of the therapeutic effect.

Objective:At present,the doctor-patient relationship is tense.The prevalence of negative emotions,such as depression and anxiety,among healthcare workers is increasing every year.Negative attitudes of medical workers toward mental problems may aggravate the doctor-patient conflict and psychological problems of medical workers.This study aims to explore the complex network relationships between outpatient medical workers'attitudes toward mental problems,doctor-patient relationships,and their depression/anxiety levels. Methods:A total of 578 outpatient medical staff from the Second Xiangya Hospital of Central South University(167 males,411 females)completed questionnaires on their attitudes toward mental problems,doctor-patient relationships,and depression/anxiety symptoms.Network analysis was conducted separately to construct the"attitude towards mental problems-doctor-patient relationship network"and"depression-anxiety related network". Results:The edge between"M15(insulting words)"and"D8(waste time)"showed the strongest strength in the"attitude towards mental problems-doctor-patient relationship network",and"M15(insulting words)"had the highest bridge strength in the network.For the analysis of emotional variables,"P1(anhedonia)"showed the most obvious association with"D10(communication difficulties)"in the doctor-patient relationship and"M2(poor quality of life)"in the psychiatric attitudes,and"P1(anhedonia)"was the key bridge symptom in the network. Conclusion:The"insulting words"may be an intervention target for medical workers'attitudes toward mental problems.The"anhedonia"in depression is the potential symptom that needs to be treated.Intervention targeting these variables may be beneficial to improve the mental health level of medical workers and the doctor-patient relationship.

Objective To study the chemical constituents from the herbs of Dendrobium crepidatum and identify its antitumor bioactive compounds.Methods The constituents were extracted by methyl alcohol and isolated by CC and Sephadex LH-20.Their structures were identified by spectroscopic analysis(1H NMR,13CNMR and EIMS).The antitumor activity of the constituents was determined by MTT assay in vitro.Results 9 compouds were obtained and identified as p-hydroxybenzaldehyde(1),p-methoxyphenol(2),p-Hydroxybenzyl alcohol(3),hydroquinone(4),(24R)-ethylcholest-5-en-3-ol-7-one(5),sinalexin(6),bis(2-ethylhexyl)benzene-1,2-dicarboxylate(7),pinoresinol(8),Chaenomin B(9).The antitumor activity of compounds 5~9 was tested,and it was found that compound 9 had good inhibitory activity against A549 cell line.Conclusion Compounds 2~5,7~8 were isolated from the herbs for the first time and compounds 6,9 were isolated from this genus for the first time.IC50 of 9 on A549 cell line is 29.35 μM.

Hip fracture is considered as the most severe osteoporotic fracture characterized by high disability and mortality in the elderly. Improved surgical techniques and multidisciplinary team play an active role in alleviating prognosis, which places higher demands on perioperative nursing. Dysfunction, complications, and secondary impact of anaesthesia and surgery add more difficulties to clinical nursing. Besides, there still lack clinical practices in perioperative nursing for elderly patients with hip fracture in China. In this context, led by the Orthopedic Nursing Committee of Chinese Nursing Association, the Expert consensus on clinical practice in perioperative nursing for elderly patients with hip fracture ( version 2023) is developed based on the evidence-based medicine. This consensus provides 11 recommendations on elderly patients with hip fracture from aspects of perioperative health education, condition monitoring and inspection, complication risk assessment and prevention, and rehabilitation, in order to provide guiding advices for clinical practice, improve the quality of nursing and ameliorate the prognosis of elderly patients with hip fracture.

Hip fractures are among the most common fractures in the elderly, presenting to be a leading cause of disability and mortality. Surgical treatment is currently the main treatment method for hip fractures. The incidence of perioperative malnutrition is increased after hip fractures in the elderly due to the comorbidities, decreased basal metabolic rate, accelerated protein breakdown, weakened anabolism and surgical stress. However, malnutrition not only increases the incidence of postoperative complications, but also leads to increased mortality, indicating an important role of perioperative nursing management of nutrition for the elderly patients with hip fractures. At present, there still lacks scientific guidance and application standards on perioperative nursing management of nutrition for the elderly patients with hip fractures. Therefore, the Orthopedic Nursing Committee of Chinese Nursing Association and the Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Expert consensus on perioperative nursing management of nutrition for elderly patients with hip fractures ( version 2023) according to evidence-based medical evidences and their clinical experiences. Fourteen recommendations were made from aspects of nutrition screening, nutrition assessment, nutrition diagnosis, nutrition intervention and nutrition monitoring to provide guidance for perioperative nursing management of nutrition in elderly patients with hip fractures.

ObjectiveTo explore the clinical effect of core muscle motor control training on postpartum diastasis recti abdominis. MethodsFrom January, 2021 to January, 2022, 30 outpatients with postpartum diastasis recti abdominis were randomly divided into control group (n = 15) and experimental group (n = 15). Manipulative therapy and breathing training were performed simultaneously in both groups. Besides, the control group received conventional core strength training, and the experimental group received core muscle motor control training, for four weeks. Their distance of diastasis recti abdominis, abdominal circumference and waist circumference were compared before and after treatment. ResultsAfter treatment, the distance of diastasis recti abdominis, abdominal circumference and waist circumference reduced in both groups (Z = 3.408, t > 5.927, P < 0.05). The reduction value of diastasis recti abdominis distance was more in the experimental group than in the control group (t = 2.328, P < 0.05). ConclusionCore muscle motor control training can effectively relieve postpartum diastasis recti abdominis, and the effect is better than conventional core strength training.

ObjectiveTo investigate the effect of neuromuscular control training on frozen shoulder. MethodsFrom February to August, 2021, 34 patients with first idiopathic frozen shoulder from the outpatients department of Rehabilitation, Beijing Tongren Hospital were randomly assigned into control group (n = 17) and experimental group (n = 17). Both groups received routine rehabilitation training, and the experimental group received neuromuscular control training in addition, one time a day, five days a week, for six weeks. The shoulder joint function was evaluated by Japanese Orthopaedic Association (JOA) score before and after training. ResultsAfter training, all the sub-item scores and total score of JOA improved in both groups (|t| > 14.835, |Z| > 2.070, P < 0.05), and the JOA sub-item scores of pain, ADL, ROM and total score were higher in the experimental group than in the control group (Z = -2.191, t > 2.060, P < 0.05). ConclusionNeuromuscular control training could improve the shoulder function of patients with frozen shoulder more effectively.

Objective:To explore the surgical technique and therapeutic effect of flap wrapping repair of soft tissue defect after digit degloving injury with free arm medial perforator flap.Methods:From October 2009 to December 2020, 15 patients with soft tissue defect after digit degloving injury were repaired with free arm medial perforator flaps. The patients were 9 males and 6 females aged 22 to 50 years old, with an average of 32 years old. Injury mechanism: 13 cases of machine strangulation and 2 cases of machine crushing. Plane of sleeve avulsion injury: at the distal metacarpophalangeal joint of thumb in 2 cases, at the middle and distal segment of single finger of 2nd to 5th fingers in 4 cases, and at distal metacarpophalangeal joint of 2nd to 5th fingers in 9 cases. Seven cases were in left hand and 8 in right hand. Emergency surgery was performed in 2 cases and scheduled surgery in 13 cases. The flap was designed in a shape of a long strip, and the size of the flap was 5.0 cm×2.5 cm-14.0 cm×3.5 cm. During the operation, the artery of the flap pedicle was anastomosed with the palmar digital proper artery or common digital artery of the recipient digit, the subcutaneous vein or the companion vein of the artery was anastomosed with the dorsal or palmar digital subcutaneous vein, and the medial brachial cutaneous nerve carried in the flap was anastomosed with the stump of palmar digital proper nerve. All the donor sites were directly sutured. After the surgery, follow-up visits were conducted regularly at outpatient clinic, or via telephone and WeChat review or by home visit. The flap appearance, sensation and function recovery of digital joints were observed together with the patient satisfaction. Results:All flaps survived successfully after surgery. Six patients with degloving defect of the whole digit had encountered poor wound healing caused by distal phalangeal necrosis. The second stage stump trimming was performed to keep the digit to the distal end of the middle segment. Two cases of thumb and 2 cases of middle finger suffered further burning and worn tears at 7-9 months after surgery and self-healed. The follow-up period was 6-28 (average 16) months. The flap was soft without bloating. The sensation recovered to S 2-S 3. The shape of fingers was good, and no secondary flap surgery was necessary. The overall movement of interphalangeal joints was poor. According to the evaluation standard of Michigan hand function questionnaire, 15 patients achieved very satisfactory with the overall appearance and function of hands. The linear scars at donor site were hidden without complications such as tenderness and contraction. Conclusion:The free medial perforator flap of the arm is easy to design and thin, hence does not affect the shape of a digit. So, it is ideal for the repair of soft tissue defect after digit degloving injury.

The fall armyworm (FAW), Spodoptera frugiperda, is a destructive pest native to America and has recently become an invasive insect pest in China. Because of its rapid spread and great risks in China, understanding of FAW genetic background and pesticide resistance is urgent and essential to develop effective management strategies. Here, we assembled a chromosome-level genome of a male FAW (SFynMstLFR) and compared re-sequencing results of the populations from America, Africa, and China. Strain identification of 163 individuals collected from America, Africa and China showed that both C and R strains were found in the American populations, while only C strain was found in the Chinese and African populations. Moreover, population genomics analysis showed that populations from Africa and China have close relationship with significantly genetic differentiation from American populations. Taken together, FAWs invaded into China were most likely originated from Africa. Comparative genomics analysis displayed that the cytochrome p450 gene family is extremely expanded to 425 members in FAW, of which 283 genes are specific to FAW. Treatments of Chinese populations with twenty-three pesticides showed the variant patterns of transcriptome profiles, and several detoxification genes such as AOX, UGT and GST specially responded to the pesticides. These findings will be useful in developing effective strategies for management of FAW in China and other invaded areas.

Objective To summarize the experience of respiratory tract management in a case of spinal muscular atrophy with neuromuscular scoliosis. Methods Respiratory tract nursing included prevention of pulmonary infection, inspiratory muscle training, rebuilding respiratory form before surgery, and assessment of sputum expectoration capacity ,pain management, airway management after surgery. Results The patient was discharged without pulmonary complications. Conclusions For patient with breathing problem, respiratory tract management can prevent pulmonary complications during perioperative period.

Objective@#To investigate the effect of lung rehabilitation exercise on cancer-related fatigue and quality of life in patients with non-small cell lung cancer (NSCLC) undergoing chemotherapy.@*Methods@#From October 2016 to October 2018, 82 patients with NSCLC treated in Zhejiang Rongjun Hospital were randomly divided into control group (41 cases) and observation group (41 cases) according to random number table method.The control group was given routine intervention, while the observation group was given lung rehabilitation exercise on the basis of the control group.Both two groups were evaluated after 8 weeks of intervention.The changes of lung function, cancer-related fatigue and quality of life before and after intervention were compared between the two groups.@*Results@#After intervention, the FEV1% and FEV1/FVC in the two groups were higher than those before intervention (observation group: t=18.565, 11.207; control group: t=9.363, 6.028, all P<0.05). After intervention, the FEV1% [(64.38±3.79)%] and FEV1/FVC (69.47±6.54) in the observation group were higher than those in the control group [(56.74±2.85)% and (60.10±4.25)] (t=10.316, 7.692, all P<0.05). After intervention, the scores of body, cognition, behavior and emotion in the two groups were higher than those before intervention (observation group: t=17.663, 15.167, 12.304, 14.750; control group: t=9.869, 6.617, 7.566, 9.553, all P<0.05). After intervention, the scores of body [(6.57±0.31)points], cognition[(6.15±0.42)points], behavior[(6.32±0.31)points] and emotion [(6.28±0.21)points] in the observation group were higher than those in the control group [(5.78±0.28)points, (5.41±0.48)points, (5.78±0.27)points and (5.82±0.25)points] (t=12.109, 7.429, 8.411, 9.021, all P<0.05). After intervention, the scores of social function, cognitive function, emotional function, role function, physical function and general health in the two groups were higher than those before intervention (observation group: t=14.487, 17.845, 22.025, 16.393, 11.665, 14.533; control group: t=9.880, 9.777, 10.797, 6.231, 6.838, 6.512, all P<0.05). After intervention, the scores of social function[(75.48±8.19)points], cognitive function[(72.43±6.32)points], emotional function [(71.93±5.74)points], role function[(63.25±7.86)points], physical function[(69.45±8.97)points] and general health[(68.39±7.54)points] in the observation group were higher than those in the control group [(67.31±6.74)points, (60.38±5.75)points, (59.64±6.56)points, (51.47±10.29)points, (58.83±7.09)points and (56.82±7.38)points] (t=4.932, 9.030, 9.028, 5.825, 5.947, 7.022, all P<0.05).@*Conclusion@#Lung rehabilitation exercise has good clinical effect on patients with NSCLC undergoing chemotherapy.It can alleviate cancer-related fatigue and improve the quality of life of patients.It is worthy of clinical reference.

To investigate the influential factors for depression and anxiety in patients with chronic wounds, and the relationship between depression and anxiety and social support or quality of life.
 Methods: We used convenience sampling method to enroll 150 patients with chronic wounds. Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS), Short Form 36 Health Survey (SF-36) and Social Support Rating Scale (SSRS) were used to assess the level of depression and anxiety, quality of life, and social support in patients with chronic wounds.
 Results: The detection rate of depression and anxiety in patients with chronic wounds was 45.3% and 14% respectively. In patients with chronic wounds, age was positively correlated with SDS and SAS scores (P<0.05), while education, eight factors of SF-36, objective support, subjective support, and social support scores of SSRS were negatively correlated with SDS and SAS scores (P<0.05). Further multiple regression analysis showed that the duration of wounds, physical functional score, role physical score and role emotional score of SF-36, objective support score, subjective support score, support utilization score, and social support score of SSRS were the independent influential factors for SDS score (P<0.05). Education, general health score, social functional score, and mental health score of SSRS were the independent factors for SAS score (P<0.05).
 Conclusion: The lower quality of life and social support of patients with chronic wounds could increase their depression and anxiety. While treating patients with chronic wounds, medical staff should also pay attention to the mutual influence between anxiety and depression, or quality of life and social support.

Objective To summarize the reason and nursing experience of one pulmonary embolism patient after total knee arthroplasty easily misdiagnosed coronary heart disease. Methods For the patient to implement risk assessment of venous thromboembolism and develop prevention plan in preoperation, then carry out anticoagulation prevention and physical prophylaxis after operation. Nurse observe the patient changes of clinical symptom and signs,focus on the complain of patient and the changes of laboratory examination results, implement the diagnosis process of pulmonary thromboembolism. Conclusions To develop the system of VTE risk assessment and implement prevention measures.Nurses need to distinguish the high risk pulmonary thromboembolism patient according to the clinical observation, it is better for the pulmonary thromboembolism patient to be early diagnosed and treated, meanwhile, to implement the effective nursing measures will contribute to the prognosis of pulmonary thromboembolism patient.

RÉSUMÉ

Objective To study the effect of lidocaine combined with local anesthesia and psychological intervention in induced abortion.Methods200 cases of artificial abortion in our hospital from March 2015 to August 2016 were selected as the research object and randomly divided into control group and experimental group,with 100 cases in each group.The control group was treated with lidocaine for local anesthesia,direct abortion,and the experimental group was given lidocaine local anesthesia and psychological intervention.The pain and satisfaction index of the two groups were compared.ResultsAfter the treatment, the satisfaction rate of cervical dilatation in the experimental group was 93 cases was 93% higher than that in the control group, and the difference was statistically significant (P<0.05).The number of patients with grade Ⅱ pain in the experimental group was 12, and the number of patients with grade I pain was 87.In the control group, the number of patients with grade Ⅱ pain was 25, and the number of patients with grade Ⅱ pain was 57.But the control group was significantly lower than the experimental group, with statistical difference (P<0.05).In the control group, patients with grade Ⅱ pain were significantly higher than those in the experimental group, the number of patients with grade I pain was significantly less than that in the experimental group, and the difference was statistically significant (P<0.05).There was no significant difference in the operation time and blood loss between the control experimental and the control group.ConclusionThe application of lidocaine local anesthesia combined with psychological intervention can effectively relieve the pain of patients and improve the therapeutic effect.It has the value of clinical popularization and Application.

RÉSUMÉ

Objective To explore the feasibility and safety of early ambulation in patients with acute deep ve-nous thrombosis. Methods The literatures about early ambulation for acute venous thrombosis were collected and the quality of the literature was evaluated by two investigators independently. Results Thirteen published studies were recruited for systematic review,including 10 RCTs and 3 CCTs. The meta-analysis showed that compared with bed rest,early ambulation did not increase the incidence of secondary pulmonary embolism in patients with acute DVT[early activity:22/1408;bed rest:34/1417;RR=0.62,95%CI(0.37,1.03),and the incidence of DVT progression [early activity:48/1523;bed rest:45/1525;RR=0.85,95%CI(0.58,1.24)]. There were no statistically significant differ-ences in mortality,swelling,and pain relief in acute DVT patients. Conclusion Early ambulation did not increase the incidence of thrombus progression and secondary pulmonary embolism in patients with acute DVT compared with bed rest. Besides,early ambulation cannot relieve symptoms of swelling and pain in DVT patients. However,it can relieve the acute pain of DVT patients with moderate to severe pain.
