Résumé

Objective To investigate the value of clinical application of mammotome under ultrasonic guidance for minimal excision and biopsy of breast tumor.Methods Excision and biopsy of 152 breast masses from 115 patients were performed using mammatome under ultrasonic guidance,and the patients were followed up.Results Total resection of 152 breast tumors were accurately performed by mammotome vacuum device and biopsy was performed.On pathological examination,86 cases were fibroadenoma,34 cases were adenosis,2 cases were fat necrosis,3 cases were gynecomastia,1 case was ductal inflammatory change and 26 cases were breast cancer.Postoperatively,hematoma was found in 3 cases,subcutaneous ecchymosis in 4 cases and skin injury in one case.All cases were followed up for a period of time and no residual or recurrent masses were found.Conclusions Mammotome vacuum system is an exact,effective and safe method.It is an ideal minimally invasive operation in treating benign breast masses with few complications.It can also be applied to biopsy for breast cancer before operation.

Résumé

Objective To explore the clinical characteristics,diagnosis,treatment,and prognosis of male breast cancer.Methods The clinical data of 38 cases of male breast cancer,who were treated in the two hospital during the past 10 years,were analyzed retrospectively.The diagnosis was mainly based on preoperative B-ultrasound,radiography with molybdenum target tube and fine needle aspiration.Among the 38 cases,radical mastectomy was conducted in 3 cases,modified radical mastectomy in 27 cases,simple mastectomy in 5 cases,and operation was refused in 3 cases.Postoperatively,radiotherapy was adopted in 4 cases,simple endocrinotherapy in 7 cases,radiotherapy plus chemotherapy in 9 cases,and chemotherapy plus endocrinotherapy in 8 cases.Results Median age at treatment was 58.5(53-82)years.TNM staging included stageⅠin 13 cases,stageⅡ in 19 cases,stageⅢ in 4 cases,and stageⅣ in 2 cases.Median follow-up was 73.8 months(2 months-10 years),and follow-up rate was 73.6%(28/38).During the follow-up,13 cases died including 8 cases died of local recurrence and metastasis,and 5 cases died of heart and brain disease.The 5-and 10-year overall survival rate was 71.6% and 55.3% respectively;5-and 10-year disease-specific survival of stageⅠ,Ⅱ was 92.6% and 78.9% respeoively;among 6 cases of stageⅢ,Ⅳ 4 were followed,and all 4 cases died with in 3 years.Conclusions Male breast cancer has a long course,poor prognosis and low survival rate.Combined therapy with modified radical mastectomy,as its basis,is the treatment of choice for male breast cancer.There is a certain correlation between the prognosis of male breast cancer and clinical stage of the disease.

Résumé

Objective To introduce the advance of the diagnosis and tretment of breast cancer(BC). Methods Reveiwing the recently world reference regarding to the diagnosis and treatment of BC was done for making an advancing paper of BC. Results The NF ?B and telomerase may be a new target of anti cancer of breast. The ECT, fiberoptic mammaroductoscopy, examine of micrometastase of BC, immuno cell chemical examine were progressing in the diagnosis of BC. The advance of the treatiment of BC included breast conservative surgery, lymphatic mapping and sentinel node biopsy, biochemical response modifier, new adjuvant chemotherapy, TAM and 1,25 (OH) 2D 3 treatmen. Conclusions The idea and mould of the diagnosis and treatment of BC have advanced obviously.

Résumé

Objective To evaluate the effect of combined therapy for inflammatory cancer of breast (ICB). Methods A retrospective analysis was made on the clinical manifestation, pathological type and treatment modalities of 25 patients with ICB from 1985 to 2000.Results All of the 25 patients were treated with mastectomy, including typical Halsted mastectomy in 12 cases, typical Halsted mastectomy with reconstruction of latissimus dorsi flap in 4, and palliative mastectomy in 9. Of the 21 patients 20 received preoperative chemotherapy; and all the 21 patients received postoperative chemotherpyl; while hormonal therapy was used in 11 cases. A mean survival was 23 months (0.5~5years) in 14 followed up patients; and now 5 patieats remain alive (2months~5 years). Conclusions A combined therapy consisting of chemotherapy, surgery and hormonal therapy may improve the prognosis of ICB.
