Résumé

Objective: To evaluate the effectiveness and safety of ultrasound-guided percutaneous cannulation for extracorporeal membrane oxygenation (ECMO) in children. Methods: In this retrospective observational study, 66 cases who underwent non-cardiac surgery ECMO in pediatric intensive care unit (PICU) of Shanghai Children's Hospital from May 2016 to April 2021 were collected. The demographics, model of ECMO support, type and size of arteriovenous cannulas, way of catheterization and complications were recorded and summarized. Patients were divided into percutaneous cannulation group and surgical cannulation group according to catheterization strategies. The demographics, duration of cannulation and ECMO support, ECMO weaning rate and hospital survival rate were compared among two groups. χ2 and nonparametric rank sum test were used for comparison. Results: Among the 66 patients who received ECMO, 38 were male and 28 were female, with age 44.5 (12.0, 83.5) months and weight 15.0 (10.0, 25.0) kg; 21 patients underwent percutaneous cannulation, with a success rate of 95% (20 cases). Point-of-care ultrasound was performed for all percutaneous cannulation cases. The duration of percutaneous cannulation was significantly shorter than that of surgical cannulation (26.0 (23.3, 30.3) vs. 57.0 (53.8, 64.0) min, Z=6.31, P<0.001). Successful percutaneous cannulation cases were aged 70.5 (23.8, 109.5) months, and their weight was 23.2 (13.6, 37.0) kg. Ten cases were initially given veno-venous (VV) ECMO support, and 10 cases were given veno-arterial (VA) ECMO support. ECMO arterial cannulas were sized from 8 F to 17 F, and venous cannulas sized from 10 F to 19 F. For VV-ECMO, the right internal jugular and femoral veins were used as vascular access, while VA-ECMO used right internal jugular vein-femoral artery or right femoral vein-left femoral artery approach. Only one patient suffered severe complication (superior vena cava perforation). There was no catheter-related bloodstream infection. Conclusion: Ultrasound-guided percutaneous cannulation for ECMO can be performed with a high rate of success and safety in children.

The development process of acupuncture and moxibustion diagnosis and treatment institutions since the founding of the People's Republic of China is summarized and analyzed. The main forms of acupuncture and moxibustion diagnosis and treatment institutions have shifted from the individual, joint clinic and outpatient department of acupuncture and moxibustion in the early years of the founding of the People's Republic of China to the present acupuncture and moxibustion departments in Chinese medicine hospitals and general hospitals.The construction and development of public and private acupuncture and moxibustion hospital and key specialty of acupuncture and moxibustion fully expand the clinical application of acupuncture and moxibustion therapy. In the future, the development of acupuncture and moxibustion diagnosis and treatment institutions should highlight its own features to satisfy the requirements of different levels of society.

Résumé

Bone marrow mesenchymal stem cells (BMSCs) are a kind of pluripotent stem cells derived from bone marrows, which can not only support hematopoiesis, but also have capabilities of multidifferentiation, high-proliferation and self-renewing. They have become one of hotspots in stem cell studies. Studies on in vitro intervention with BMSCs with TCMs have made remarkable progress in recent years. According to the findings, some traditional Chinese medicines can promote proliferation of BMSCs, some can inhibit the apoptosis of BMSCs, while others can induce BMSCs to differentiate into multiple cell types, such as osteoblast. Furthermore, some studies also involved relevant action mechanisms. The authors summarized the advance in relevant studies by reference to relevant literatures of this field.

Résumé

<p><b>OBJECTIVE</b>To study the central modulation mechanism on the relative specificity of the Heart and Lung Meridians and to provide the experimental evidence for deeply study on correlation between meridian-viscera and brain.</p><p><b>METHODS</b>Ten healthy students in Anhui College of TCM were chosen and a modified block design was adopted. After 32 time points of resting and 32 of rotation needling, then 48 of resting and 32 stimulating, and 16 resting time points, functional imagings were collected at last. All the process would last for 10 min 44 sec. Acupuncture work was finished by one acupuncturist with extensive experience by acupuncture at the left Shenmen (HT 7) or Taiyuan (LU 8) with the disposable sterile stainless steel needle, and uniform reinforcing-reducing method was used with frequency of 1 Hz and depth of 1.0 cm. After the experiment, the sensation of acupuncture and the other feeling or psychic process were inquired and recorded detailedly. These data were analyzed by AFNI software.</p><p><b>RESULTS</b>Acupuncture at Taiyuan (LU 8) could excite the contralateral frontal lobe, apical lobe, cerebral ganglion, VI, VIII areas and inferior semilunar lobule of cerebellum, and restrain bilateral callosal gyrus and homolateral gyrus rectus. Acupuncture at Shenmen (HT 7) could excite the contralateral IV-VIII areas of cerebellum, and homolateral VI, VII areas of cerebellum, and restrain parts of homolateral apical lobe.</p><p><b>CONCLUSION</b>Acupuncture at Shenmen (HT 7) of the Heart Meridian and Taiyuan (LU 8) of the Lung Meridian can excite or restrain different brain areas, indicating that there are relatively specific corresponding brain areas for the Heart Meridian and Lung Meridian.</p>

Résumé

Hepatocyte growth factor (HGF) pretreatment could protect multiple cell types from apoptosis induced by various damages including oxidative stress. The present study was designed to investigate the protective effect of HGF on rat cortical neurons against apoptosis induced by hydrogen peroxide (H2O2) in culture, and then to explore whether HGF could influence the mitochondrial pathway of apoptosis. Primary rat cortical neurons were isolated from Sprague-Dawley rats and cultured in serum free medium containing 2% B27 and Neurobasal-A. To mimic the oxidative stress damage, cortical neurons were exposed to 100 mumol/L H2O2 for 4 h. To explore the effects of HGF on the neurons subjected to H2O2 injury, cells were pretreated with HGF 15, 30, 60 ng/mL for 24 h, respectively, and then exposed to 100 mumol/L H2O2 for 4 h. The cell viability was measured by MTT colorimetric assay and cell injury was evaluated by lactate dehydrogenase (LDH) leakage rate. Apoptotic cells were detected by Hoechst 33258 staining and Annexin V-FITC/PI double labeled flow cytometry. The caspase-3 activity was assessed by colorimetry. The alteration of transmembrane potential of mitochondria was determined by confocal laser scanning microscopy. The expression of cytochrome C protein was measured by Western blot analysis. The results showed that H2O2 treatment significantly decreased the cell viability, increased LDH leakage rate and the percentage of apoptotic cells. Pretreatment of HGF at different concentrations (15-60 ng/mL) could remarkably increase the cell viability of neurons. Compared with that of H2O2 group (53.4%+/-7.4%), the cell viabilities of neurons treated with 15, 30, and 60 ng/mL HGF significantly increased to (69.3+/-6.4)%, (77.5+/-6.1)% and (82.9+/-9.3)% (P<0.05), respectively. HGF preincubation also evidently decreased the LDH leakage rate in cortical neurons damaged by H2O2. The results of Hoechst staining revealed that HGF pretreatment could significantly reduce the apoptotic rate of neurons. The apoptotic rate of H2O2 group was (62.8+/-7.1)%, while that of HGF groups decreased significantly to (34.8+/-8.4)%, (23.5+/-3.2)% and (18.6+/-4.5)% (P<0.05), respectively. The data from caspase-3 activity assay indicated that HGF preconditioning could also remarkably decrease the caspase-3 activity of neurons. In addition, in the presence of various concentrations of HGF, the decrease of transmembrane potential of mitochondria in neurons caused by H2O2 injury could be reversed. Moreover, as detected by Western blot analysis, HGF downregulated the expression of cytochrome C protein in neurons. These results suggest that HGF has a protective effect on rat cortical neurons against apoptosis induced by H2O2, which might be related to the inhibition of the mitochondrial apoptotic pathway and the suppression of the caspase-3 activity.

Some issues including medical support for mass burn casually and treatment regime in future, such as the prevention of burn calamities, further elevation of the cure rate and lowering in the rate of disability, further development in network of burn care and preliminary scheme of rescue of mass burn casualties and their treatment, accelerating the development and study on the substitutes of allo-skin graft were discussed.

Résumé

<p><b>OBJECTIVE</b>To analyze the mechanisms of surrogate tolerogenesis induced by chimeric donors.</p><p><b>METHODS</b>Hematopoietic stem cells (HSCs) from human cord blood were transplanted into fetal rats via intrauterine injection and infused into the liver of the neonatal rats to establish chimeric rat models with human HSCs. Four weeks after birth, flow cytometry was performed to analyze the percentages of human CD45 (hCD45), CD55 (hCD55) and CD59 (hCD59)-positive cells in the peripheral blood cells of the chimeric rats. The distributions of hCD55- and hCD59-positive cells in different hCD45/SSC gating regions were observed. The resistance of the peripheral blood lymphocytes to complements-mediated cytolysis was assessed by complement-dependent cytotoxicity (CDC) test in the chimeric rats and compared with that in control rats. The correlation between CDC and the human complement-regulating proteins in the chimeric rats were analyzed statistically.</p><p><b>RESULTS</b>On hCD45/SSC gating, the percentages of hCD55- and hCD59-positive cells in hCD45-positives region were (53.69-/+18.23)% and (31.8-/+27.5)%, accounting for (2.0-/+1.32)% and (0.76-/+0.56)% of the total cell population, respectively, which were significantly lower than the cell percentages in the extensive region (t=2.71, P=0.043 and t=3.64, P=0.015). The cytolytic rate of PBLs incubated with normal human serum was (22.32-/+15.10)% in the chimeric rats, significantly lower than that in the non-chimeric rats [(60.7-/+22.65)%, t=4.16, P<0.001). In the chimeric rats, hCD55-positive cell percentage was inversely correlated in the peripheral blood karyocytes the cytolysis rate in CDC (r=-0.679, P=0.031), and positively correlated to hCD45-positive cell percentage (r=0.658, P=0.038).</p><p><b>CONCLUSION</b>The hCD45-positives region is the cluster of chimeric human cells expressing human complement-regulating proteins. The peripheral blood lymphocytes from chimeric donor can resist the cytolysis mediated by human complement. The presence of allogenic CD55 and CD59 antigens in chimeric donors may be the basis of surrogate tolerogenesis for xenotransplantation.</p>

<p><b>OBJECTIVE</b>To explore the feasibility of transplanting the skin from chimeric rats to rabbits.</p><p><b>METHODS</b>Chimeric rats were produced by transplanting the haematopoietic stem cells (HSCs) from rabbit marrows into fetal rats in uterus and followed by injecting the HSCs into the livers of the rats at newborn stage. After six weeks, the skin from chimeric rats was transplanted to the rabbits. In group A, the skin grafts from chimeric rat donors were transplanted to the HSCs donating rabbits, with the skin from non-chimeric rat to normal rabbits were used as control. In group B, the skin grafts from chimeric and non-chimeric rats were transplanted to the HSCs donating rabbits at the same time. Gross observation and the surviving time of heterogenic-skin graft were observed. The wound healing time was also recorded.</p><p><b>RESULTS</b>In group A, the surviving time and the wound healing time of non-chimeric grafts were (9.3 +/- 1.8) days and (20.9 +/- 2.1) days, respectively, while those in chimeric grafts were (15.1 +/- 2.6) and (18.5 +/-1.3) days, respectively. In group B, the surviving time and the wound healing time of non-chimeric grafts were similar to those of group A. Compared with those in non-chimeric grafts, the surviving time of chimeric grafts in both groups were prolonged (P < 0.01), and the wound healing time shortened (P < 0.05 or 0.01). Most of the wounds healed quickly after rejection of chimeric grafts, while the wounds with non-chimeric grafts were re-opened with exudation and some necrotic tissue.</p><p><b>CONCLUSION</b>Immunologic tolerance for skin graft can be induced by the skin from chimeric donors, which can prolong the surviving time of skin grafts and shorten the wound healing time.</p>

<p><b>OBJECTIVE</b>To reveal the mechanism of acupuncture at the Heart meridian in treatment of myocardial ischemia from gene level.</p><p><b>METHODS</b>A model of acute myocardial ischemia was made by ligation of left anterior descending branch of the coronary artery. Changes of gene expression pattern of the Lung meridian group, the Heart meridian group and the model group in the heart were compared.</p><p><b>RESULTS</b>Differential expression genes and expression sequence tags (ESTs) were 14 with signal log ratio > or = 1 and 20 with signal log ratio < or = -1, mainly included immunological and inflammatory reaction relative genes, cell signal transduction and transferrin relative genes, and others, in the Lung meridian group vs the model group. Differential expression genes and ESTs were 20 with signal log ratio > or = 1 and 70 with signal log ratio < or = -1, mainly included ion channel and transport protein relative genes, cell apoptosis and stress reaction protein relative genes, metabolism relative genes, cell signal transduction and transferrin relative genes, DNA's binding, transcription and transcriptional factor relative genes, immunological and inflammatory reaction relative genes, and others, in the Heart meridian group vs the model group.</p><p><b>CONCLUSION</b>In number and types of differential expression genes and ESTs with signal log ratio > or = 1 or < or = -1, the changes in the Heart meridian group vs the model group were obviously different from those in the Lung meridian group vs the model group, indicating that the Heart meridian has relative specificity in protecting against acute myocardial ischemia.</p>

Résumé

<p><b>OBJECTIVE</b>To explore the mechanism of electroacupuncture combined with medication for treatment of compound model of hypertension and hyperlipemia (CMHH).</p><p><b>METHODS</b>CMHH rat model was made by the way of "2K1C" combined with intragastric perfusion of high fat diet, and a normal group and a pseudosurgery group were set up. After modeling for 4 weeks, the successful model rats who had synchronously increase of blood pressure (BP) and blood lipids were randomly divided into a model group, a medication group , an electroacupuncture group and an acupuncture plus medication group. After interference of 4 weeks, changes of BP, total cholesterol (TC) and thiglyceride (TG) and contents of serum vWF, tissue plasminogen activator (t-PA ) and plasminogen activator inhibitor-1 (PAl-1) were observed.</p><p><b>RESULTS</b>After interference, the levels of BP, TC, TG, vWF, t-PA and PAl-1 significantly changed in all the treatment groups as compared with those in the model group (P < 0.05 or P < 0.01) with most significantly changed in the electroacupuncture plus medication group.</p><p><b>CONCLUSION</b>Both electroacupunctore and electroacupuncture combined with medication can down-regulate levels of BP, TC, TG, and decrease plasma vWF and PAl-1 levels, increase t-PA content, so as to effectively prevent and treat CMHH and possibly induced cerebral diseases.</p>

<p><b>OBJECTIVE</b>To probe into thinking and prospect of application of gene chip technique to research of the channel-viscera correlativity.</p><p><b>METHODS</b>Adopt literature analytic method, review and analyze current state of application of gene chip in biology and medicine, and achievements in studies of channel-viscera correlativity.</p><p><b>RESULTS</b>In biological and medical fields, gene chip can be applied to high flux expression parallel analysis, large-scale gene discovery and gene analysis, gene polymorphous analysis, genome study, etc, especially, it has important application values in gene expression map; channel-viscera correlativity need stereo-crossing studies by multiple subjects, multiple systems, multiple directions and multiple levels, being more systematic and sequential, and gene chip technique is applicable to this requirement.</p><p><b>CONCLUSION</b>Wide application of gene chip technique will comprehensively promote deep study of channel-viscera correlativity.</p>

Résumé

<p><b>AIM</b>To study the biochemistry of lanthanides, the cooperative action of inorganic and organic anti-tumor drugs.</p><p><b>METHODS</b>A series of rare earth complexes were synthesized with Ln(NO3) 6H2O, Phen and 5-Fu. Their anti-tumor activity was measured by the improved MTT, SRB methods.</p><p><b>RESULTS</b>The formula of complex Ln[(Phen)2(5-Fu)3(NO3)](NO3)2(Ln = Y, La, Ce, Sm, Gd, Dy, Er; Phen = 1, 10-phenanthroline; 5-Fu = fluorouracil) was characterized by elemental analyses, molar conductivity, IR, TGA, and 13C NMR spectra. The preliminary biological activity studies indicated that Lanthanide complex has strong anti-tumor activity in vitro.</p><p><b>CONCLUSION</b>The complex might have anti-tumor cooperation action.</p>
