Résumé

Vaginal Brenner tumor is extremely rare. Only five cases have been reported in the English literature to date. Here we report a vaginal Brenner tumor in a 76-year old postmenopausal woman, who presented with a 2.5cm-sized sessile vaginal polyp. Microscopically, it showed characteristic features of Brenner tumor consisting of three components; transitional islands, glands, and dense fibrous stroma. The epithelial tumor cells were positive for GATA-3, p63 and ER, but negative for PAX8. The origin of Brenner tumors in the vagina is unclear, but previous reports suggested of Müllerian origin. However, our case revealed that vaginal Walthard nests could be possible precursor lesions based on their immunohistochemical staining results.

Résumé

Walthard cell rests are benign cluster of epithelial cells found in connective tissue of fallopian tubes and also seen in the mesovarium, mesosalpinx and ovarian hilus. It was proposed that Walthard cell rests may be a source of Brenner's tumor or primary urothelial cell carcinoma because of similar histogenetic origin of their cells. Here we report a case which not only during gross examination showed peculiar peritubal glistening tiny nodules but also demonstrated attention grabbing clusters of cell on histopathology, mimicking ? A tumor. A diagnosis of Walthard cell rest was made. One should always consider a differential diagnosis as Brenner's tumor (primary) or serosal implant from gynaecological, genitourinary or other tumors before making a confirmatory diagnosis of Walthard cell rest.

Résumé

Bilateral malignant Brenner tumour of ovary is extremely rate. A case of malignant Brenner tumour involving both the ovaries with mctastasis to mesentery in a 48 year femalc is presented. Grossly ovarian masses were firm with soft areas, encapsulated and having bosselated external surfaces. Cut sections showed yellowish white surface with peripheral cysts (in both tumours). Microscopy revealed transitional cell carcinoma with squamoid differentiation at places. Metastatic deposits were found in the mesentery. Endometrium showed cystic glandular hyperplasia.
