Background Cornea astigmatism can be effectively corrected by implanting Toric intraocular lens (IOL) during cataract surgery and therefore improve visual acuity of patients.However,the decentration and rotation position errors were inevitable sometime.What's the difference of effect of position errors on quality of image between spherical IOL and Toric IOL needs further research.Objective This study was to evaluate the optical performance and wavefront with rotation and decentration of Toric IOL.Methods Different decentration for SN60AT IOL(spherical IOL) and Toric IOL in Hwey-Lan Lion model eyes was set with the role as follows:decentration 0.25 mm to 0.75 mm in a 5°-interval from 0° to 90°.Furthermore,Toric IOL was rotated at 5° and 10°,respectively.Then the image performances of SN60AT IOL and Toric IOL at different decentration distances and rotated degrees were evaluated with modulation transfer function (MTF) and value of wavefront aberration under all conditions.Results At the centration,the MTF curves of spherical IOL and Toric IOL were similar under 3,4 and 5 mm pupil diameter at each spatial frequency.Under the condition of 4 mm pupil diameter,when the decentration was 0.25 mm,the MTF values of SN60AT IOL at 6 c/d and 12 c/d were 0.581 087 and 0.411 960,respectively.T3 IOL were 0.454 259 and 0.382 313,T4 IOL were 0.426 020 and 0.360 490,T5 IOL were 0.425 606 and 0.359 877.When the decentration was 0.50 mm,the MTF values of SN60AT at 6 c/d and 12 c/d were 0.573 073 and 0.412 787,respectively.T3 IOL were 0.450943 and 0.379481,T4 IOL were 0.423 153 and 0.356 664,T5 IOL were 0.422 881 and 0.356 230.When the decentration was 0.75 mm,the MTF values of SN60AT at 6 c/d and 12 c/d were 0.560 038 and 0.413 624,respectively.T3 IOL were 0.445 597 and 0.374 322,T4 IOL were 0.418 522 and 0.350 087,T5 IOL were 0.418 468 and 0.349 976.When the IOL decentralized along 0°,5°,10°,90°meridian line,the MTF values were almostly same.The root mean square (RMS) of spherical IOL and Toric IOL was increased when the IOL decentralized from 0 mm to 0.75 mm,with the most increasing level in coma aberration and slight increase in trefoil aberration.When the T4 IOL decentralized from centre to 0.75 mm,the coma increased from 0 to C(3,-1)-0.049 79 μm,C (3,1)-0.037 59 μm and the trefoil aberration increased from 0 to C (3,3) 0.005 72 μm,C (3,-3) 0.004 64 μm.With the increase of rotation degrees (from 5°to 10°) of Toric IOL,the MTF was worse at high spatial frequency.Toric IOL rotation caused the increase of astigmatism and residual astigmatism and spherical error,but not high order aberration.Conclusions The tolerance of Toric IOL to decentration is very close to the spherical IOL,and optical performance is only associated with the amount of decentration but not direction.The aberration caused by Toric IOL decentration is mainly coma.The rotation of Toric IOL causes astigmatism error but not high order aberrations.

Purpose: To assess the reliability and reproducibility of static cyclotorsion correction (SCC) measurements made using the Schwind Amaris Excimer laser in patients undergoing LASIK or PRK, and compare the outcomes of treating astigmatism with and without SCC. Methods: Eighty eyes of 40 patients were included in this study. All eyes underwent 2 or 3 sets of five measurements: before and after speculum placement, and after flap-lift (in LASIK cases). We assessed the reproducibility, accuracy, and the percentage of "no catch" measurements. The astigmatism was calculated pre-and 3-months-postoperatively by vector analysis. Results: The mean age of the patients was 23.67 ± 4.19 years. Preoperative spherical equivalent and astigmatism were -2.56 ± 2.86 D and +1.36 ± 0.98 D, respectively. The mean measurement time was 15.1 seconds per measurement. The percentages of "no catch" were: 63.8%, 14.9%, and 26.9%; pre-speculum, post-speculum, and post flap-lift, respectively. Cyclotorsion of ≥±2º was seen in 41.25% and 66% of the cases before and after the flap-lift, respectively. Significant cyclotorsion (≥±5º) was seen in 12.50% and 18% of the eyes pre and post flap-lift. The mean astigmatism dropped from +1.53 D@1º to +0.34D@3º when SCC was used and from +1.86D@1º to +0.23D@7º when SCC was not used. No statistical difference was noticed between the groups (p>0.05) in the postoperative residual astigmatism. A postoperative astigmatism of ≥1 D was seen in 10% and 20% of eyes with and without SCC, respectively (p<0.01). Conclusion: Although not always feasible, the SCC measurement is a simple and useful tool. Postoperative astigmatism showed less variability when SCC was used. .


Objetivo: Avaliar a confiabilidade e reprodutibilidade da correção da medida de ciclotorção estática (SCC), realizada com o excimer laser Schwind Amaris em pacientes submetidos a LASIK ou PRK, e comparar os resultados do tratamento de astigmatismo com e sem SCC. Método: Oitenta olhos (40 pacientes). Todos os olhos foram submetidos a 2 ou 3 séries de 5 medições: antes e após a colocação do espéculo, e depois do levantamento do flap (nos casos de LASIK). Foram avaliadas a reprodutibilidade, a precisão e a porcentagem de medições "não obtidas". O astigmatismo foi avaliado no pré-operatório e aos 3 meses de pós-operatório, por meio de análise vetorial. Resultados: A idade foi 23,67 ± 4,19 anos. O equivalente esférico e o astigmatismo pré-operatórios foram -2,56 ± 2,86 D, e 1,36 ± 0,98 D, respectivamente. O tempo de medição médio foi 15,1 segundos por medição; as porcentagens de medidas "não obtidas" foram: 63,8%, 14,9% e 26,9%; pré, pós-espéculo, e pós-levantamento do flap, respectivamente. Ciclotorção ≥±2º foi observada em 41,25% e 66 % dos casos pré e pós-levantamento do flap. Ciclotorção significativa (≥±5º) foi observada em 12,50% e 18% pré e pós-levantamento do flap. A média do astigmatismo diminuiu de 1,53D @ 1º para 0,34D @ 3º quando SCC foi usado e de 1,86D @ 1º a 0,23D @7º quando SCC não foi usado. Résumé

Purpose: To compare the effectiveness of limbal relaxing incisions (LRI) with that of toric intraocular lens (IOL) implantation for the treatment of astigmatism during phacoemulsification using nonvectorial (predictability, safety, and efficacy) and vectorial analyses. Methods: This longitudinal observational case series assessed 62 eyes of 31 consecutive cataract patients with preoperative corneal astigmatism of 0.75-2.50 diopters (D) in both eyes. Patients were randomly assigned to 2 groups: a toric IOL group, which received toric IOLs in both eyes, and an LRI group, which received spherical IOLs associated with LRI placement in both eyes. All patients were evaluated at 1, 3, and 6 months after surgery, when refractive astigmatism analysis was performed using both nonvectorial and Alpins vectorial methods. Outcomes were assessed within each group and compared between groups. Results: The proportion of eyes within ±0.50 D of the intended correction at 3 and 6 months after surgery was 75% and 71.88%, respectively, in the LRI group and 40% and 66.67%, respectively, in the toric IOL group. In the remaining period, the proportion was greater in the toric IOL group. The safety index showed no difference at any time point. The efficacy index at 1 and 3 months after surgery was significantly higher in the toric IOL group (0.43 and 0.44, respectively) than in the LRI group (0.31 and 0.36, respectively). At 6 months after surgery, the status of eyes in the LRI group was as follows: 53.13% were undercorrected, 43.74% achieved the intended correction, and 3.13% were overcorrected. In the toric IOL group, 16.76% eyes were undercorrected, 76.67% achieved the intended correction, and 6.67% were overcorrected. The success rates for astigmatic surgery, astigmatism reduction at the intended axis, and astigmatism corrected were 43%, 62%, and 64%, respectively, in the LRI group and 57%, 81%, and 94%, respectively, in the toric IOL group. Objetivo: Comparar incisões relaxantes limbares (IRL) e lentes intraoculares (LIO) tóricas tanto em termos não-vetoriais (efetividade, segurança e eficácia), quanto vetoriais no tratamento do astigmatismo por ocasião da facoemulsificação. Métodos: Estudo observacional longitudinal (série de casos) no qual foram avaliados 62 olhos de 31 pacientes consecutivos de catarata com astigmatismo corneano pré-operatório entre 0,75 e 2,50 dioptrias (D) para ambos os olhos. Os pacientes foram aleatoriamente distribuídos entre 2 grupos: "1" submetido a implante de lentes intraoculares AcrySof ToricTM em ambos os olhos e "2" com implante bilateral de lentes intraoculares tóricas AcrySof NaturalTM complementada por incisões relaxantes limbares. Todos os pacientes foram reavaliados com 1, 3 e 6 meses de pós-operatório, sendo feitas análises do astigmatismo refracional por métodos não-vetoriais, como pela análise vetorial de Alpins, interessando os resultados dentro de cada grupo e entre os grupos. Resultados: O porcentual de olhos entre ±0,50 D da correção pretendida no grupo incisões relaxantes limbares foi de 75 e 71,88%, respectivamente, em comparação aos 40 e 66,67% do grupo lentes intraoculares tóricas aos 3 e 6 meses de pós-operatório. Nos outros períodos avaliados, os porcentuais foram favoráveis ao grupo lentes intraoculares tóricas. O índice de segurança não demonstrou diferença em nenhum dos períodos. O índice de eficácia foi estatisticamente maior para o grupo lentes intraoculares tórica com 1 e 3 meses de pós-operatório (0,43 e 0,44), em comparação ao grupo incisões relaxantes limbares (0,31 e 0,36).
