Introdução: O objeto do estudo foi a Career Anchors Self-Assessment no contexto de enfermeiros brasileiros sob a ótica da saúde do trabalhador. Objetivos: Adaptar transculturalmente a Career Anchors Self-Assessment para o contexto brasileiro; Analisar evidências de validade de conteúdo, processo de resposta e estrutura interna da versão brasileira da Career Anchors Self-Assessment em uma amostra de enfermeiros; Discutir a validação da Career Anchors Self-Assessment para a realidade brasileira e a utilização da escala para reflexão sobre a saúde do trabalhador enfermeiro. Método: Estudo psicométrico, dividiu-se em duas etapas: 1 - Obtenção da versão brasileira da escala Career Anchors Self-assessment Adaptação Transcultural/validade de conteúdo e de processo de resposta. 2 - Evidências de validade relacionadas à estrutura interna da versão brasileira da Career Anchors Self-assessment. Resultados: Obteve-se a versão adaptada para o contexto de enfermeiros brasileiros da área hospitalar, do instrumento denominado de Autoavaliação de Âncoras de Carreira, composto 42 itens. Quanto à avaliação das evidências de validade da estrutura interna, foram encontrados dados satisfatórios para a AFC, AFE e Confiabilidade. Uma análise reflexiva pode ser feita a partir dos resultados, o modelo teórico das âncoras de carreira, originalmente criado com o perfil de administradores indicou para o autor, oito tipos de âncoras de carreira. Porém, ao aplicar para a enfermagem, teve-se uma evidência maior nas âncoras: Segurança e Estabilidade (SEG), Senso de serviço/ Dedicação a uma causa (SER), Autonomia e Independência (AUT)/Criatividade Empreendedora (CRI) e Competência Gerencial (GER). Considerações Finais: A versão brasileira, denominada Autoavaliação das Âncoras de Carreira, tem boa qualidade de tradução e evidências de validade de conteúdo e foram encontrados valores satisfatórios para o instrumento com 08 fatores latentes, confirmando a hipótese.


Introduction: The object of the study was the Career Anchors Self-Assessment in the context of Brazilian nurses from the perspective of occupational health. Objectives: To adapt the Career Anchors Self-Assessment cross-culturally to the Brazilian context; To analyze evidence of content validity, response process and internal structure of the Brazilian version of the Career Anchors Self-Assessment in a sample of nurses; To discuss the validation of the Career Anchors Self-Assessment for the Brazilian reality and the use of the scale for reflection on the health of the nursing worker. Method: Psychometric study, divided into two stages: 1 - Obtaining the Brazilian version of the Career Anchors Self-Assessment scale Cross-cultural adaptation/content validity and response process. 2 - Evidence of validity related to the internal structure of the Brazilian version of the Career Anchors Self-Assessment. Results: The adapted version for the context of Brazilian nurses in the hospital area was obtained, of the instrument called Self-Assessment of Career Anchors, composed of 42 items. Regarding the evaluation of the evidence of validity of the internal structure, satisfactory data were found for the AFC, AFE and Reliability. A reflective analysis can be made from the results, the theoretical model of career anchors, originally created with the profile of administrators indicated for the author, eight types of career anchors. However, when applied to nursing, there was greater evidence in the anchors: Security and Stability (SEG), Sense of Service/Dedication to a Cause (SER), Autonomy and Independence (AUT)/Entrepreneurial Creativity (CRI) and Management Competence (GER). Final Considerations: The Brazilian version, called Self-Assessment of Career Anchors, has good translation quality and evidence of content validity and satisfactory values were found for the instrument with 08 latent factors, confirming the hypothesis.

Reducing low-value care is a key measure to optimize service model for cancer patients and improve the quality of and value of care.We summarizes the conceptual connotation of low-value medical services for cancer patients,compares the relevant measurement indicators at home and abroad,explains the logic of low-value medical services use for cancer patients at the three levels of"individual-institution-system",and proposes strategies for reducing low-value medical services for cancer patients in five aspects:construction of service value framework,dissemination of evaluation results,connection of drug access mechanisms,moni-toring of cancer care services,and empowering shared decision-making.

Objective:To analysis the job preference and heterogeneity of medical students by distinguishing their birthplaces,and to provide reference for optimizing the management of primary health care resources.Methods:Using a cluster sampling method,an online survey of discrete choice experiment was conducted with 925 medical students from six teaching hospitals in Beijing,741 valid questionnaires were obtained,the effective recovery rate was 80.1%.The mixed logit model was used to perform regression analysis on six job attributes and estimate the willingness to pay.Results:There were significant differences in the choice of work location among medical students from different birthplaces.The subgroup results showed that compared to medical students from city,undergraduates from rural and county district preferred a work with sufficient career development opportunities.The results of undergraduate subgroup showed that undergraduates from rural district preferred a work with good environment than those from other birthplaces.Conclusion:There is heterogeneity in job preferences of medical students from different birthplaces.Policy makers should pay attention to the medical students'birthplace,also take the educational level into account to optimize the diversified job attributes,formulating targeted intervention to attract primary health care talents.

Objective:This study discusses the job preferences of Center for Disease Control and Prevention(CDC)staffs at the prefectural-level,and provides a basis for the development of an effective incentive mechanism.Method:This study used a combination of stratified sampling and purposive sampling to research online 455 staffs from six prefectural-level CDCs in Shandong Province,analyzed the data using a mixed logit model and latent class model,and calculated willingness to pay and relative importance.Result:In the mixed logit model,income,benefit level,establishment,workload,recognition and respect from the public,personal career development opportunities,and training opportunities all had significant influences(P<0.05)on the job selection preferences of the CDC staffs,with hygiene factors such as establishment(β =2.636)and income(β =0.083)having a greater degree of influence than motivation factors.The latent class model shows that relatively young CDC staffs with lower monthly incomes value income more;older CDC staffs with higher monthly incomes value establishment more.Conclusion:Prefectural-level CDC staffs prefer jobs with establishment,higher incomes,very good benefit levels,recognition and respected from the public,lower workloads,many opportunities for personal career advancement and abundant training opportunities.It is recommended that the total number of establishments be rationally controlled and dynamically adjusted to balance the differences between working conditions within and outside the establishment and that the financial input to CDC be increased and the pay performance system be improved;that attention be paid to both hygiene factors and motivation factors,and that a variety of measures work together to incentivize CDC staffs development;and that differentiated incentives be adopted for different categories of CDC staffs.

OBJECTIVE To evaluate the psychological dependence of the extrasynaptic GABAA receptor(GABAAR)agonist gaboxadol and compare it with the synaptic GABAAR modulator midazolam in the two-bottle free-choice model of mice.METHODS ① Male C57BL/6J mice were ig administered with vehicle,midazolam(59.0,73.7,92.2,115.2,144.0 and 180.0 mg·kg-1)or gaboxadol(8.4,10.5,13.1,16.4,20.5,25.6 and 32.0 mg·kg-1),and the loss of righting reflex was observed.The median effective dose(ED50)was obtained from the dose-response curve.② A two-bottle free-choice model was used to find out whether gaboxadol and midazolam induced preference behavior in mice.The mice were divided into normal control,gaboxadol or midazolam groups.During the habituation stage(the first day to the third day,D1-D3),both test and vehicle bottles contained water.During the trail stage(D4-D5),4%sucrose solution was provided in both bottles.During the test stage(D6-D15),test bottles contained vehicle,gaboxadol(3.9×10-6 mol·L-1)or midazolam(1.4×10-5 mol·L-1)in sucrose solu-tions,while other bottles contained the corresponding vehicle in sucrose solutions.Bottles were placed on the two sides of the home cage,to which mice had free access,and their consumption from each bottle was recorded daily.Total consumption,accumulated daily consumption,relative consumption,and accumulated relative consumption during the test stage were calculated.The weight of the mice was also recorded.RESULTS ① Midazolam and gaboxadol dose-dependently increased the rate of loss of right reflex in mice,with ED50 of 105.3 mg·kg-1(95%CI:96.4-115.2 mg·kg-1,R2=0.9796),13.7 mg·kg-1(95%CI:12.6-15.0 mg·kg-1,R2=0.9773),respectively.② Compared with the normal control group,there was no significant difference in the total consumption in the gaboxadol and midazolam groups.Compared to the vehicle bottles,the daily consumption from test bottles in the midazolam group increased significantly on D11-D15 of the test stage(P<0.05),while daily consumption from gaboxadol test bottles was significantly higher than that of vehicle bottles(P<0.01).Compared with the normal control group,the daily relative consumption in the gaboxadol group was significantly increased on D9(P<0.05),and the accumulative relative consumption was significantly higher than in the normal control group(P<0.01).There was no significant change in body weight across the groups over the test stage.CONCLU-SION Like midazolam,gaboxadol exhibits psychological dependence potential in a two-bottle free-choice model.

The shared decision marking between doctors and patients is a moral requirement,which stems from the basic rights of medical staff to accept patients or their families,such as autonomy,informed consent and choice.In order to achieve the shared decision marking between doctors and patients,medical staff and patients play different roles.The task of medical staff is to introduce professional knowledge and specific experience,provide patients with multiple recommendations for diagnosis and treatment choices,and the task of patients is to choose treatment plans recognized in the shared decision marking based on their understanding of medical and ethical knowledge such as treatment goals,health values,and basic rights.From the ethical perspective,based on the shared decision marking between doctors and patients,this study deeply analyzes its ethical significance.Centering on the basic requirements of ethics,this study explores various practical models such as the doctor-patient joint participation model,the doctor-patient-family joint participation model,the doctor-nurse-patient joint participation model,the high-risk case conversation model of medical intervention,and the multidisciplinary team collaboration model from multiple perspectives and depths,to further promote the implementation of the shared decision marking between doctors and patients and to provide some reference basis.

Objective@#To determine the heterogeneity for caries prevention service preferences among children in Anhui Province, so as to provide reference for the promotion and popularization of caries prevention services for school age children.@*Methods@#Based on a discrete selection experiment, a face to face questionnaire survey was administered using a multi stage sampling method among 785 parents with children 3-12 years of age who were hospitalized in the stomatology clinics of 7 prefectures and cities in Anhui Province from October 2021 to October 2022. A mixed Logit model was used to evaluate caries prevention service preferences for children.@*Results@#Four discrete choice experiment attributes included in the study were statistically significant for choice preference ( P <0.05). Compared with the control group, parents with a high school education or above preferred caries prevention services with 70%-<80% preventive effectiveness, 2-<5 and <2 km from the service point, and a high service cost ( β =0.38, 1.66, 1.64, 0.00); female parents preferred preventive services with 70%-<80% preventive effectiveness and a high service cost ( β =0.35, 0.01 ); parents of children <7 years of age preferred services with 70%-<80% preventive effectiveness ( β =0.75); parents of children with oral health preferred preventive services during winter and summer vacations ( β =-0.28); parents of children with caries preferred preventive services with a high cost per denticle ( β =0.00)( P <0.05).@*Conclusions@#Parents with different education levels, gender, child age, and oral health status have heterogeneity in dental caries prevention service preferences. The provision of targeted and precise services can improve the participation and coverage of caries prevention services for school age children.

Objective:To discuss the possibility of modifying nonfunctional distractors to improve the quality of multiple-choice questions in medical examination through analyzing the changes in the difficulty and discrimination of items and the selection rate of modified distractors.Methods:Thirty-two multiple-choice questions involving nonfunctional distractors from a medical examination were studied. According to the item-writing guidelines, experts modified nonfunctional distractors based on actual measurements. We analyzed the changes in the difficulty and discrimination of items and the selection rate of modified distractors before and after item modification using the paired Wilcoxon test, and investigated the correlation of item difficulty and discrimination before and after item modification by Spearman's rank correlation analysis.Results:Six questions were modified due to technical factors, and 26 questions were modified due to weak interference in knowledge content. Before and after nonfunctional distractor modification, the difficulty values of the questions were 0.676 (0.558, 0.893) and 0.637 (0.531, 0.839), respectively; the discrimination values (point-biserial correlation) were 0.261 (0.150, 0.316) and 0.262 (0.138, 0.358), respectively; and the discrimination index values were 0.215 (0.113, 0.352) and 0.259 (0.138, 0.346), respectively. There were significant differences in the difficulty and discrimination index of items before and after modification. The difficulty and discrimination of items before and after modification were both significantly correlated. The selection rates of modified distractors were 0.009 (0.003, 0.015) and 0.044 (0.021, 0.092) before and after modification, respectively, which were significantly different.Conclusions:Nonfunctional distractors in this study were mainly caused by reasons in the contents of the test questions, less in technical defects. Through the analysis and modification of nonfunctional distractors, the selection rate of nonfunctional distractors can be effectively increased, and item difficulty and discrimination can be improved.

Objective:To understand the major cognition, major choice motivation and the relationship between the two of medical students, and provide references and suggestions for the selection of talents in various majors of medical schools and the effective development of enrollment work.Methods:This study selected undergraduates of Batch 2019 from Peking University Health Science Center as the survey objects, conducted a questionnaire survey on their major cognition, major choice motivation and influencing factors, and used principal component analysis and Spearman rank correlation analysis.Results:The study found that the major cognition scores of 640 undergraduates of Batch 2019 from Peking University Health Science Center were clinical medicine (3.24±0.89) > stomatology (2.89±1.00) > basic medicine (2.66±1.02) > pharmacy (2.54±0.97) > preventive medicine (2.29±0.93) > nursing medicine (2.21±0.99) > medical laboratory (1.98±0.95) > medical English (1.95±0.93). Six major motivation factors for professional choice were school and professional strength, professional learning and job prospects, own factors, Peking University sentiments and the influence of others, medical factors, school policies, and the contribution rates were 34.60%, 12.97%, 7.42%, 6.00%, 5.59% and 5.37%, respectively. Major cognition scores and major choice motivation factors were positively correlated with each other to some extent.Conclusions:At present, students' major cognition level of medical majors still has a large room for improvement, and the motivational factors of major choice are more complicated, among which "the school and professional strength" and "the prospects of study and work" are important factors. Medical schools should focus on strengthening major publicity, improving students' major cognition, attracting aspiring students to apply for medical majors from many aspects, and improving the training quality of medical professionals.

En la presente publicación se consideró la compilación de información disponible en sitios de la red informática al respecto de las diferentes instituciones públicas y privadas oferentes de postgrados en áreas de la odontología, en la intención de presentar un contexto a considerar antes de decidirse por un postgrado en alguna de las diferentes disciplinas estomatológicas disponibles en la República Mexicana. Considerando además las expectativas profesionales, académicas y económicas, vinculadas a la pertinencia, factibilidad y calidad de los postgrados. Material y métodos: se analizó la información disponible de 17 de las principales universidades ofertantes de los postgrados que han obtenido reconocimiento por su calidad académica, considerando sus ubicaciones, costos relativos y posicionamiento en el Ranking Nacional de Universidades en México. Por otro lado, se identificaron las expectativas expresadas por un grupo de estudiantes de licenciatura en Cirujano Dentista de la Universidad Autónoma de Chihuahua, al momento de elegir un postgrado, información obtenida de un estudio previo realizado en el año 2018 (AU)


In the following publication, a compilation of information available from various network sites were considered in regards to the different public and private institutions that offer a postgraduate program in the dental field. Also, the information collected was used to format all of the contextual considerations a prospective graduate must acknowledge like feasibility and quality of the program; as well as, professional, academic benefits and expenses, when deciding to enroll and pursue a dental specialty in Mexico. Material and methods: a total of 17 universities in the country were analysed and aspects like their location, general expenses (academic and living) academic quality awards received, as well as, their national ranking were taken into consideration. In addition, the results obtained from a previous study carried out in 2018 concerning a series of expectations expressed by a group of students from the Autonomous University of Chihuahua Dental School with respect to choosing a postgraduate Advance dental program were also included to enrich the following conclusions (AU)

Resumen La soledad individualista humana impele a vivir en pareja y a alcanzar complementariedad hombre/mujer, afecto y reci procidad. El periodismo científico de este artículo parafrasea el contenido de su bibliografía al revisar la elección de pareja, su satisfacción y complicaciones e instrucción preventiva de circunstancias que ocasionan ruptura. La falla -desde la elección-, desconoce los mecanismos entre razón e instinto sexual. El problema del amor no es ser amado sino desarrollar tal capacidad, pues lo extingue la inadaptación y monotonía. Se describe el acto elector/elegido y sus razones simultáneas: conscientes (homogeneidad/heterogeneidad del encuentro; emulación del modelo animal; tipo, dinámica vincular y redes sociales; aspectos sociodemográficos; ubicación geográfica y accesibilidad; compartición sexual y expectativas inmediatas/ futuras), e inconscientes (problemática electora, interacción diádica y antecedentes parentales; mecanismos de defensa, intercambio de disociaciones; idealización; reciprocidad negativa, lucha de poder, identificación proyectiva, búsqueda de aprecio por el otro; narcisismo, falso sí mismo, mecanis mos de dilación/evasión de responsabilidad, inseguridad, intrusión y colusión). Una pareja saludable, entiende/atiende la creación y mantenimiento del "nosotros", elimina la lucha de poder y produce conciliación permanente. El acuerdo y comprensión tolerante son indispensables para una relación emocional satisfactoria, estable y duradera.


Abstract Human individualistic loneliness impels us to live as a couple and achieve complementarity between man and woman, af fection, and reciprocity. The science journalism of this article reviews the choice of partner, their satisfaction and complications, and preventive instruction of circumstances that cause rupture. The failure from the election ignores the mechanisms between reason and sexual instinct. The problem of love is not to be loved but to develop such a capacity, for it is extinguished by maladjustment and monotony. The voting/elected act and its simultaneous reasons are described: Conscious (homogeneity/heterogeneity of the encounter; emulation of the animal model; type, dynamic link and social networks; Sociodemographic aspects; sexual sharing and immediate/future expectations). Unconscious (electoral problems, dyadic interaction, and parental background; defense mechanisms, exchange of dissociations; idealization; negative reciprocity, power struggle, projective identification, search for appreciation for the other; narcissism, false self, mecha nisms of procrastination/evasion of responsibility, insecurity, intrusion, and collusion). A healthy couple, understands/at tends to the creation and maintenance of the "we", eliminates the power struggle and produces permanent conciliation. Tolerant agreement and understanding are indispensable for a satisfying, stable, and lasting emotional relationship.

ABSTRACT Introduction: Although not mandatory, medical residency has become a sine qua non condition for practicing in most medical specialties in Brazil. Residency programs are hosted mainly by university accredited academic centers and hospitals in the national public healthcare system, under guidance and accreditation by a national commission. Despite the importance of these programs for the development of the hematology workforce, few studies have addressed their characteristics and impact on society. Methods: We performed a comprehensive cross-sectional survey of a 35-year alumni cohort from a hematology academic residency program in Brazil. Results: In total, 86/98 (87.8%) responded to the survey. The mean age at residency completion was 28.5 years, 60.5% of the alumni were women and sixty-four (74.4%) self-declared their skin color as white. Higher rates of parental education attainment and low rates of trainee financial dependence were observed and these patterns were stable over time. While the proportion of trainees from other states increased steadily, the number of hematologists practicing in other states remained stable. Approximately half of the alumni worked both in the private and public sectors, mainly in malignant hematology and in outpatient clinics. Twenty-five percent of the alumni reported prior leadership and teaching positions, mainly as directors of transfusion services. Conclusion: Our results provide data that can be potentially useful for policymakers and curricular development in the planning of strategies concerning the future workforce of hematologists.

Throughout the world, forensic odontology academic programmes have been designed and permeate discussions about international or local standard operational protocols, principles or guidelines requiring current technical and scientific knowledge. The heterogeneous groups of students who aim to pursue this career might have high educational and occupational aspirations that will be confronted with the labour markets. In this report, the authors briefly present aspects of the education and training that comprise the choice of a career in forensic odontology nowadays. In conclusion, the individual who opts for forensic odontology as a career must be prepared to find the confidence and resilience to practice professional skills in a unique and challenging field to comply with the society's expectation


Em todo o mundo, vários programas acadêmicos de Odontologia Legal têm sido criados e permeiam discussões sobre protocolos, princípios ou diretrizes operacionais, padrão internacional ou local, que exigem conhecimento técnico e científico atualizado. Os grupos heterogêneos de estudantes que pretendem seguir esta carreira poderão ter elevadas aspirações educativas e profissionais que serão confrontadas com os mercados de trabalho. Neste contexto, os autores apresentam brevemente aspectos da educação e formação que compõem a escolha de uma carreira em Odontologia Legal nos dias de hoje. Em conclusão, o indivíduo que opta pela Odontologia Legal como carreira deve estar preparado para encontrar confiança e resiliência para exercer habilidades profissionais em um campo único e desafiador para atender às expectativas da sociedade

Recent work in decision neuroscience suggests that visual saliency can interact with reward-based choice, and the lateral intraparietal cortex (LIP) is implicated in this process. In this study, we recorded from LIP neurons while monkeys performed a two alternative choice task in which the reward and luminance associated with each offer were varied independently. We discovered that the animal's choice was dictated by the reward amount while the luminance had a marginal effect. In the LIP, neuronal activity corresponded well with the animal's choice pattern, in that a majority of reward-modulated neurons encoded the reward amount in the neuron's preferred hemifield with a positive slope. In contrast, compared to their responses to low luminance, an approximately equal proportion of luminance-sensitive neurons responded to high luminance with increased or decreased activity, leading to a much weaker population-level response. Meanwhile, in the non-preferred hemifield, the strength of encoding for reward amount and luminance was positively correlated, suggesting the integration of these two factors in the LIP. Moreover, neurons encoding reward and luminance were homogeneously distributed along the anterior-posterior axis of the LIP. Overall, our study provides further evidence supporting the neural instantiation of a priority map in the LIP in reward-based decisions.

Objective:To find the preferences of nursing staff when they provide " Internet + nursing service" for elderly patients.Methods:By means of the discrete choice experiment, a questionnaire was designed to investigate the preferences of nursing staff when they provide " Internet + nursing services" for elderly patients. In-service nursing staff from 8 medical and health institutions in Ningbo city and Wenzhou city were selected by random sampling, for an online questionnaire survey conducted from May to June 2022. The preferences of nursing staff on their service income, service content, service distance, service continuity and residence status of elderly patients when providing " Internet + nursing service" were analyzed by mixed logit regression.Results:A total of 420 valid questionnaires were collected. Compared to 50 yuan/order, nursing staff preferred to a price of 150 yuan/order ( β= 1.22, P<0.001) nursing services; Compared to specialized nursing services, nursing staff preferred to the routine care ( β= 0.86, P<0.001) and health promotion ( β= 0.86, P<0.05) service; Compared to<5 km, nursing staff were unwilling to provide nursing services for elderly patients at distances of 5-10 km and 11-15 km ( β=-0.66, P<0.05; β=-0.95, P<0.001) ; Compared to 1-2 visits per month, nursing staff preferred not to provide continuing care services ( β=-0.70, P<0.05); Compared to homestay with the patient family, nursing staff preferred to provide nursing services for elderly patients residing in nursing homes ( β= 1.21, P<0.001) . Conclusions:Considering the preference of nursing staff tend to provide " Internet + nursing service" for the elderly patients with services featuring appropriate price, non-specialist care, close distance, low continuity (moderate intensity used as the reference) and security assurance for practice.

@#Introduction: The Healthier Choice Logo (HCL) was introduced in 2017 by the Ministry of Health Malaysia. This paper analysed acceptance of HCL, effectiveness of HCL in encouraging healthier product reformulation, and factors affecting reformulation among food industries. Methods: An online self-administered questionnaire consisting of four sections utilising multiple choice and 5-point Likert scale questions was distributed to food industries in Malaysia. Sample size calculation yielded 100 respondents. Results: Food industries had a higher acceptance of the processes and requirements involved in HCL implementation. HCL was highly effective in encouraging product reformulation among food industries in Malaysia. Meeting consumer demand, improving brand image, public health, more awareness around nutrition labelling, logo and national nutrition target, more technical knowledge and budget were found to motivate healthier product reformulation. However, product suitability, consumer acceptability, difficulties maintaining taste and shelf life, and limited budget were the challenges faced in product reformulation. There was no correlation between HCL acceptance and factors encouraging or inhibiting reformulation. Conclusion: These findings are expected to help relevant authorities or stakeholders make changes, if necessary, towards processes and requirements involved in HCL application to ensure wider HCL implementation. Future research should identify the relationship between HCL implementation and public health improvement among the Malaysian population.

Purpose/Significance The online health community provides a new channel for communication and interaction between patients and doctors.To study the influencing factors of patients'choice of doctors has practical significance for improving patient deci-sion-making efficiency,motivating doctors to improve services,and maintaining the healthy development of online health community.Method/Process Based on trust theory,perceived value theory and other theories,the hypothesis of influencing factors of pa-tients'medical choice behavior is put forward and a research model is constructed.Web crawler tools are used to collect relevant informa-tion about doctors provided by online health community,and regression analysis is used to test the hypothesis.Result/Conclusion Pa-tients'medical choice behavior is influenced by group size,online reputation,functional value,etc.,and online trust plays a mediating role in the impact of the above factors on patients'medical choice behavior.The study provides references for patients'medical choice,doctors'improvement of their own service level,and platform optimization of doctor evaluation mechanisms.

The secondary motor cortex (M2) encodes choice-related information and plays an important role in cue-guided actions. M2 neurons innervate the dorsal striatum (DS), which also contributes to decision-making behavior, yet how M2 modulates signals in the DS to influence perceptual decision-making is unclear. Using mice performing a visual Go/No-Go task, we showed that inactivating M2 projections to the DS impaired performance by increasing the false alarm (FA) rate to the reward-irrelevant No-Go stimulus. The choice signal of M2 neurons correlated with behavioral performance, and the inactivation of M2 neurons projecting to the DS reduced the choice signal in the DS. By measuring and manipulating the responses of direct or indirect pathway striatal neurons defined by M2 inputs, we found that the indirect pathway neurons exhibited a shorter response latency to the No-Go stimulus, and inactivating their early responses increased the FA rate. These results demonstrate that the M2-to-DS pathway is crucial for suppressing inappropriate responses in perceptual decision behavior.

Responsiveness optimisesthe system-based approach to meeting legitimate demands by healthcare recipients. This study assessed the responsiveness of orthopaedic services at the University of Port Harcourt Teaching Hospital (UPTH) from the perspectives of the care recipients. Methods:Descriptive cross-sectional study among 442 consecutively recruited recipients of orthopaedic services at UPTH from March to June 2020. Close-ended questionnaire with responsiveness conceptualised by five constructs: dignity, autonomy, confidentiality, quality of basic amenities and choice of care provider, each measured along 4-point response scale. The internal consistency reliability of the responsiveness scale was determined by the Cronbach's alpha coefficient. Descriptive (frequency, percentages, bar charts) and inferential (ordinal logistic regression) statistics were conducted and p-values ≤ 0.05 were considered statistically significant. Results: Response rate was 97.3% and the Cronbach's alpha coefficient for the responsiveness scale was 0.83. Participants' mean age was 38.5±14.8years with more being males (55.8%), privately employed (34.9%) and completed secondary education (82.5%). Proportion of respondents who gave excellent ratings across responsiveness domains were dignity (32.8%), autonomy (34.2%), confidentiality (26.3%), amenities (25.8%) and no excellent rating for choice of provider. Marital, employment and visit status were the most consistent factors associated with feedback on autonomy, choice of providers, confidentiality domains.Conclusion: More orthopaedic patients were pleased with the level of autonomy and dignity than choice of providers and quality of basic amenities. There is the need for enhanced responsiveness of orthopedic services to meeting the unique needs of patients and achieving improved quality of care and patient outcomes

Abstract Solubility of pharmaceutical drugs in organic solvents is one of the important parameters to understand the equilibrium concentration of solute-solvent, which helps optimize and design crystallization conditions to obtain the desired product crystals. In the present study, Chlorzoxazone (CHZ) is used as a model pharmaceutical compound to investigate the equilibrium solubility, the influence of solvent and the operating conditions on the shape, and the size distribution. The solubility of CHZ is determined in organic solvents like Isopropanol, Ethanol, and 2-Ethoxyethylacetate, Ethylacetate and Ethyllactate using shake flask method from -5ºC to 60ºC. The solubility of CHZ in these solvents shows an increasing trend as the temperature increases in the following order: ethyllactate + water (0.5+0.5) < ethylacetate < isopropanol < ethanol < 2-ethoxyethylacetate < ethyllactate + water (0.75+0.25). The solvents, isopropanol, ethanol, and ethyl lactate, produce needle-shaped crystals, while 2-ethoxyethylacetate and ethyl acetate tend to produce plate shaped crystals. CHZ crystals obtained from 2-ethoxyethylacetate tend to have plate shaped crystals with a lower aspect ratio and are selected for batch cooling crystallization experiments performed at different cooling rates, and agitation. It is found that the agitation at 300 rpm and the cooling rate 0.2ºC/min produce more uniform crystal size distribution
