Mechanical Failure in Distraction Osteogenesis Using the Intramedullary Skeletal Kinetic Distractor / 대한정형외과학회잡지

In an effort to overcome the drawbacks of distraction osteogenesis using the Ilizarov external fixator, intramedullary lengthening devices have been developed and applied for long bone distraction osteogenesis. Several successful cases have been reported, leading to the next generation of distraction osteogenesis. However, intramedullary lengthening devices have their own problems, such as device failure, difficulty of control of the lengthening degree. The authors report on a case of device failure during distraction osteogenesis using ISKD(R) (Orthofix Inc.) in the distraction phase, and the strategy that involved switching to a lengthening over nail system, which uses the Ilizarov external fixator, in order to obtain the target length.

Simultaneous Bilateral Quadriceps Tendon Rupture in Patient with Chronic Renal Failure

Simultaneous bilateral spontaneous rupture of the quadriceps tendon is a very rare condition and only a few cases have been reported in the literature. The etiology is not clear yet. But it occurs infrequently in patients with chronic metabolic disorders. A 30-year-old female patient with simultaneous bilateral spontaneous quadriceps tendon rupture visited our hospital. She had chronic renal failure and her parathyroid hormone level was elevated due to parathyroid adenoma. We report a surgical repair of both quadriceps tendons of a patient with chronic renal failure as well as management of hyperparathyroidism.
