Objective:To explore the effect of attentional bias training on the attentional bias, psychological craving and relapse rate of alcohol dependent patients during rehabilitation.Methods:A randomized controlled trial was used to select 90 alcohol-dependent patients and randomly divide them into an intervention group and a control group. The alcohol cue pictures and neutral pictures were imported into E-Prime software at a ratio of 1∶1.The patients in the two groups were trained for eight times with attentional bias and without attentional bias by point delection paradigm. After each training, the software automatically records the response time and correct rate of the cue pictures and neutral pictures, and before intervention(T0), after 4 interventions(T4), and after 8 interventions(T8), the alcohol craving score was assessed by visual analogue scale and the relapse situation was followed up 1 month after the intervention. SPSS 24.0 software was used for statistical analysis.Repeat measure ANOVA, χ2 test and t-test were used to compare the response time, accuracy and psychological craving of the two groups. Results:Repeated measurement analysis of variance showed that there were interaction between group and time in cue-map response time, line chart accuracy and psychological craving score of the two groups ( F=5.218, 6.939, 147.14, all P<0.01). Simple effect analysis showed that there was no significant difference in cue pictures response time, cue pictures accuracy and psychological craving score between the intervention group and the control group at T0(all P>0.05), but there was significant difference at T4 and T8 (all P<0.05), and there was significant difference in neutral pictures reaction only at T8 ( P<0.05). The comparison between two groups showed that the intervention group showed a downward trend in cue pictures response, T4 and T8 were lower than T0, with statistical difference (both P<0.001), while there was no difference in the control group. In the intervention group, the correct rate of cue pictures increased, T4 and T8 were higher than T0, and there were statistical differences (both P<0.001), while there was no difference in the control group( P>0.05). The psychological craving scores of both the intervention group and the control group showed a downward trend, T4 and T8 were lower than T0, there were statistical differences (both P<0.001), but the difference between T4 and T8 in the control group remained unchanged. One month after the end of the intervention, the difference in the rate of relapse between the intervention group and the control group was marginal (11.10%, 26.70%, P=0.059). The total number of days of abstinence in the intervention group was longer than that in the control group ((28.33±4.99)d, (26.47±6.66)d, P=0.010). Conclusion:Attentional bias training can improve the mental craving and attentional bias of alcohol-dependent patients during the rehabilitation period, and the relapse rate decreased one month after the intervention.

@#Objectives: Background- Chronic alcohol use causes not only physical consequences but causes cognitive deficits also. 50 and 80% of the Alcohol dependent syndrome patients present with cognitive deficits. These cognitive deficits will significantly contribute to poor functional outcomes which lead to reduced health related quality of life. Objectives- To assess Cognitive deficits in Alcohol Dependent syndrome (ADS). To find the relationship between early onset of alcohol use, duration of Alcohol use and large quantity of alcohol use with Cognitive deficit pattern. MethodA cross sectional study was done on 38 patients with ADS, aged 18- 45 years, education ≥7th std, with no major psychiatric or medical co- morbidities. AUDIT scale was administered to look for dependence pattern, CIWA scale was applied to look for withdrawal symptoms. MoCA scale was applied to screen for cognitive impairment in patient with MMSE score (25 and above). In subjects with cognitive impairment on MoCA scale (25 and below), neuropsychological assessment was done using neuropsychological battery of test from NIMHANS Neuropsych battery standardised on Indian Population. Results- There was impairment in the Cognitive domains as follows, speed of processing (BACS - 100%), attention (DF- 97%) working memory (DF-97%, DB-100%, LNS - 84%), visual memory (100%), visuospatial memory (78%), executive functions (CT2 -92%), reasoning and problem solving (maze- 81%) in majority and a significant relation between early onset of alcohol use, duration of Alcohol use. Conclusions- There is a significant impairment in the areas of speed of processing, attention, executive functions, reasoning and problem solving domains. Significant association between Age of onset of ADS (Early onset) with Executive functions. Attention domain was found to be associated with longer duration of alcohol intake, whereas working memory deficits was associated with large quantity of alcohol intake.

ObjectiveTo explore the attentional biases in male alcohol dependent (AD) patients and the correlations between the attentional bias and alcohol-related factors.MethodsA total of 30 recently detoxified male individuals with alcoholism were compared with 37 healthy controls ( HC ) on the Chinese Emotional Stroop Task using negative,neutral,and alcohol-related words.ResultsThe comparison between AD group( ( 1382.13 ±323.38) ms,( 1365.76 ±313.03)ms,( 1433.20 ±342.23) ms,respectively) and HC group( (797.27 ±216.97)ms,( 794.11 ± 209.41 ) ms,(799.40 ± 215.82 ) ms respectively) on the reaction time of neutral,negative and alcohol-related words were significant ( t =8.822,P < 0.001 ; t =8.922,P < 0.001 ; t =9.234,P < 0.001 ).The error number of of the neutral and negative- related words of the patients ( ( 3.70 ± 2.56) time,( 4.23 ± 2.53 ) time)was worse than that of HC group( ( 2.11 ± 1.87 ) time,( 1.92 ± 1.82 ) time) ( t =2.939,P =0.005 ; t =4.355,P <0.01 ).Error number of the alcohol- related words between two groups were not significant;Its alcohol-related words attentional bias negative correlation to the age of initial alcohol use(P< 0.05 ),and positive correlation to continue drinking and score of self-rating depression scale (P<0.05).Age of addiction and the score of self-rating anxiety scale enter the regression equation of alcohol-related words.ConclusionThese results demonstrate that alcoholics have attentional bias in alcohol-related words and reward-related brain regions may be associated with craving among male patients with attentional bias.

The 2008 National Addiction Survey demonstrated the existence of 39 million alcohol drinkers, of whom 4.2 million are excessive drinkers and 4.8 million are alcohol dependents. No reports of the comorbidity of psychiatric disorders in alcohol consumers in our country exist. Nevertheless, 40% to 50% of alcohol-dependent patients from other countries have some sort of psychiatric disorder, such as major depression. Serotonergic function is a key mediator of mood states, impulsiveness, and addictive behavior, including alcohol consumption. Several studies have noted alterations in the serotonergic system in alcoholics (as demonstrated by an increase in the shooting frequency of raphe nuclei serotonergic neurons, an increase in serotonin levels in the accumbens nuclei, and a loss in serotonergic neurons in the raphe nuclei) and depressed patients (decreases in the density of serotonin reuptake transporter [5-HTT] and serotonin levels [5-HT]). Clinical studies have documented that excessive alcohol intake reduces 5-HT levels and that this condition potentiates psychiatric disorders, such as anxiety, major depression, and alcohol dependence. These data demonstrate an association between alcoholism, psychiatric disorders, and alcohol dependence. By molecular biology techniques, genetic risk factors have been identified and candidate genes, such as 5-HTT, have been selected. This gene is associated with a greater susceptibility to onset of alcohol-dependence and major depression. The 5-HTT gene lies in the SLC6A4 locus of 1 7q1 1.1-q12 and encodes a 600-amino-acid integral membrane protein. This transporter regulates serotonergic neurotransmission through removal of 5-HT from the synaptic space. Pharmacological research has shown that selective reuptake inhibitors (5-HTT blockers) reduce alcohol intake in alcohol-dependent and major depression patients. Serotonergic system receptors, such as 5-HTT, 5-HT1, and 5-HT2, are expressed in nervous system and immune system cells; thus it is likely that both systems have functional similarities. Due to this property, peripheral blood mononuclear cells (PBMCs) can be used to research neurodegenerative, psychiatric, and alcohol dependence disorders. The aim of this study was to assess 5-HTT expression levels in the PBMCs from alcohol-dependent patients and patients with comorbid alcohol-dependence and major depression disorder. A nivel mundial varios estudios indican que los pacientes con dependencia al alcohol (40 a 50%) presentan comorbilidad con algún tipo de padecimiento psiquiátrico. La función serotoninérgica es un mediador clave en los estados de ánimo, la impulsividad y las conductas adictivas, entre ellas el consumo de alcohol. Se ha reportado que el consumo excesivo de alcohol etílico disminuye los niveles de serotonina, aumenta la frecuencia de disparo de las neuronas serotoninérgicas en el núcleo del rafé y aumenta los niveles de serotonina en el núcleo accumbens. Las técnicas de biología molecular han permitido identificar factores de riesgo genético y se han seleccionado genes candidatos del sistema serotoninérgico, siendo uno de ellos el gen para el transportador de serotonina (5-HTT), el cual se ha demostrado que se encuentra asociado tanto a una mayor susceptibilidad para el establecimiento de la dependencia al alcohol como a la depresión mayor. Los receptores del sistema serotoninérgico como el 5-HTT, el 5-HT1 y el 5-HT2 se expresan tanto en las células del Sistema Nervioso como en las células del sistema inmunológico, lo que sugiere una similitud funcional de ambos sistemas. Es por ello que las células mononucleares de sangre periférica (PBMC) han sido utilizadas como un modelo de estudio en los trastornos de dependencia al alcohol y en los psiquiátricos. OBJECTIVES: The decline in general neurocognitive functions of alcoholics has been previously reported, especially in the area of memory, problem-solving and executive function which are important for occupational performance or adaptation to enviroment. On the other hand, young alcoholics under the age of 40 with relatively short duration of problem drinking were reported of no significant decline in neurocognitive functions consisting of memory, problem-solving and executive function. The objective of this study is to investigate whether there is a decline in neurocognitive functions around memory, problem-solving and executive function in abstinent young alcoholics. METHODS: 24 male and female alcohol dependent patients from the age of 19 to 39 years, who were detoxified in the hospotal were recruited as subjects. Normal control group of 20 males and females who matched for age, sex, educational level were selected. Clinical characteristics of the subjects were examined by demographic variables, alcohol history, Beck's Depression Inventory, Michigan Alcohol Screening Test and Standard Progressive Matrices. Also, subtests of Wechsler Memory Scale-Revised, Block design from the Korean-Wechsler Adult Intelligence, Wisconsin Card Sorting Test were performed in order to evaluate the subjects' neurocognitive functions. RESULTS: Compared to normal controls, significant decline in neurocognitive functions such as memory, problem-solving and executive function were observed in young alcohol dependent patients with an average age of 32. Parts of the neuropsychological tests showed significant correlation with duration of lifetime drinking and lifetime alcohol consumption. CONCLUSION: This study shows that, unlike previous reports, decline in memory, problem-solving and executive function occur in young alcoholics as in chronic alcoholics. This implies that young alcoholics can experience difficulty in occupational performance and adaptation to environment, and may be less responsive to treatment due to neurocognitive damage. The results of this study inclicate the importance of early identification, intervention, and planning therapeutic strategies in young alcoholics.
