Résumé

@#Synovial sarcoma of the hypopharynx is an uncommon malignancy, with less than 100 cases reported in available journals. We report a case of a 22-year-old female presenting with dysphagia and enlarging hypopharyngeal mass, clinically diagnosed as hypopharyngeal malignancy, right, at least stage III. Histopathologic examination including immunohistochemistry study with TLE1 and SS18 Fluorescence In Situ Hybridization (FISH) confirm the diagnosis of synovial sarcoma. This is the first reported case of synovial sarcoma of the hypopharynx in the Philippines confirmed by SS18 FISH. Due to the size of the mass, chemoradiotherapy followed by surgery is the current plan of management for this patient.

SUMMARY: Refixation of the damaged acetabular labrum is a method of surgical treatment of the hip joint that can promote the repair of joint function after injury and prevent premature osteoarthritis. We sought to determine the condition of the hip joint in rabbits 4 months after excision of the acetabular labrum and the condition of the joint after labral refixation. The articular cartilage of the femoral head and acetabulum was examined by histological methods, multipoint measurement of cartilage thickness, and the ratio between cartilage matrix and chondrocytes lacunae, and the condition of cartilage according to the OARSI grading scale was carried out. On this model, a correlation analysis was performed between the results of the OARSI grading scale and the data of linear morphometry. All these parameters made it possible to better assess changes in articular cartilage. The ratio between matrix and chondrocyte lacunae turned out to be a method that allows establishing early cartilage damage when erosion, fibrosis or deformation did not occur. We found significant differences between the condition of the cartilage after exicion of acetabular labrum and after labral refixation, which give hope to confirm that this surgical technique can delay or prevent progressive changes in the cartilage of the damaged hip joint.


La refijación del labrum acetabular dañado es un método de tratamiento quirúrgico de la articulación coxal, que puede promover la reparación de la función articular después de una lesión y prevenir la osteoartritis prematura. Intentamos determinar el estado de la articulación coxal en conejos de 4 meses después de la escisión del labrum acetabular y observar el estado de la articulación después de la refijación del labrum. El cartílago articular de la cabeza femoral y el acetábulo se examinó por métodos histológicos, se midió a través de multipunto el grosor del cartílago y se realizó la relación entre la matriz del cartílago y las lagunas de condrocitos, y se llevó a cabo la condición del cartílago según la escala de clasificación OARSI. Sobre este modelo se realizó un análisis de correlación entre los resultados de la escala de calificación OARSI y los datos de la morfometría lineal. Todos estos parámetros permitieron evaluar mejor los cambios en el cartílago articular. La relación entre la matriz y las lagunas de condrocitos resultó ser un método que permite establecer temprano el daño del cartílago cuando no se presentó erosión, fibrosis o deformación. Encontramos diferencias significativas entre la condición del cartílago después de la extirpación del labrum acetabular y después de la refijación del labrum, lo que da la esperanza de confirmar que esta técnica quirúrgica puede retrasar o prevenir cambios progresivos en el cartílago de la articulación coxal dañada.

Résumé

A farmer-participatory research, which lasted for seven weeks, was carried out at Machache, in the Foothills Agro-Ecological Zone of Lesotho to examine the effect of forage based diets supplementary feeding on the performance response and milk quality of lactating ewes during dry lambing season. A total of 270 lactating merino ewes were randomly distributed among four dietary treatments: basal diet (T1) which was the range land pasture used as control, cereal forage based diet (T2), leguminous forage based diet (T3) and mixed forage based diet (T4). The feed value of the supplementary diets T2 (4.70% CP and 9.94MJ/kg ME), T3 (12.31% CP and 10.27 MJ/kg ME), T4 (11.90% CP and 10.47 MJ/kg ME)] was superior to that of the range land pasture T1 (2.80% CP and 8.61MJ/kg ME). Ewes on forage supplemented diets performed significantly (P<0.05) better than the control group in feed intake, live body weight and live weight change. The milk quality evaluation showed that solids-non-fat (SNF), protein and lactose were highly significant (P<0.05) for T3 and T4 than T1 and T2 which had high milk fat. The study revealed that diets T1 and T2 lacked the nutritional capacity to meet the nutrient requirements of lactating ewes as evidenced by slight body weight improvement. It is concluded that supplementary diets T3 (leguminous forage based diet) and T4 (mixed forage based diet) contained adequate nutrients that can meet the requirement of lactating ewes during dry lambing season. This was verified by high voluntary feed intake and good nutrients utilization as resulted by improving of body weight and high milk quality response of lactating ewes.

Children under the age of five are prone to foodborne diseases, resulting from poor food hygiene because of their less effective immune systems. This study aimed to assess the knowledge, attitude and practice of food hygiene among nursing mothers attending under-five clinics.Method: This study was a descriptive cross-sectional study, involving 330 nursing mothers attending the under-five clinic in health care facilities. Data were collected using interviewer-administered questionnaires and analyzed using Statistical Package for Social Sciences (SPSS) version 21.A p-value < 0.05 at 95% confidence interval was considered statistically significant.Result: Eighty-two percent of the respondents had good knowledge of food hygiene; 95.2% had a good attitude and 82.7% demonstrated good practice. Knowledge on food hygiene was significantly associated with the age of the respondents (P=0.008) and their educational status (P=0.001), those with tertiary education doing better. Similarly, respondents with tertiary education had significantly better attitude (P=0.001 and practice (P=0.001). The predictors of knowledge on food hygiene were education status (odds ratio [OR] =0.249, 95% confidence interval [CI]: 0.126, 0.492) and occupation of the respondents (OR = 2.158, 95% CI: 1.156, 4.026). Educational status was also the predictor for positive attitude (OR =0.143, 95% CI: 0.031, 0.667) and practice of food hygiene (OR =0.394, 95% CI: 0.210, 0.738).Conclusion: The knowledge, attitude and practice of food hygiene among the respondents was good. Predictors of knowledge included level of education and occupation. Government at the State and Local Government levels should organize regular health promotion on food hygiene.

Résumé

Background: COVID-19 vaccine is one of the most effective public health intervention approaches for prevention of COVID-19. Despite its well-known efficacy and safety, significant proportion of frontline COVID-19 healthcare workers remain hesitant about accepting the vaccine for whatever reasons. This study aimed to determine acceptance rate and determinants of vaccine refusal among doctors in Cross River State, Nigeria. Methodology: This was a cross-sectional survey of doctors using structured online questionnaire administered via the WhatsApp platform of the medical doctors' association, in order to assess their rate of acceptance of COVID-19 vaccines, and reasons for vaccine refusal. The predictors of vaccine acceptance were analysed by univariate and multivariate logistic regression analyses. Results: Of the 443 medical doctors targeted on the WhatsApp platform, 164 responded to the questionnaire survey, giving a response rate of 37.0% (164/443). The mean age of the respondents is 38 ±6.28 years, 91 (55.5%) are 38 years old and above, 97 (59.1%) are males and 67 (40.9%) are females, giving a male-to-female ratio of 1.4:1. The greater proportion of the respondents are physicians (70/148, 47.3%) and about three-quarter of the participants (127/164, 77.4%) had received COVID-19 vaccine. The proportion of physicians who had received COVID-19 vaccine (57/70, 81.4%) was more than the proportion of general practitioners (31/42, 73.8%) and surgeons (24/35, 68.6%). Low perceived benefit of vaccination was the main reason given for COVID-19 vaccine refusal (45.9%, 17/37). No significant association was found between vaccine refusal and suspected predictors (p>0.05). Conclusion: Our study revealed high rate of COVID-19 vaccine acceptance among medical doctors especially among the physicians, with the surgeons showing lowest acceptance rate. A significant proportion would not take vaccine because they perceived it lacks much benefits. To raise vaccine acceptance among doctors, more efforts on vaccine literacy that would target doctors from all sub-specialties especially surgeons and incorporate vaccine benefits should be made.

Background: Pre-exposure prophylaxis (PrEP) is a highly effective preventive measures against HIV infection but its success is strongly based on adherence, which in turn depends on willingness to use. This study is aimed at assessing the level of awareness and willingness to use PrEP to prevent HIV infection among female sex workers (FSWs) in Anambra State, Nigeria, and to identify factors that influence willingness to use PrEP. Methodology: The study was a cross-sectional survey involving 265 brothel-based FSWs recruited through snowballing technique. A structured questionnaire was used to collect relevant information on demographic characteristics, awareness of HIV/AIDS and transmission route, attitudes/behavior related to HIV/AIDS, and awareness of and willingness to use PrEP. Univariate and bivariate analyses with Chi square test (with Odds ratio and 95% confidence interval) was used to determine association of socio-demographic and predictive factors with willingness to use PrEP. Statistical significance was considered when p value was less than 0.05. Results: Of 265 FSW respondents, only 81 (31.2%) have heard of PrEP, 10 (3.9%) indicated they have previously used PrEP while 91.0% indicated willingness to use PrEP. Univariate analysis showed that FSWs in Onitsha had a significantly higher odds (OR=28.6, 95% CI=1.718-476.82, p=0.0006) while those from Awka had a significantly lower odds (OR=0.184, 95% CI=0.0704-0.1812, p=0.0004) of willingness to use PrEP. Also, FSWs with monthly income less than 18,000 Naira had a lower odd of willingness to use PrEP (OR=0.3980, 95% CI=0.1593-0.9945, p=0.08). Bivariate analysis shows that FSWs who wish to have more knowledge of HIV/AIDS had higher odd of willingness to use PrEP than those who did not wish to have more knowledge (OR=4.235, 95% CI=1.577­11.374, p=0.0066). Similarly, FSWs who are worried of being discriminated against have a lower odd of willingness to use PrEP than those who are not worried of being discriminated against (OR=0.3921, 95% CI=0.1582-0.9718, p=0.0439). Conclusion: Our study showed low awareness but high willingness to use PrEP among FSWs in Anambra State,Nigeria. Cost, HIV/AIDS knowledge and fear of discrimination are significant predicting factors of willingness to use HIV PrEP and should be considered when formulating PrEP policy. Adequate enlightenment on PrEP should be emphasized among FSWs.

Introduction: Mental health disorders in undergraduates are often undetected and may predispose to other academic and social complications. The objective of the study is to determine the prevalence of probable psychiatric morbidity among students of University of Ilorin, Nigeria and the psycho-social factors that are associated with psychiatric morbidity in them. Methods: Socio-demographic questionnaire and the 12-item General health questionnaire (GHQ-12) were administered to 3,300 undergraduate students to assess psychosocial variables and psychiatric morbidity respectively. Results: About 23.5% of respondents scored >3 using the GHQ-12 questionnaire, signifying a likehood of psychiatric morbidity. Students from polygamous families were 1.3 times more likely to have GHQ scores of >3 than those from monogamous (OR=1.276, P=0.026). Those who had unemployed fathers were twice more likely to have a GHQ > 3 than those with employed fathers. (OR=2.084, P=0.005).Those who lived in houses with shared toilet facilities were 1.3 times more likely to have GHQ >3 (OR=1.310, P=0.028) Conclusion: This study calls for a careful consideration and modification of the various psychosocial factors associated with pschiatric morbidity in order to ensure a mentally healthy and vibrant student community

Résumé

Hepatocellular carcinoma (HCC) is the twelfth most common cancer and the fifth leading cause of worldwide cancer related death. Chronic hepatitis B infection, caused by the hepatitis B virus (HBV) and exposure to aflatoxins is fundamental in the formation of HCC in developing countries. This review of scientific publications aims to establish the detrimental effects of aflatoxin-contaminated foods and highlights the correlation between aflatoxin and hepatitis B viral-associated hepatocellular carcinoma. Research has shown a significant increase in the occurrence of HCC in HBV-infected individuals exposed to fungal toxins. HBV demonstrates the ability to integrate and bind to p53 protein in the host DNA and propagate hepatocyte vulnerability through carcinogenic aflatoxin B1 (AFB1) damage. Although there has been clear evidence about the synergistic interaction of exposure to AFB1 and HBV infection in the induction of HCC, other literature has shown otherwise, mainly because incomplete and vague findings and hypotheses were made in regions where AFB1 and HBV pose a public health risk. Vaccination against hepatitis B and measures such as robust food safety systems to avoid hepatotoxicity and hepatocellular carcinogenesis induced by AFB1 is the most effective methods in the prevention of HCC induced by HBV and AFB1

Résumé

This study investigated the effect of extrusion process on engineering properties of noodles analogue from water yam, yellow maize, and African yam bean. Flour blend from water yam, yellow maize, and African yam bean were produced using the standard method of dry milling and was used to extrude noodles analogue using a brabender single screw laboratory extruder (Duisburg DCE 330 model) fitted with 3.0mmdie nozzle diameter. A central composite rotatable design (CCRD) with three variables, namely barrel temperature, feed moisture content and screw speed and five level coded – a, -1, 0, +1,, +a, was used for the study and data was generated at (p<0.05). Results of result of the engineering properties of noodles analogue showed that toque ranged from 22.4 to 60.50Nm/s, mass flow rate ranged from 33.90 to 109g/min-1, specific mechanical energy ranged from 100.19 to 385.97kj/kg, and the residence time ranged from 22 to 55s in this study. The CCRD was effective in explaining the effect of the process conditions on noodles analogue as influenced by barrel temperature, feed moisture content, and screw speed of the extruder. The data obtained from the study could be used for control of product characteristics.It was generally observed that the changing of feed moisture content, barrel temperature, and screw speed influenced an acceptable torque, mass flow rate, and residence time during the extruder operation. The noodles analogue produced at this cheap machine operating conditions could be projected for commercial production.

Aims: To evaluate the anti-hyperglycaemic efficacy and safety of the methanol extract of the combination of Senecio biafrae leaf, Xylopia aethiopica fruit, Carica papaya seed and Spondias mombin stem bark mixed together in ratio 1:1:1:1 Study Design: Extract of medicinal plants was assayed using glucose and streptozotocin-induced herperglycaemic rats model. Place and Duration of Study: Department of Pharmacognosy, Obafemi Awolowo University, IleIfe, Nigeria, between May, 2019 and January, 2022. Methodology: The extract of the combined plant parts was tested for toxicity in rats while its effects on glucose level, blood and biochemical components were also assessed. Its in-vitro anti-hyperglycaemic activity was assayed in ?-amylase and ?-glucosidase inhibitory models while its in-vivo effects were tested in glucose and streptozotocin-induced hyperglycaemic rats. The antioxidant activity of the extract was also carried out. Results: The extract did not show any adverse effects on blood sugar levels, haematological and biochemical parameters in normal rats in sub acute toxicity tests. The extract gave comparable (p > 0.05) ?-amylase and ?-glucosidase inhibitory effects to acarbose. In glucose-induced hyperglycaemic rats, its 100 mg/kg was the most effective dose with 19, 40, 43, and 57% activity that was significantly higher (p < 0.05) than the 10, 18, 24, and 40% activity given by glibenclamide (5 mg/kg) at the same time points. In streptozotocin-induced diabetic assay, its 50 mg/kg showed 31, 85, 85 and 82 % effects on days 4, 7, 10 and 14, respectively that was significantly higher than its 100 mg/kg and glibenclamide on days 7 and 10. The extract also elicited high free radical scavenging effects in all the antioxidant assays. Conclusion: The extract of the combination of four Nigerian antidiabetic plants mixed together in equal ratio gave significantly better antidiabetic activity at low doses than the individual plants without toxic effects.

This research work evaluates the production of biscuits from blends of acha and pigeon pea flour. Acha and pigeon pea flour were blended in the ratio 95:5, 90:10, 85:15, 80:20 and 100:0 (control) to produce biscuits. Biscuits made from this blend were analyzed for proximate analysis, antinutrient composition, physical attributes, and sensory qualities. The moisture, protein, fat, crude fiber, ash, and carbohydrate content of the biscuits ranged from 7.87-9.84%, 7.36-8.14%, 21.99-26.33%, 0.39-0.59%, 0.87-1.24%, and 58.06-63.01%, respectively. The antinutrient composition varied from 4.24-6.82% for tannin and 3.85-4.68% for phytate. The physical attributes of the biscuits ranged from 8.34-10.98 g, 0.56-0.85 cm, 4.77-5.03, 5.99-8.59 for weight, thickness, diameter and spread ratio, respectively. The sensory evaluation showed that the biscuit sample with the blend ratio of 95% and 5% (acha to pigeon pea flour) was the most acceptable because it gave the best colour, appearance, crispness and overall acceptability. The result shows that acceptable biscuits with improved nutritional attributes can be produced from the blend of acha and pigeon pea flour. This will eliminate or reduce the problems associated with protein-energy malnutrition common in most local communities, and reduce wheat importation, thereby increasing the use of the most underutilized sources of flour in most developing countries.

Résumé

An experiment was conducted to assess the effect of natural antioxidants (black pepper, green tea, roselle and their combinations) on meat quality of broiler chickens. A total of 270 1 - day old Arbor Acre broiler chicks were randomly distributed into nine treatments of three replicates each (10 birds in each replicate) in a 2 x 5 factorial arrangement for 2 inclusion levels (0.5g and 1.0g per kg of feed) of natural antioxidants (Control (CT), Green tea (GT), Roselle (RS), Black pepper (BP) and combination (CM) of the 3 antioxidants). At the end of the feeding trial (at 8 weeks), nine birds per treatment were immobilized, slaughtered, dressed, weighed and cut into primal cuts. The growth (initial and final body weight gain, average daily feed intake and weight gain, and feed conversion) and blood assay (haematology and serum biochemistry) of the birds were monitored while the breast and thigh meat cuts were subjected to physico-chemical and sensory analysis. The result indicated that, among examined natural antioxidants, BP improved the bird抯 live weight. High Density Lipoprotein value was highest (p<0.05) in control and closely followed by birds on GT, CM, BP and RS. The lowest blood (p<0.05) cholesterol was recorded in RS which was closely followed by GT and CM. Carcass evaluation showed that birds fed BP had better (p<0.05) live weight (2.05kg) and highest acceptability (p<0.05) for organoleptic properties. The breast meat weight was also highest (p<0.05) in BP. It was concluded that the natural antioxidants increased live weight, improved performance and reduced abdominal fat. RS reduced blood cholesterol while RS, CM (GT + RS + BP) inclusion improved serum total protein of broiler chickens. Inclusion of natural antioxidant in the diets of broiler is hereby advocated for achieving optimum broilers performance and meat quality.

A feeding trial, which lasted for seventy days, was conducted in which palm oil mill sludge and biodegraded sweet orange peel mixture was fed to substitute maize in broiler chicken diet at 0%, 5%, 10%, 15%, 20% and 25%. Sweet orange (Citrus sinensis) fruit peel was fermented by soaking for 48 h in retted cassava waste water (CWW) and sundried, to obtain biodegraded sweet orange peel (BSOP). Palm oil mill effluent was filtered with a 0.30 mm pore plastic mesh sieve, poured into a 0.75 ?m pore fine cheesecloth bag and allowed to stand for five hours to produce a paste of palm oil mill sludge (POMS). The POMS was mixed with BSOP in ratio 1:1, sundried, milled to produce a POMS-BSOP mixture. One hundred and eighty day-old Cobb 700 broilers divided into six equal parts, and three replicates of 10 birds each were used. Each part was assigned to one of 6 diets compounded with 0% (T1), 5% (T2), 10% (T3), 15% (T4), 20% (T5) and 25% (T6) of POMS-BSOP mixture. The microbial composition of retted CWW, chemical composition of the POMS-BSOP mixture, and the digestibility of nutrients by the broiler chickens were determined. Isolated from CWW were; Staphylococcus aureus, Streptococcus spp., Salmonella spp., Escherichia coli (bacteria), Aspergillus spp. (fungus) and Candidia spp. (yeast). POMS-BSOP was high in energy (4415.69 kcalME/kg), ether extract (41.50%), crude fibre (25.63%) and dry matter (92.28%), moderate in crude protein (6.83%), low in indigestible lignin (4.90% ADL), alkaloid (0.01%), tannin (0.02%), saponin (0.03%), phytate (0.05%), oxalate (0.15%) and flavonoid (0.17%). Dietary treatments significantly (P<0.05) affected digestibility of ether extract and metabolisable energy and crude protein digestibility by broiler chickens. Dietary maize can be replaced at up to 25% with a POMS-BSOP mixture to improve energy digestibility by broiler chickens.

Objective: The COVID-19 pandemic has had tremendous effect on the medical and scientific community. Measures instituted to curb the spread of the disease such as physical distancing and ban on large gatherings have significantly altered conferencing activities of medical professional associations. With no end in sight to the pandemic, it is certain that medical professional associations may have to make do with purely virtual or hybrid conferencing for some time to come. The objective of this paper is to highlight the benefits, disadvantages and challenges of virtual conferencing Methods: We searched Pubmed, Embase, Scopus, and AJOL databases from January 1, 2021 to December 31, 2021 for publications describing the benefits, disadvantages and challenges of virtual conferencing during the pandemic. This, together with authors' experience formed the resource base for this work. Conclusion: We propose ways the scientific community in Nigeria can maximize the virtual conferencing while the pandemic lasts. We also advocate increased discussion about how to improve the virtual conferencing culture and the development of guidelines for purely virtual or hybrid scientific conferences


Objectif de l'étude: La pandémie de COVID-19 a eu un effet considérable sur la communauté médicale et scientifique. Les mesures instituées pour freiner la propagation de la maladie, telles que l'éloignement physique et l'interdiction des grands rassemblements, ont considérablement modifié les activités de conférence des associations professionnelles médicales. Sans fin en vue de la pandémie, il est certain que les associations professionnelles médicales devront se contenter de conférences purement virtuelles ou hybrides pendant un certain temps encore. L'objectif de cet article est de mettre en évidence les avantages, les inconvénients et les défis de la conférence virtuelle. Méthodes de l'étude : Nous avons effectué des recherches dans les bases de données Pubmed, Embase, Scopus et AJOL du 1er janvier 2021 au 31 décembre 2021 pour trouver des publications décrivant les avantages, les inconvénients et les défis des conférences virtuelles pendant la pandémie. Ceci, combiné à l'expérience des auteurs, a constitué la base de ressources pour ce travail. Conclusion : Nous proposons des moyens pour que la communauté scientifique du Nigéria puisse maximiser les conférences virtuelles pendant la durée de la pandémie. Nous préconisons également une discussion accrue sur la manière d'améliorer la culture des conférences virtuelles et l'élaboration de lignes directrices pour les conférences scientifiques purement virtuelles ou hybrides

BACKGROUND: The exponential rise in the prevalence of Type 2 Diabetes (T2DM) necessitates the introduction of strategies for early diagnosis to reduce the burden of the disease. This study assessed the prevalence of prediabetes and also determined the 10-year risk of developing T2DM in Southern Nigerian rural communities by adopting the validated Finnish Diabetes Risk Score (FINDRISC) tool. METHODS: 273 participants from 3 Southern rural communities aged 18 years and older were recruited in this cross-sectional study.Data in the FINDRISC stratification tool and Random Blood Glucose (RBG) variables were obtained for the participants. IBM SPSS version 21 was used to analyze the data with a level of significance put at p< 0.05. RESULTS: The participants' mean age was 54.20±16.61 years. The prevalence of prediabetes among the study participants based on RBG was 4.8% (3.8% of males and 6.6% of females, respectively).Most of the study participants (41%) had a low risk of developing T2DM which meant that 1 in 100 participants would become diabetic in a 10-year period, based on the FINDRISC scale. Amongst the male participants, the Total Diabetes Risk Score (TDRS) showed significant positive correlation with the RBG (r=0.315, p=0.001); similarly, a weak positive correlation between TDRS and RBG was noted among female participants. CONCLUSION: The propensity of developing T2DM in 10 years was indisputably low amongst rural dwellers in the Southern Nigerian rural communities studied. Further studies to compare the risk of developing T2DM between rural and urban communities would be required

BACKGROUND: Colorectal cancer (CRC) is the fourth most common cancer in Nigeria, and it affects mostly persons in their middle age. In a bid to gain some insight into the molecular characteristics of CRC in our environment, we set out to investigate the expression of COX-2 and HER-2 among Nigerian subjects. OBJECTIVES: To evaluate the expression of COX-2 and HER2 and determine their correlation with clinicopathologic parameters in surgically resected histologically diagnosed cases of colorectal cancer. METHODS: Fifty-three paraffin-embedded tissue blocks of colorectal resections and corresponding patient information were retrieved from the archives of the Anatomic and Molecular Pathology Department of Lagos University Teaching Hospital.A 4-micron slide section was obtained from each specimen and immunohistochemistry for COX-2 and HER-2 expression was performed. RESULTS: Mean age of cases was 53.9years with an almost equal M:F ratio of 1.12:1. Half of the cases were moderately differentiated adenocarcinoma and 17% were high grade tumors.Eighty three percent of the tumours showed positive cytoplasmic COX-2 expression and extremely low membranous HER-2 positivity was observed in 2%. There was no significant correlation between COX-2 expression and age, gender, tumour location, tumour size, depth of invasion or lymph node status.However, COX-2 expression revealed a significant correlation with tumour grade (p= 0.013). CONCLUSION: This study detects a high COX-2 and low HER2 expression in colorectal cancer using immunohistochemistry,suggesting a possible role for COX-2 in CRC pathogenesis.This report should trigger further investigations of both markers vis-à-vis the management of CRC in our environment. SUMMARY Aims: This study investigated the bioactivity of the crude leaf extract (CLE) and fractions hexane (HX) and ethyl acetate (EtOAc) from Talinum paniculatum alone and in association with fluconazole (FLC) against reference strain and clinical isolates of FLC-resistant Candida albicans. Furthermore, the antioxidant capability, chemical composition of this plant, and the effect's underlying mechanisms were evaluated. Methods: The antifungal activity was evaluated using checkerboard assay to establish the minimum inhibitory (MIC) and minimum microbicidal concen trations (MMC). During FLC and plant products challenges, the reactive oxygen species (ROS) generation (hydroxyl radicals [HO●]) were detected in C. albicans cells using the membrane-permeable fluorescent probes APF and HPF. High-performance liquid chromatography (HPLC) profile, quantitative analysis of antioxidant compounds, and free radical scavenging activity (DPPH assay) tests were performed. Results: The CLE and fractions presented outstanding antifungal activity and selectivity against C. albicans cells but had no synergistic effect's with FLC. The MIC values for CLE and its fractions against C. albicans reference strain were in the order of HX (31.25 µg ml-1) < EtOAc (62.5 μg ml-1) < CLE (500 μg ml-1), and against FLC-resistant C. albicans HX (125 μg ml-1) = EtOAc < CLE (500 μg ml-1). CLE and its fractions had more potent antifungal activities than FLC against the clinical isolates. Moreover, fungicidal effect's for these plant products were demonstrated against FLC-resistant C. albicans, which further conirmed an antifungal potential. Conversely, during association, plant products were shown to cause an increase in FLC MIC anywhere from 2- to 16-fold. FLC exposure led to an increase in the steady-state levels of ROS (HO●) in C. albicans cells. Next, we found that the increases in FLC MICs were owing to action of antioxidants containing-CLE and its fractions in preventing FLC-induced ROS-mediated growth inhibition of C. albicans. Conclusion: T. paniculatum can be a source of bioactive compounds with antifungal potential. However, because of the common use of its edible leaf, caution is advised during therapy with FLC (since it can decrease FLC susceptibility).


RESUMEN Objetivos: este estudio investigó la bioactividad del extracto de hoja en bruto (EHB) y las fracciones hexano (HX) y acetato de etilo (AcOEt) de Talinum paniculatum solo y en asociación con fluconazol (FLC) contra cepas de referencia y aislados clínicos de Candida albicans resistente a FLC. Además, evaluó la capacidad antioxidante, la composición química de esta planta y los mecanismos subyacentes del efecto fungicida. Métodos: la actividad antifúngica se evaluó mediante microdilución en caldo para establecer las concentraciones inhibitorias mínimas (CIM) y microbicidas mínimas (CMM). Durante el tratamiento con FLC y productos vegetales se detectó la generación de especies reactivas de oxígeno (ERO) (radicales hidroxilo [HO●]) en células de C. albicans utilizando las sondas fluorescentes permeables a la membrana APF y HPF. El perfil de cromatografía líquida de alta resolución (CLAR), el análisis cuantitativo de compuestos antioxidantes y el ensayo DPPH fueron evaluados. Resultados: el EHB y las fracciones presentaron una excelente actividad antifúngica y selectividad contra las células de C. albicans, pero no tuvieron efectos sinérgicos con FLC. Los valores de CIM para EHB y sus fracciones contra la cepa referencia de C. albicans fueron del orden de: HX (31,25 μg ml-1) < AcOEt (62,5 μg ml-1) < EHB (500 μg ml-1), y contra C. albicans resistente a FLC: HX (125 μg ml-1)= AcOEt < EHB (500 µg ml-1). EHB y sus fracciones fueron más potentes antifúngicos que FLC contra los aislados clínicos. Además, estos productos vegetales tienen efectos fungicidas contra C. albicans resistentes a FLC, esto conirmó el potencial antifúngico. Por el contrario, durante la asociación se demostró que los productos vegetales causan un aumento en la CIM de FLC de 2 a 16 veces. La exposición a FLC aumentó los niveles de ERO (HO●) en las células de C. albicans. Los aumentos en las CIM de FLC se debieron a la acción de los antioxidantes presentes en EHB y sus fracciones para prevenir la inhibición del crecimiento mediada por ERO inducida por FLC en C. albicans. Conclusión: T. paniculatum puede ser una fuente de compuestos bioactivos con potencial antifúngico. Sin embargo, debido al uso común de su hoja comestible, se recomienda usarla con precaución durante la terapia con FLC (ya que puede disminuir la susceptibilidad a FLC).

According to World Health Organization, health not only refers to the absence of physical diseases, but also includes psychological and social well-being of individuals.[1] Quality of life (QoL) on the other hand, lacks a single, universally accepted definition.[2] However, QoL may be defined subjectively as "an individual's perception of his/her life in the context of the culture and value system in which they live, and in relation to their goals, expectations, standards and concerns.[3] QoL also has an objective component which can be measured objectively using validated instruments (e.g. questionnaires) in which a score is assigned to specific indices or characteristics in different domains along a scale in a continuum.[4],[5],[6]Visual function is important for optimal orientation in functional and social life and has effects on physical, psychological, mental and emotional well-being of the individual.[7] Visual function therefore is a domain under which QoL may be assessed. This is referred to as vision-specific or vision-related QoL. The National Eye Institute Visual Function Questionnaire (NEI-VFQ) is an example of an instrument designed and validated for the objective assessment of vision-specific QoL.[8]Broman et al.[9] revealed that a visual acuity of at least 6/12 was found to negatively impact on n individual's quality of life. This was also reiterated by Bekibele et al.[10] The Proyecto VER study[9] observed that visual impairment was associated with a decrease in the quality of life among elderly patients, and that the severity of ocular diseases was related to the level of visual impairment. In relation to persons with no visual impairment, persons with bilateral mild and unilateral or bilateral moderate or severe visual impairment report greater difficulties in performing most vision-dependent daily activities, experience vision-related dependency and poorer vision-related mental health.[11]Ee Munn Chia et al.[12] further disclosed that the impact of visual impairment on mental domains was much greater compared to co-existing medical conditions like stroke.Therefore, data is needed on the effect of ocular diseases on vision-related quality of life among the elderly for the purpose of making recommendations for improved eye care with resultant improvement in their quality of life

@#Treatment Failure with chloroquine is one of the challenges that faced the dedicated efforts to eradicate malaria This study aims at investigating the impact of treatment failure with chloroquine on the progression of the disease-induced histo-pathogenic and immunogenic outcomes. To achieve this, Rane’s protocol with modifications was applied on a model of Plasmodium berghei ANKA infected ICR mice to determine the dose response curve of chloroquine and to screen the treatment impact on the disease progression. Chloroquine was given at 1, 5, 10, 15 and 20 mg/kg once the parasitemia reached to 20-30% (the experimental initiation point). During the subsequent days, the mice were monitored for changes in the clinical signs, hematology parameters and the progress of the parasitemia until the parasitemia reached to 60-70% (the experimental termination point) or up to 10 days after chloroquine administration in case of achieving a complete eradication of the parasite. At the end, the mice were exsanguinated and their blood and organs were collected for the biochemistry and the histology study. A complete eradication of the parasite was achieved at 20 mg/kg while recrudescence was observed at the lower doses. At 1 mg/kg, the parasite growth was comparable to that of the positive control. The histo-pathogenic and immunogenic changes were stronger in the groups that experienced recrudescence (at 5 and 10 mg/kg). All in all, the study highlights the possibility of having a worsened clinical condition when chloroquine is given at its sub-therapeutic doses during malaria treatment.

Metastasis is the most frequent cause of mortality in cancer patients and symptoms related to a secondary deposit are a common form of presentation in malignancies. Cancers of the prostate commonly present with metastasis to the vertebrae, and less commonly to the lungs and liver. Metastasis to soft tissue is extremely rare and a highly unlikely form of presentation. However, with rising cancer rates in the developing world, secondary tumors should be excluded when soft tissue masses are encountered
