Résumé

The occurrence and development of many eye diseases are closely related to genetic and environmental factors, among which epigenetic modification is an important bridge connecting genetic and environmental factors. It can affect the levels of related genes by influencing gene transcription or translation, thereby playing a role in the pathogenesis of ocular diseases. DNA methylation is an important part of epigenetic modification which is usually regulated by three processes: de novo methylation, maintenance methylation, and demethylation, and plays an essential role in regulating gene expression. At present, researchers have conducted that DNA methylation plays an important role in repair of damage to corneal endothelium, mitochondrial dynamics regulation and diabetic retinopathy, oxidative stress response and cataracts and other eye diseases, providing new ideas in the treatment of related ocular diseases. This study presented a brief review of the role of DNA methylation in the development of related ocular diseases and provided new perspectives and directions for the screening, diagnosis, and treatment of eye diseases.

OBJECTIVE@#To investigate the effect of internal external fixator assisted O-arm navigation imaging in the treatment of unstable pelvic fractures.@*METHODS@#From May 2019 to November 2019, 15 patients with unstable pelvic fractures were treated by intraoperative O-arm navigation imaging using INFIX technology. There were 6 males and 9 females. The age ranged from 24 to 66 years old. The course of disease ranged from 2 to 14 days. According to Tile classification, there were 1 case of B1 type, 8 cases of B2 type, 3 cases of C1 type, and 3 cases of C2 type. According to Young-Burgess classification, there were 8 cases of LC, 1 case of APC, 4 cases of VS, 2 cases of CM. Preoperative routine pelvic anteroposterior film, entrance position, exit position and pelvic CT three-dimensional reconstruction were performed. Intraoperative O-arm navigation system three-dimensional reconstruction and triplane scanning imaging were used to evaluate the effect of intraoperative reduction. The anterior pelvic ring was fixed with internal external fixator, and the posterior ring was fixed with sacroiliac screw, plate screw or lumbar iliac screw. The operation time, intraoperative bleeding and nail placement were observed and recorded. The quality of fracture reduction was evaluated by Matta standard, and the postoperative function was evaluated by Majeed function score.@*RESULTS@#Wound healing was good in all patients without vascular, nerve and local irritation complications. All the 15 patients were followed up for 10 to 16 months. The fracture reduction was evaluated according to the Matta scoring standard, 9 cases were excellent results, 5 cases were good, and 1 case was medium. The Majeed functional score was 0 to 95 points.@*CONCLUSION@#The built-in external fixator assisted O-arm navigation imaging system in the treatment of unstable pelvic fractures. The reduction effect is evaluated in advance, the operation time is shortened, and the accuracy of internal fixation is improved. The operation is simple, safe and less bleeding. The operation is in line with the principles of minimally invasive medical treatment and precision medical treatment in orthopedics, which is conducive to the recovery of patients' postoperative function and rapid recovery.

OBJECTIVE@#To explore clinical effect of arthroscopic debridement combined with platelet-rich plasma (PRP) injection for Kellgren-Lawrence(K-L) gradeⅠ-Ⅲ knee osteoarthritis (KOA) .@*METHODS@#Totally 117 patients with KOA who underwent arthroscopic debridement combined with injection from November 2015 to January 2019 were retrospectively analyzed. According to different injection drugs, the patients were divided into sodium hyaluronate group(group A) and PRP group(group B). In group A, there were 60 patients, including 27 males and 33 females, aged from 49 to 67 years old with an average age of (54.1±4.8) years old;12 patients with gradeⅠ, 23 patients with gradeⅡand 25 patients with grade Ⅲ according to K-L clssification, 2 ml(20 g) sodium hyaluronate was injected into knee joint after intraoperative, 1, 2, 3 weeks after operation for 4 times. In group B, there were 57 patients, including 25 males and 32 females, aged from 47 to 70 years old with an average of (55.8±5.0) years old, 10 patients with gradeⅠ, 20 patients with gradeⅡand 27 patients with grade Ⅲ according to K-L classification, injected 5 ml PRP at the same time. Postoperative complications was recorded between two groups. Postoperative visual analogue scale(VAS) and Lysholm score at 3, 6, 12 months were used to evaluate improvement of knee pain and joint function.@*RESULTS@#All patients were followed up for 12 to 19 months with an average of (14.1±1.6) months. There was no significant difference in postopertaive complications between group A and group B (P>0.05). Postoperative VAS score in group A at 3, 6, 12 months were 3.0±0.8, 2.0±0.8, 2.6±0.9 respectively, and 2.9±0.8, 1.9±0.7, 2.2±0.8 in group B respectively; and no differnece at 3 and 6 months after operation between two groups (P<0.05), while VAS score in group B was higher than group A at 12 months after operation(P<0.05). Postoperative Lysholm score in group A at 1, 6, 12 months (86.6±1.8, 93.1±2.0, 86.7±1.7) were lower than group B(88.9±1.9, 95.0±2.0, 89.0±1.9)(P<0.05).@*CONCLUSION@#Arthroscopic debridement combined with sodium hyaluronate or PRP injection for K-L gradeⅠ-Ⅲ KOA could effectively relieve pain and improve joint function with higher safety in short term, but the medium-long-term effect of PRP injection is stable.

<p><b>OBJECTIVE</b>To evaluate the outcome and indication of the reconstruction of oral and maxillofacial postoperative defects by submental artery island myocutaneous flaps.</p><p><b>METHODS</b>Sixty eight cases with the reconstruction of oral and maxillofacial defects by submental artery island myocutaneous flaps from January 2006 to May 2010 were analysed retrospectively. Primary lesions included carcinomas originating from tongue (28 cases), palate (13 cases), mouth floor (9 cases), gingiva (4 cases), buccal mucosa (6 cases), lip (3 cases), and other malignant or benign tumors (5 cases). The ages ranged from 25 to 84 years (mean 58 years); 47 males and 21 females. The sizes of skin paddle varied from a minimum of 4 cm × 4 cm to a maximum of 15 cm × 10 cm.</p><p><b>RESULTS</b>Of the 68 flaps, 62 were survival, 4 had partial necrosis but healed with treatments, and 2 failed due to complete necrosis. Appearance and functions of recipient sites were satisfactory. The followed-up time was 3 - 24 months, local recurrence occurred in 5 cases and cervical lymph node metastases were found in 15 patients.</p><p><b>CONCLUSION</b>Submental island flap is reliable for the reconstruction of postoperative defects in early oral cancer without regional lymph node metastasis or in benign tumor.</p>

Résumé

Oral squamous cell carcinoma (OSCC) has a high incidence of cervical micrometastases and sometimes metastasizes contralaterally because of the rich lymphatic intercommunications relative to submucosal plexus of oral cavity that freely communicate across the midline, and it can facilitate the spread of neoplastic cells to any area of the neck consequently. Clinical and histopathologic factors continue to provide predictive information to contralateral neck metastases (CLNM) in OSCC, which determine prophylactic and adjuvant treatments for an individual patient. This review describes the predictive value of clinical-histopathologic factors, which relate to primary tumor and cervical lymph nodes, and surgical dissection and adjuvant treatments. In addition, the indications for elective contralateral neck dissection and adjuvant radiotherapy (aRT) and strategies for follow-up are offered, which is strongly focused by clinicians to prevent later CLNM and poor prognosis subsequently.

<p><b>OBJECTIVE</b>To assess the reliability and usefulness of the extended supraclavicular fasciocutaneous island flap (eSIF) for reconstructing soft tissue defects followinng tongue cancer ablation.</p><p><b>METHODS</b>Twenty-two tongue defects in 22 patients with tongue cancer were repaired with eSIF after resection of the tongue malignant tumours. The sizes of the skin paddle ranged from (4 cm × 6 cm) to (8 cm × 14 cm) (average: 5.5 cm × 8.7 cm).</p><p><b>RESULTS</b>The eSIF survived in twenty cases, including two cases with minimal partial necrosis. The patients were follwed up for 5 - 16 months. Eighteen patients were alive with disease free, two were alive with disease and two died of a lung and liver metastasis.</p><p><b>CONCLUSIONS</b>The eSIF is reliable for reconstructing oral defects following the ablation of advanced oral malignant tumors.</p>

<p><b>OBJECTIVE</b>To examine the expression of Ezrin in human salivary gland adenoid cystic carcinoma and investigate the effects of Ezrin gene silence on cell proliferation, apoptosis and invasion of adenoid cystic carcinoma (ACC)-M.</p><p><b>METHODS</b>The expression of Ezrin was detected by immunohistochemistry in normal salivary gland tissue (n=15), pleomorphic adenoma (n=40) and salivary gland adenoid cystic carcinoma (n=43). The Ezrin Stealth RNAi Duplex, containing Stealth RNAi Negative Control Duplex were constructed and transfected into ACC-M cells by Lipofectamine 2000. The expression levels of Ezrin were detected by RT-PCR and immunohistochemistry. The cell cycle and apoptosis rate were analyzed by flow cytometry (FCM). The cell proliferation was detected by methyl thiazolyl tetrazolium (MTT) and cell invasion by Transwell test.</p><p><b>RESULTS</b>The positive rate of Ezrin expression in ACC was significantly higher than that in normal salivary gland tissue and pleomorphic adenoma (P<0.05). After transfection of Ezrin Stealth RNAi Duplex, the mRNA and protein expression of Ezrin were down-regulated, the cell proliferation activity was inhibited, the G0-G1 Phase cells were increased, and the apoptosis rate of Ezrin Stealth RNAi Duplex group was higher than that in control groups and cell invasion ability was decreased.</p><p><b>CONCLUSIONS</b>Over expression of Ezrin in human salivary gland adenoid cystic carcinoma may promote genesis, development and metastasis of tumors. Ezrin Stealth RNAi Duplex could efficiently down-regulate the expression of Ezrin gene, and partly inhibited proliferation of ACC-M cells, induce apoptosis and decrease invasion ability of these cells in vitro.</p>

<p><b>OBJECTIVE</b>Observe the effect of Ribavirin on reducing the EV71 replication, inactivating EV71 and protecting the RD-A cells against the EV71 infection in vitro.</p><p><b>METHODS</b>Using the EV71 isolated from Anhui Fuyang HFMD outbreak, the effect of Ribavirin on RD-A cells during the EV71 infection was observed.</p><p><b>RESULTS</b>In the experiment of Ribavirin inhibiting the EV71 replication, comparing with the no-Ribavirin-dealed virus control group, the group of 1:640 dilution of Ribavirin delayed the CPE for 2 days, while the normal cell group was growing very well. In the experiment of protecting cell from EV71 infection, comparing with the no-Ribavirin-dealed virus control group, the group of 1:8 dilution of Ribavirin delayed the CPE for 4 days. In the experiment of Ribavirin effect on the inactivation of EV71, the group of 1:40 dilution of Ribavirin delayed the CPE for 2 days comparing with the virus control group.</p><p><b>CONCLUSION</b>Ribavirin could inhibit the replication of the EV71 and inactivate the EV71 in vitro. Additionally, Ribavirin could protect RD-A cells from EV 71 infection. The observation will contribute to EV71 infection control and quick medicine therapy.</p>

<p><b>OBJECTIVE</b>To study the situation of 1- 5-years-old children's antibody against Coxsackievirus A group 16 strain (CVA16) in Guangdong, Heilongjiang,Yunnan Province and Xinjiang Uygur Autonomous Regions, China, 2005, it can offer scientific evidences for preventing and controlling CVA16 causative hand-food and mouth disease.</p><p><b>METHODS</b>Using microneutrilization test, to study 503 serum samples randomly selected from sera collected in 2005.</p><p><b>RESULTS</b>Positive rate of anti-CVA16 antibody were 41.90%, 9.40%, 40.00% and 34.40% in Guangdong, Heilongjiang,Yunnan and Xinjiang, respectively. Antibody titer was relative low (average, 1: 6.1) and there was no statistical difference of geometry mean of antibody titer (GMT) among Guangdong, Heilongjiang, Yunnan (F = 0.97, 0.40, 1.06, respectively; P > 0.05), while there had statistical difference of GMT between Heilongjiang and other three regions( F = 10.61, P < 0.00).</p><p><b>CONCLUSIONS</b>There had probably existed local epidemic in some regions of Guangdong, Heilongjiang, Yunnan Province and Xinjiang Uygur Autonomous Regions, China, 2005 or even before, but the area and degree of transmission and epidemic had difference. Children aged from 1- 5-years-old were relatively susceptible population of CVA16 infection.</p>

<p><b>OBJECTIVE</b>To compare electrical sensory threshold (EST) and electrical pain threshold (EPT) of bilateral masseters, temporal muscles and finger region of chronic masticatory muscle pain (MMP) patients and the controls, and to analyze the mechanism of pain threshold change.</p><p><b>METHODS</b>Twelve patients with MMP and 12 matched controls participated in this study. EST and EPT of bilateral masseters, bilateral temporal muscles, and left middle finger were recorded and compared within group and between groups.</p><p><b>RESULTS</b>Compared to the controls, the EPTs of patients significantly decreased [left masseter (5.9 +/- 1.1) mA,right masseter (5.7 +/- 1.0) mA, left temporal muscle (5.3 +/- 1.4) mA, right temporal muscle (5.5 +/- 1.7) mA,finger (5.2 +/- 1.0) mA] (P < 0.001). There was no significant difference in EST between patients and the controls. No relationship was found among VAS, ESP, and EPT.</p><p><b>CONCLUSIONS</b>Decreased pain thresholds of masseters, temporal muscles and fingers in MMP indicated that peripheral sensitization was induced in MMP patients. Decreased pain threshold in patients meant that MMP might influence the central modulation of nervous system.</p>

<p><b>BACKGROUND</b>Ossification of the posterior longitudinal ligament (OPLL) has a strong genetic background. Previous studies have shown that bone morphogenetic protein-2 (BMP2) and BMP2 mRNA are expressed in ossifying matrix and chondrocytes adjacent to cartilaginous areas of OPLL tissues and mesenchymal cells with fibroblastic features in the immediate vicinity of the cartilaginous areas. It is suggested that BMP2 plays different roles in the different stages of development of OPLL. However, it remains unknown which factors induce ligament cells to produce BMP2.</p><p><b>METHODS</b>OPLL patients (n = 192) and non-OPLL controls (n = 304) were studied. Radiographs of the cervical spine were analyzed for extent of OPLL. We investigated whether single nucleotide polymorphisms of exons 3 (-726) T/C and 3 (-583) A/G in the BMP2 gene are statistically associated with genetic susceptibility to OPLL in Chinese Han subjects.</p><p><b>RESULTS</b>There was no statistical difference between the occurrence of exons 3 (-726) T/C and 3 (-583) A/G and the occurrence of OPLL in the cervical spine. However, there was a significant association between occurrence of exon 3 (-726) T/C polymorphism and occurrence of OPLL in males of cases and controls in the cervical spine. In addition, no significant association was found between the exons 3 (-726) T/C and 3 (-583) A/G with number of ossified cervical vertebrae in OPLL patients.</p><p><b>CONCLUSIONS</b>Exon 3 (-583) A/G polymorphism in BMP2 gene is not associated with the occurrence and the extent of OPLL in the cervical spine. Chinese Han male patients with TC and CC genotypes in exon 3 (-726) T/C have genetic susceptibility to OPLL but not to more extensive OPLL in the cervical spine.</p>

<p><b>OBJECTIVE</b>To establish a whole-body visualization model of breast cancer with high hepatic metastatic potential in nude mice and observe the development and metastasis of breast cancer by real-time imaging.</p><p><b>METHODS</b>pEGFP-N1 plasmid was transfected into human breast cancer cell line MDA-MB-231 to obtain pEGFP-MDA-MB-231 cells that emitted fluorescence. pEGFP-MDA-MB-231 cells were inoculated orthotopically in BALB/C nude mice and cultured in vivo through serial passage, thereby establishing the mouse model bearing tumors with high hepatic metastasis potential. The fluorescence emitted from the tumors was quantitatively detected and imaged with a fluorescence stereo microscope for real-time visualization of the tumor growth and metastasis.</p><p><b>RESULTS</b>The transfected breast cancer cells stably and efficiently expressed EGFP. After inoculation of the transfected cells in nude mice, 20% of the first-generation cells showed hepatic metastasis, and the rate increased to 80% among the second-generation and up to 100% among the third-generation cells. The reliability of this visualization model was validated with conventional pathological methods.</p><p><b>CONCLUSION</b>The whole-body visualization model bearing breast cancer with high hepatic metastasis potential provides a reliable means for studying the mechanisms of hepatic tumor metastasis, and can be instrumental in the exploration of novel means for breast cancer treatment.</p>

<p><b>OBJECTIVE</b>To investigate the relation of vascular endothelial growth factor receptor 1 (VEGFR1)-positive hematopoietic progenitor cell with the metastasis of human colorectal carcinoma.</p><p><b>METHODS</b>Human colorectal cancer cells SW480/M5 were implanted into BALB/c nude mice, and the tumor tissue blocks were re-implanted into the colon of nude mice. The quantity and percentage of VEGFR1-positive hematopoietic progenitor cells and human colorectal cancer cells in the pre-metastatic location were observed with flow cytometry, and the expression of metastasis-related factors was detected using Western blotting.</p><p><b>RESULTS</b>Bone marrow-derived hematopoietic progenitor cells expressing VEGFR1 were found in the common pre-metastatic sites and formed cell clusters before the arrival of the tumor cells, but these cells were not detected in nude mice without tumor implantation. In the course of tumor metastasis, the expression of the proteinases including matrix metalloproteinase 9 and stromal cell-derived factor 1 gradually intensified within the metastatic niche.</p><p><b>CONCLUSION</b>The formation of VEGFR1-positive hematopoietic progenitor cellular clusters is accompanied by the metastasis of human colorectal cancer, and may enhance the expression of metastasis-related factors, suggesting its important role in the invasion and metastasis of colorectal cancer.</p>

<p><b>OBJECTIVE</b>To evaluate the anatomy basis and clinical effects of the lower trapezius myocutaneous flap to reconstruct defects in the head and neck.</p><p><b>METHODS</b>From Jan. 2000 to Jan. 2004, lower trapezius myocutaneous flaps which were used to reconstruct the defects in the head and neck after radical tumors resection in twenty four cases in Center of Craniomaxillofacial Surgery of the Second Affiliated Hospital of Sun Yat-sen University were analyzed retrospectively. Male was 14,female was 10. Ages ranged from 35 to 76 (median = 54). The causes of the defects were malignant neoplasm of the tongue (n = 10), buccal (n =4), oral floor (n = 2), parotid (n = 1 ), oropharynx (n = 2), gingiva (n = 3), maxillary sinus (n = 1), zygomatic and frontal part (n = 1). Fifteen patients were primary, nine patients were recurrent. The flaps ranged from 8 cm x 7 cm to 12 cm x 10 cm.</p><p><b>RESULTS</b>Twenty one flaps survived fully and the other three had partial necrosis at the distal portion. Twenty two donor sites were closed primarily. Follow up 6 to 24 months, all flaps were good, the texture and color of the flaps were satisfying.</p><p><b>CONCLUSIONS</b>Lower trapezius myocutaneous flap is a thin, pliable and simple myocutaneous with a long pediculi and wide arc of rotation, and it is preferred for reconstructing huge soft-tissues defects in the head and neck regions simultaneously.</p>

<p><b>OBJECTIVE</b>To evaluate trigeminal function of patients with chronic masticatory muscle pain (MMP) using trigeminal somatosensory evoked potential (TSEP) and analyze mechanism of changes compared to the controls.</p><p><b>METHODS</b>Twelve chronic MMP patients and 12 matched controls participated in this study. TSEPs of bilateral masseter regions and inferior alveolar nerves were measured and compared between patient and control groups. The collected waves were concluded and those with appearance rate over 75% - 100% were selected. The latencies of waves were analyzed by use of the paired t test and independent-samples t test.</p><p><b>RESULTS</b>Five stable-appearing waves were obtained and named P1, N1, P2, N2, P3. The latency of P1 was significant reduced in patients (P < 0.05). The latencies of other waves were prolonged, but only P2 and N2 in left were significantly changed (P < 0.05).</p><p><b>CONCLUSIONS</b>TSEP latencies of chronic MMP patients changed in certain regular pattern and TSEP could be used as a parameter to disclose the trigeminal system abnormalities of the patients. The reduced latency of P1 might indicate the increasing conductivity of trigeminal nerves under level of ganglion Gasseri and the prolongation of latency of P2 was possibly related to inhibitory effects of the brain stem.</p>

<p><b>OBJECTIVE</b>To evaluate the reliability of the reverse facial artery-submental artery island myocutaneous flap for repairing the oral and maxillofacial defects.</p><p><b>METHODS</b>Eighteen oral and maxillofacial defects were repaired with reverse facial artery-submental artery island myocutaneous flaps following resection of malignant tumors. The ages of the patients ranged from 28 to 90 years; 11 were male and 7 were female. Primary sites of the lesions were the tongue squamous cell carcinoma (SCC) (7 cases), buccal mucosa SCC (4 cases), palate SCC (3 cases), oropharyngeal SCC (2 case), and facial skin basal cell carcinoma (2 cases). The sizes of skin paddle varied from a minimum of 4.0 cm x 12.0 cm to a maximum of 5.0 cm x 15.0 cm. The followed-up period was 9 to 18 months (11.8 months on average).</p><p><b>RESULTS</b>The postoperative outcome for the flaps was 17 cases surviving, one complete necrosis, and one temporary palsy of the marginal mandibular branch of the facial nerve. The form and function of recipient sites were well recovered. The donor site left a well-hidden scar. One case of cervical lymph node metastasis was observed.</p><p><b>CONCLUSIONS</b>Reverse facial artery-submental artery island myocutaneous flap is reliable for reconstructing medium-sized oral and maxillofacial defects.</p>

<p><b>OBJECTIVE</b>To investigate the effects of occlusal rehabilitation on chewing patterns of patients with extensive tooth wear.</p><p><b>METHODS</b>29 patients with severe tooth wear were selected and divided into two groups: Group I with complete posterior tooth support (15 cases) and group II with one-side or both side posterior support lost (14 cases). 15 normal old persons were also selected as control group. The surface electromyography (EMG) of masseter(MM), anterior temporalis (TA) and posterior temporalis (TP) during chewing movement were recorded in the stage of pre-treatment, 1 month after temporary restoration and 1 month after permanent restoration. The EMG activity, total cycle duration (TCD) and ratio of activation period to relaxation period (AP/RP) of chewing cycles were measured and compared.</p><p><b>RESULTS</b>1) Before treatment, the TCD of these two groups were longer than normal group, but the differences were not significant (P > 0.05). All of the AP/RPs of MM, TA and TP in group II were significantly higher than that of normal group (P < 0.01), and EMG activity of TA of group II was group I were both significantly lower than that in pre-treatment stage. 3)After permanent restoration, the TCD of group I were significantly lower than that in temporary restoration stage (P < 0.01). In group II, the TCD was continuously slightly shortened, while the AP/RP were significantly lower (P < 0.01) and EMG activities were significantly higher(P < 0.01) than that in the temporary restoration stage.</p><p><b>CONCLUSION</b>Occlusal rehabilitation could obviously change the chewing patterns and the EMG activities of patients with severe tooth wear.</p>

@#ObjectiveTo investigate the effect of low frequency stimulation on motor function of stroke patients.Methods50 stroke patients were randomly divided into control group and treatment group with 25 cases in each group. Patients of control group received neuromedical and Bobath technical treatment; cases of treatment group received low frequency stimulation besides the above two methods. All patients were assessed respectively with the Fugl-Meyer Assessment and Clinical Nerve Function Impaired Scale at the admission and on 20 days after treatment.ResultsThe motor functions of both groups were improved (P<0.05). But the recovery of treatment group was superior to the control group (P<0.05).ConclusionLow frequency stimulation can promote the stroke patients' motor function recovery.

<p><b>OBJECTIVE</b>To investigate the effect of the restoration for pathological tooth wear.</p><p><b>METHODS</b>A total of 48 pathological weared premolars and molars with restorable space in occlusal surface in 10 patients were restored by composite resins. The patients' masticatory function (biting force, masticatory efficiency, the surface electromyographic (EMG) activity of masseter muscles and anterior temporalis) were compared before and after the treatment. At the same time, the restorations were evaluated according to the modified the United States Public Health Service (USPHS) criterion.</p><p><b>RESULTS</b>After the treatment, patients' clinical syndrome disappeared, the biting force was improved (P < 0.05) and the masticatory efficiency was increased significantly (P < 0.01). EMG activity, the asymmetry index and the action index of the tested muscles during different status had no significant change (P > 0.05). The composite resin restorations of the 48 teeth were all evaluated as A after three month and one year.</p><p><b>CONCLUSIONS</b>Restoring the pathological weared posterior teeth with composite resins is an effective treatment to relieve the patients' syndrome, improve their masticatory efficiency and biting force.</p>

<p><b>OBJECTIVE</b>To explore the clinical value and safety of using rib-major pectoralis myocutaneous flap carrying costal parietal pleura in combined repair of large soft and hard tissue defect caused by radical surgery of advanced tongue cancer.</p><p><b>METHODS</b>Six patients with advanced tongue carcinoma involving the floor of mouth and mandible were performed combined radical neck dissection with glossectomy and mandibulectomy, which caused large soft and hard tissue defect. Six rib-major pectoralis myocutaneous flaps carrying costal parietal pleura were transferred for immediate repair of the large defects. The rib flaps were applied for the repair of mandible, and the major pectoralis myocutaneous flaps were applied for the reconstruction of tongue and floor of mouth.</p><p><b>RESULTS</b>Six patients recovered well after operation. Six rib-major pectoralis myocutaneous flaps carrying costal parietal pleura survived well; the wounds of surgical incision of the oral cavity, neck, and chest healed up. The reconstructed tongue and the lower face appearance were satisfactory, the occlusion relationships were normal; the speaking as well as swallowing functions recovered.</p><p><b>CONCLUSIONS</b>It's safe and reliable to use rib-major pectoralis myocutaneous flap carrying costal parietal pleura to repair large soft and hard tissue defect in oral and maxillofacial region. Opening pleural cavity and harvest costal parietal pleura would not influence patients' thoracic movement and breath function and would not cause other complications. It's simple and safe for harvesting the composite flap. Carrying costal parietal pleura assures the sufficient blood supply of rib in the composite flap.</p>
