Objective To analyze the efficacy of plasma adjuvant therapy on coagulation disorders in patients with traumatic brain injury and its relation with the prognosis.Methods Prospective study was performed on patients with traumatic brain injury and coagulation disorders,admitted to our hospitals from January 2010 to June 2012; these patients were divided into conventional treatment group and plasma adjuvant therapy group.Cranial CT was performed and blood coagulation function was checked at admission,and then,coagulation function was re-checked again 3 days after therapy and cranial CT within 3 days.The Glasgow Outcome Scale (GOS) was marked 21 days after treatment.The improvement of coagulation disorders,secondary bleeding and prognosis were compared between the two groups.Results As compared with those in the conventional treatment group,the prothrombin time,partial thromboplastin time,secondary hemorrhage rate in the plasma adjuvant therapy group were significantly reduced (P<0.05).The differences of D-dimer content and GOS scores between the two groups were not statistically significant (P>0.05).Conclusion Plasma adjuvant therapy effectively improves the coagulation function,reduces the incidence of intracranial secondary bleeding,but can not obviously improve the prognosis of patients with traumatic brain injury.

RÉSUMÉ

Objective To explore the factors that may influence the prognosis of patients with multiple intracranial aneurysms (MIA) in subarachnoid hemorrhage (SAH). Methods A retrospective review was performed to analyze the medical records of 93 patients who had been managed in our department from January 2000 through January 2011 for MIA in SAH. Results The single factor analysis showed that the gender, preoperative Hunt-Hess grade, treatment protocol and a history of hypertension had significant influences on the prognosis of the patients (P<0.05). The multiple linear regression analysis revealed that the preoperative Hunt-Hess grade and treatment protocol were 2 independent risk factors of the prognosis of the patients (P<0.05). Conclusions The preoperative Hunt-Hess grade and the treatment protocol appear to be related to the prognosis of patients with MIA in SAH. Early diagnosis and treatment of ruptured aneurysms, as well as careful observation and proper intervention ofunruptured aneurysms,can result in a satisfactory prognosis in most patients with MIA in SAH.

Objective To investigate the effects of transient axonal glycoprotein-1 (TAG-1) on activity of U251 cells and expressions of AICD, p53 and EGFR genes in the cells. Methods The viability of U251 cells was tested by MTT assay at 48 h following the addition of various concentrations of TAG-1 (0, 5, 10 and 20 μg/mL). The expression ofamyloid precursor protein (ALP) was detected by immunofluorescent staining. The apoptotic cells were examined by TUNEL. Real-time PCR was employed to detect the influence of TAG-1 on the expressions of AICD, p53 and EGFR genes in U251 cells. Results TAG-1 did not play an inhibitory effect on the proliferation of the U251 cells. APP was abundantly expressed on membrane of the U251 cells. U251 cells did not show apoptotic cells but increased expressions of AICD, p53 and EGFR genes were noted when U251 cells were exposed at 10 μg/mL of TAG-1. Conclusion TAG-1 plays an important role in regulating the proliferation of glioma and may not induce the apoptosis of U251 cells through the signal pathway of TAG-1/APP/AICD/p53 or TAG- 1/APP/AICD/EGFR.

Objective To map the vision cortex of rats by dynamic manganese-enhanced functional magnetic resonance imaging and provide a method for researching the nervous function. Methods Six adult male Wistar rats were chosen and the process was divided into 4 continuous phases. No agent was injected into the rats in the first phase (5 min). Disrupting the BBB with marmitol and injecting manganese chloride were performed in the fight internal carotid artery (ICA) in the second phase (10 min). In the third phase (15 min), manganese chloride was administrated into theright ICA and vision stimulation was performed before the imaging process. The mixed liquor of manganese chloride and glutamate was injected into the rats in the forth phase (5 min). MRI was performed instantly after the handles in each phase. SPM and Matlab software were employed to help analyze the imaging data. Region-of-interest (ROI) was recorded to observe the stimulated regions and compare the signal intensity in the visual cortex. Results No specific enhanced region was found in the rat brain in the first and second phases. The right visual cortex was enhanced specifically on T1WI in the third phase. Many brain regions of the right hemisphere, the sites that agents was injected, were obviously enhanced in the forth 2008A1-E4011)phase. ROI analysis showed that the signal intensity in the third phrase (1.897±0.172) was significantly stronger as compared with that in the second phrase(1.549±0.163)(P<0.05). Conclusion The dynamic manganese-enhanced functional magnetic resonance imaging can analyze the functional activities of the vision cortex in rats and provide a new method for researching the function of the nervous system.

Objective To investigate the establishment of synapses between the cortical neurons and the neuron-like cells difierentiated from the marrow stromal cells(BMSCs)in a simulated transplantation system in vitro.Methods The BMSCs from green fluorescent protein(GFP)transgenic mice(GFP-GM-BMSCs) were isolated, cultured and purified in vitro.The third passage of GFP-GM-BMSCs were co-cultured with primary cultured cortical neurons and gliai cells in a simulated transplantation system in serum-free medium conmining 2%B27 supplemented with 20 ng/mL basic fibroblast growth factor(bFGF)and 20 ng/mL epidermal growth factor(EGF).On day 10 of the co-culture,FM1-43,a fluorescent dye specific to active synaptic vesicles,was used to observe synapses formation between the cells under fluorescence microscope. Results The GFP.GM-BMSCsco-cultured with the neural cells in the Serum-free medium containing bFGF and EGF differentiated into neuron-like cells 7 days after the co-culture.On day 10 ofthe co-culture,FM1-43 dye-positive synaptic vesicles were foundin the cell culture,locating mostly in the cell body,processes and terminal sffuctures ofthe neuron-like cells. Conclusions The neuron-like cells derived from GFP-GM-BMSCs can form synapses with the coRical neurons in the simulated cell transplantation system in vitro.

Objective To analyze the feasibility of local LINGO-1 polyclonal antibody administration for treatment of spinal cord injury in adult rats. Methods Twenty-four adult female SD rats were randomized into sham-operated group, rabbit IgG group and LINGO-1 antibody group. In the latter two groups, partial transaction of the T9 segment of the spinal cord was performed to completely sever the dorsal eorticospinal tract, followed immediately by administration of rabbit IgG and anti-LINGO polyclonal antibody via a mini-osmotic pump, respectively. At 3 and 28 days after the operation, the T8~10 segments of the spinal cord were harvested to prepare cryosections, and immunofluorescence staining was used to analyze the penetration of LINGO-1 polyclonal antibody into the spinal cord tissue and its specific binding to LINGO-1 molecules. Results In LINGO-1 antibody group, the presence of rabbit antibodies was detected at the injured sites of the spinal cord at 3 and 28 days after the operation. The mean immunofluorescence density was significantly lower in L1NGO-1 antibody group than in rabbit IgG group at 3 days after the operation (P<0.05). In rabbit IgG group, the mean immunofluorescence density for LINGO-1 in the crysections pre-treated with LINGO-1 polyclonal antibody was significantly lower than that in sections pre-treated with rabbit IgG(P<0.05). Conclusion Locally administered LINGO-1 polyclonal antibody can penetrate into the injured sites in the spinal cord in a wide time window and recognizes LINGO-1 molecule specifically, suggesting the feasibility of passive immunotherapy for spinal cord injury.

Objective To identify the risk factors of delayed cerebral vasospasm following aneurysmal subarachnoid hemorrhage (SAH). Methods The clinical data of 74 patients with SAH were retrospectively analyzed. The severity of delayed cerebral vasospasm in these patients was assessed according to the findings by digital subtraction angiography, and the risk factors for delayed cerebral vasospasm following SAH were analyzed. Results The patients' age, number of SAH episode, histor yof smoking, Hunt-Hess grade, Fisher grade and peak white blood cell count were found to significantl ycorrelated to the occurrence of delayed cerebral vasospasm. Among these factors, the patients' age, number of SAH, Hunt-Hess grade, and Fisher grade were identified as the independent risk factors of delayed cerebral vasospasm. Conclusions Patients with younger age, SAH more than twice, and Hunt-Hess or Fisher grades over Ⅲ are at higher risk of delayed cerebral vasospasm. Close monitoring fo rearly detection of delayed cerebral vasospasm and timely management are crucial in these patients.

RÉSUMÉ

Objective To construct an aneurismal model with estrogen deficiency and investigate the mechanism of estrogen deficiency in the formation and development of intracranial aneurysm. Methods Female Wistar rats were randomly divided into experimental, sham-operative and blank control groups (n=10). Rats in the experimental group were ovariectomized and those in the sham-operative group were removed the adipose issue nearby the ovary only; while the rats in the blank control group were done nothing. Two weeks after the ovariectomized or sham operation, elastase dropped around the right external carotid artery and the crotch of the carotid artery and the carotid artery was ligated by two lines at 1.5 mm far from the crotch, and then sheared between the two lines to successfully induce the aneurysm. At 6 weeks of the successful construction of aneurysm model, the estrogen was detected and the aneurysm was harvested for pathological staining. Results The experimental group showed a lower estrogen level (105.00±12.96 pmol/L) than the sham-operative group (178.50±25.96 pmol/L) and the blank control group (180.40±18.70 pmol/L, P<0.05). Aneurismal length dilatation rates in the experimental group and the sham-operative group were (131.31±6.63)% and (109.90±3.44) %, respectively (P<0.05). Aneurismal diameter dilatation rates in the experimental group and sham-operative group were (125.10±5.49) % and (106.82±2.49) %, respectively (P<0.05). Conclusion Estrogen deficiency may promote the formation and development of the intracranial aneurysm. This experiment provides a simple model for investigating the relationship between estrogen deficiency and aneurysm development.

<p><b>OBJECTIVE</b>To explore the association of blood and cerebrospinal fluid (CSF) IgG contents and the severity of craniocerebral injury.</p><p><b>METHODS</b>Totalling 143 patients with craniocerebral injury were divided into 3 groups according Glasgow Coma Scale (GCS) scores, namely the mild injury group with GCS score of 12-15 (n=41), moderate injury group with GCS score of 9-11 (n=71) and severe injury group (GCS score 3-8, n=32). Another 9 patients with congenital hydrocephalus were also recruited as the control group. The CSF and blood samples were collected from these patients to measure the IgG contents 4 and 14 days and 1, 2, and 6 months after the injury, respectively. Physical disabilities of the patients were estimated with Rappaport's disability rating scale (DRS), whose correlations with CSF and blood IgG contents were analyzed.</p><p><b>RESULTS</b>In the early stage of moderate to severe brain injury, the IgG content was lowered significantly in the blood but increased in CSF as compared with the control patients (P<0.05), and the changes in CSF and blood IgG displayed a significant correlation with the severity of the injury (r=0.950, P<0.01). During the recovery of severe brain injury, DRS score was in inverse correlation with blood IgG content but in positive correlation with CSF IgG content (Spearman's correlation coefficient of 0.800, P<0.05).</p><p><b>CONCLUSION</b>In the early stage of brain injury, detection of blood IgG content may help with the assessment of the injury severity. During the recovery of the injury, dynamic monitoring of blood and CSF IgG contents provides clues of the outcome of the patients and benefit the modification of the treatment plan.</p>
