Objective To treat calcaneal fracture with plate fixation combined with 64-slice CT 3D reconstruction and to analyze the efficacy.Methods Totally 86 patients with Sanders Ⅲ and Ⅳ types of calcaneal fractures underwent surgical treatment,and 64-slice CT 3D reconstruction was used to record the length,height,width,Bohler angle and Gissane angle of calcaneus before and 1 d,2 d,3 d and 1 a after the treatment.Postoperative evaluation was executed based on AOFAS Ankle Hindfoot Scale.Results All the fractures healed,of which,there were 60 ones scored as excellence,19 ones in good condition and 7 ones in satisfactory condition with the excellence and goodness rate being 91.9%.Complications occurred in 4 patients,with 3 cases of delayed healing and one case of infection.Tbere were obviously differences between the lengths,heights,widths,Bohler angles and Gissane angles before and after the treatment,while the differences were not significant between the measured values 1 d and 1 a after the treatment.Conclusion 64-slice CT 3D reconstruction is an excellent clinical auxiliary tool,and is of great value for fracture typing,operational plan preparation,postoperative evaluation and late efficacy assessment.

To put forward spatial resolution detection requirements of 64-slice helical CT based on analyzing the present situation of CT spatial resolution detection. The requirements for spatial resolution in the existing industrial standards were analyzed, and some test data were used to prove the deficiency of the standards. Spatial resolu-tion detection requirements were proposed for 64-slice helical CT, and the factors affecting spatial resolution were ex-plored. The deficiencies in the original standards were eliminated. The proposed requirements may facilitate the spatial resolution detection of 64-slice CT, and thus can enhance image quality for clinical diagnosis and treatment.

Objective:To investigate the value of 64-slice spiral computed tomography (64SCT) in preoperative evaluation of spinal vascular intervention.Methods: Seventeen patients with spinal vascular malformations(SVM) scheduled for spinal vascular interventional therapy in our hospital between January 2012-December 2014 were collected, all received 64-detector spiral CT angiography (64-MDCTA) before surgery, image postprocessing technologies such as volume rendering (VR), maximum intensity projection(MIP) and multiplanar reconstruction(MPR) were applied to conduct three-dimensional reconstruction, the traveling case of spinal cord feeding artery was analyzed and were compared with DSA test results.Results: All of the 17 patients clearly showed the abnormality of spinal cord blood vessels and lesions scope, the SVM classification and lesions scope were exactly the same as that of DSA and intraoperative performance, the coincidence rate was 100%;1 case of patient was not shown fistula, the rest of the patients clearly shown the draining veins around the spinal cord, open position and flow tendency of radiculomedullary artery, the coincidence rates were 100.00%, 94.12%%, 100.00%.Conclusion: 64-MDCTA is important for SVM patients to preoperative evaluation of spinal vascular intervention, for it can fully and accurately reflect the open position and flow tendency of radiculomedullary artery, spatial relationship with the surrounding vessels, spinal cord and vertebral situation, should be widely applied.

Objective To explore the diagnosis reliability of the 64-slice spiral CT to the patency of coronary arterial stents after PCI .Methods From September 2009 to December 2012 ,84 patients with 132 coronary artery stents accepted 64-slice spiral CT coronary artetry imaging and coronary angiography .The patency of the stents and the vascular were assessed compared with coro-nary angiography images ,and the diagnostic sensitivity ,specificity and accuracy were analyzed and compared .Results By calculat-ed the number of patients ,the stent occlusion sensitivity and accuracy of 64-slice spiral CT coronary artetry imaging were both 100% .The stent occlusion and stenosis (>50% ) sensitivity ,false negative and positive predictive value were 96 .7% ,3 .4% and 100% .64-slice spiral CT reconstruction imaging of the stent occlusion and stenosis (>50% ) compared with coronary angiography had no statistical difference(χ2 =0 .022 ,P=0 .883) .Conclusion Compared with coronary angiography ,64-slice spiral CT coronary artetry imaging has the higher accuracy and reliability .

BACKGROUND: This study was to investigate the feasibility of real-time 3-dimensional echocardiography (RT3DE) for the analysis of biventricular ejection fractions and volume measurements in patients with cor pulmonale and the correlations of RT3DE results with 64-slice multi-detector cardiac computed tomography (64-MDCT) results. METHODS: This study included a total of 22 patients (59.3 +/- 16.6 years of age; 10 males and 12 females) who showed flattening or reverse curvature of the interventricular septum and severe pulmonary hypertension [mean right ventricular (RV) systolic pressure = 66.8 +/- 19.7 mmHg] on 2-dimensional transthoracic echocardiography due to cor pulmonale. Biventricular end-diastolic and end-systolic volumes were measured by RT3DE and 64-MDCT. The severity of D-shaped deformation was evaluated by using left ventricular (LV) eccentricity index (ratio of diameters parallel/perpendicular to the interventricular septum on parasternal short axis images of the papillary muscle level). RESULTS: There were moderate correlations between biventricular volumes measured by RT3DE and 64-MDCT except for LV end-systolic volume (59.8 +/- 17.1 vs. 73.2 +/- 20.2 mL, r = 0.652, p = 0.001 for LV end-diastolic volume; 30.6 +/- 9.1 vs. 30.8 +/- 12.5 mL, r = 0.361, p = 0.099 for LV end-systolic volume; 110.1 +/- 42.9 vs. 171.1 +/- 55.3 mL, r = 0.545, p = 0.009 for RV end-diastolic volume; and 80.9 +/- 35.0 vs. 128.7 +/- 45.1 mL, r = 0.549, p = 0.005 for RV end-systolic volume respectively). CONCLUSION: This study suggests that RT3DE may be a modest method for measuring distorted biventricular end-systolic and end-diastolic volumes in patients with cor pulmonale.

OBJECTIVE: The purpose of this study is to determine the effects of propranolol on the left ventricular (LV) volume during CT coronary angiography. MATERIALS AND METHODS: The LV volume of 252 normal Chinese subjects (126 subjects with propranolol medication and 126 age- and gender-matched Chinese subjects without medication) was estimated using 64 slices multi-detector CT (MDCT). The heart rate difference was analyzed by the logistic linear regression model with variables that included gender, age, body height, body weight, systolic blood pressure (SBP), diastolic blood pressure (DBP) and the dosage of propranolol. The following global LV functional parameters were calculated: the real-end diastolic volume (EDV), the real-end systolic volume (ESV) and the real-ejection fraction (EF). RESULTS: The female subjects had a greater decrease of heart rate after taking propranolol. The difference of heart rate was negatively correlated with the dosage of propranolol. The real-EDV, the real-ESV and the real-EF ranged from 48.1 to 109 mL/m2, 6.1 to 57.1 mL/m2 and 41% to 88%, respectively. There was no significant difference in the SBP and DBP between the groups without and with propranolol medication (123 +/- 17 and 80 +/- 10 mmHg; 120 +/- 14 and 80 +/- 11 mmHg, respectively). The real-EDV showed no significant difference between these two groups, but the real-ESV and real-EF showed significant differences between these two groups (69.4 +/- 9.3 and 70.6 +/- 8.9 mL/m2; 23.5 +/- 5.7 and 25.6 +/- 3.7 mL/m2, 66.5 +/- 5.1% and 63.5 +/- 4.6%, respectively). CONCLUSION: The difference of heart rate is significantly influenced by gender and the dosage of propranolol. Propranolol will also increase the ESV, which contributes to a decreased EF, while the SBP, DBP and EDV are not statistically changed.

Objective To evaluate the clinical diagnostic value of 64-slice computed tomography coronary angiography (CTCA) for the detection of significant coronary artery stenosis ( ≥50% lumen reduction). Methods We enrolled 61 patients (47 male, 14 female; mean age 62.8 ± 9.7 years) with suspected coronary artery disease into the study and performed selective coronary angiography (SCA) within 2 weeks after CTCA examination. Results One patient was excluded from the analysis because of unsuccessful CTCA evaluation as a result of severe coronary artery calcification in 4 vessels. 240 vessels of remaining 60 patients were achieved good coronary artery imaging by CTCA. Sensitivity,specificity and positive and negative predictive value of CTCA for detecting significant coronary artery sternosis on a per-vessel basis were 90.0 % ,91.9% ,84. 7% and 94.8% ,respectively. Conclusions Our data indicated that 64-slice CTCA is a noninvasive method to reliably rule out significant coronary sternosis with high diagnostic value in negative prediction.

ObjectiveTo study the types of anatomical variations of hepatic arteries. Methods Hepatic arteries of 64-slice spiral CT scanning data were three-dimensional constructed by using self-designed software. The types of anatomical variations were analyzed and classified with Michels' classification criteria. Results The model presented with realistic profile of hepatic arteries which allowed vivid three-dimensional observation. Of these patients, 40 had normal hepatic arteries (60.61%), 26 had variations (39.39%), and 5 had infrequent aberrant hepatic arteries that was not included in Michels' classification (7.58%). Conclusion Three-dimensional model of hepatic arteries can volumetricly display the anatomical variations of hepatic arteries.

Objective To explore the diagnostic performance of 64-slice spiral CT combined with exercise treadmill test for coronary artery disease ( CAD). Methods Eighty-six patients suspected of CAD were divided into low risk group, intermediate risk group and high risk group according to estimated pretest probabilities of CAD. All patients underwent coronary angiography, 64-slice spiral CT and exercise treadmill test. With coronary artery stenosis rate >50% as positive findings, the sensitivity, specificity, positive predictive value ( PPV), negative predictive value ( NPV) and accuracy of 64-slice spiral CT and 64-slice spiral CT combined with exercise treadmill test in diagnosis of CAD were calculated. Results With coronary angiography as the "golden criteria", the sensitivity, specificity, PPV, NPV and accuracy of 64-slice spiral CT in diagnosis of CAD were 95.2% , 88.6% , 88.9% , 95.1% and 91.9% , respectively. The sensitivity of low risk group, intermediate risk group and high risk group was 100% , 100% and 92.6%, specificity was 94.4% , 94.1% and 66.7%, PPV was 80.0%, 91.7% and 89.3%, NPV was 100% , 100% and 75.0%, and accuracy was 95.5% , 96.4% and 86.1%, respectively. The sensitivity, specificity, PPV, NPV and accuracy of 64-slice spiral CT combined with exercise treadmill test in diagnosis of CAD were 97.6%, 97.7%, 97.6%, 97.7% and 97.7%, respectively. Conclusion 64-slice spiral CT combined with exercise treadmill test works well in screening CAD, especially for those with a low or intermediate estimated pretest probability.

Objective To discuss the value of nursing work in 64-slice CT urography. Methods 240 patients participated in the 64-slice CT urography, inquiring medical history and iodine allergic history, paying attention to their psychological state and finishing iodine preliminary test before examination, ob-serving patients carefully, pre-judging any possible adverse effect and formulating corresponding measures during the examination, giving expectant treatment according to the various condition and nursing instruc-tion. Results 239 patients passed through the examination smoothly, agents exosmosis happened in one case and led to failure, but satisfactory image was obtained after rescanning. Conclusions Nursing oper-ation penetrates every step of 64-slice CT urography, which is a non-traumatic, low-expense and high-safety examination. Effect nursing directly influences the accuracy of the results, proficient nursing opera-tion and patient psychological nursing is the important part of it.

Objective To compare the image quality and radiation dose between prospective ECG-trigering and retrospective ECG gating technique in coronary artery imaging. Methods 33 patients suspected coronary artery disease were included in this study and divided into experimental group (prospective ECG-triggering coronary artery imaging, heart rate < 65 bpm, 16 patients)and control group (retrospective ECG gating coronary artery imaging, heart rate < 75 bpm, 17 patients). Radiation dose was recorded for statistical analysis. At the same time, image quality was evaluated. Results There were no significant differences in image quality 3.2 msy. Effective dose reduced 76.47 %. Conclusions Prospective ECG gating coronary artery imaging can obtain the similar image quality compared with prospective ECG gating coronary artery imaging but the effective dose reduced 76.47 %. Prospective ECG gating coronary artery imaging has clinical value to peoples who are able to not accept high radiation dose and with low heart rate.

Objective To discuss clinical value of 64-slice spiral CT scanning in fast evaluation of severe multiple trauma.Methods A retrospective study was carried out on data of 72 multiple trauma patients diagnosed with systemic 64-slice spiral CT scanning in our hospital from January 2006 to June 2008.Of all,28 patients with traffic injuries,24 with falling injuries,10 with blunt injuries,6 with stab injuries and 4 with explosive injuries.Results There were 72 multiple trauma patients with mean scanning time of 10.2 seconds.The CT scanning showed both craniocerebral and thoracic injuries in 30 patients,brain injuries in 16,thoracoabdominal injuries plus pelvic injuries in 10,extremity and brain injuries in 14 and systemic soft tissue injuries in 2.Isotropic characteristics and postprocessing function of 64-slice spiral CT scanning could accurately diagnose and evaluate injury severity of multiple trauma.Conclusions 64-slice spiral CT scanning is a fast and effective method for fast evaluating injury severity of multiple trauma,for it can not only shorten checking time,reduce movements of the patients and relieve the pain of the patients,but also can avoid defects induced by respiratory movement and provide fairly integrated imaging materials by its isotropic characteristics and pestprecessing function.

@# Objective To explore the application of 64-slice spiral CT angiography (CTA) in the evaluation of cerebral vascular disease. Methods 160 cases with suspected cerebral vascular disease underwent CTA with 64-slice spiral CT. Two dimensional and three dimensional reformation were performed in all cases including multiplanar reconstruction (MPR), maximum intensity projection (MIP), volume rendering (VR). Both axial and reformatted images were analyzed by two radiologists. Results 94 cases of 160 were diagnosed as abnormal, including 50 stenosis or obstructive disease, 21 aneurysm, 1 post-intravascular stent, 4 cliped aneurysm, 3 arteriovenous malformation, 4 moyamoya disease, 4 carotid-cavernous fistula, 4 venous sinus thrombus, 1 straight sinus occlusion with perpetual falcial sinus, 1 great cerebral venous aneurysm, 1 perpetual sublingual artery. Conclusion 64-slice spiral CTA is a valuable diagnostic method for various cerebral vascular disease.

Objective To discuss the feasibility of displaying the extrahepatic feeding arteries in hepatocellular carcinoma with the help of multi-slice spiral CT 3D reconstruction and to assess the clinical value of this technique. Methods Triple-phase enhanced CT scanning with a 64-slice spiral CT scanner was performed in 89 patients with advanced primary hepatocellular carcinoma (HCC). Three-dimensional reconstruction techniques,including maximum intensity projection (MIP) and volume rendering (VR),with arterial phase images,were used to display the origination and course of both the intrahepatic and extrahepatic supplying arteries of HCC. The results were compared with the angiographic findings. Results Of 59 cases with massive type HCC,extrahepatic supplying arteries were found in 33. In 21 cases of diffuse type HCC four showed extrahepatic supplying arteries,and in nine cases of nodular type HCC only one had extra-hepatic supplying arteries. The HCC could get their extrahepatic blood supply via eight pathways. A total of 44 extrahepatic supplying arteries were detected,and 19 anomalously originated hepatic arteries were found. Conclusion The extrahepatic supplying arteries in hepatocellular carcinoma are common findings and their supplying pattern are extremely varied,which may be associated with the type and location of the tumors. Three-dimensional reconstruction technique with the help of triple-phase enhanced CT scanning on a 64-slice spiral CT scanner can provide excellent images as vivid and ideal as angiography can afford. Therefore,the times of angiography examination,the use of contrast media as well as the dose of radiation to both the patients and the physicians can be reduced as far as possible. The detailed information about extrahepatic blood supply is very useful for improving the therapeutic result of HCC.

Objective To evaluate the diagnostic value and limits of 64-slice spiral coronary artery imaging,by com- parison with selective coronary angiography,in detection of coronary heart disease.Methods Fourty-two patients sus- pected CAD were performed 64-slice spiral CT coronary imaging and selective coronary artery angiography in two weeks, comparative analysis of results were progressed consequently.Results The sensitivity,specificity and positive and nega- tive predictive value to identify≥50% stenosis branches was 90.5%,96.6%,85.9% and 97.8%,respectively.The sen- sitivity,specificity and positive and negative predictive value to identify≥75% stenosis branches was 93.5%,98.9%, 87.9% and99.4%,respectively.Conclusion As a noinvasive quantitative assessment of coronary artery stenoses exami- nation,64-slice spiral CT is a valuable method to detect and diagnose the disease of coronary artery,but its clinical use maybe presently be limited due to image quality in a number of cases.

Objective To investigate the value of multi planar reconstruction (MPR) with 64-slice spiral CT for diseases of lumbar disc, vertebra and accessory etc. Methods 50 cases of MPR were compared with 64-slice spiral CT on protrusion of lumbar disc at radom. Results MPR with 64-slice spiral CT showed not only the protrusion of lumbar disc but also the diseases of the vertebra,the accessory, and the adjacent tissue. Conclusion MPR with 64-slice spiral CT has a good value in diagnosis of disease of lumbar disc, vertebra, accessory and the adjacent tissue.

The 64 slices PET/CT is significant in directing coronary heart disease diagnosis and treatment. The PET of the 18F-FDG cardiac muscle metabolism and coronary artery imaging can be operated by this equipment at the same time, which can confirm the activeness of the cardiac muscle and the blood-supply for the coronary arterial distributing. This has a determinative meaning for the choosing of Rx.

Objective To explore the diagnosis evaluation of 64-slice spiral CT angiography(CTA) in cerebral vascular disease.Methods 64-slice spiral CT angiography of 48 patients with suspected clinically cerebral vascular diseases were analyzed retrospectively.The source images were got by TOSHIBA Aquilion VCT Scanner,and all axial images were transferred to an external workstation.The reconstructed images were processed into volume rendering(VR),multiplanner reconstruction(MPR) and maximum intensity projection(MIP),the values were valuably compared with the results of surgery or DSA.Results 30 cases were aneurysms,4 cases were AVM,8 cases were cerebral artery stenosis or occlusion,6 cases were normal.Conclusion The 64-slice spiral CTA is a valuable diagnostic method for various cerebral vascular diseases.
