@#A fungal carotid aneurysm is an infrequently occurring infectious aneurysm that is usually treated by antifungal therapy plus surgical debridement of the infected vessel. We herein report an extremely rare case involving a patient with a medical history of bladder cancer treated by Bacillus Calmette– Guérin (BCG) who developed a fungal aneurysm of the internal carotid artery and thrombosis of the superior ophthalmic vein caused by maxillary Aspergillus sinusitis. The patient was successfully treated by antifungal, anticoagulant, and antiplatelet drugs combined with internal carotid artery ligation therapy. Internal carotid artery fungal aneurysm is associated with a high mortality rate, but the present case suggests that it can be successfully treated by antifungal therapy combined with a less invasive endovascular therapy.

Objective To investigate the roles of Pipeline embolization device ( PED) in combination with coils in the treatment of large and giant unruptured internal carotid artery aneurysms. Methods From January 2014 to December 2016,51 patients with large (diameter 10-25 mm) and giant (diameter >25 mm) internal carotid artery aneurysms treated at the Department of Neurosurgery,Xuanwu Hospital,Capital Medical University were analyzed retrospectively. Among them,35 were females and 16 were males, the average age was 53 ±13 years (range 15-71). They were divided into a PED group (n=31) and a PED combined with coil group (n=20) according to whether using coils for packing aneurysm cavity or not. The treatment of two groups of patients and ≥6 months of imaging follow-up effect were analyzed. The embolic rate was evaluated by Raymond grade. Results Thirty-one patients with 32 aneurysms ( mean diameter 15 ± 3 mm) in the PED group were treated with 33 PEDs,including 15 with cavernous sinus aneurysm and 17 with supraclinoid aneurysm;20 patients with 21 aneurysms ( mean diameter 17 ± 4 mm) were treated with 22 PEDs in the PED combined with coil group,including 10 cavernous sinus aneurysms and 11 supraclinoid carotid aneurysms. Six months after procedure,imaging follow-up revealed that the incidence of residual aneurysm ( Raymond grade Ⅱ-Ⅲ) in the PED group was significantly higher that that in the PED combined with coil group (9. 7% [n =3] and 0%,respectively). The incidence of thrombosis in PED combined with coil group was significantly higher than that in the PED group,there was significant difference between two groups (P<0. 05). Conclusions When using PED to treat large and giant internal carotid artery aneurysms,packing aneurysm cavity in combination with coils can reduce the incidence of residual aneurysm,but it may have a higher risk of thrombotic events. A prospective randomized controlled trial of large samples is still needed to prove it.

Objective To discuss the hemodynamic features of internal carotid aneurysm,and study the relationship between hemodynamics and occurrence,development and rupture of internal carotid aneurysm .Methods The 3D model of internal carotid aneurysm was established by materialise's interactive medical image control system(MIMICS).The finite element analysis software ANSYS was used to carry out numerical simulation.Results Low wall shear stress of internal carotid aneurysms were located at the same area.The wall shear stress at different region were positive correlation with the blood flow velocity.Conclusion The blood flow velocity and wall shear stress at the top of aneurysm are relatively low.This position is consistent with the rupture position.The significant changes of hemodynamic at the top of aneurysm may trigger a series of mechanical biological effects on the internal carotid aneurysm wall.The changes alters cells'gene and protein's expression,causes rupture of the aneurysm.

Résumé

Extracranial aneurysms are a rare presentation accounting for only less than 5% of all peripheral artery aneurysms. The left common carotid artery aneurysm is considered even rarer. We present a case of a 46-year-old gentleman who presented to casualty unit with a neck mass, stridor and acute respiratory distress. Prior to this admission patient was being investigated for Takayasu arteritis (TA). Patient was subsequently intubated due to respiratory compromise. A CT angiography of the thorax was done showed a proximal left common carotid artery aneurysm with contained leakage of aneurysm with severe compression of the trachea. Patient was subsequently transferred to the vascular team in National Heart Institute for further management.

PURPOSE: Oculomotor disturbance is the common manifestation of intracavernous carotid aneurysm cases. Intracavernous carotid aneurysms causing compressive optic neuropathy with no oculomotor disturbance are relatively rare due to their anatomical characteristics. We experienced a case of intracavernous carotid aneurysm which resulted in a shifted supraclinoid segment of the internal carotid artery, presenting with visual loss and visual field defect with no oculomotor disturbance. CASE SUMMARY: A 40-year-old woman presented with loss of vision in the right eye. A relative afferent pupillary defect was observed in this eye. Visual field test showed quadranopsia in the right eye. Magnetic resonance imaging revealed that the intracavernous carotid aneurysm had shifted the supraclinoid segment of the internal carotid artery to the superomedial position. The right optic nerve was directly molded by the shifted supraclinoid segment of the internal carotid artery at the point of the bifurcation between the anterior cerebral artery and the middle cerebral artery. A Guglielmi detachable coil (GDC) embolization was performed successfully with no operational complications. Six months after coiling, best corrected visual acuity of the right eye was 1.0, and the visual field defect had recovered in all except the superior temporal field. CONCLUSIONS: Oculomotor disturbance is frequently associated with intracavernous carotid aneurysms. Nevertheless, optic neuropathy without oculomotor disturbance may be the only sign in patients with an intracavernous carotid aneurysm that causes shifting of the supraclinoid segment of the internal carotid artery.

Objective To investigate effects of endovascular stents with different structures and wire cross section shapes on the treatment of internal carotid aneurysm and its influence on hemodynamics and flexibility. Method Based on the same model of internal carotid aneurysm, five models with different stent intervention treatment were constructed, which had different stent structions or wire cross section shapes while their porosity rates were approximately the same. Numerical simulations were performed using finite volume method to get quantitative information of biomechanics. Results Among the five models, the mean flow rate in aneurismal cavity decreased maximally in the model with stent of rectangular cross section. Wall shear stress in aneurismal dome and aneurismal neck were found to decrease much more in models with circular cross section and rectangular spiral stent. The flexibility of mesh stent was far better than that of the spiral stent. Conclusions Mesh stent with rectangular cross section has better biomechanical influence on the treatment of internal carotid aneurysm. These findings may help clinicians to select a proper stent when treating arterial aneurysm.

Los aneurismas se definen como dilataciones localizadas y permanentes que suponen un incremento del calibre del vaso mayor del 50%. En la carótida interna son poco frecuentes, de etiología casi siempre aterosclerótica y aparecen con mayor frecuencia en mujeres mayores de 60 años. La porción cavernosa de la arteria carótida interna raramente sufre ruptura y tiene una clínica variada que incluye cefalea, diplopía, parálisis de los nervios oculomotores y dolor facial. Presentamos el caso clínico de un paciente del Hospital Vargas de Caracas con aneurisma gigante de arteria carótida derecha en seno cavernoso, quien inició de forma súbita cefalea holocraneana, acompañada de oftalmoplejía, midriasis, disminución de la agudeza visual y sin signos de irritación meníngea, que se manejó de forma médica conservadora con evolución satisfactoria


Aneurysms are defined as permanent expansions of the artery with an increase of arterial blood of 50 %. The ethiology is mostly atherosclerotic, and they appear more frequently in 60-year old women. The clinical presentation is more frequent with migraine, diplopya, paralysis of the oculomotor nerves and facial pain. We present a case of a patient with a gigantic aneurysm of the cavernous sinus of the carotid artery treated at the Hospital Vargas de Caracas, Venezuela. This patient had a sudden severe migraine, accompanied of ophthalmoplegia, mydriasis, impaired vision, without signs of meningeal irritation. The treatment was medical with satisfactory progress of the patient

Résumé

The morphology of the Sella Turcica is important both as a reference in cephalometric studies and in the pituitary gland, internal carotid artery and cavernous sinus surgical boarding. The present report describes a case of partial ossification of the sella turcica walls in a dry skull of 40-year-old Brazilian individual. We analyze the morphometric characteristics of the region.


La morfología de la silla turca tiene importancia tanto como referencia en los estudios cefalomé trieos como en los abordajes quirúrgicos de la glándula hipófisis, carótida interna y seno cavernoso. En el presente reporte se describe un caso de osificación parcial de las paredes de la silla turca, en un cráneo seco de un individuo brasilero de 40 años. Se analizan las características morfométricas de la región.

Résumé

Intracranial aneurysms focus very often in the polygonal of Willis. They appear at the site of carotid-posterior communicating artery with high rate. Headache usually appears in these lesions. Subarachnoid hemorrhage is an early complication. Cerebral angiography plays a very important part in the diagnosis of these lesions but enhanced CT-Scanner can reveal them easily. We used sugita clip in the treatment of this aneurysm and had a good result. This spring clip that remains international standards is applied in spreading around the world.
