Résumé

The effects of topically applied bunazosin hydrochloride, Which has been developed as a highly selective alpha-1 adrenergic antagonist, on intraocular pressure(IOP), pupillary diameter, conjunctival injection, ptosis, and blood pressure were investigated in 14 normal volunteers. It is known that the mechanism of lOP reduction appears to be an increase in uveoscleral outflow and/or a decrease in episcleral venous pressure. The purpose of this experiment is to observe the effects of 0.1 % Bunazosin(R) on the lOP, pupil size, lid, conjunctival change and blood pressure in the normal human subject. The effects of 0.1% topical bunazosin(R) have been observed with placebo-controlled, double-blind single-dose study. The drug reduced lOP for 10 hours, and the decrease was statistically significant(p value <0.05). The lOP was decreased maximally at 3 hours after the instillation of the drug and the amount of the lOP reduction was 4.2mmHg. There were no significant changes of pupil size, lid configuration, conjunctival injection, and blood pressure in this experiment.

Résumé

Bunazosin is a alpha-1 blocker that is more selective and effective than other alpha-1 receptor blockers. Bunazosin has been shown to lower intraocular pressure but it's mechanism is uncertain. The purpose of this study is to know the morphological changes in the ciliary epithelium by bunazosin. 0.1% Bunazosin was instilled on the eye of the rabbits and the eyes were enucleated on 1, 2, 6 and 24 hours after instillation to find out the tissue response under the electronmicroscopy. As a result, widening of the cell membranes and swelling of the mitochondria were observed in the pigmented and non-pigmented epithelium and these changes were recovered with time and seem to be the morphological changes of bunazosin against aqueous formation in the ciliary body.

Résumé

Antihypertensive effect of Bunazosin was evaluated in 23 patients with essential hypertension. Systolic blood pressure decreased from 175.5+/-16.9mmHg to 167.5+/-30.2mmHg after 4 weeks and to 165.4+/-29.7mmHg(p<0.05) after 8 weeks of medication. Diastolic blood pressure decreased from 105.6+/-9.7mmHg to 98.6+/-11.6mmHg(p<0.05) after 4 weeks and to 98.1+/-12.2mmHg(p<0.05) after 8 weeks of medication. Heart rate and serum lipid profiles did not change significantly. Transient side effects such as nausea(two cases), headache (one case), and weakness(one case) were observed, but there were no significant side effects. Therefore Bunazosin seemed to be effective in lowering blood pressure without reflx tachycardia, changes of serum lipie profiles, and significant side effects.
