Résumé

Objective:To compare the effects of two methods of marking surface landmarks on the patient’s positional stability when using a multifunctional body board in combination with thermoplastics to fix the abdominal and pelvic areas for radiotherapy patients.Methods:50 subjects who underwent positional fixation using a multifunctional body board in combination with thermoplastics from August 2022 to January 2023. The subjects were divided into two groups, A and B, with 25 cases each, according to the different methods of body surface marking. In group A, landmarks were marked on the body surface on the top edge of the thermoplastics. In group B, three sets of surface landmarks were marked on the patient’s body according to the laser line on the projection of the patient’s body surface when the thermoplastics were completed. Manual registration is performed using L3 to L5 as the main registration targets. The pre-treatment CBCT image is used to analyze the first-time positioning pass rate, setup errors in the x-, y-, and z-axis directions, and the distribution of positive and negative setup errors in both groups of patients. Results:The pass rates of the first-time positioning of patients in Groups A and B were 76.9% and 86.1%, respectively, which met the clinical requirements. Group B had a better first-time positioning pass rate than group A, and the difference between the two groups was statistically significant ( P < 0.05). The pendulum errors of group B were smaller than those of group A in both the x-axis and y-axis (all P < 0.05), and the difference between the two groups in terms of the pendulum errors in the z-axis direction was not statistically significant (all P > 0.05). The difference in the frequency distribution of the pendulum error in the positive and negative directions of the x- and z-axis between the two groups was not statistically significant (all P > 0.05). The difference in the frequency of distribution of the pendulum error in the positive and negative directions of the y-axis between the two groups was statistically significant ( P < 0.05). Conclusions:The proposed two methods of surface landmark marking are generally in line with the positioning requirements for conventional fractionation radiotherapy for abdominal and pelvic patients. Using a laser line on the projection of the patient’s body surface for three sets of surface landmark markings produces smaller setup errors and is better than using the top edge of the thermoplastics for surface landmark markings, improving the positional stability of abdominal and pelvic patients.

Objective:To study the impact of the Varian real-time position management (RPM) respiratory gating system on radiotherapy planning dosimetry.Methods:The radiotherapy plans of 40 cases with thoracic or abdominal tumors were retrospectively selected in this study. The motion phantom for quality control was adopted to generate respiratory gating signals, and the 30%-60% stable phase at the end of expiratory was selected as the respiratory gating window. The dose verification for the abovementioned radiotherapy plans was performed using the Portal Dosimetry (PD) system under RPM respiratory gating mode with the Edge accelerator. Afterwards, dose analysis was performed with different γ passing rate criteria and the distribution characteristics of γ values were analyzed. Finally, the verification results between the non-gating mode and the gating mode were compared.Results:Under the respiratory gating mode, the passing rates of all intensity-modulated radiation therapy/volumetric-modulated arc therapy (IMRT/VMAT) plans with or without flattening filters were over 95.5% by γ criteria of (3%, 3 mm) or (3%, 2 mm) and were over 90% by stricter γ criteria of (2%, 2 mm). All plans met the clinical requirements recommended by the American Association of Physicists in Medicine (AAPM). The passing rates of dose verification under non-gating mode were slightly better than those under respiratory gating mode, and the differences between the two modes were statistically significant (3%/3 mm, Z =-1.45; 3%/2 mm, Z =-2.86; 2%/2 mm, Z =-3.70; 1%/1 mm, Z =-4.52; P<0.05). There was no significant difference in the minimum and maximum values of γ and the share of γ > 1.5 of plan verification result under the two modes. However, the average value and standard deviation of the γ were generally smaller under the non-gating mode. Conclusions:The impact of the introduction of RPM respiratory gating technology on dose is clinically acceptable, and the execution of these plans in this gating mode is safe and reliable.

Objective: To compare setup errors between patients using the customized Klarity AccuCushion® with a thermoplastic fixation mask and patients using a thermoplastic fixation mask or vacuum fixation cushion alone while receiving radiotherapy. Methods: A total of 66 patients with head and neck (H&N) tumors (n=27) or thoracic and abdominal tumors (T&N) tumors (n=39) were included during Jaurnary 2018 to December 2019. 15 H&N cancer patients using only a single head-neck-shoulder mask were categorized into group A; 12 patients using a customized Klarity AccuCushion® and head-neck-shoulder mask were categorized into group B. Among T&A cancer patients, 19 patients using only a vacuum fixation cushion were classified into group A; the remaining 20 patients using a customized Klarity AccuCushion® and thermoplastic fixation mask were classified into group B. Cone-beam computed tomography was performed, and the setup errors were evaluated. The setup errors in the left-right (LR) direction, superior-inferior (SI) direction, anterior-posterior (AP) direction, and for rotation were compared between groups A and B. Results: Among H&N cancer patients, the setup errors in group B in the LR direction, SI direction, and for rotation were 0.06±0.06 cm, 0.08±0.07 cm, and 0.12±0.17°, respectively, which were smaller than those in group A (0.10±0.11 cm, 0.13±0.14 cm, and 0.25±0.47°, respectively). The differences in setup errors in the LR direction, SI direction, and for rotation were significant between the two groups (P0.05). For T&A cancer patients, significant differences were found in setup errors between the two groups (P<0.05) in the LR direction (group B vs. group A: 0.10±0.08 cm vs. 0.14±0.12 cm) and for rotation (group B vs. group A: 0.09 ± 0.18° vs. 0.22 ± 0.39°). No significant differences were observed in the setup errors in the SI and AP directions. Conclusions: Compared with the immobilization techniques using only a thermoplastic mask and only a vacuumed fixation cushion, the technique using a customized Klarity AccuCushion® with a thermoplastic fixation mask can improve repeatability, stability, and setup errors in radiotherapy.

ABSTRACT: This report provides an unpublished account of an intra-abdominal myolipoma in a 11-year-old female Poodle with hyperadrenocorticism and diabetes mellitus. The patient was well controlled for both endocrine diseases; however, was suffering for severe abdominal discomfort. Ultrasound scan revealed a huge abdominal mass. At necropsy a pinkish mass (35cm x 28cm x 12cm) of elastic consistency attached to the broad ligament of the left uterine horn was reported. Histologically, this mass was constituted of a mixed population of well-differentiated adipocytes and of spindle-shaped mesenchymal cells. Both tumor cells were positive for vimentin at immunohistochemistry, while the spindle-shaped cells were positive for desmin and actin, compatible with smooth muscle cells. Immunohistochemistry was crucial for myolipoma characterization.


RESUMO: Este relato apresenta um caso inédito de miolipoma intra-abdominal em um poodle fêmea de 11 anos de idade com hiperadrenocorticismo e diabetes mellitus concomitante. A paciente apresentava um bom controle das doenças endócrinas, porém, grande desconforto abdominal. Ao ultrassom abdominal evidenciou-se uma massa de grandes proporções. Na necropsia uma massa rosácea (35cm x 28cm x 12cm) de consistência elástica unida ao ligamento largo do corno uterino esquerdo foi identificada. Histologicamente esta massa era constituída de uma população mista de adipócitos bem diferenciados e células fusiformes mesenquimais. Ambos tipos celulares foram positivos para vimentina na imuno-histoquímica, enquanto as células fusiformes foram positivas também para desmina e actina, compatível com músculo liso. A imuno-histoquímica foi crucial para o diagnóstico do miolipoma.

Résumé

Background: Abdominal masses are often incidentally discovered by a parent while bathing the child, palpated unexpectedly on routine physical examination, or detected on abdominal imaging. Aim: The objective of the present study was to observe intra-abdominal tumors in children less than 12 year. Materials and methods: Total 17 intra-abdominal tumors of both sexes under 12 years of age was collected and analyzes to determine the various types of intra-abdominal tumors in relation to age and sex. Study was done for a period of 4 years. Results: 1 to 5 years are more common pediatric age group with tumors in abdomen with 52.7%. Age under 5 years age group with 70.6%. Males are most commonly observed with pediatric tumors with 58.8% of total subjects. Male: female ratio is 1.2:1. Neuroblastoma was the most common tumor constituting 41.18 % of all cases, followed by Wilms’ tumor (23.53%), hepatoblastoma (11.76%), teratoma and granulosa cell tumor (11.76%). Conclusion: Neuroblastoma was the most common tumor. Most of the tumors were noted in children less than 5 years of age.

ABSTRACT: In cattle, abdominal tumors leading to partial or total intestinal obstruction are rare. Here, it is described the clinico-pathological findings of two cows with intestinal obstruction by neoplasms. A crossbred cow presented apathy, weight loss, constipation, enophthalmos, ruminal and intestinal hypomotility, and abdominal distension in the right ventral quadrant. Pathological examination revealed bilateral ovarian teratoma. The other case was a cow that in the past 2 years had several episodes of squamous cell carcinoma in the third eyelid and vulva. During the 6 days prior to investigation, clinical signs of constipation and tenesmus were noted. Nodular and friable masses were located in the vulva, right third eyelid and cranially to the right kidney that were identified as poorly differentiated metastatic carcinoma of the adrenal cortex. To our knowledge, these are probably the first reports of ovarian teratoma and adrenal carcinoma causing clinical signs of intestinal obstruction in cattle.


RESUMO: Em bovinos, tumores abdominais que levam à obstrução intestinal parcial ou total são raros. Neste relato, descrevemos os achados clínico-patológicos de duas vacas com obstrução intestinal por neoplasias. Uma vaca mestiça apresentava apatia, perda de peso, constipação, enoftalmia, hipomotilidade ruminal e intestinal e distensão abdominal no quadrante ventral direito. O exame patológico revelou teratoma ovariano bilateral. O outro caso foi uma vaca que, nos últimos 2 anos, teve episódios de carcinoma de células escamosas na terceira pálpebra e vulva. Durante 6 dias antes da investigação, foram observados constipação e tenesmo. Massas nodulares e friáveis foram localizados na vulva, terceira pálpebra direita e cranial ao rim direito, que foram identificadas como carcinoma metastático pouco diferenciado do córtex adrenal. Ao nosso conhecimento, estes são os primeiros relatos de teratoma ovariano e carcinoma adrenal causando sinais clínicos de obstrução intestinal em bovinos.

El quiste mesentérico es una enfermedad rara en la etapa neonatal, y suele confundirse imagenológicamente con múltiples patologías de los órganos intraabdominales. Se presenta un caso clínico con diagnóstico prenatal por imagen anecoica del hemiabdomen superior. El paciente requirió múltiples intervenciones quirúrgicas por presentar un quiste mesentérico que coexistía con atresia intestinal ileal en forma de cáscara de manzana, y el tratamiento clínico y nutricional fue difícil.


Mesenteric cyst is a rare condition in neonatal stage and often is confounded with multiple pathologies of intra-abdominal organs. A clinical case diagnosed with prenatal diagnosis by anechoic image of superior hemi-abdomen. Patient required of many surgical interventions due to a mesenteric cyst coexisting with an apple shell ileal intestinal atresia. Clinical and nutritional treatment was difficult.
