{L-End}Objective To investigate the current status of sleep quality and its influencing factors among coal miners in a company in Shanxi Province. {L-End}Methods A total of 1 047 coal miners from a coal mine company in Shanxi Province were selected as the study subjects by convenient sampling method. The occupational stress, occupational burnout and sleep quality of the study subjects were investigated using Occupational Stress Core Scale, Maslach Burnout Inventory-General Survey and Pittsburg Sleep Quality Index Scale. {L-End}Results The detection rates of occupational stress, occupational burnout, sleep disorder were 58.9%, 59.1% and 57.9%, respectively. The result of multivariate logistic regression analysis showed that education level, alcohol consumption, work shift, duration of dust-exposure, phase of respiratory symptoms, self-assessment of health, occupational stress and occupational burnout were independent influencing factors of sleep disorders in the coal miners (all P<0.05). Among them, the risk of sleep disorders in drinkers was higher than that in non-drinkers (P<0.05); the risk of sleep disorders was higher in miners working in a rotating work shift with two shifts than in those with three shifts (P<0.05); the higher the education level, the longer the duration of dust-exposure, the more serious the phase of respiratory symptoms, the worse the self-assessment of health, the higher the degree of occupational stress and the higher the degree of occupational burnout, the higher the risk of sleep disorders (all P<0.05). {L-End}Conclusion The incidence of sleep disorders in coal miners in this company is relatively high. Occupational stress, occupational burnout, education level, alcohol consumption, work shift, duration of dust-exposure, respiratory symptoms and health status are risk factors for sleep disorders in coal miners.

Vesicles derived from Chinese medicinal herbs (VCMH) are nano-vesicular entities released by the cells of Chinese medicinal herbs. VCMHs have various biological effects and targeting characteristics, and their component chemicals and functional activities are closely related to the parent plant. VCMH differs from animal-derived vesicles in three ways: stability, specificity, and safety. There are a number of extraction and isolation techniques for VCMH, each with their own benefits and drawbacks, and there is no unified standard. When two or more approaches are used, high quantities of intact vesicles can be obtained more quickly and efficiently. The obtained VCMHs were systematically examined and evaluated. Firstly, they are generally saucer-shaped, cup-shaped or sphere, with particle size of 10-300 nm. Secondly, they contain lipids, proteins, nucleic acids and other active substances, and these components are an important part for intercellular information transfer. Finally, they mostly have good biocompatibility and low toxicity, with anti-inflammatory, antioxidant, anti-tumor and anti-fibrotic effects. As a new drug carrier, VCMHs have outstanding active targeting capabilities, and the capsule form can effectively preserve the drugs, considerably enhancing drug delivery efficiency and stability in vitro and in vivo. The modification of its vesicular structure by suitable physical or chemical means can further create more stable and precise drug carriers. This article reviews the extraction and purification techniques, activity evaluation and application of VCMH to provide information for further research and application of new active substances and targeted drug carriers.

OBJECTIVE:To study the effects and active ingr edients of Tibetan medicine Lamiophlomis rotata against rheumatoid arthritis (RA). METHODS :Fifty-six SD rats were randomly divided into normal control group (0.5% sodium carboxymethylcellulose),model group (0.5% sodium carboxymethylcellulose ),methotrexate group (positive control group ,3 mg/kg),L. rotata water extract low-dose ,medium-dose and high-dose groups (0.5,1,2 g/kg,by crude drug ),L. rotata total flavonoid group (200 mg/kg,by flavonoid extat ),with 8 rats in each group. Except for normal control group ,other groups were given Freund ’s complete adjuvant (FCA)on the rat ’s right hind footpad to induce adjuvant-induced arthritis model. The next day after injection of FCA ,rats in all groups were given relevant medicine intragastrically ,once a day (once every 3 days for methotrexate group )for 30 days. At 15th and 30th day of administration ,the degree of paw swelling of left hind foot was measured,and the arthritis index ,spleen index were calculated. At the end of 30th day of administration ,the levels of TNF-α,IL-1β, IL-6 and IL- 10 in rat serum were determined by ELISA assay ,the thymus index and spleen index were calculated ,the pathological changes of the ankle joints were observed by HE staining. RESULTS :Compared with normal control group ,degree of paw swelling,arthritis index at 15th and 30th day of administration as well as the spleen index and the levels of TNF-α,IL-1β, msxmX0146) IL-6 in serum at 30th day of administration were significantly com increased in model group (P<0.01);while the thymus indexyunbinji- and IL- 10 level at 30th day of administration were ang@swu.edu.cn significantly decreased(P<0.01);synovial cell proliferation and infiltration of articular cavity were observed in ankle joint. Compared with model group ,degree of paw swelling ,arthritis index at 15th and 30th day of administration as well as the spleen index and the levels of TNF-α,IL-1β,IL-6 in serum at 30th day of administration were significantly decreased in each medicine group (P<0.01);while the thymus index and IL- 10 level at 30th day of administration were significantly increased (P<0.01);the pathological changes of arthritis were significantly improved. Compared with L. rotata water extract high-dose group ,there were no significant differences in degree of paw swelling at 15th day of administration as well as the arthritis index ,spleen index ,levels of inflammatory cytokines and pathological changes of ankle joint in L. rotata total flavonoid group (P>0.05). CONCLUSIONS :Tibetan medicine L. rotata shows well anti-RA activity ,and total flavonoids may be the active ingredients of its efficacy.

Objective@#To study the correlation between low back pain (LBP) and occupational stress in coal miners.@*Methods@#From January 2015 to December 2016, a total of 472 front-line workers in a mining area of a large-scale coal mining enterprise in Shanxi, China were enrolled in the study. The general information, condition of LBP, and occupational stress level of the subjects were obtained by questionnaire survey. Dichotomous logistic regression (DLR) was used to analyze the correlation between LBP and occupational stress.@*Results@#Of the 472 subjects, 186 subjects experienced LBP in the past year; the prevalence rate of LBP was 39.41%. The scores of the Occupational Role Questionnaire were significantly higher for workers in the LBP group than in the non-LBP group (P<0.01) . As revealed by the DLR analysis, age, marital status, length of service, occupational stress role, labor intensity, keeping single posture during work, and working in shifts were risk factors of LBP (P<0.01) , while smoking may be a protective factor against LBP (P<0.01) .@*Conclusion@#Occupational stress role is a potential risk factor for LBP in coal miners (P<0.01) . Reasonable work assignment and timely alleviation of occupational stress may be one of the effective approaches to prevent LBP.

Objective@#To understand the prevalence of dyslipidemia and risk factors among coal miners under different work conditions.@*Methods@#The survey was conducted from April 2016 to June 2016. 759 mine workers were divided into three groups (group of the front line miner, underground auxiliary and ground) . Questionnaire and physical examination were used to collect related information of workers. Logistic regression model was used to analyze relative factors.@*Results@#The overall prevalence of dyslipidemia was 43.2% in coal miners. The prevalence rate of the front line miner and underground auxiliary miners was 46.6%. Ground workers had the lowest prevalence rate of 36.4%. Multiple Logistic regression analysis showed that higher body mass index (BMI) was risk factors for underground workers (OR=2.18, 95%CI:1.51~3.13) . Smoking (OR=1.99, 95%CI:1.17~3.38) , drinking (OR=1.85, 95%CI:1.11~3.06) , hypertension (OR=1.79, 95%CI:1.00~3.22) and higher waist and hip ratio (OR=1.06, 95%CI:1.04~1.09) were risk factors for underground auxiliary workers. For ground workers, those with higher BMI (OR=2.64, 95%CI:1.68~4.16) were at higher risk of dyslipidemia and female workers had lower risk (OR=0.35, 95%CI:0.18~0.65) than male workers.@*Conclusion@#The dyslipidemia rate of coal mine workers is related to work environment and behavior. Health education may be needed to reduce the dyslipidemia rate of coal mine workers.

Objective@#To investigate the features of literature on hand-transmitted vibration in China, 1990-2016.@*Methods@#In September 2017, the studies on hand-transmitted vibration in China, which were published in Chinese or English during 1990-2016, with "China" and "Taiwan" as the places where author affiliations were located, were retrieved. A bibliometric analysis was performed to investigate the type of articles, publication time, the journals in which articles were published, author affiliations, author regions, and funding.@*Results@#A total of 205 articles on hand-transmitted vibration were retrieved. There were 7.59 articles on average published annually from 1990 to 2016. In the 205 articles, 114 (55.61%) were published in the journals indexed in one or two core journal databases. In the 64 journals, 22 (34.38%) were indexed in one or two core journal databases. The first authors were from 22 provincial regions (provinces, autonomous regions, or centrally administered municipalities) in China, with 152 articles (74.15%) by the authors in the top five regions. There were a total of 876 authors, and the co-authorship degree was 4.27 (876/205). Most of the first authors (136 articles, 66.34%) were affiliated with universities or institutes for prevention and control of occupational diseases. Among the 205 articles, 103 (50.24%) were original articles or investigations, and 72 (35.12%) were funded.@*Conclusion@#The studies on hand-transmitted vibration fluctuated and increased from 1990 to 2016, with a relatively concentrated distribution in terms of sources, regions, and institutions. Interregional and international academic exchange should be strengthened.
