Objective@#Based on the principle of evidence-based nursing, we searched, studied and summarized a nursing pathway for patients who after transverse tibial bone transportation, and explored its effect.@*Methods@#With A set of targeted nursing intervention program, 17 patients with type 2 diabetic foot patients after transverse tibial bone transportation were nursed. After 2 months, the percentage of ulcer decreased, the visual analogue scale and the temperature of foot dorsum were used to evaluate the nursing effect.@*Results@#The average temperature of foot dorsum was (28.49±1.12) ℃, the average visual analogue scale was (4.06±1.40) . And after 2 months, the average temperature of foot dorsum was (29.97± 1.26)℃, the average visual analogue scale was (1.94±1.92), and the area of ulcer in 15 cases (2 cases could not be counted due to toe amputations in treatment) decreased by 25%~100%, with an average of 59%, compared with that before intervention.@*Conclusions@#The implementation of evidence-based nursing can provide effective nursing intervention, relieve the pain of patients, and promote the rehabilitation of diabetic foot ulcers after transverse tibial bone transportation.

Objective To evaluate the efficacy of microsurgical techniques combined with bone transportation for osteomyelitis following open fracture on the leg.Methods From April 2007 to April 2016,25 patients with osteomyelitis caused by severe open injury on the leg were treated at Orthopaedic Department,General Hospital of Rocket Force.They were 22 men and 3 women,aged from 19 to 65 years (average,36.5 years).Their open injury was located at upper tibia in 6 cases,at middle tibia in 6 and at lower tibia in 13.Eight patients received internal fixation with plate and 17 unilateral external fixation.By the Cierny-Mader classification,their chronic osteomyelitis was of type Ⅲ in 14 cases and of type Ⅳ in 11.The tibial defects after excision of infectious segment ranged fromn 5 to 14 cm in length (average,8.8 cm).After infectious tissues were removed,antibiotic bone cement chains were inserted into the defects.The open wounds were covered with microsurgical pedicled or free flaps.Bone transportation with unilateral external fixation was used to deal with the bone defects after removal of the infectious bone.Results Bone infection was controlled in the 25 patients.The rate of infection control after one debridement procedure was 76.0% (19/25),and that after 2 debridement procedures was 24.0% (6/25).Flap repair led to early closure of the open wounds.The successful rate of wound closure after one operative procedure was 84.0% (21/25),and that after secondary operation was 16.0% (6/25).Bony union of tibial defects was achieved in all the 25 patients with no recurrence of osteomyelitis.The patients were followed up for 18 to 36 months (average,24.6 months).Re-fracture happened in one case and was treated with another operation.Bone union time starting from bone transportation ranged from 8 to 18 months (average,10.5 months).Time for external fixation ranged from 12 to 20 months (average,18.4 months).Conclusion For osteomyelitis caused by severe open injury on the leg,microsurgical techniques can be used to repair open wounds and bone transportation to reconstruct bone defects after removal of infectious bone.

Son activados principalmente por el operador diariamente; teniendo como ventaja estos últimos la ausencia de cicatrices cutáneas permanentes. El objetivo principal de este estudio fue diseñar y fabricar un prototipo de distractor curvo de sínfisis, intraoral para reconstrucción mandibular. Materiales y Métodos: Se realizaron pruebas técnicas: modelo elementos finitos del distractor y cuerpo mandibular, generando una malla formada por 80.000 elementos con restricciones en el condilo mandibular, dos tornillos a cada lado de la placa y cuatro a cada lado del distractor en todos los grados de libertad; se aplicaron fuerzas de 248N a la zona. Finalmente, se realizó la prueba de instalación en cadáver para verificar la técnica quirúrgica y su ergonomía. Resultados: Se observó una distribución uniforme de esfuerzos para todo el sistema mandíbula- distractor, garantizando su resistencia y estabilidad estructural. Conclusiones: El dispositivo es resistente a las fuerzas de masticación, es de facial instalación y activación; permite la reconstrucción de segmentos curvos de amplia longitud.


Introduction and Objective: Currently, there are extra-and intra devices, which are mainly triggered by the operator daily, taking advantage as the latter no permanent scars skin. The main objective of this study was to design and manufacture a prototype of intraoral curve symphyseal distractor for mandibular reconstruction. Materials and Methods: The following technical tests were carried out: finite element model of the distractor and mandibular body creating a net of 80,000 elements with restrictions in the mandibular condyle, two screws on each side of the plate and four on each side of the distractor in every degree of freedom. 248N forces were applied to the zone. Finally, the installation test on a cadaver was done in order to verify the surgical technique and its ergonomy. Results: A uniform distribution of strains was observed for all the mandibular-distractor system, guarantying its resistance and structural stability. Conclusions: The device is resistant to masticatory forces; it is easy to install and to activate and allows the reconstruction of wide range curved segments.
