Objective@#To explore the diagnosis, treatment and prognosis of synovial chondroma in the temporomandibular joint and to provide a reference for clinical diagnosis and treatment.@*Methods @#A case of right temporomandibular joint synovial chondromatosis admitted to the Stomatological Hospital, Southern Medical University was reported, and the related literature was reviewed and analyzed.@*Results @#During the movement of the mandibular opening, noise and obvious pain were found. The imaging features showed that several free calcification shadows of different sizes were seen around the condyle in the right articular fossa. The right temporomandibular joint mass resection and articular disc reduction were performed under general anesthesia, and the postoperative pathological results showed synovial chondromatosis. The patient′s symptoms were relieved 3 months after the surgery, and the imaging examination showed no residual lesions. A review of the literature shows that synovial chondromatosis usually occurs in large joints, such as the knee, hip, elbow, and shoulder joints, and rarely occurs in the temporomandibular joint. It occurs in middle-aged patients, manifesting as pain, swelling, and limited movement. Imaging studies play an important role in diagnosis, but the final diagnosis requires pathological diagnosis. Surgical treatment is effective, and synovial chondromatosis does not easily relapse.@*Conclusion@#The diagnosis of TMJ synovial chondroma should be combined with clinical manifestations, imaging features, and pathological examination. Surgery is an effective treatment. After completely removing the diseased and affected tissues, the disease has a good prognosis and does not easily relapse.

RESUMEN: El término "Transtornos Temporomandibulares" (TTM) incluye numerosos problemas clínicos asociados con las articulaciones temporomandibulares, músculos de la masticación y otras estructuras asociadas. El bruxismo, un hábito oral parafuncional, consiste en rechinar o apretar de manera rítmica e involuntaria los dientes, lo que puede conducir a trauma oclusal y problemas articulares. El objetivo del estudio fue determinar la frecuencia y prevalencia de signos y síntomas de TTM y bruxismo en niños escolares de 6 a 12 años, en una población de San Luis Potosí, México. Se empleó un diseño transversal descriptivo en una muestra de 314 participantes, estudiantes de una escuela primaria de la ciudad de San Luis Potosí, México. Los participantes fueron seleccionados aleatoriamente en forma estratificada y polietápica. La muestra total consistió en 153 femeninos y 161 masculinos. Los signos y síntomas de TTM más frecuentes fueron la alteración de la función de la ATM (35 %), seguido por los ruidos y chasquido articulares (29,2 %). En los sujetos con diagnóstico de bruxismo, la alteración más frecuente fueron molestias asociadas en la ATM (19,4 %), dolor de cabeza (17,8 %) y atrición dental (16,5 %).


ABSTRACT: The term "Temporomandibular disorders" (TTM) includes numerous clinical problems associated with joint temporomandibular, muscles of mastication and other associated structures. Bruxism, oral parafunctional habit, consists of grinding or clenching of involuntary, rhythmic manner, which can lead to trauma, occlusal and problems joint. The objective of the study was to determine the frequency and prevalence of signs and symptoms of DTM and bruxism in school children aged 6 to 12, in a population of San Luis Potosí, México. A crosssectional descriptive design was used in a sample of 314 participants, students of a primary school in the city of San Luis Potosi, Mexico. The participants were randomly selected in tiered form and conglomerated. The total sample consisted of 153 female and male 161. The signs and symptoms of TMD frequent were alteration of the function of the ATM (35 %), followed by noise and snap joint (29.2 %). In subjects with a diagnosis of bruxism, the most frequent alteration was associated with TMJ discomfort (19.4 %), headache (17.8 %) and dental attrition (16.5 %).

Résumé

Objective To analyse the MRI characteristics of the chronic clicking temporomandibular joints(TMJs).Methods 20 TMJs in 10 young chronic clicking individuals (12 clicking TMJs)and 60 TMJs in 30 young healthy volunteers were examined by MRI scanner, and sagittal oblique opened-mouth and closed-mouth position images were observed.Then comparing the MRI findings of the acquired j oints between two groups.Results The ratio of the non-double concave articular disc,slightly high signal intensity in the articular disc, abnormal changes of the condyle,articular fluid,anterior displacement of the articular disc,and increasedDraceangles in the clicking group were significantly higher than those in the control group(P<0.05).The result revealed statistically significant differences.Conclusion The MRI anatomical characteristics can be an indicator for the TMJ anomalys,and provide an obj ective radiological basis for clinical diagnosis and treatment to some extent.

This study was performed to evaluate the clinical features and treatment results on 55 temporomandibular disorder patients who had treated by conservative treatment using occlusal splint. The results were as follows; 1. The ratio of men to women was about 1:2.2 and most of the patients were second and third decades. 2. Pain was the most frequent symptom, followed by clicking and mouth opening limitation. 3. The number of acute and chronic groups on the basis of 6 months duration of symptoms were similar. 4. Most of patients had Angle's Class 1 molar relationships(78.2%), followed by Class 3 and Class 2. 5. Centric Relation splints were used alone for treatment of 34 patients and 21 patients were treated with Centric Relation splints and Anterior Repositioning splints. The treatment duration of the patients who had complained pain was average 9.8 weeks and 6 patients of them had slight pain continuously during follow-up. 6. Eleven patient's maximum mouth opening who had mouth opening limitation was improved from 30.7mm to 43.0mm during procedures.

The purpose of this study was to evaluate the diagnostic value of SONOPAK* in internal derangement of TMJ by comparing the spectral analysis data of TMJ sounds recorded by SONOPAK with the results of MRI. From the patients who came to Department of Orthodontics, Seoul National University Dental Hospital for treatment of malocclusion, eighteen adult patients (five males and thirteen females) with TMD symptoms were selected. After joint sounds were checked by a pediatric stethoscope, they were recorded and analyzed by the SONOPAK. The spectral analysis of the SONOPAK provided SONOPAK INTERPRETATION data about the stage of internal derangement, which were compared with the results of MRI. Among the patients whose disc positions were diagnosed as 'normal' by MRI, there were no false positive diagnosis by the SONOPAK INTERPRETATION. But in the cases of anterior disc displacements(reducible and/or nonreducible), most of SONOPAK INTERPRETATION data did not coincide with MRI results. In conclusion, it is not adequate to try to differentiate reducible and non-reducible anterior disc displacements on the basis of joint sounds. And it is recommended not to determine the stage of internal derangement according to the nature of sounds. We suggest that the diagnostic value of the SONOPAK will be enhanced when clinicians combine some other informations such as clinical signs/symptoms and other supplementary diagnostic tools), and that more data be incoporated into the SONOPAK INTERPRETATION software.

El objetivo este estudio, fue identificar la importancia que le da a diferentes aspectos relacionados con la aparición de ruidos articulares (RA) la articulación en temporomandibular (ATM), un grupo de 290 odontólogos. A los odontólogos se les aplicó una encuesta, en donde se les preguntó acerca de la importancia que les merecía la aparición de RA (clicking y crepitación) y su tratamiento, en ausencia o presencia de otros signos o síntomas; como también, acerca de la relación causal entre maloclusión y RA y sobre la posibilidad de considerarlos normales. Los resultados mostraron que para estos odontólogos, la aparición de RA y su tratamiento, son importantes; ya sea que se presenten, o no, asociados con otros signos o síntomas; y además, consideran que la relación causal entre maloclusión y aparición de RA, es alta.


The purpose of this study was to identify the value of different aspects related with the apperance of TMJ sounds by the dentists. A survey was conducted to the dentists and they were enquired about the importance attached to joint sounds (clicking and crepitus) and their treatment when other signs or symptoms are present or absent. Also, they were asked about the causal relation between malocclusion and joint sounds and the possibility of being considered normal. The results showed that the presence of TMJ sounds and their treatment are important for the dentist, whether the sounds are associated or not with other signs or symptoms. Morover, they considered the causal relation between malocclusion and presence of joint sounds high.
