AIM: To investigate the application of biological amniotic membrane soaked in pirfenidone(PFD)and to evaluate its anti-scarring effect and toxic side effects on glaucoma model of rabbit eyes.METHODS: The right eyes of 72 healthy New Zealand white rabbits were randomly divided into 0.5%PFD+ biological amniotic membrane group, biological amniotic membrane group, mitomycin C(MMC)group and blank control group after the glaucoma model was established by anterior chamber injection of compound carbomer solution, and 18 rabbits in each group underwent trabeculectomy, in which the 0.5% PFD+ biological amniotic membrane group was placed with 0.5% PFD solution-soaked biological amniotic membrane under the scleral flap, and the biological amniotic membrane group was placed with normal saline-soaked rehydrated biological amniotic membrane under the scleral flap. In the MMC group, a cotton pad soaked in MMC was placed under the scleral flap for 3 min and immediately rinsed with normal saline, while the blank control group received no implant after the scleral flap was made. The intraocular pressure(IOP), filtration blebs, toxic side effects and complications were evaluated, and the histopathological changes in the filtration area were observed by hematoxylin-eosin(HE), Masson staining and immunohistochemical staining.RESULTS: The mean IOP at 14, 21 and 28 d after trabeculectomy were 0.5%PFD+ biological amniotic membrane group<MMC group<biological amniotic membrane group<blank control group(all P<0.05). At 28 d after trabeculectomy, 0.5%PFD+ biological amniotic membrane group had the best effect of anti-inflammatory hyperplasia and inhibition of collagen formation, the highest survival rate of filtration blebs, and the inflammatory reaction was mild.CONCLUSION: Biological amniotic membrane soaked in pirfenidone has more obvious anti-scarring effect on glaucoma model, with less toxic side effects and good safety.

Objective:To investigate the expression of integrin ανβ3 in conjunctival scar after glaucoma filtration surgery and to explore its role in the formation of filtering scar in rats.Methods:Fifty-five SPF male SD rats aged 6-8 weeks were selected to establish the model of glaucoma filtration surgery, and 15 unsuccessful modeling rats were excluded.The remaining rats were divided into control group, transforming growth factor (TGF) group, integrin ανβ3 receptor antagonist LM609 group and TGF+ LM609 group, with 10 rats in each group.The conjunctival flap of different groups was treated with cotton piece containing 0.9% sodium chloride, TGF-β2 (2 mg/ml), LM609 (40 μg/ml), TGF-β2 (2 mg/ml)+ LM609 (40 μg/ml) solution for two minutes, respectively, according to grouping.The intraocular pressure (IOP) was monitored with an iCARE tonometer before operation and at 3 days, 1 week, 2 weeks after operation, and the average IOP ratio ([postoperative IOP/preoperative IOP]×100%) was calculated.At 3 days, 1 week and 2 weeks after operation, slit lamp microscopy was used to observe the morphological changes of filtering blebs and calculate the survival rate of filtering blebs.Two weeks after operation, the operative eyeball tissues were removed and made into paraffin section.The inflammatory changes, fibrosis and collagen deposition of bulbar conjunctiva in the operation area were observed by hematoxylin-eosin staining and picric acid-sirius red staining, respectively.Western blot was used to detect the expression of integrin αν and β3.Immunohistochemical staining was used to detect the expression of integrin ανβ3 in the conjunctival tissue of operation area.The use and care of the rats complied with the Instruction Notions with Respect to Care for Laboratory by State Ministry of Science and Technology.The study protocol was approved by an Ethics Committee of The Daping Hospital, Army Medical University (No.2017-06).Results:The IOP of rats in each group was significantly decreased on the third day after operation compared with before operation, and gradually recovered to the preoperative level in the first to second week after operation.The mean IOP ratios of the control group, TGF group, LM609 group and TGF+ LM609 group were (101.80±4.44)%, (103.90±5.65)%, (94.10±7.45)% and (100.60±4.61)%, respectively, showing a statistically significant difference among them ( F=4.571, P=0.010). The mean IOP ratio of LM609 group was lower than that of the control group, TGF group and TGF+ LM609 group (all at P<0.05). Two weeks after operation, the filter blebs became flat and the bulbar conjunctival tissue was slightly thickened in the control group and TGF+ LM609 group, and bulbar conjunctival tissue was significantly thickened with a large number of blood vessels growing in the TGF group, and localized small filter blebs as well as less vascularization were seen in the LM609 group.The survival rate of filtering bleb in the TGF group was significantly lower than that in the control group, LM609 group and TGF+ LM609 group, and the survival rate of filtering bleb in the LM609 group was significantly higher than that in the control group and TGF+ LM609 group (all at P<0.05). Two weeks after operation, there were fewer infiltrated inflammatory cells and looser collagen fiber deposition in the conjunctival tissue in the LM609 group and TGF+ LM609 group compared with the TGF group.The relative expression levels of integrin αν and β3 under conjunctival tissue in the TGF group were significantly higher than those in the control group, LM609 group and TGF+ LM609 group (all at P<0.05). Immunohistochemical staining showed that integrin ανβ3-positive cells were mainly in the conjunctival tissue around the operation area and microtubules.The number of integrin ανβ3-positive cells in the conjunctival tissue of the operation area in the TGF group were significantly greater than those of other groups. Conclusions:Integrin ανβ3 is highly expressed in the scar of conjunctival tissue after filtering surgery in rats.The inhibition of integrin ανβ3 can reduce the deposition of fibers and prolong the survival of filtering blebs, suggesting that integrin ανβ3 may be involved in the formation of conjunctival scar.

@#AIM: To analyze the effect of ocular massage in patients after glaucoma filtering surgery.<p>METHODS: Databases including the CNKI, VIP Information, WanFang and CBM were electronically searched for all randomized controlled trials(RCTs)about comparing the intraocular pressure(IOP), forming efficiency of functional filtering blebs and success rate of filtering surgery between patients with and without ocular massage after receiving filtering surgery. Two reviewers independently screened literature according to the inclusion and exclusion criteria, and evaluated the included studies. Then, Meta-analysis was performed using RevMan5.3 software.<p>RESULTS: Totally 20 eligible studies involving 1 757 eyes of 1 750 individuals were included to investigate the effect of ocular massage in patients after glaucoma filtering surgery. Ten of the studies were related with IOP. The results of Meta-analysis showed that IOP was better controlled in patients with ocular massage in the postoperative 2wk, 1 and 3mo \〖(<i>WMD</i>= -0.96, 95%<i>CI</i>(-1.83, -0.09), <i>P</i><0.05\〗, \〖<i>WMD</i>= -2.68, 95%<i>CI</i>(-3.81, -1.55), <i>P</i><0.05\〗 and \〖<i>WMD</i>= -3.98, 95%<i>CI</i>(-5.00, -2.96), <i>P</i><0.05\〗, respectively. Fourteen of the studies were related with the forming efficiency of functional filtering blebs. The results of Meta-analysis showed that patients with ocular massage act better in forming functional blebs \〖<i>RR</i>=1.33, 95%<i>CI</i>(1.23, 1.44), <i>P</i><0.05)\〗. Nine of the studies were related with the success rate of surgery. The results of Meta-analysis showed that patients with ocular massage had higher rate of surgery success \〖(<i>RR</i>=1.41, 95%<i>CI</i>(1.28, 1.55), <i>P</i><0.05)\〗.<p>CONCLUSION: Ocular massage can help patients after filtering surgery to control IOP more effectively and promote the formation of functional filtering blebs, so as to improve the success rate of surgery.

Résumé

A lady who underwent lensectomy for microspherophakia and pars plana vitrectomy for retinal detachment in her left eye developed recurrent filtering blebs at the site of sclerotomies. Filtering blebs were managed by suturing the sclerotomies. Targeted gene sequencing identified a variant of ASPH gene (p.Arg688Gln) which is not known to be associated with Traboulsi syndrome. But considering the paucity of cases with genetic analysis, it would be possible that p.Arg688Gln is a pathogenic variant. This is the first case report of Traboulsi syndrome due to an ASPH variant not reported earlier that can lead to recurrent filtering blebs.

Résumé

We present a case of B-acute lymphoblastic leukemiain an elderly patient who presented with severe weakness and pancytopenia. The patient was a 75 year old Female whose blasts had an unusual morphology in form of coarse azurophilic granules and cytoplasmic blebs and on flow cytometry the blasts were present in the bright CD45 zone with a high side scatter. Bone marrow aspirate sample was subjected to multicolour flow cytometry using Beckman Coulter Navios® which is an 8 colour flow cytometer.Flow cytometricanalysis of the bone marrow aspirate showed blasts in the monocytic zone with a precursor B cell immunophenotype. Complete blood counts showed pancytopenia with peripheral blood film not showingany blasts. Bone marrow aspirate smears showed 20% blasts with coarse azurophilic granules and cytoplasmic blebs.The position of the blasts in this case which were in monocytic zone giving them a bright expression of CD45 and a high side scatter on the CD45 side scatter. This is not the usual positionfor blasts in B-acute lymphoblastic leukemia as these blasts are less complex. A bright expression of CD45 by blasts in B-acute lymphoblastic leukemia is known to be associated with a poor prognosis but the clinical significance of blasts being bright CD45 with a high side scatter is a very rare occurrence and more number of cases with a similar presentation are required to determine a prognostic significance.

Purpose: The aim of this study is to investigate the efficacy and safety of needling-revision augmented with a high dose of mitomycin C (MMC) in failing or failed blebs after trabeculectomy in Indian eyes. Methods: Prospective, noncomparative, interventional study. All patients (>18 years) who had raised intraocular pressure (IOP) following trabeculectomy (>6 weeks and <2 years), who had a flat bleb, bleb encapsulation, and/or required antiglaucoma medication (AGM) for IOP control were eligible for inclusion. MMC was injected subconjunctivally at least ½ hour before the needling procedure was carried out at the slit lamp in the outpatient's clinic. Results: Thirty-nine eyes of 38 patients were included. The median follow-up was 20 months and time interval between trabeculectomy and needle revision was 113 days. Initially, in all cases, aqueous flow was re-established with a raised bleb; 7 eyes required repeat needling. IOP decreased from median 24 mmHg (Q1 21, Q3 27, interquartile range [IQR] 6, range 18–35) preneedling to median 14 mmHg (Q1 10, Q3 16, IQR 6, range 6–18) postneedling at last follow-up (P < 0.0001, 95% confidence interval [CI]: 8.2–13.0). The use of AGM reduced from median 1 (Q1 0, Q3 3, IQR 3, range 0–4) preneedling to median 0 postneedling (P < 0.0001, 95% CI: 1–2). Complete success was seen in 28 eyes (71.8%, 95% CI: 71.1%–96.4%); another 5 eyes (12.8%) were controlled with AGM (qualified success) with overall success of 84.6%. Most complications were transient in nature with resolution within 1 week. One patient developed hypotony, and another developed a late bleb leak. Conclusion: Needling revision augmented with high-dose MMC, at the slit lamp, effectively rescues failing or failed filtration, and appears to be safe.

Objetivo: determinar la efectividad a mediano plazo de la revisión con aguja asociada a bevacizumab (avastin) en ampollas de filtración encapsuladas postrabeculectomía. Métodos: se realizó un estudio observacional descriptivo-prospectivo, de serie de casos (14 ojos, 14 pacientes), con antecedentes de quiste de Tenon a corto plazo postrabeculectomía (2 meses - 1 año). Se realizó revisión con aguja asociada a inyección subconjuntival de avastin (1 mg) hasta completar 3 dosis en días alternos. Se evaluaron la presión intraocular y el uso de colirios hipotensores preoperatorio y posoperatorio a la semana, 1 mes, 3 meses y 1 año. Se definió el éxito total (si en el posoperatorio la PIO era menor o igual a 21 mmHg sin necesidad de colirios hipotensores); parcial (si la PIO era menor o igual a 21 mmHg con el uso de hasta dos fármacos); y sin éxito (si la PIO era menor o igual a 21 mmHg con 3 colirios hipotensores o mayor que 21 mmHg). Se registraron las complicaciones quirúrgicas. Resultados: la edad media fue de 59,3 ± 8,4 (43-75). Predominó el sexo femenino (57 por ciento) y no existieron diferencias en cuanto al color de piel (7/7). El grado de reducción de la PIO fue regular a la semana y al mes, y malo a los 3 meses y al año, lo que requirió una nueva cirugía en 11 casos (78,6 por ciento). Las complicaciones más frecuentes fueron: hemorragia subconjuntival (71,4 por ciento de casos) y Seidel positivo (21,4 por ciento). Conclusiones: la revisión con aguja asociada al uso de avastin subconjuntival como tratamiento de la ampolla encapsulada no logra una reducción significativa de la presión intraocular(AU)


Objective: determine the medium-term effectiveness of needle revision with bevacizumab (avastin) in encapsulated filtration blebs after trabeculectomy. Methods: an observational descriptive prospective case-series study was conducted of 14 eyes of 14 patients with a history of Tenon cyst shortly (2 months - 1 year) after trabeculectomy. Needle revision was performed by avastin subconjunctival injection (1 mg) until completing 3 doses on alternate days. Evaluation of intraocular pressure and the use of hypotensive collyriums before and after surgery was conducted at 1 week, 1 month, 3 months and 1 year. Definitions were made of total success (IOP minor or equal 21 mmHg in the postoperative period without hypotensive collyriums), partial success (IOP minor or equal 21 mmHg with up to two drugs), and no success (IOP minor or equal 21 mmHg with 3 hypotensive collyriums or greater than 21 mmHg). Surgical complications were recorded. Results: mean age was 59.3 ± 8.4 (43-75) years. Female sex prevailed (57 percent) and no differences were found based on skin color (7/7). The degree of IOP reduction was fair at one week and one month, and bad at 3 months and one year, 11 cases (78.6 percent) requiring new surgery. The most common complications were subconjunctival hemorrhage (71.4 percent of the cases) and positive Seidel (21.4 percent). Conclusions: needle revision with subconjunctival avastin to treat encapsulated blebs does not achieve a significant reduction in intraocular pressure(AU)

Objetivo: determinar la efectividad a mediano plazo de la revisión con aguja asociada a mitomicina C en la ampolla de filtración encapsulada postrabeculectomía. Métodos: estudio observacional descriptivo, prospectivo de serie de casos (30 ojos, 30 pacientes), con antecedentes de quiste de Tenon a corto plazo postrabeculectomía (1 mes-1 año). Se realizó revisión con aguja asociada a inyección subconjuntival MMC 0,01 % hasta completar 3 dosis en días alternos. Se evaluaron la presión intraocular y el uso de colirios hipotensores preoperatorio y posoperatorio a la semana, 1 mes, 3 meses, 1 año y 2 años. Se definió éxito si la presión intraocular era menor de 21 mmHg sin colirio hipotensor (éxito total) y con colirios hipotensores (éxito parcial). Se registraron las complicaciones quirúrgicas. Resultados: hubo descenso significativo de la presión intraocular media preoperatoria de 27,06 ± 5,2 mmHg a 13,10 ± 3,65 mmHg, 14,83 ± 3,68 mmHg y 16,70 ± 3,38 mmHg a la semana, 1 y 3 meses posoperatorios respectivamente, y se mantuvo estable hasta 2 años (16,70 ± 2,18 mmHg); p< 0,001 para cada comparación preoperatorio vs. posoperatorio), lo que representó una reducción de la presión intraocular del 51,8 por ciento a la semana; 44,4 por ciento al mes y 37 por ciento hasta los 2 años. La media del número de colirios hipotensores se redujo significativamente de 2,0 ± 0,0 (preoperatorio) a 0,4 ± 0,0 (2 años posoperatorios), p< 0,001. El éxito fue total en el 70 por ciento de los casos y parcial en el 30 por ciento, tras 2 años posoperatorios. Las complicaciones fueron: hemorragia subconjuntival (100 por ciento de casos), atalamia (40 por ciento) y Seidel positivo (26,7 por ciento). Conclusiones: la revisión con aguja asociada a mitomicina C subconjuntival como tratamiento de la ampolla encapsulada, logra reducción del 37 por ciento de la presión intraocular durante los dos primeros años posoperatorios con un mínimo de complicaciones(AU)


Objective: to determine the medium-term effectiveness of subconjuntival mitomicyn-C associated to needle revision on encapsulated filtering bleb after trabeculectomy. Methods: prospective, descriptive and observational case series study (30 eyes, 30 patients) with a history of Tenon cyst after short term trabeculectomy (1month-1year). These patients underwent needle revision associated to subconjuntival 0,01 percent mitomycin injection to completing three doses in every other day. The intraocular pressure and the use of hypotensive drops preoperatively and postoperatively were evaluated seven days, one month, three months, one year and two years after the procedure. The success of intervention was defined as total when the intraocular pressure was less than 21mmHg without hypotensive drops and partial with hypotensive drops. Surgical complications were recorded. Results: the mean preoperative intraocular pressure decreased significantly from 27,06 ± 5,2 mmHg to 13,10 ± 3,3 mmHg, 14,83 ± 3,68 mmHg and 16,70 ± 3,38 mmHg one week, one month and three months after surgery and remained stable (16,70 ± 2,18 mmHg) for 2 years, p< 0,001 for each preoperative and postoperative comparison; this represented an intraocular pressure reduction of 51,8 percent; 44,4 percent and 37 percent after one week, one month, and up to 2 years, respectively. The mean number of hypotensive drops lowered significantly from 2,0 ± 0,0 (preoperative) to 0,4 ± 0,0 (two years after surgery), being p< 0,001. Total success was attained in 70 percent of cases and partial in 30 percent after 2 years. Surgical complications were subconjuntival haemorrhage (in all patients), atalamy (40 percent) and positive Seidel index (26,7 percent). Conclusion: subconjuntival mitomicyn-C injection-associated needle revision for encapsulated bleb reduces intraocular pressure by 37 percent during the first 2 years after the procedure with minimal complications(AU)

Background The scarring of conjunctival filtering blebs after glaucomatous surgery is a leading cause of operation failure.Exploring the balance between matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of matrix metalloproteinase-2 (TIMP-2) in conjunctival filtering bleb is very important for the study on pathogenesis of postoperative scarring.Objective This study was to evaluate the role of MMP-2 and TIMP-2 in wounding healing of subconjunctival tissue after filtering surgery in rats.Methods Sixty-three clean male SD rats were divided into normal control group and postoperative 1-day,3-day,5-day,7-day,14-day and 28-day group.The drainage tube was monocularly implanted into the anterior chamber of the rats to establish the filtering surgery models,and the operative response of the eyes was examined under the slit lamp microscope.The animals were sacrificed in corresponding time points,and the frozen sections of eyeballs were prepared,and the expressions of MMP-2 and TIMP-2 in conjunctival and subconjunctival tissues were detected using immunofluorescence technique.Western blot was employed to assay the dynamic expressions of MMP-2 and TIMP-2 proteins in conjunctival and subconjunctival tissues in different groups.The use and care of the rats complied with the Instruction Notions with Respect to Care for Laboratory by State Ministry of Science and Technology.Results Filtering blebs were formed in all the operated eyes 1 day after surgery and remained for 7-17 days,with the average survival time of 12 days.Western blot assay revealed that the expression levels of MMP-2 protein in the filtering blebs of the normal control group were 121.67 ±4.37,and the expressions were gradually elevated from the postoperative 3-day group (183.67±5.61) until the postoperative 7-day group (230.50±8.48) and then gradually declined till the postoperative 28-day group (172.33 ± 8.43),showing a significant difference among the groups (F=280.18,P<0.05).In addition,the expression levels of TIMP-2 protein in filtering blebs of the normal control group were 102.50 ±6.25.The expression levels were gradually raised in postoperative 3-day group (162.67±7.00) and peaked in the postoperative 5-day group (232.00± 11.03),and then the levels gradually reduced till the postoperative 28-day group (150.50±6.41),with a significant difference among the groups (F =145.34,P < 0.05).Immunofluorescent staining showed that MMP-2 and TIMP-2 were weakly expressed in the conjunctival epithelium of the normal rats,while in the operated rats,strong fluorescence for MMP-2 and TIMP-2 was seen in both conjunctival epithelium and subconjunctival tissues of filtering blebs.Conclusions The expression trends of MMP-2 and TIMP-2 in the filtering blebs are consistent to the fibrosis process of conjunctival tissue,indicating that MMP-2 and TIMP-2 participate in the scar formation of conjunctival filtering bleb after glaucoma filtering surgery.

Background Package and tissue patch implantation are common methods for repair of filtering bleb leaking after anti-glaucoma surgery.But the scarring or re-leakage of filtering bleb probably occur again.Objective This study was to investigate the repair effect of acellular dermal matrix (ADM) on filtering bleb leaking in rabbit model and compare the effectiveness among ADM,amniotic membrane and conjunctival overlap.Methods Trabeculectomy was performed on 48 eyes of 24 New Zealand rabbits,and models of filtering bleb leaking were established.The models were randomized into ADM group,amniotic membrane group and conjunctival covering group based on randomized number table.ADM patches with 4 mm×4 mm were implanted across lamellar cornea and sclera at a bridge in the ADM group,and the same size of amniotic membranes were used in the amniotic membrane group,and conjunctiva was sutured to limbus in the conjunctiva overlap group.The intraocular pressure (IOP) was measured before surgery and 1 day,1 week,1 month,3 months and 6 months after surgery.The biocompatibility of materials above was assessed under the slit lamp microscope,and the status of filtering bleb was evaluated and compared with anterior segment optical coherent tomography (AS-OCT) 1 day,1 week,1 month,3 months and 6 months after surgery.Results Before surgery and 1 day,1 week,1 month,3 months and 6 months after surgery,the IOP was (26.9±4.3),(16.6±5.1),(22.1 ±6.2),(18.3±6.5),(22.7±2.5),(23.4±1.4) mmHg in the AMD group,(29.9±5.4),(14.9 ± 6.4),(21.6 ± 7.8),(26.3 ± 4.1),(26.0 ± 4.2) and (23.0 ± 5.3) mmHg in the amniotic membrane group,and (28.7 ±4.3),(15.7 ±7.0),(22.0±6.3),(28.2±4.1),(24.7 ±4.1),(23.0±2.7) mmHg in the conjunctival overlap group,showing significant differences among different groups and various time points (Fgroup =8.419,P=0.011 ;Ftme =15.543,P=0.000).The IOPs were significantly lower from 1 day through 3 months after operation than those before operation in the AMD group (P =0.000,0.000,0.006,0.045) ; while the IOPs were reduced only from 1 day through 1 week after operation in comparison with before operation in the amniotic membrane group and the conjunctival overlap group (P =0.000,0.001).One month after surgery,the IOPs were significantly declined in the ADM group compared with the amniotic membrane group and the conjunctival overlap group (P =0.001,0.000).The grafts were clear under the slit lamp microscope and exhibited the valid filtering bleb until 3 months after operation under the AS-OCT in the ADM group.However,the valid filtering bleb remained only 1 month after surgery in the amniotic membrane group and the conjunctival overlap group.Neovascularization on the filtering bleb was found 3 months in the AMD group but 1 month in the amniotic membrane group and the conjunctival overlap group.Conclusions Compared with amniotic membrane and conjunctival tissue,ADM patch for the repair of filtering bleb leakage can increase the survival duration of filtering bleb and remain lower IOP.

Objetivo: describir los hallazgos de la microscopia confocal de barrido láser y su relación con la morfología de la bula de filtración.Métodos: estudio observacional descriptivo de corte transversal en 100 ojos. Se les realizó microscopia confocal de barrido láser con el HRT II y el módulo corneal de Rostock. Se aplicó la prueba de significación estadística de Mc. Nemar para una p=0,05.Resultados: predominó el grupo entre 61 y 80 años de edad (55,8 porciento) y el color de la piel blanca (46,8 porciento). En el grupo de descenso de la presión intraocular en más del 30 porciento se ubicó la mayor cantidad de bulas de todos los tamaños. Las bulas de mediano tamaño fueron las que más disminuyeron las cifras de presión intraocular, con 24 ojos (55,8 porciento, p=0,00), seguidas por las de pequeño tamaño (p=0,14). Las bulas de filtración aplanadas fueron las más frecuentes en 55 porciento de los casos (p=0,00), y 67,4 porciento de estas se ubicaron en el grupo de descenso de presión intraocular de más de 30 porciento (p=0,01). Las bulas medianas presentaron la mayor cantidad de estroma en malla porosa (60 porciento) y de microquistes epiteliales (56 porciento) p=0,00.Conclusiones: la configuración aplanada y el tamaño mediano de la bula de filtración se relacionaron con la presencia de variables histológicas que infieren buen funcionamiento de la bula (microquistes epiteliales y estroma en malla porosa). También se relacionaron con un mayor descenso de la presión intraocular


Objective: to describe the findings in the confocal laser scanning microscopy and its relation with the morphology of the filtering bleb. Methods: cross-sectional, observational and descriptive study of 100 eyes that underwent confocal laser scanning microscopy with the Retinal Heidelberg Tomography and the Rostock corneal module. Mc NemarÆs statistical significance test was made to obtain p=0,05. Results: the 61-80 years old age group (55,8 per cent) and the caucasians predominated. The group of eyes with over 30 per cent decrease of the intraocular pressure showed the highest amount of filtering blebs of all sizes. The medium-size blebs were the ones that decreases the IOP in a higher percentage (24 eyes; 55,8 per cent) for p=0,00, followed by the small blebs (p=0,14). The flat blebs were predominant in 55 per cent of cases (p=0,00) and the 67,4 per cent of them were consistent with the major range of decrease of the intraocular pressure (p=0,01). The medium size blebs presented the major number of stromas in porous mesh (60 per cent), and of epithelial mycrocists (56 per cent) for p=0,00. Conclusions: the flat configuration and the medium size of the blebs were related with the presence of the histological variables that infer adequate function of the filtrating blebs (stroma in porous mesh and epithelial mycrocists). They were also associated with higher decrease of intraocular pressure

Sin embargo, el fallo de la ampolla de filtración puede ocurrir y provocar un incremento de la presión intraocular. Esto conllevaría a una intervención médica o quirúrgica adicional. Se realiza una revisión bibliográfica sobre la cistitomía transconjuntival, asociada al uso de antimetabolitos (mitomicina C o 5-fluorouracilo) o no. Se demuestra que es mínimamente invasiva, con escasas complicaciones y que constituye un método eficaz, fácil y barato para restablecer el drenaje acuoso y disminuir la presión intraocular


Trabeculectomy is usually a successful procedure to reduce intraocular pressure in glaucoma patients. However, the failure of the filtering bleb can occur, leading to increased intraocular pressure and to a further medical or surgical intervention. A literature review on transconjunctival cystotomy, with or without the use of mitomycin C or 5-fluorouracil, was made. It was demonstrated that this method is minimally invasive with few complications, effective, easy-to-apply and inexpensive to restore aqueous drainage and lowering intraocular pressure

BACKGROUND: The current modality of choice for the treatment of Port-wine stains (PWS) is laser photocoagulation. Laser therapy for the treatment of PWS, especially with a pulsed dye laser (PDL), has been proven safe and effective; however, because penetration of the PDL is too shallow for an effective ablation of the blebs, treatment of blebbed PWS, using PDL, may be insufficient. OBJECTIVE: We demonstrated the clinical efficacy of a 1,064 nm long pulsed Nd:YAG laser with contact cooling device for blebbed PWS. METHODS: Twenty one patients with blebbed PWS (Fitzpatrick skin types II-V) underwent a treatment, using a 1,064 nm long pulsed Nd:YAG laser with a contact cooling device at 8-week intervals. Treatments were done using 5~6 mm spot sizes at 20~30 ms and 95~170 J/cm2. Laser parameters were adjusted in order to meet the needs of each individual patient's lesions. RESULTS: All subjects tolerated the treatments well, and showed clinical improvement from blebs. Of the 21 patients, 18 of them experienced either moderate or excellent response. CONCLUSION: Use of a 1,064 nm long pulsed Nd:YAG laser results in a greater depth of vascular coagulation. A 1,064 nm long pulsed Nd:YAG laser with contact cooling device may be regarded as a promising therapeutic option for the treatment of blebbed PWS.

Presentamos un caso de neumotórax espontáneo primario recurrente en un adolescente de 13 años de edad previamente sano . El paciente presentó súbitamente dolor punzante en el hemitórax izquierdo y disnea de medianos y grandes esfuerzos. En la radiografía de tórax se evidenció un neumotórax izquierdo. El paciente fue referido al Hospital del Niño para su manejo. Se refiere un episodio similar al descrito seis meses antes del evento actual. Se encontraba asintomático al momento de su hospitalización. Era evidente una elevación anterior del hemitórax izquierdo y disminución de los ruidos pulmonares en el campo pulmonar izquierdo. En la radiografía de tórax se encontró un neumotórax izquierdo de 70% y una bula en el lóbulo superior izquierdo. Se consideró el diagnóstico de un neumotórax espontáneo primario izquierdo recurrente. Fue tratado con oxigeno húmedo al 100%, resección de las bulas y pleurodesis química. El centrol radiográfico mostró resolución del neumotórax. Egresó y continuo controles en la consulta de cirugía y neurología.


The authors describe a thirteen years old boy with a recurrence fo a primary spontaneous pneumothorax . He was a healthy boy without underlying lung disease. The main complain was the sudden onset of an acute sharp and diffuse pain located on the left hemithorax. Besides, the patient had medium and big efforts dyspnea. A chest radiograph was made and a left pneumothorax was detected. The patient was referred to the Hospital del Niño for his management and treatment. The patient had a history of a similar clinical picture six months before the actual event. The patient was asymptomatic at the moment of the hospital admission . Physical findings were asymmetry of the left hemithorax and diminished breath sounds on the affected side. There was evidence, on the chest radiograph, of a left pneumothorax of 70% with collapse of the lung and the presence of an apical sub pleural bulla in the upper left lobe. The diagnosis of a recurrence of a primary spontaneous pneumothorax was made by the patient´s history. The administration of humidified 100 per cent oxygen was indicated and the patient was evaluated by a surgeon for surgical treatment. The surgical procedure was resection of blebs by thoracoscopy and chemical pleurodesis. The control chest radiograph showed evacuation of the pneumothorax and reexpansion of the left lung. The chest tube was removed two days after surgery. The patient was discharged nine days after the hospital admission with follow up in the out patient clinic.

Objective To evaluate the therapeutic effects of needling revision with mitomycin C(MMC) subconjuctival injection on early failed filtering blebs after trabeculectomy for glaucoma. Methods Needling revision with MMC 0.2 mL(0.04 mg) subconjuctival injection was performed on 86 eyes of 76 patients with failed filtering blebs 2 to 6 weeks after trabeculectomy for glaucoma.An average of 1.88 times of treatment was performed.The intraocular pressure(IOP),blebs and side effects were observed,and follow-up was conducted for 6 months. Results Two to six weeks after trabeculectomy,there were 50 eyes with thickened and focalized blebs,32 eyes with encapsulated blebs and 4 eyes with no bleb.Six months after needling revision with MMC subconjuctival injection,blebs of 61 eyes turned into functional ones.The blebs were thinned and multicysted in 24 eyes,diffused and elevated in 37 eyes,thickened and focalized in 6 eyes,encapsulated in 13 eyes and disappeared in 6 eyes.Three months after treatment,the mean IOP was(15.2?6.1) mmHg,and there were 57 eyes with IOP

Adjunctive use of Mitomycin C (MMC)in trabeculectomy has greatly improved the success rate. Trabeculectomy with MMC, however, has resulted in a cystic and thin-walled filtering bleb, which may be more susceptible to infection. Late onset bleb-related endophthalmitis developed in a glaucoma patient at 40 months after MMC trabeculectomy. Filtering bleb in this patient had been avascular, thin-walled and cystic prior to the occurrence of endophthalmitis. Patient complained of decreased visual acuity with ocular discomfort and conjunctival injection and showed a mucopurulent infiltrate within the bleb, hypopyon and vitritis. Visual acuity was hand motion. An intensive treatment included anterior chamber irrigation, lensectomy,par plana vitrectomy, intravitreal and subconjunctival antibiotics (vancomycin,gentamycin) injection and topical and systemic corticosteroid (dexamethasone) and antibiotics,which led to a resolution.
